The Adjunctive Role of Resectional Surgery for the Treatment of Multidrug-Resistant Pulmonary Tuberculosis (다제내성 폐결핵의 치료에서 폐절제술의 보조적인 역할)

  • Background : Many patients with isoniazid and rifampin-resistant pulmonary tuberculosis have organisms that are also resistant to other first-line drugs. Despite of aggressive retreatment chemotherapy, the results are often unsuccessful, with a failure rate approaching 40%. Recently, there has been a revival of resectional surgery for the treatment of multidrug-resistant pulmonary tuberculosis. Methods : A retrospective analyses of the case records and radiographic findings were done. Between January 1991 and December 1995, 14 human immunodeficiency virus (HIV)-seronegative patients with multidrug-resistant pulmonary tuberculosis were selected for resection to supplement chemotherapy. All patients had organisms resistant to many of the first-line drugs, including both isoniazid and rifampin. Results : Despite of aggressive therapy for median duration of 9.5 months, 12 of the 14 patients (86%) were still sputum smear and/or culture positive at the time of surgery. The disease was generally extensive. Although main lesions of the disease including thick-walled cavities were localized in one lung, lesser amounts of contralateral disease were demonstrated in 10 of 14 (71%). Types of surgery performed were pneumonectomy including extrapleural pneumonectomy in six patients, lobectomy or lobectomy plus in six patients, and segmentectomy in two patients. The resected lung appeared to have poor function ; preoperative perfusion lung scan showed only 4.8% of the total perfusion to the resected portion of the lung. There were no operative deaths. Two patients had major postoperative complications including empyema with bronchopleural fistula and prolonged air leak, respectively. Of the 14 patients, 13 (93%) remained sputum-culture-negative for M. tuberculosis for a median duration of 23 months and one remained continuously sputum smear and culture positive for M. tuberculosis. Conclusion : On the basis of comparison with historical controls, adjunctive resectional surgery appears to play a significant beneficial role in the management of patients with multidrug-resistant pulmonary tuberculosis if the disease is localized and there are adequate reserve in pulmonary function.

  • A case of breast sparganosis was confirmed by surgical excision of a worm (fragmented into 5 pieces) in a 59-year-old Korean woman suffering from a palpable mass in the left breast. Mammography and ultrasonography characteristically revealed the presence of several well-defined, isodense and hypoechoic tubular masses, in the upper quadrant of the left breast, each mass consisting of a continuous cord- or worm-like structure. During surgery, a long segment of an actively moving sparganum of Spirometra sp. and 4 small fragments of the same worm, giving a total length of 20.3 cm, were extracted from the upper outer quadrant of the left breast and the axillary region. The infection source remains unclear, because the patient denied ingesting any snake or frog meat or drinking untreated water.

A Neonatal Form of Alexander Disease Presented with Intractable Seizures and Obstructive Hydrocephalus

  • Alexander disease is a rare degenerative leukodystrophy caused by dominant mutations in glial fibrillary acidic protein (GFAP). The neonatal form of Alexander disease may manifest as frequent and intractable seizures or obstructive hydrocephalus, with rapid progression leading to severe disability or death within two years. We report a case of a 50-day-old male who presented with intractable seizures and obstructive hydrocephalus. His initial magnetic resonance imaging (MRI) suggested a tumor-like lesion in the tectal area causing obstructive hydrocephalus. Despite endoscopic third ventriculostomy and multiple administrations of antiepileptic drugs, the patient experienced intractable seizures with rapid deterioration of his clinical status. After reviewing serial brain MRI scans, Alexander disease was suspected. Subsequently, we confirmed the de novo missense mutation in GFAP (c.1096T>C, Y366H). Although the onset was slightly delayed from the neonatal period (50 days old), we concluded that the overall clinical features were consistent with the neonatal form of Alexander disease. Furthermore, we also suspected that a Y366 residue might be closely linked to the neonatal form of Alexander disease based on a literature review.

Study on the Protective Mechanism of Vitamin C in the SH-SY5Y Cell Death Induced by the Streptozotocin (스트렙토조토신으로 유도된 신경세포사멸에서 비타민 C의 보호 기전 연구)

  • In this study, we analyzed the protective effects of the vitamin C in the streptozotocin (STZ)-induced apoptosis using the SH-SY5Y, a neuroblastoma cell line. The cells were pretreated with the vitamin C ($100{\mu}g$) for 30 min, followed by the 24-hr treatment with the 2.5-mM STZ. The cell-viability assay using the Cell Counting Kit (CCK)-8 revealed the cell-survival rate increased by 15% following the vitamin-C pretreatment compared to the STZ-only treatment. Moreover, we conducted the western-blot analysis to determine the protective effect of the vitamin C regarding the apoptosis. Compared to those in the STZ-only-treatment group, the p-ERK and Bcl-2 expressions increased in the vitamin-C-pretreatment group, whereas the p-JNK and Bax expressions decreased. The vitamin-C pretreatment increased the expression of the SOD-1, an antioxidant enzyme, by more than 30%, indicating its protective role in the STZ-induced oxidative stress. Also, we found both the intrinsic- and extrinsic-pathway mechanisms of the STZ-induced apoptosis. The results of this study $s{\mu}ggest$ vitamin C may help prevent the neurodegenerative diseases.

Analytical Application of Glow Discharge Atomic Absorption Spectroscopy (GD-AAS) Using Three Types of Jet Configurations Under Power Mode

  • Three anode configurations of six-jet, cone-jet and cylindrical-jet are tested for their analytical performance under power mode operation. The effect of pressure, power and gas flow rate on atomic absorption signals have been studied. The increase of atomic absorption signal of sample element is observed at a fixed pressure in all configurations as the gas flow rate increase up to 300-600 seem, and as the power dissipated in the glow discharge cell increase. The lower the pressure is in the glow discharge cell at a fixed discharge power and argon flow rate, the greater the absorbance of sample element is. The optimum conditions are taken from these data and a calibration curve of Cu in low-alloy steel sample is obtained. In this calibration curve, six-jet configuration shows the best analytical results varies as the sample element.

Quality of Life in Patients Underwent Microlaryngeal Surgery (후두미세수술을 시행 받은 환자의 삶의 질)

  • Objectives: Prospective study of quality of life in patients underwent microlaryngeal surgery for dysphonia was performed. Materials and Methods : 51 patients with dysphonia took part in the study, and patients with malignancies or functional voice disorder were excluded. Patients were asked to complete the SF-36 questionnaire before surgery and within 6-12 months after surgery. Preoperative and postoperative SF-36 scores were compared with data on 46 age-matched healthy controls. Results The most of SF-36 subscale scores showed significantly improvement after surgery, except of PF (physical functioning) and MH (mental health). Preoperative scores had significantly poorer than the normal controls on 6 subscales, but there is no statistically significant differences between postoperative scores and normal controls on 5 subscales. Conclusion In the study, patients with organic voice disorder show improvement in quality of life after microlaryngeal surgery.

  • Machine learning model gained the popularity in recent years as multi-national companies have incorporated machine learning in their services. Such service is called machine learning as a service (MLaSS). Such services are provided to users based on charge-per-query which triggers the motivations for adversaries to steal the trained victim model to reduce the cost of using the service. Therefore, it is important for companies that provide MLaSS to protect their intellectual property (IP) against adversaries. It has been arms race between the attack and defence in a context of the privacy of machine learning models. In this paper, we provide a comprehensive study of recent development in protecting privacy of machine learning models.

Metastatic intestinal adenocarcinoma with osseous metaplasia in two Domestic Korean Shorthair cats

  • Two Domestic Korean Shorthair cats presented with dyschezia and vomiting. Computed tomography revealed a colonic mass with calcification and lymph node metastasis in case 1, and a small intestinal mass with disseminated mesenteric metastasis and calcification in case 2. Histopathology revealed intestinal adenocarcinoma with osseous metaplasia. Case 1 died two months after surgery from distant metastasis; and case 2 showed no metastasis for five months but presented with anorexia, euthanized seven months after diagnosis. Metastatic intestinal adenocarcinoma with bone formation should be considered as differential diagnosis for calcification on imaging, and lymph node metastasis at diagnosis may indicate poor prognosis.

  • Amylose contents and amylopectin chain architecture of sorghmn and waxy sorghum starches were determined and compared with those of other common cereal and tuber starches. Also, in vitro digestibility of sorghum starch was estimated using a novel methodology. The absolute amylose content of sorghum starch was similar to that of com and wheat starches. The side chain length distribution patterns for sorghum and waxy sorghum amylopectin were very similar to those of com and waxy com, respectively. The $k_{cat}/K_m$ values for sorghum and potato amylopectin did not show a significant difference. The kinetic parameters could be used as novel indicators for starch digestibility.
