Genetic characterization of Shigella flexneri isolated from the diarrheic patients in Seoul region (서울지역 설사환자로 부터 분리된 Shigella flexneri의 성상과 유전적 특성)

  • The shigellae are common etiological agents of bacillary dysentery in humans and primates. During four years from 2002 to 2005, 22 strains of Shigella spp. were isolated from the diarrheic patients in Seoul region. All of them were identified as S. flexneri by biochemical tests and serotyping. The prevalence of serotypes were variable by year, but the major serotypes were 2a and 3a. In an antimicrobial susceptibility test, all of the isolates were resistant to streptomycin and tetracycline, and susceptible to amikacin, kanamycin, cefoxitin, and gentamicin. All of the isolates showed the multi-resistant patterns over 3 drugs. By analysis of the plasmid profile the isolates were classified into 7 groups (P1~P7). Serotypes 2a and 2b were distributed to P1, P2, P3, and P4. Serotype 3a was differentiated to P5 and serotype 3b, to P6 and serotype 4a, to P7. PCR results showed that all isolates were positive for two virulence genes, ipaH and ial, but none of the strains had stx gene. The set1A and set1B genes were detected from 12 isolates (54.5%) that belonged to serotype 2a and 2b. The sen gene was detected from 19 isolates (86.4%). The 22 isolates showed 12 to 17 DNA fragments in the sizes ranging from 20.5 kb to 1135 kb, resulting in 13 patterns by the PFGE with Not I digestion. The PFGE patterns of the isolates showed the close relation with the serotypes, but no relations with year of isolation and antimicrobial resistance.

Prevalence of Toxin Genes and Profiles of Antibitoc Resistance in Vibrio vulnificus Isolates from Fish, Fish Tanks, and Patients (어류, 수족관수 및 환자에서 분리된 Vibrio vulnificus의 독소유전자 분포 및 항생제 내성)

  • Prevalence of toxin genes and profiles of antibiotic resistance in Vibrio vulnificus were investigated for prevention of Vibrio sepsis and selection of effective antibiotics. A total of 23 V. vulnificus strains were isolated from Vibrio sepsis patients, fish, and water samples collected from fish tanks in restaurants in Jeonnam province during 2015-2017 period. Prevalence of toxin genes including, RtxA, viuB and vvhA were assessed and susceptibilities to 15 different antibiotics were determined. As a result of the toxin gene profile, the RtxA toxin gene was detected in 19 (82.6%) out of 23 strains, and vvhA and viuB toxin genes were positive in all strains. These results showed that V. vulnificus tested in this study possessed at least one more toxin gene, and the toxin gene detection rate was higher than in previous reports. Therefore, there is always a risk of Vibrio sepsis through eating fish or having contact with aquarium water at seafood restaurants. Especially, it was deemed necessary to provide preventive education about Vibrio sepsis for workers in such restaurants. The results of antibiotic susceptibility tests presented 94.4% resistance to cepoxitin antibiotics but all strains showed susceptibility to 14 kinds of antibiotics including chloramphenicol and tetracycline. The currents antibiotic therapy using chloramphenicol and teteracycline against Vibrio sepsis was judged to be useful.

  • The purpose of this study were to illustrate the various medication error types and causes and identified to related drugs to provide basic data for preventing nurses' medication error by analysing 73 cases of AJN 'medication Error' column(1993, Oct -2000, Nov). Nurses' types of medication error were classified into 7 types. The most frequent error types are wrong medication$(21.9\%)$ and the wrong dose$(21.9\%)$ together. The others are wrong $time(4.1\%)$, $omission(2.7\%)$, mechanical $error(2.7\%)$, incorrect IV $rate(1.4\%)$. wrong route $administration(1.4\%)$ in order. Nurses' causes of medication error were 9 kinds. The most frequent type is confusing between similar drug shape, color, size, name, injection devices and patient's $name(43.9\%)$ and the others are lack of knowledge about $drugs(26.8\%),\; slips(7.3\%),\; miscalculating\;dose(4.9\%)$, incorrect adjusts $devices(4.9\%)$, difficulty to read or illegible decimal $point(4.9\%),$ $abbreviation(2.4\%)$, fatigue with $overwork(2.4\%)$ and no communication with $patient(2.4\%)$ in order. Related drugs with medication error are as follows. - dose unit(IU. minims. mcg/min. mEq) : Heparin. insulin. synthetic calcitonin, some enzymes and hormones, vitamins, some antibiotics, tuberculin injection. MgSO4 injection. nitroglycerin - similar size, color and shape drug : $0.9\%$ N/S and acetic acid $0.25\%$ for irrigation. premixed 2mg lidocaine sol. and $0.9\%$ N/S, gentamycin 20mg/2mL for children and 80mg/2mL for adult, dextroamphetamine 5mg and 10mg capsule. sedatives chloral hydrate 250mg/5mL and 500mg/5mL - similar name :Aredia(pamidronate disodium) and Adriamycin(doxorubicin), Lamictal (lamotrigine) and Lamisil 250mg. Isolation and antimicrobial susceptibility of microorganisms from milk samples of healthy postpartum sows (모돈 유즙으로부터 분리한 세균의 분포 및 항균제 감수성 조사)

  • The growth rate of piglets will be decreased and the mortality of piglets will be increased in the postpartum sow with mastitis. The aim of this study was to isolate microorganisms from milk samples found in postpartum sows with suckling piglets and to further investigate antimicrobial susceptibility against isolated bacteria. Milk samples were collected from 567 udders of 42 lactating sows and the isolated bacteria was then identified. Isolated bacteria were tested with 12 antimicrobial agents. Bacteria were isolated from 260 milk samples (64.5%). Staphylococcus spp. were the most common microorganisms (74.5%) isolated from sow milk, followed by Streptococcus hyicus (53.9%), Staphylococcus epidermis (11.8%), Staphylococcus aureus (9.8%), and Staphylococcus haemolyticus (53.9%). In the Staphylococcus spp., Streptococcus pneumoniae (5.1%) was isolated as was Streptococcus sanguinis (3.8%) and Streptococcus ovis (2.9%). Results indicated that bacteria isolated from healthy sow milk were mostly susceptible to cephalothin (88.0%), amikacin (85.3%) and cefoxitin (82.4%), but were resistant to streptomycin (10.0%), kanamycin (33.1%) and tetracycline (19.6%).

RAPD Analysis and Antimicrobial Susceptibility of Streptococcus equi subsp. zooepidemicus Isolated from Thoroughbred Horses (더러브렛 말에서 분리한 Streptococcus equi subsp. zooepidemicus의 RAPD 분석 및 약제 감수성)

  • A total of 68 samples were collected including vaginal mucosa (n=66) from Jangsu stud farm, an equine aborted fetus (n=1), and uterine contents (n=1) from Jeju island. Seventeen Streptococcus equi subsp. zooepidemicus (S. zooepidemicus) strains isolated from horses in Korea were identified as S. zooepidemicus by biochemical tests and sodA.seeI specific multiplex PCR. All isolated strains were divided into 4 clusters: group 1 (No. 2, 3, 5, 6, 7, 11, 12, 13, 14, 15), group 2 (No. 4, 9), group 3 (No. 10, 16, 17), and group 4 (No. 1, 8) by RAPD typing. In group 3, No. 10 isolate that was isolated from vaginal mucosa was indistinguishable from No. 16 and 17 isolates, which were isolated from the equine uterine contents and the equine aborted fetus, respectively. The results of this study suggest that a limited epidemiological relationship exists between the strains from Jangsu (No. 10) and Jeju (No. 16 and No. 17). All isolates showed a high susceptibility to ampicillin, cefoxitin, ceftiofur, cephalothin, florofenicol, gentamicin, nalidixic acid, oxacillin, penicillin, tiamulin, tylosin and vancomycin in antimicrobial susceptibility tests. These results may provide the basic information needed to establish strategies for the treatment and prevention of reproductive diseases in mares in Korea.

Distribution and Characterization of Vibrio vulnificus Isolated in Coastal Areas of Chungcheongnam-do Province (충청남도 해안지역의 비브리오 패혈증균 분포 및 분리주의 특성 분석)

  • Background: Vibrio vulnificus has been frequently detected in seawater, fish, and shellfish mainly in the coastal areas of Chungcheongnam-do Province. Objectives: This study was conducted to investigate the analyzed biochemical properties, genetic characteristics, and distribution of Vibrio vulnificus isolated from environmental sources in coastal areas of Chungcheongnam-do Province from 2019 to 2020. Methods: A total of 1,510 samples were obtained from six different sites in Chungcheongnam-do Province. Isolated strains from the samples were identified by a VITEK 2 system and MALDI-TOF. Antibiotic susceptibility testing for 85 isolates was done by microdilution minimum inhibitory concentration methods, and 11 isolates were analyzed for 16s rRNA sequences in multiple alignments. Results: Among the 1,510 samples taken during the investigation period, 306 strains were isolated and the detection rate of V. vulnificus was 20.3%. One hundred eighty-eight strains (24.6%) from seawater and 118 strains (15.8%) from mud flats were isolated. It was mainly detected in July (17.3%), August (36.5%), and September (28.8%), and the proportion was 82.0%. Based on the CLSI-recommended breakpoints, V. vulnificus isolates were all susceptible to amoxicillin/clavulanic acid, amikacin, gentamicin, ciprofloxacin and trimethoprim/sulfamethoxazole. However, nonsusceptible isolates to ampicillin, ampicillin/sulbactam, cefazolin, cefoxitin, imipenem, tetracycline and chloramphenicol were identified. In the analysis of the nucleotide sequences for 16s rRNA of V. vulnificus isolates, it was confirmed that mutations frequently occurred between nucleotide number 922 and 952, and 98.2% to 100% nucleotide identities between isolates was verified. Conclusions: The results of this study can be used as a basic data for infection control and prevention of Vibrio vulnificus infection by describing the distribution and characteristics of Vibrio vulnificus strains isolated in coastal areas of Chungcheongnam-do Province.

Analysis of whole genome sequencing and virulence factors of Vibrio vulnificus 1908-10 isolated from sea water at Gadeok island coast

  • Vibrio vulnificus is an aquatic bacterium causing septicemia and wound infection in humans. To understand this pathogen at the genomic level, it was performed whole genome sequencing of a cefoxitin-resistant strain, V. vulnificus 1908-10 possessing virulence-related genes (vvhA, viuB, and vcgC) isolated from Gadeok island coastal seawater in South Korea. The genome of V. vulnificus 1908-10 consisted of two circular contigs and no plasmid. The total genome size was estimated to be 5,018,425 bp with a guanine-cytosine (GC) content of 46.9%. We found 119 tRNA and 34 rRNA genes respectively in the genome, along with 4,352 predicted protein sequences. Virulence factor (VF) analysis further revealed that V. vulnificus 1908-10 possess various virulence genes in classes of adherence, antiphagocytosis, chemotaxis and motility, iron uptake, quorum sensing, secretion system, and toxin. In the comparison of the presence/absence of virulence genes, V. vulnificus 1908-10 had fur, hlyU, luxS, ompU, pilA, pilF, rtxA, rtxC, and vvhA. Of the 30 V. vulnificus comparative strains, 80% of the C-genotype strains have all of these genes, whereas 40% of the E-genotype strains have all of them. In particular, pilA were identified in 80% of the C-type strains and 40% of the E-type strains, showing more difference than other genes. Therefore, V. vulnificus 1908-10 had similar VF characteristics to those of type C strains. Multifunctional-autoprocessing repeats-in-toxin (MARTX) toxin of V. vulnificus 1908-10 contained 8 A-type repeats (GXXGXXXXXG), 25 B.1-type repeats (TXVGXGXX), 18 B2-type repeats (GGXGXDXXX), and 7 C-type repeats (GGXGXDXXX). The National Center for Biotechnology Information (NCBI) Basic Local Alignment Search Tool (BLAST) showed that the RtxA protein of V. vulnificus 1908-10 had the effector domain in the order of cross-liking domain (ACD)-C58_PaToxP-like domain- α/β hydrolase-C58_PaToxP-like domain.

Antibiotic Sensitivity to the Major Causative Organisms of Acute Urinary Tract Infection in Children (소아 요로감염증의 주요 원인균과 항생제 감수성)

  • Purpose : We studied the changes in antibiotic sensitivity to the causative organisms of urinary tract infection(UTI), in order to provide useful information on the choice of adequate drugs in the treatment of UTI. Methods : We retrospectively analyzed the major causative organisms and their antibiotic sensitivities in 69 patients diagnosed with UTI in the Department of Pediatrics, Samsung Cheil Hospital from 2002 to 2003. Results : The frequency of UTI was the highest in infants younger than 1 year of age(88.4 percent). The male to female ratio was 3.05 : 1. Escherichia coli was the most frequent organism(78.3 percent), followed by Klebsiella(11.6 percent), Pseudomonas(2.9 percent), Proteus(2.9 percent), Enterobacter, Morganelle, and Enterococcus(1.4 percent) in descending order. Antibiotic sensitivity of gram negative organisms was above 90 percent against imipenem, amikacin, 80 percent against aztreonam, cefepime, ceftriaxone, 50-70 percent against gentamicin, trimethoprime-sulfamethoxazole (TMP/SMX), and 23 percent against ampicillin(23.4 percent). Conclusion : Antibiotict sensitivity of gram negative organisms was high to amikacin and third generation cephalosporins but low to ampicillin, gentamicin and TMP/SMX. The use of ampicillin or TMP/SMX, as the first choice of the empiric and prophylactic treatment for UTI, should be reconsidered and investigated further.

The Changes of Intestinal Normal Flora in Neonates for Seven Days Postnatally (정상 신생아의 대변에서 생후 1주일 동안 장내세균총의 변화)

  • Purpose: Microbial colonization of the intestine begins just after birth and development of the normal flora is a gradual process. The first bacteria colonizing the intestine in newborns are Staphylococcus, Enterobacteriaceae and Streptococcus. For several days after birth, the number of Bifidobacterium spp. increase. The aim of this study was to investigate the changes of microflora for seven days postnatally in neonatal stool. Methods: Fifteen neonates (breast : formula : mixed feeding 1 : 8 : 6, vaginal delivery : cesarean section 3 : 12) who were born at the Kangdong Sacred Heart Hospital, Hallym University were enrolled. First meconium and stools of postnatal 1-, 3-, and 7-day were innoculated. Blood agar plates for total aerobes, trypton bile X-glucuronide agar for E. coli, phenylethyl alcohol agar for gram positive anaerobes, MRS agar for Lactobacillus spp., bifidobacterium selective agar for Bifidobacterium spp. and cefoxitin-cycloserine-fructose agar for Clostridium difficile were used in the general incubator ($CO_2$ free incubator), $CO_2$ incubator or the anaerobic chamber for 48 or 72 hours at $37^{\circ}C$ and then colony forming units were counted. Results: No microflora was identified in the first meconium. Total aerobes, E. coli, and gram positive anaerobes were significantly increased with advancing postnatal days. In only one baby, Lactobacillus acidophilus was detected $2{\times}10^5CFU/g$ in the seven-day stool. Bifidobacterium spp. was detected in two babies. Clostridium difficile was not detected during the seven days. There were no significant differences in the bowel flora depending on the delivery pattern and feeding method. Conclusion: This study shows many changes in the intestinal normal flora in neonatal stool during seven days postnatally. If these findings are confirmed with larger studies, the data may be preliminary findings to support use of probiotics in neonates.

Antibiotic Susceptibility of Salmonella spp. Isolated From Diarrhoea Patients in Seoul From 1996 to 2001 (서울 시내 설사환자에서 분리한 살모넬라의 항생제 감수성의 년도별 변화 추이)

  • In order to investigate the classification and antibiotic resistance of Salmonella species,718 isolates were isolated from patient in Seoul from 1996 to 2001. The two hundred and ninety eight isolates (41.5%) were identified as Sal. Enteritidis, followed by Sal. Typhi 218 isolates (30.4%), and Sal. Typhimurium 87 isolates (12.1%). The identified Salmonella species were most resistant to tetracycline (32.7%), followed by streptomycin (28.0%), ticarcillin (18.1%) and ampicillin (12.4%). Among isolates,34.7% of Sal. Enteritidis were resistant to tetracycline, 32.3% to streptomycin,23.2% to ticarcillin,13.5% to ampicillin, respectively. 13.8% of Sal. Typhi were resistant to streptomycin,10.6% to tetracycline, respectively.66.7% of Sal. Typhimurium were resistant to tetracycline, 42.5% to streptomycin, 28.7% to ticarcillin, 26.4% to ampicillin and 17.2% to chloramphenicol, respectively. Of 718 isolates, 324 isolates (45.1%) were resistant to 1 or more drugs and 64 isolates (19.8%) were resistant to 1 drug, 132 isolates (40.7%) were resistant to 2 drugs,50 isolates (15.4%) were resistant to 3 drugs, 27 isolates (8.3%) to 4 drugs,27 isolates (8.3%) to 5 drugs,22 Isolates (6.8%) to 6 drugs. The most prevalent multiple resistant pattern was tetracycline-kanamycin (35.5%), followed by tetracycline-kanamycin-ticarcillin (8.3%), and tetracycline-kanamycin-ticarcillin-ampicillin (7.4%) . Antibiotic resistant rate of Sal. Typhimurium was 73.6%,1311owe4 by Sal. Enteritidis 53.7% and Sal. Typhi 19.3%. Most Sal. Enteritidis was resistant to 1 drug o.2 drugs, whereas Sal. Typhi. and Sal.. Typhunurium were more .resistant to 5 (16.7%) or 6 drugs (26.6%). The old generation antibiotics such as ampicillin, tetracycline, and streptomycin were annually more resistant than the new generation antibiotics such as ceftriaxone, ciprofloxacin or cefoxitin.
