Suggestion of Optimal Radiation Fields in Rectal Cancer Patients after Surgical Resection for the Development of the Patterns of Care Study (Patterns of Care 연구 개발을 위한 직장암의 수술 후 방사선치료 시 적정 방사선치료 조사영역 제안)

  • Purpose: To suggest the optimal radiation fields after a surgical resection based on a nationwide survey on the principles of radiotherapy for rectal cancer in the Korean Patterns on Care Study. Materials and method: A consensus committee, composed of radiation oncologists from 18 hospitals in Seoul Metropolitan area, developed a survey format to analyze radiation oncologist's treatment principles for rectal cancer after a surgical resection. The survey format included 19 questions on the principles of defining field margins, and was sent to the radiation oncologists in charge of gastrointestinal malignancies in all korean hospitals (48 hospitals). Thirty three ($69\%$) oncologists replied. On the basis of the replies and literature review, the committee developed guidelines for the optimal radiation fields nor rectal cancer Results: The following guidelines were developed: superior border between the lower tip on the L5 vertebral body and upper sacroiliac joint; inferior border $2\~3$ cm distal to the anastomosis in patient whose sphincter was saved, and $2\~3$ cm distal to the perineal scar In patients whose anal sphincter was sacrificed; anterior margin at the posterior lip of the symphysis pubis or $2\~3$ cm anterior to the vertebral body, to include the internal iliac lymph node and posterior margin $1.5\~2$ cm posterior to the anterior surface of the surface, to include the presacral space with enough margin. Comparison with the guidelines, the replies on the superior margin coincided in 23 cases ($70\%$), the inferior margin after sphincter saving surgery in 13 ($39\%$), the inferior margin after adbominoperineal resection in 32 ($97\%$), the lateral margin in 32 ($97\%$), the posterior margins in 32 ($97\%$) and the anterior margin in 16 ($45\%$). Conclusion: These recommendations should be tailored to each patient according to the clinical characteristics such as tumor location, pathological and operative findings, for the optimal treatment. The adequacy of these guidelines should be proved be following the Korean Patterns of Care Study.

Real Option Analysis to Value Government Risk Share Liability in BTO-a Projects (손익공유형 민간투자사업의 투자위험분담 가치 산정)

  • The BTO-a projects is the types, which has a demand risk among the type of PPP projects in Korea. When demand risk is realized, private investor encounters financial difficulties due to lower revenue than its expectation and the government may also have a problem in stable infrastructure operation. In this regards, the government has applied various risk sharing policies in response to demand risk. However, the amount of government's risk sharing is the government's contingent liabilities as a result of demand uncertainty, and it fails to be quantified by the conventional NPV method of expressing in the text of the concession agreement. The purpose of this study is to estimate the value of investment risk sharing by the government considering the demand risk in the profit sharing system (BTO-a) introduced in 2015 as one of the demand risk sharing policy. The investment risk sharing will take the form of options in finance. Private investors have the right to claim subsidies from the government when their revenue declines, while the government has the obligation to pay subsidies under certain conditions. In this study, we have established a methodology for estimating the value of investment risk sharing by using the Black - Scholes option pricing model and examined the appropriateness of the results through case studies. As a result of the analysis, the value of investment risk sharing is estimated to be 12 billion won, which is about 4% of the investment cost of the private investment. In other words, it can be seen that the government will invest 12 billion won in financial support by sharing the investment risk. The option value when assuming the traffic volume risk as a random variable from the case studies is derived as an average of 12.2 billion won and a standard deviation of 3.67 billion won. As a result of the cumulative distribution, the option value of the 90% probability interval will be determined within the range of 6.9 to 18.8 billion won. The method proposed in this study is expected to help government and private investors understand the better risk analysis and economic value of better for investment risk sharing under the uncertainty of future demand.

Comparative Analysis Per Area of the Liver Diseases in Ultrasound Diagnosis of Healthcare Center (건강검진 초음파에서 지역별 간질환 유병률 비교 분석)

  • The study find out developmental factors of the liver diseases in 29,531 subject of the healthy adults who diagnosed by using ultrasound at healthcare centers located at 6 cities around the nation. The results are as follows. Based on the result of the study, the liver diseases diagnosed by using ultrasound revealed to show 43.1% of prevalence, and the occurrence was significantly higher in male(23.3%) than in female (19.8%)(p<0.001). The disease revealed to show highest prevalence rate in regionally, Dae-jeon was found to record the highest prevalence by showing 49.6% (p<0.001). The order of liver disease prevalence was the highest in Dae-jeon(49.6%),which was followed by Seoul(48.2%), Busan(42.9%), Incheon(36.2%), Daegu(36.2%), Kangju(32.1%)(p<0.001). The order of hepatic disease prevalence was the highest in fatty liver (69.7%), which was followed by intrahepatic cysts (13.2%), intrahepatic calcification (9.6%), intrahepatic hemangioma (4.5%), intrahepatic tumor (1.4%), diffuse liver disease (1.0%),and liver cirrhosis (0.5%) (p<0.001). The liver disease revealed to show highest prevalence rate in 40s and regionally, disease was found to record the highest prevalence from 30s to 50s (p<0.001). The distribution of the liver diseases by sex revealed to show the higher occurrence of the fatty liver and the diffuse liver diseases in male, but the intrahepatic cysts, intrahepatic calcification and intrahepatic tumors were highly distributed in female. In the regional evaluation, the fatty liver was most often found in Busan, and intrahepatic cysts, intrahepatic hemangioma, intrahepatic calcification and intrahepatic tumors were highly found in Dae-jeon and Seoul revealed to show higher occurrence of diffuse liver disease than other regions (p<0.001). A Study for Plant Community Structure and Management Plan of Pinus densiflora Forest in Byeonsanbando National Park (변산반도국립공원 소나무림 식물군집구조 및 관리방안 연구)

  • This study is intended to provide basic date for the efficient management of Pinus densiflora community by analyzing ecological characteristics of Pinus densiflora inhabiting Byeonsanbando National Park. According to investigations, P. densiflora community and P. densiflora-Quercus community are widely distributed, occupying 40.3% of the total area. 21 sites ($400m^2$per site) are selected for TWINSPAN analysis, and the result indicates that the whole community of P. densiflora and P. densiflora-Quercus can be classified into 8 types, and the age of P. densiflora is 40-50 years, which is similar to that of deciduous broad-leaved trees. The 8 community types are: P. densiflora community which competes with Quercus variabilis; P. densiflora community in which Fraxinus sieboldiana and Quercus serrata grow in the understory layer; P. densifloa community in which Q. serrata grow in the understory layer and Smilax china var. microphylla in the shrub layer respectively; P. densifloa community in which P. densiflora and F. sieboldiana grow in the understory layer; P. densiflora community which competes with Q. serrata and Carpinus tschonoskii; P. densiflora community which competes with Q. variabilis and Q. serrata; P. densiflora community in which Prunus sargentii grow; P. densiflora community in which Abies holophylla grow. P. densiflora community which competes with Q. variablis and C. tschonoskii seems to be in a stage of succession to deciduous broad-leaved community. The analysis indicates that Shannon diversity index is 0.2756-1.3879. It also indicates that there is a negative correlation between P. densiflora and Q. variabilis and C. tschonoskii; there is a positive correlation between P. densiflora and F. sieboldiana and Rhododendron schlippenbachii. These investigations show that the transformation of vegetation is already under way. There is a possibility that ecological succession can take place in 30.4% of the total area from P. densiflora to Quercus and deciduous broad - leaved trees. Therefore, it is recommended that the preservation and maintenance of P. densiflora be implemented by taking control of competing species which undermine the stability of P. densiflora forest community.

Comparison of the Vegetation Structure between Base Mountain and Residual Mountain in the Urban Area, Wonju (원주시 도심 거점산림과 잔존산림의 식생구조 비교 연구)

  • This study analyzed and compared characteristic of vegetation structure in Bonghwa mountain, the base Mt. outside urban area, and Hakseoung neighborhood park, Ilsan neighborhood park and Musil neighborhood park, disconnected residual Mt. inside urban area of Wonju. Regarding vegetation of urban forest in Wonju, Quercus mongolica forest and Robinia pseudo-acacia forest were widely distributed in the base Mt. outside urban area. Quercus acutissima forest and Robinia pseudo-acacia forest were widely distributed in the disconnected residual Mt. inside urban area. Forests in both areas were managed as part of forest management campaign. Analysis on importance value of a community classified by DCA and DBH class categorized communities into three groups according to the expected ecological succession. Community B was the one to be maintained as Pinus densiflora community. Community A, D and d were expected to be maintained as Quercus spp. Community. Community C, E, F, e, f and g were classified as exotic plant community. Ecological succession was suspended in both base Mt. outside urban area and disconnected residual Mt. inside urban area. However, importance value of Quercus spp. which appeared in the understory layer and shrub layer and their DBH class showed that the potential for ecological succession of exotic plant community was higher in base Mt. outside urban area than disconnected residual Mt. inside urban area. According to the analysis on number of species and individuals and species diversity, understory and shrub layer were reduced due to Forest Management campaign and plant species which appear at an earlier stage of succession varied as time went by since the campaign began. Species diversity comparison between natural and artificial forest showed no clear difference in both base Mt. outside urban area and disconnected residual Mt. inside urban area due to man-made disruption such as thinning and brush cutting. Six communities in base Mt. outside urban area had 17.76~52.22% of similarity index. Four communities in disconnected residual Mt. inside urban area showed 13.34~37.01% of similarity index, which was lower than base Mt. outside urban area. Among naturalized plants, Tupatorium rugosum and Phytolacca americana appeared more in disconnected residual Mt. inside urban area.

An Interpretation of Symbols in Water Gardens of Old Palaces - Based on the Archetype Theory of Jung - (융(Jung)의 원형론의 관점에서 본 궁궐 수공간의 상징성 - 공간구조와 디테일에 나타난 상징의미를 중심으로 -)

  • The purpose of this paper is to provide a unified examination of apparently quite different gardens in terms of Carl Jung's psychological concepts such as Archetypes, Individualization, and a natural tendency towards balance or wholeness. In Jung's psychological framework, Archetypes are innate, universal prototypes for ideas and function as the first original models upon which all other similar persons, objects or concepts are derived, copied or patterned. Jung proposes that Individualization be achieved through a natural tendency towards balance, especially the balance between the conscious and the unconscious. This paper deals with three gardens, each of which represents a distinct cultural region: Bu-Yong Ji(芙蓉池) at the Changdeok Palace(Oriental), the Patio of the Lions at the Alhambra(Islamic), and the Fountain of Apollo at the Versailles Palace(Western). It is argued that all of three have in common a natural tendency towards balance and symbolize mandala, the archetype of wholeness. Bu-Yong Ji is in the form of quadrangle which embodies Yin and Yang. In the Patio of the Lions, the basin at the center and the four channels, which symbolize the waterway of the Garden of Eden and the four rivers in Paradise respectively, are constructed in the form of a quadripartite composition. The overlapped circle and rectangular shaped pond of the Fountain of Apollo also represents mandala. Symbols representing the same archetype can vary from culture to culture. This explains the differences among the three gardens with respect to specific aspects of external forms. In other words, an archetypal image can give rise to various forms in different cultures, and thus quite different mediums of design or design details may be developed. In conclusion, the three gardens give us a good example as to how an archetypal image can be expressed in different ways from culture to culture and how seemingly different design details can be analyzed in a unified way.

Development of a Stability Formula for Tetrapod by Using M5' Model Tree (M5' Model Tree를 이용한 Tetrapod 안정식 개발)

  • Tetrapod, one of the famous armor blocks for rubble mound breakwaters, has been widely used in the world. In order to evaluate the required weight of Tetrapod, many researchers have proposed various stability formulas. Since the stability formulas were proposed by curve-fitting the experimental data, some uncertainties are included in the formulas. The main uncertainties are associated with experimental data, derivation of the formula, and variability of the design variables. In this study, a new stability formula is developed by using M5' model tree, which reduces the uncertainty in the derivation of the formula. The index of agreement is used to evaluate the performance of the developed formula. The index of agreement for the new formula is higher by about 0.1 than the previous formula. The performance of the previous formula was not good when the predicted stability number is greater than about 3.0. However. the new formula is accurate regardless of the magnitude of stability number. As a result, the new formula performs better than the previous formula, while expressed in the form of a tree but still in an explicit form.

Simulation Skills of RegCM4 for Regional Climate over CORDEX East Asia driven by HadGEM2-AO (CORDEX 동아시아 지역에서 HadGEM2-AO를 경계조건으로 처방한 RegCM4의 상세 지역기후 모의성능)

  • In this study, 27-year (1979-2005) regional climate over the CORDEX East Asia domain was reproduced using a regional climate model, RegCM4, driven by HadGEM2-AO output, and the model's simulation skill was evaluated in terms of surface air temperature and precipitation. The RegCM4 reasonably simulated the spatial distribution and interannual variability and seasonal variability of surface air temperature, while it had systematic biases in the simulation of precipitation. In particular, simulated rainband of East Asian summer monsoon was southward shifted below $30^{\circ}N$ as compared with the observation, thereby, summer mean precipitation over South Korea was significantly underestimated. Simulated temperature from the RegCM4 driven by the HadGEM2-AO output was comparable to that driven by the reanalysis. However, the RegCM4 driven by the HadGEM2-AO had prominently poor skill in the simulation of precipitation. This can be associated with the distorted monsoon circulations in the driving data (i.e., HadGEM2-AO) such as southward shifted low-level southwesterly, which resulted in the erroneous evolution of East Asian summer monsoon simulated by RegCM4.

A study on the personality characteristics of orthognathic surgery patients through the MMPI. (MMPI를 이용한 악교정 수술환자의 인성에 관한 연구)

  • This study was designed to evaluate the Personality characteristics of orthognathic surgery patients preoperatively and postoperatively through the MMPI. Comparison was made between the personality characteritics of preoperative and postoperative groups. Motivation for undergoing orthognathic surgery and postoperative satisfaction were examined through the questionnaire. Subjects used in this study consisted of 50 patients under presurgical orthodontic treatment and 22 patients who had received orthognathic surgery. From this study, the results were followings. 1. In preoperative patients group, all T-scores were within the normal range except for K-scale. The T-score of K-scale is 57.7, this showed that these patients underwent some or less stress. 2. In postoperative patients group, all T-scores were within the normal range. 3. Only in K-scale, there was statistically significant increase after surgery (p<0.05). 4. In male patients group, all T-scores were within the normal range except for that of preoperative K-scale. Postoperatively, there were statistically significant increase in K and Hs scales(p<0.05) 5. In female patients group, all T-scores were within the normal range. 6. Motivation for undergoing orthognathic surgery could be divided into 4 categories. esthetic goal($28.2\%$), functional goal($32.1\%$), TMJ problem($9.0\%$), and psychosocial reason($30.7\%$). 7. The degree of postoperative satisfaction was quite high($85.7\%$). $14.3\%$ of all patients were very satisfied, $71.4\%$ were satisfied, $9.5\%$ were neither satisfied or dissatisfied, and $4.8\%$ were very dissatisfied.

Ultrasonic Pulses Characteristics in Lightweight Fine Aggregate Concrete under Various Load Histories (하중 이력에 따른 경량 잔골재 콘크리트의 초음파 특성)

  • One of the widely used NDT(Non-destructive techniques) is the ultrasonic pulse velocity (USPV) method, which determines the travel time of the ultrasonic pulse through the tested materials and most studies were focused on the results expressed in time domain. However, the signal of ultrasonic pulse in time domain can be transformed into frequency domain, through Fast fourier transform(FFT) to give more useful informations. This paper shows a comparison of changes in the pulse velocity and frequency domain signals of concrete for various load histories using lightweight fine aggregates. The strength prediction equation for normal concrete using USPV cannot be used to estimate lightweight fine aggregate concrete strength. The signals in frequency domain of ultrasonic pulse of lightweight fine aggregate concrete does not show any significant difference comparing with those of normal concrete. The increases in stress levels of concrete change the pulse velocities and maximum frequencies, however the apparent relationship between themselves can not be found in this experiment.
