Clinical Outcomes after Upfront Surgery in Clinical Stage I-IIA Small Cell Lung Cancer

  • Background: Upfront surgery followed by systemic treatment is recommended to treat clinical stage I-IIA small cell lung cancer (SCLC), but data on the clinical outcomes are sparse. Thus, this study evaluated the stage migration and long-term prognosis of surgically treated clinical stage I-IIA SCLC. Methods: We retrospectively reviewed 49 patients with clinical stage I-IIA SCLC who underwent upfront surgery between 2000 and 2020. Additionally, we re-evaluated the TNM (tumor-node-metastasis) staging according to the eighth edition of the American Joint Committee on Cancer staging system for lung cancer. Results: The clinical stages of SCLC were cIA in 75.5%, cIB in 18.4%, and cIIA in 6.1% of patients. A preoperative histologic diagnosis was made in 65.3% of patients. Lobectomy and systematic lymph node dissection were performed in 77.6% and 83.7% of patients, respectively. The pathological stages were pI in 67.3%, pII in 24.5%, pIII in 4.1%, and pIV in 4.1% of patients. The concordance rate between clinical and pathological stages was 44.9%, and the upstaging rate was 49.0%. The 5-year overall survival (OS) rate was 67.8%. No significant difference in OS was found between stages pI and pII. However, the OS for stages pIII/IV was significantly worse than for stages pI/II (p<0.001). Conclusion: In clinical stage I-IIA SCLC, approximately half of the patients were pathologically upstaged, and OS was favorable after upfront surgery, particularly in pI/II patients. The poor prognosis of pIII/IV patients indicates the necessity of intensive preoperative pathologic mediastinal staging.

Analysis of Meteorological Characteristics by Fine Dust Classification on the Korean Peninsula, 2015~2021 (2015년~2021년 한반도 고농도 미세먼지 사례의 유형분류에 따른 기상학적 특징 분석)

  • From 2015 to 2021, high-concentration fine dust episodes with a daily average PM2.5 concentration of 50 ㎍ m-3 or higher were selected and classified into 3 types [long range transport (LRT), mixed (MIX) and Local emission and stagnant (LES)] using synoptic chart and backward trajectory analysis. And relationships between the fine particle data (PM2.5 and PM10 concentration and PM2.5/PM10 ratio) and meteorological data (PBLH, Ta, WS, U-wind, and Rainfall) were analyzed using hourly observation for the classification episodes on the Korean Peninsula and the Seoul metropolitan area (SMA). In LRT, relatively large particles such as dust are usually included, and in LES, fine particle is abundant. In the Korean peninsula, the rainfall was relatively increased centered on the middle and western coasts in MIX and LES. In the SMA, wind speed was rather strong in LRT and weak in LES. In LRT, rainfall was centered in Seoul, and in MIX and LES, rainfall appeared around Seoul. However, when the dust cases were excluded, the difference between the LRT and other types of air quality was decreased, but the meteorological variables (Ta, RH, Pa, PBLH, etc.) were further strengthened. In the case of the Korean Peninsula, it is difficult to find a clear relationship because regional influences (topographical elevation, cities and coasts, etc.) are complexly included in a rather wide area. In the SMA, it is analyzed that the effects of urbanization such as the urban heat island centered on Seoul coincide with the sea and land winds, resulting in a combination of high concentrations and meteorological phenomena.

  • Purpose : The aim of this study was to evaluate the clinical efficacy of two commercially available desensitizing agents over 3 weeks on patients with dentin hypersensitivity. Materials and Methods : An oxalate-based solution, Sensblok (NIBEC Inc. Seoul, Korea) & Superseal (Pheonix Inc. Michigan, USA), and a distilled water placebo were compared in a clinical setting. Seven volunteers exhibiting three or more teeth that were sensitive to tactile & air were enrolled in the study (35 teeth total). A visual analog scale (VAS) was used for recording each patient s level of sensitivity to tactile & air stimuli. A Florida probe was rubbed across the exposed dentin three times with a constant pressure 20 grams to measure the tactile stimulus. The air stimulus was generated by dental unit air syringe for 1-second blast from 1/2 inch distance. The desensitizing agents were applied according manufacture s guideline. VAS scores were recorded at baseline, 1 minute after treatment, at 1 week, 2weeks, 3weeks. VAS score were analyzed using a repeated measures ANOVA ( =0.05). Results : The Sensblok, Superseal, and distilled water placebo all decreased dentin sensitivity. A statistically significant difference existed between Sensblok and placebo when tactile stimulus was applied (P<0.05), but no significant difference was found among other desensitizing agents and tactile and air stimuli.

  • Objective: We evaluated the clinical characteristics and prognoses of patients with septic shock who transferred to the emergency department (ED) in a tertiary referral center. Methods: This study was performed using a prospective, multi-center registry of septic shock, with the participation of 11 tertiary referral centers in the Korean Shock Society between October 2015 and February 2017. We classified the patients as a transferred group who transferred from other hospitals after meeting the inclusion criteria upon ED arrival and a non-transferred group who presented directly to the ED. Primary outcome was hospital mortality. We conducted multiple logistic regression analysis to assess variables related to in-hospital mortality. Results: A total of 2,098 patients were included, and we assigned 717 patients to the transferred group and 1,381 patients to the non-transferred group. The initial Sequential Organ Failure Assessment score was higher in the transferred group than the non-transferred group (6; interquartile range [IQR], 4-9 vs. 6; IQR, 4-8; P<0.001). Mechanical ventilator (29% vs. 21%, P<0.001) and renal replacement therapy (12% vs. 9%, P=0.034) within 24 hours after ED arrival were more frequently applied in the transferred group than the non-transferred group. Overall hospital mortality was 22% and there was no significant difference between transferred and non-transferred groups (23% vs. 22%, P=0.820). Multivariable analysis showed an odds ratio for in-hospital mortality of 1.00 (95% confidence interval, 0.78-1.28; P=0.999) for the transferred group compared with the non-transferred group. Conclusion: The transferred group showed higher severity and needed more organ support procedures than the non-transferred group. However, inter-hospital transfer did not affect in-hospital mortality.

The Reproductive Patterns of Endangered Captive Orangutans by Analysing the Sex Hormones in Feces (분변내 성호르몬의 분석을 통한 멸종위기 사육 오랑우탄에서의 번식형태)

  • To monitor the reproductive patterns of endangered orangutan, Time-resolved fluorescence immuno assay (TR-FIA) were used to analyze metabolites of sex hormones in feces. Orangutan had long-term interbirth intervals (amenorrhea) during lactation period in which the secretion of sex hormones was repressed. The concentration of progesterone in the serum of pregnant orangutan was 30fold higher than that in non-pregnant orangutan. However, the concentration of hCG during pregnant period was different from the result of other primates. The present study suggested that age is not the important factor in determining the reproduction capability, because it is rather greatly influenced by the combination of various factors. Tracing metabolites of sex hormones in orangutan feces seems to be provide the effective solution for the infertility in orangutan. This study result shows the basic data in operating conservation project for endangered orangutan.

  • It is important for nursing faculties to try to improve their professional knowledge and skill in nursing education. Thus it is essential for them to participate in clinical training program. The purpose of this study was to identify the effect and satisfaction of faculty clinical training programs which were managed by Korean Academic Society of Nursing Education and 4 Hospitals(Samsung Medical Center, Seoul National University Hospital, Seoul, Asan Medical Center, Yeonsei Sebrance Medical Center) in Seoul. The data collection was done from July, 1999 to December, 1999 and the 125 subjects were used for the analysis. The purpose of participation of professor to clinical training course was found that to observe of up-to-date treatment method($3.57{\pm}.60$) and to connect between knowledge and practice ($3.63{\pm}.50$). The professors were highly satisfied with 'can see the high-tech institution and facilities($3.45{\pm}.62$)' and 'can confirm in advanced nursing field and practice ($3.38{\pm}.58$)' in clinical training program. And it was found that nursing professors applied the experience of participation in clinical training program to lecture and clinial practice for students. But they were dissatisfied with 'they had no or not enough time to practice'. In conclusion, the clinical training program was useful for nursing professor to educate and instruct the students. It is suggested that the more advanced course and practical program have to be developed for nursing faculties.

What are the Differences Between Scientifically Gifted and Normal Students in the Aspects of Creativity? (과학 영재 학생과 일반 학생은 창의성에서 어떻게 다른가? - 서울대학교 과학영재교육센터 학생들을 중심으로 -)

  • In the present study we have compared and analyzed three different measures of creativity in 135 gifted and 161 normal students to understand the nature of creativity and to propose the guideline for measuring creativity. The instruments used to measure creativity in this study are the Torrance Test of Creativity Thinking (TTCT), the Test of Creative Problem solving and Finding in Science (CPFS), and the Creative Behavior Checklist in Science (CBCS). The TTCT is the most widely used divergent thinking test and measures creativity in the aspect of domain-generality. The CPFS and the CBCS were developed for the purpose of this study and measure domain-specific creativity in the area of Science. The findings of this study revealed that gifted students are significantly more creative compared to normal students in all measures of creativity used in the study. The biggest difference between the gifted and normal students was found in the aspect of CPFS. This study implies that creativity, which is considered less useful to identify gifted students compared to achievement or IQ scores, is the important factor to consider for judging giftedness. The low correlations revealed among the TTCT, CPFS, and CBCS imply that the three measures of creativity address relatively different aspects of creativity. The results also suggest that it is essential to consider multiple criteria of creativity not to overlook potential creative students in the area of science. Implications of the study in connection with the identification and educational practices for gifted education program is discussed.

  • The Korean Society of Heart Failure (KSHF) guidelines aim to provide physicians with evidence-based recommendations for the management of patients with heart failure (HF). After the first introduction of the KSHF guidelines in 2016, newer therapies for HF with reduced ejection fraction, HF with mildly reduced ejection fraction, and HF with preserved ejection fraction have since emerged. The current version has been updated based on international guidelines and research data on Korean patients with HF. Herein, we present Part II of these guidelines, which comprises treatment strategies to improve the outcomes of patients with HF.

Concentrations of $C_2$~$C_9$ Volatile Organic Compounds in Ambient Air in Seoul (서울 대기 중에서 $C_2$~$C_9$ 휘발성 유기화합물의 농도)

  • Volatile organic compounds (VOCs) from Ca to C9 were investigated with nine ambient air samples collected in April 26, August 17, 1996 and January 23, 1997 in a Seoul site. On each sampling day, three 2-hr integrated canister samples were collected in early morning, early afternoon and late afternoon, respectively to study temporal . variation of VOCs. Most of VOC species showed diurnal variation with higher concentrations in the early morning and lower concentrations in the afternoon. The concentrations of light alkanes were high, probably due to the emission from liquefied petroleum gas (LPG) and evaporation of gasoline. Especially, the concentration of propane was the highest in the morning samples. The concentrations of propane, ethylene, acetylene, and toluene were prominent in their hydrocarbon groups, respectively. These components were the main source of car exhaust, gasoline evaporization, LPG, or solvent usage.

Korean Small Telescope Network (소형망원경 네트워크)

  • In this talk, we will give an overview of the small telescope network project in Korea. The small telescope network is a project in planning that would gather 0.4m-1.0m telescopes in Korea together for a common use in research and education, and the project is being led by the Optical/IR Astronomy Division of KAS. Even in the era of giant telescopes, small telescopes are still competitive for various research topics that require rapid response or long-term, steady monitoring. There are quite a few small telescopes in Korea, but the research use of these telescope has been very limited. By organizing these telescopes together, the small telescope network hopes to bring these telescopes in full operation and offer Korean astronomers competitive observational resources. In this talk, we will outline the project, describe potential resources, and several science cases such as multi-messenger astronomy, supernovae, and AGN. We will also introduce how this project might be run, with the expected operation of the small network starting at 2020.
