Comparison of Novel Telemonitoring System Using the Single-lead Electrocardiogram Patch With Conventional Telemetry System

  • Background and Objectives: Although a single-lead electrocardiogram (ECG) patch may provide advantages for detecting arrhythmias in outpatient settings owing to user convenience, its comparative effectiveness for real-time telemonitoring in inpatient settings remains unclear. We aimed to compare a novel telemonitoring system using a single-lead ECG patch with a conventional telemonitoring system in an inpatient setting. Methods: This was a single-center, prospective cohort study. Patients admitted to the cardiology unit for arrhythmia treatment who required a wireless ECG telemonitoring system were enrolled. A single-lead ECG patch and conventional telemetry were applied simultaneously in hospitalized patients for over 24 hours for real-time telemonitoring. The basic ECG parameters, arrhythmia episodes, and signal loss or noise were compared between the 2 systems. Results: Eighty participants (mean age 62±10 years, 76.3% male) were enrolled. The three most common indications for ECG telemonitoring were atrial fibrillation (66.3%), sick sinus syndrome (12.5%), and atrioventricular block (10.0%). The intra-class correlation coefficients for detecting the number of total beats, atrial and ventricular premature complexes, maximal, average, and minimal heart rates, and pauses were all over 0.9 with p values for reliability <0.001. Compared to a conventional system, a novel system demonstrated significantly lower signal noise (median 0.3% [0.1-1.6%] vs. 2.4% [1.4-3.7%], p<0.001) and fewer episodes of signal loss (median 22 [2-53] vs. 64 [22-112] episodes, p=0.002). Conclusions: The novel telemonitoring system using a single-lead ECG patch offers performance comparable to that of a conventional system while significantly reducing signal loss and noise.

  • Background and Objectives: Menkes disease (MNK) is a rare X-linked recessive disease, caused by mutations in the copper transporting ATP7A gene that is required for copper homeostasis. MNK patients experience various clinical symptoms including neurological defects that are closely related to the prognosis of MNK patients. Neural stem cells (NSCs) in the hippocampal dentate gyrus (DG) produce new neurons throughout life, and defects in DG neurogenesis are often correlated with cognitive and behavioral problems. However, neurodevelopmental defects in the DG during postnatal period in MNK have not been understood yet. Methods and Results: Mottled-brindled (MoBr/y) mice (MNK mice) and littermate controls were used in this study. In vivo microCT imaging and immunohistochemistry results demonstrate that blood vasculatures in hippocampus are abnormally decreased in MNK mice. Furthermore, postnatal establishment of NSC population and their neurogenesis are severely compromised in the DG of MNK mice. In addition, in vitro analyses using hippocampal neurosphere culture followed by immunocytochemistry and immunoblotting suggest that neurogenesis from MNK NSCs is also significantly compromised, corresponding to defective neurogenic gene expression in MNK derived neurons. Conclusions: Our study is the first reports demonstrating that improper expansion of the postnatal NSC population followed by significant reduction of neurogenesis may contribute to neurodevelopmental symptoms in MNK. In conclusion, our results provide new insight into early neurodevelopmental defects in MNK and emphasize the needs for early diagnosis and new therapeutic strategies in the postnatal central nerve system damage of MNK patients.

Polymorphisms of the NR3C1 gene in Korean children with nephrotic syndrome (한국 신증후군 환아에서 NR3C1 유전자 다형성 분석)

  • Purpose : Idiopathic nephrotic syndrome (NS) can be clinically classified as steroid-sensitive and steroid-resistant. The detailed mechanism of glucocorticoid action in NS is currently unknown. Methods : In this study, we investigated 3 known single nucleotide polymorphisms (SNPs) (ER22/23EK, N363S, and BclI) of the glucocorticoid receptor gene (the NR3C1 gene) in 190 children with NS using polymerase chain reaction-restriction fragment length polymorphism and analyzed the correlation between the genotypes and clinicopathologic features of the patients. Results : Eighty patients (42.1%) were initial steroid nonresponders, of which 31 (16.3% of the total) developed end-stage renal disease during follow-up. Renal biopsy findings of 133 patients were available, of which 36 (31.9%) showed minimal changes in NS and 77 (68.1%) had focal segmental glomerulosclerosis. The distribution of the BclI genotypes was comparable between the patient and control groups, and the G allele frequencies in both the groups were almost the same. The ER22/23EK and N363S genotypes were homogenous as ER/ER and NN, respectively, in all the patients and in 100 control subjects. The BclI genotype showed no correlation with the NS onset age, initial steroid responsiveness, renal pathologic findings, or progression to end-stage renal disease. Conclusion : These data suggested that the ER22/23EK, N363S, and BclI SNPs in the NR3C1 gene do not affect the development of NS, initial steroid responsiveness, renal pathologic lesion, and progression to end-stage renal disease in Korean children with NS.

Irregular Pronunciation Detection for Korean Point-of-Interest Data Using Prosodic Word

  • This paper aims to propose a method of detecting irregular pronunciations for Korean POI data adopting the notion of the Prosodic Word based on the Prosodic Phonology (Selkirk 1984, Nespor and Vogel 1986) and Intonational Phonology (Jun 1996). In order to show the performance of the proposed method, the detection experiment was conducted on the 250,000 POI data. When all the data were trained, 99.99% of the exceptional prosodic words were detected, which shows the stability of the system. The results show that similar ratio of exceptional prosodic words (22.4% on average) were detected on each stage where a certain amount of the training data were added. Being intended to be an example of an interdisciplinary study of linguistics and computer science, this study will, on the one hand, provide an understanding of Korean language from the phonological point of view, and, on the other hand, enable a systematic development of a multiple pronunciation lexicon for Korean TTS or ASR systems of high performance.

The Effect of Appendages of a Water-Jet Propelled High Speed Vessel on the Course Keeping Ability (워터젯 추진 고속선의 부가물이 침로안정성에 미치는 영향)

  • It has been often reported that a water-jet propelled high speed vessel lost the course keeping ability in seaway. In this study, model tests of a high speed vessel were performed to measure the running attitude and to check the course keeping ability. The model ship may lose the course keeping ability due to bad running attitudes such as bow drop. So model tests were carried out to improve the running attitude by changing the position of longitudinal center of gravity and using appendages at the bow and the stern of a model. The position of lateral center of pressure moved toward stern and the course keeping ability was improved by modifying the transom wedge angle.

Types of Violence and Coping Methods Experienced by General Hospital Nurses (종합병원 간호사가 경험한 폭력 유형과 대처방식)

  • Purpose: The purpose of this study was to identify the types of violence and coping methods experienced by general hospital nurses. Methods: Data were collected from March 17 to 24, 2014, using self-report questionnaires. Responses from 449 nurses were analyzed. Results: The majority of the respondents experienced violence from patients, visitors, doctors, and other nurses. Verbal violence was more frequent than physical threats and physical violence. Most violence happened in ERs, followed by surgical units, and ICUs. The most frequent response by nurses after violence was an emotional response, especially 'anger' ($4.01{\pm}1.059$). Based on general characteristics, the responses were significant for professional experience (F=2.935, p=.013) and work areas (F=2.290, p=.021). The most frequent coping method for nurses after violence had occurred was to 'just complete their duties as if nothing happened'. Conclusion: Most nurses are exposed to frequent violence, but they feel defenseless. These results suggest that hospital should improve the respective organizational cultures and develop promotional programs and administrative policies to prevent violence. In addition, educational programs should be provided for nurses to improve their attitudes and abilities to cope with violence. Also, hospitals should offer sufficient support, stress reduction programs and counseling programs for nurses.

  • Angiotensin I, a model decapeptide, was glycosylated and partially hydrolyzed with HCl (6 N, 80 $^{\circ}C$, 4 h), aminopeptidase, and carboxypeptidase Y. A single peptide mass map obtained from truncated peptides in the partial acid hydrolysate of angiotensin and its glycosylation product mixture by matrix-assisted laser desorption/ionization time-of-flight (MALDI-TOF) mass spectrometry enabled sequencing of angiotensin by a combinatorial procedure. MALDI-TOF and electrospray ionization (ESI) tandem mass spectrometric results indicate that both the N-terminal amino group of aspartic acid and the guanidinium group of the second residue arginine are glycosylated.

  • Rod-like polymerizable LC molecules having two different reactive groups, i.e. acryl and diacetylene groups were prepared. 4-Hydroxyphenyldiacetylenes were synthesized by the coupling reaction of 1-bromoalkynes with 4-ethynylphenol and then reacted with 4-(6-acryloyloxyalkyloxy)benzoic acid to give polymerizable LC molecules 4a-d. The mesomorphic properties of compounds 4a-d were investigated by differential scanning calorimetry, polarized optical microscopy and X-ray diffractometry. Compounds 4a-c exhibited smectic and nematic phases, but compound 4d having a longest alkyl tail among the series formed only a smectic phase. Photopolymerizability of acryl and diacetylene groups was investigated by IR spectroscopy. An anisotropic polymer film could be prepared by selective polymerization of acryl groups with 365 nm UV light in the presence of a photoinitiator (2,2-dimethoxy-2-phenylacetophenone). The subsequent reaction of diacetylene groups with 254 nm UV light disrupted the anisotropic structure, suggesting that these LC molecules could be used for imaging on the film.

Effect of Rapid Thermal Annealing on Growth and Field Emission Characteristics of Carbon Nanotubes

  • The effect of rapid thermal annealing (RTA) treatment on the growth characteristics of CNTs was investigated. We observed that Ni catalyst film was agglomerated by RTA treatment, resulting in the formation of Ni nanoparticles. The well aligned CNTs were grown from the Ni nanoparticles by plasma enhanced chemical vapor deposition (PECVD). It is shown that the size and distribution of the nanoparticles depend mainly on the annealing temperature and initial thickness of the metal layer. Also, it was found that CNTs grown through optimal RTA treatment had the more improved field emission characteristics than those of as-grown CNTs.

Effect of Wet Cleaning on Shrinkage and Detergency of Wool and Rayon Fabrics (웨트클리닝이 양모, 레이온 직물의 치수 안정성과 세탁성능에 미치는 영향)

  • This study focuses on the optimal washing conditions for dry cleaning recommended fabrics to minimize dimensional changes using wet cleaning. We suggest water-based alternatives to a perchloroethylene based cleaning process. Wool and rayon fabrics were laundered under various washing conditions and then air-dried for 24hrs. All specimens were extended after spinning and shrunk after drying. This is probably because the fibers were swollen and extended by wetting. The wool fabrics were shown to be acutely influenced by washing temperature and mechanical force. The optimal washing conditions for wool fabric to minimize the dimensional change implied a normal washing temperature and minimized mechanical force. For rayon specimens, dimensional change by a hand wash showed a remarkable decrease compared with a machine wash. Rayon fabric seemed to be influenced by the quantity of water contained in the fabric after spinning and washing time. Therefore, the desirable washing conditions for rayon fabric are to reduce the time required for washing and to increase the spin speed.
