지르코니움 세라믹에서 표면 처리 방법이 레진 시멘트의 접착력에 미치는 영향 (Effects of Surface Treatments of The Zirconium-Based Ceramic on the Bond Strength of Resin Cement)

  • Recently, the need for esthetic results has increased the interest for all-ceramic crown prosthesis. Furthermore, the development of zirconium core via CAD/CAM system has allowed the all ceramic restorations to be applied to almost all fixed prosthesis situations. But, the increased strength has been reported to increase in proportion with the bond strength of cement, and recently, the tribochemical system which increases the bond strength through, silica coating and silanization has been introduced. The purpose of this study was to compare the $Rocatec^{TM}$ system and $CoJet^{TM}$ system with the traditional acid etching and silanization method of the irconium based ceramic. The surface character was observed via SEM(X2000), and the bond strength with the resin cement were measured. 50 In-Ceram Zirconia (Adens, Korea) discs were fabricated and embedded in resin, group 1 was treated with glass-bead blasting and cleaning, group 2 was treated with 20% HF for 10 minutes and silanized, group 3 was treated with the $Rocatec^{TM}$ system, and group 4 was treated with the $CoJet^{TM}$ system. Each group was comprised of 10 specimens. The specimens were cemented to a $3mm{\times}5mm$ resin block with Super-Bond C&B. The shear bond strength was measured with the $Instron^{(R)}$ 8871 at a crosshead speed of 0.5mm/min. The results were as follows. 1. According to SEM results, there were little difference between group 1 & group 2, but in group 3 and 4, silica coating was detected and there was increase in surface roughness. 2. The shear bond strength decreased in the order of group 3(46.28MPa), group 4(42.04MPa), group 2(31.56MPa), and group 1(27.46MPa). 3. There was significant differnce between group 1&2 and group 3&4(p<0.05). From the results above, it can be considered that the conventional method of acid etching and silane treatment cannot increase the bond strength with resin cements, and that by applying the tribochemical system of $Rocatec^{TM}$ system and $CoJet^{TM}$ system, we can achieve a stronger all ceramic restoration. Further studies on surface treatments to increase the bond strength are thought to be needed.

  • By the concerns of esthetic restoration were increased recently. many all ceramic crowns were developed. But they except In-Ceram Alumina were used only single crown. In-Ceram Alumina, developed by Dr. Sadon, was revealed to have high flexural Strength(450MPa). So it could be used not only anterior bridges but also posterior bridges. But In-Ceram Alumina was seen to be opaque, a little green color in transillumination light by high content of alumina oxide(85%). So new all ceramics with high strength and high translucence were needed. Spinell($MgAl_2O_3$) have a high melting point, high flexural strength, low heat conductivity, high light conductivity. In-Ceram Spinell offers glasslike light transmission by using the spinell cores instead of the alumina cores. And they have a high translucency like to natural tooth, an excellent margin integrity and a high strength(350MPa). The purposes of this study are 1) to know about the construction method of In-Ceram Spinell System, 2) to investigate the its clinical possibiliy through patients and literature reviews.

마이크로파 재가열이 쌀밥의 품질에 미치는 영향 (Effect of Microwave Reheating on the Quality of Cooked Rice)

  • Changes in quality of cooked rice after microwave reheating were evaluated. Ceramic coated microwave oven reduced a reheating time(1 min). Storage temperature, storage time and microwave reheating did not affect the color value of cooked rice after microwave reheating. Hardness of cooked rice after microwave reheating decreased in frozen storage treatment. Photomicrographs of cross-section of cooked rice after microwave reheating showed restoration of starch. Effect of storage temperature and time were not significant in appearance, aroma, taste and texture of cooked rice after microwave reheating for sensory evaluation test.

  • In order to recover the esthetic of the missing anterior area, we must consider color, shape of a tooth and the functions of a prosthesis that is harmonized with the surrounding factors. When the tooth of a natural tooth is exposed, we face to challenge to restore them. several factors cause resorption of bones, in flammation, and gum trouble, and a defective prosthesis damages the aesthetics of the gingiva. There are two ways how to solve these problems. One can be done by periodontal surgery and the other can be done by prosthesis. Most patients want aesthetic restoration that is harmonized with their teeth and gingiva. So if prosthesis is fabricated to expose the root exposed, it is far from the patient's expectation.

Nanofilled 복합레진으로 와동 충전 시 flowable 레진 사용 유무에 따른 피로시험 후의 미세 변연 누출 비교 (Comparison of microleakage after load cycling for nanofilled composite resin fillings with or without flowable resin lining)

  • 연구 목적: 제2급 와동에서 nanofilled 레진 충전 시 기구 조작 방향에 따라 변연 누출 가능성이 있다. 따라서 기구 조작에 따른 변연 누출을 줄이고자 인접면변연에 flowable 레진의 사용 유무에 따라 반복 하중 전후에 미세 변연 누출의 차이를 보고자 하였다. 연구 재료 및 방법: 건전한 상태의 24개의 발치 된 치아를 제2급 와동으로 형성한 뒤 그룹F는 nanofilled 레진으로 충전 시 flowable 레진을 liner로 사용하였고, 그룹 NF는 nanofilled 레진으로만 충전 하였다. 300N의 하중으로 $10^4$, $10^5$, $10^6$번의 반복하중을 가했다. 반복하중 전후로 모든 시편에서 전체 미세 변연 누출 길이 (T), 축면 미세 변연 누출 길이 (A), 치은 미세 변연 누출 길이 (G), 협측 미세 변연 누출 길이 (B), 설측 미세 변연 누출 길이 (L)를 측정하여, 각변연누출비율을 비교하였다. 통계는 Mann-Whitney test와 Wilcoxon signed ranks test로 하였다. 결과: 하중 전에 그룹F의 전체 미세 변연 누출 백분율은 0.6%이고 그룹 NF는 5.6%이며, 하중 후에는 그룹 F는 13.0%이고 그룹 NF는 36.3%로 그룹 NF에서 미세 변연 누출이 컸고, 반복 하중 횟수가 증가할수록 더욱 커졌으며, 부위상으로는 치은 변연의 미세 변연 누출이 컸다. 통계적 분석 결과 그룹NF의반복횟수별 하중 전후 비교에서 경계성으로 유의한 차이를 보였다. 결론: 제2급 와동에서 nanofilled 레진 충전 시 인접면 변연에 flowable 레진을 liner로 사용할 경우 반복 하중 전후에 미세 변연 누출의 감소에 영향을 주었다. 그리고 liner로 사용된 flowable 레진은 300 N의 반복 하중에서도 견딜 수 있었다.

두 종류의 CAD/CAM 지르코니아 전부도재관의 도재 축성 전후의 변연적합도 비교 (Comparison of marginal fit before and after porcelain build-up of two kinds of CAD/CAM zirconia all-ceramic restorations)

  • 연구목적: 변연적합도는 보철수복물의 성공을 좌우하는 중요한 요소 중의 하나이다. McLean과 von Fraunhofer등에 의하면 120 ${\mu}m$ 이하를 임상적으로 허용할 수 있다고 했는데, 이를 넘는 정확하지 않은 변연은 지각과민을 일으키거나 이차 우식을 일으키고, 치태 축적을 용이하게 하여 보철물의 수명을 단축시키게 된다. 이에 최근에 우리나라에 소개되어 임상적으로 사용되고 있는 CAD-CAM 전부도재 수복물 중 $LAVA^{(R)}$ 시스템 (3M ESPE, St. Paul, MN)과 $EVEREST^{(R)}$ 시스템 (KaVo Dental GmbH, Biberach, Germany)으로 제작한 지르코니아 전부 도재관들이 도재전장 금속주조 수복물과 비교해 보아 코핑 상태일 때와 도재축성 후 각각 변연적합도의 차이가 있는지를 살펴보고자 하였다. 재료와 방법: 상악 제1소구치 덴티폼 치아를 high speed diamond bur 로 교합면 2.0 mm, 순설면 1.0 mm, 그리고 변연은 chamfer margin을 6도 측면 경사 절삭 기계로 균일하게 제작해서 이를 인상 채득하여 금속 지대치를 제작하였다. 이를 덴티폼 모형상에 장착한 후 전악인상을 채득하여 $LAVA^{(R)}$ 수복물 코핑, $EVEREST^{(R)}$ 수복물 코핑, 그리고 도재전장 금속주조 수복물 코핑을 각각 8개씩 제작하였다. 이들 코핑을 염색제를 섞은 Fit-checker $II^{(R)}$ (GC Cor., Tokyo, Japan)를 이용하여 금속 지대치에 접합시킨 뒤, $Microhiscope^{(R)}$ system (HIROX KH-1000 ING Plus, Seoul, Korea)을 이용하여 300배율로 관찰하면서 변연 간격을 ${\mu}m$단위로 측정하였다. 도재축성 후에도 마찬가지 방법으로 모든 수복물의 변연간격을 측정하여 통계 처리를 하였다. 결과: 코핑 상태에서의 평균 변연간격 값은 $EVEREST^{(R)}$ 에서 $52.00{\pm}11.94\;{\mu}m$, $LAVA^{(R)}$$ 56.97{\pm}10.00\;{\mu}m$, 그리고 도재 전장 금속 주조 수복물은 $97.38{\pm}18.54\;{\mu}m$를 보였고, 도재축성 후에 평균 변연 간격값은 $EVEREST^{(R)}$$61.69{\pm}19.33\;{\mu}m$, $LAVA^{(R)}$$70.81{\pm}12.99\;{\mu}m$, 그리고 도재 전장 금속 주조 수복물은 $115.25{\pm}23.86\;{\mu}m$를 보였다. 결론: 1. $LAVA^{(R)}$$EVEREST^{(R)}$ 수복물은 도재 전장 금속 주조 수복물과 비교하여 각각 코핑상태일 때와 도재 축성 후 모두 유의성 있는 (P < .05) 차이를 보이는 우수한 변연 적합도를 나타내었다. 2. $LAVA^{(R)}$$EVEREST^{(R)}$ 수복물 간의 평균 변연 간격 비교에서 코핑 상태일 때와 도재 축성 후에 모두 유의성 있는 차이는 없었다 (P > .05). 3. $LAVA^{(R)}$$EVEREST^{(R)}$ 그리고 PFM 은 각각 코어 상태일 때와 비교하여 도재 축성 후 변연 간격에서 약간 증가되었으나 유의성 있는 차이를 보이지는 않았다 (P > .05).

복합 치관-치근 파절의 교정적 정출술을 이용한 심미적 수복 (Esthetic Restoration of Complicated Crown-Root Fractures Utilizing Orthodontic Extrusion)

  • 영구 전치의 복합 치관-치근 파절은 심미적, 기능적 문제 뿐 아니라 심리적 문제도 발생시킨다. 그러므로 파절에 대한 치료는 매우 중요하며, 여러 분야에 걸친 종합적 치료가 요구된다. 본 증례는 두 환자에서의 외상으로 인한 상악 전치의 복합 치관-치근 파절에 대한 임상적 치료 과정을 서술하였다. 근관치료와 근첨형성술이 각 환자에게 시행되었으며, 파절선을 치은 상방으로 노출시키고 생물학적 폭경을 재확립하기 위해 교정적 정출을 시행하였다. 이후 유지기간을 두었고, 그 기간 동안 치아에 임시수복을 시행하였다. 최종적으로 CAD/CAM을 이용한 세라믹 크라운 수복을 시행하였다. 이후 정기 검진 때에도 해당 치아들은 만족스러운 기능적, 심미적 결과를 보였으며, 두 환자 모두에서 재발은 나타나지 않았다. 치주조직 또한 건강한 상태를 보였다.

Comparison of three-dimensional digital technique with two-dimensional replica method for measuring marginal and internal fit of full coverage restorations

  • PURPOSE. This study compared digital (reference point matching) and replica methods for measuring marginal and internal fit of full coverage restorations. MATERIALS AND METHODS. A maxillary left first molar typodent was fixed on to an aluminum base and prepared to receive all-ceramic full coverage restoration. The model was scanned with an intraoral scanner (CEREC Omnicam, Sirona, York, PA, USA). Twelve crowns were fabricated from lithium disilicate blocks (IPS emax CAD, Ivoclar Vivadent) and then crystalized. Marginal and internal fit of each restoration was measured by two examiners using replica and a new digital three-dimensional technique. Reliability between the two methods and two examiners was assessed by correlation and Cronbach's Alpha coefficient (P<.05). A Bland-Altman assessment for agreement was used to compare the two methods. RESULTS. Bland-Altman assessment showed that the mean of difference for marginal, absolute marginal, and axial gap was respectively -1.04 ㎛, -41.9 ㎛, and -29.53 ㎛ with limit of agreement (LOA) between -37.26 to 35.18 ㎛ for marginal, -105.85 to 22.05 ㎛ for absolute marginal and -80.52 to 22.02 ㎛ for axial gap. Positive correlation for repeatability (P<.05) in determining marginal and internal gaps by the two examiners in both techniques was revealed. Reliability of both techniques in all sites of measurements was at least good (0.8 ≤ α < 0.9). CONCLUSION. Both measuring techniques appeared highly reliable for evaluating fit of fixed dental restorations, while reference point matching provided higher values in axial and absolute marginal gap assessment.

Clinical performance and failures of zirconia-based fixed partial dentures: a review literature

  • PURPOSE. Zirconia has been used in clinical dentistry for approximately a decade, and there have been several reports regarding the clinical performance and survival rates of zirconia-based restorations. The aim of this article was to review the literatures published from 2000 to 2010 regarding the clinical performance and the causes of failure of zirconia fixed partial dentures (FPDs). MATERIALS AND METHODS. An electronic search of English peer-reviewed dental literatures was performed through PubMed to obtain all the clinical studies focused on the performance of the zirconia FPDs. The electronic search was supplemented by manual searching through the references of the selected articles for possible inclusion of some articles. Randomized controlled clinical trials, longitudinal prospective and retrospective cohort studies were the focuses of this review. Articles that did not focus on the restoration of teeth using zirconia-based restorations were excluded from this review. RESULTS. There have been three studies for the study of zirconia single crowns. The clinical outcome was satisfactory (acceptable) according to the CDA evaluation. There have been 14 studies for the study of zirconia FPDs. The survival rates of zirconia anterior and posterior FPDs ranged between 73.9% - 100% after 2 - 5 years. The causes of failure were veneer fracture, ceramic core fracture, abutment tooth fracture, secondary caries, and restoration dislodgment. CONCLUSION. The overall performance of zirconia FPDs was satisfactory according to either USPHS criteria or CDA evaluations. Fracture resistance of core and veneering ceramics, bonding between core and veneering materials, and marginal discrepancy of zirconia-based restorations were discussed as the causes of failure. Because of its repeated occurrence in many studies, future researches are essentially required to clarify this problem and to reduce the fracture incident.

Comparative analysis of transmittance for different types of commercially available zirconia and lithium disilicate materials

  • PURPOSE. Translucency and colour stability are two most important aspects for an aesthetic dental restoration. Glass ceramic restorations are popular amongst clinicians because of their superior aesthetic properties. In the last decade, zirconia has generated tremendous interest due to its favorable mechanical and biological properties. However, zirconia lacks the translucency that lithium disilicate materials possess and therefore has limitations in its use, especially in esthetically demanding situations. There has been a great thrust in research towards developing translucent zirconia materials for dental restorations. The objective of the study was to evaluate and compare the transmittance of a translucent variant of zirconia to lithium disilicate. MATERIALS AND METHODS. Two commercially available zirconia materials (conventional and high translucency) and 2 lithium disilicate materials (conventional and high translucency) with standardized dimensions were fabricated. Transmittance values were measured for all samples followed by a microstructural analysis using a finite element scanning electron microscope. One way analysis of variance combined with a Tukey-post hoc test was used to analyze the data obtained (P=.05). RESULTS. High translucency lithium disilicate showed highest transmittance of all materials studied, followed by conventional lithium disilicate, high translucency zirconia and conventional zirconia. The difference between all groups of materials was statistically significant. The transmittance of the different materials correlated to their microstructure analysis. CONCLUSION. Despite manufacturers' efforts to make zirconia significantly more translucent, the transmittance values of these materials still do not match conventional lithium disilicate. More research is required on zirconia towards making the material more translucent for its potential use as esthetic monolithic restoration.
