병원간호사가 인식한 고위험 정맥주사 투약오류 원인 분석 (Analysis of the causes of high-risk intravenous medication errors recognized by hospital nurses)

  • 병원간호사의 고위험 정맥주사 약물의 투약오류에 대한 인식과 경험을 파악하고 투약오류의 원인과 개선방안을 파악하기 위해 시도되었다. 연구 대상은 D시에 위치한 일개 대학병원에 근무하는 고위험 정맥투약 관련 업무경험이 있는 간호사를 대상으로 2021년 5월 16일~30일 간 자료 수집하였다. 연구 결과 고위험 정맥주사 투약안전 문제점의 핵심요인으로 병동 별 주요 약물의 투약 protocol 부재, 투약 주입기기의 작동 교육 부족, 표준화 된 고위험 정맥주사 투약수행 절차 미확립, 간호사 대상의 개별화 투약교육 부족, 병원 자체 약물집 부족 혹은 미비치, 비슷한 용기의 포장 약물 확인 부족의 6가지가 도출되었다. 간호실무적 차원에서 고위험 정맥주사 투약안전 프로그램을 적용하고 안전결과 지표를 확인할 수 있는 추후 연구 수행을 제언한다.

사업장의 기계재해 위험성평가에 관환 연구 (A study on quantitative risk assessment of mechanical accidents)

  • Recently the occurrence ratio of mechanical accidents in industrial disaster of korea is increased, but very little accident information has been given regarding the prevention of mechanical accidents. In this study, mechanical accidents by the analysis of industrial accident case was examined. And it was proposed the risk assessment method and building database for investigation of risk factors in mechanical accident. As a result, from mechanical accidents database, it was found that the occurrence ratio of mechanical accident by constriction and falling etc. is very high and death ratio by mechanical accident is larger than that of usual accident. And we applied the quantitative risk assessment method proposed by this study in the mechanical accident analysis of a domestic manufacturer. From the results, the risk factors due to constriction, curling, falling and scattering are principal causes of mechanical accidents. These result is similar to that of the analysis of mechanical accidents for recent 12 years in korea.

  • Since the diversification, complication of the construction industry, construction conflicts have been increased these days. Expansion of scale of construction has brought conflict treatment cost increase. Thus, needs of the study on construction conflict risk factors and its management is getting important. But the existing studies about construction conflicts and risk factors that need to be cared to decrease construction cost are almost focused on analysis of number of conflict proposed. It means those studies can be helpful when want to know how many conflict have been instituted, however, they do not contain a close examination between types of conflict and cost of conflict treatment. The purpose of this study, therefore, is an extraction of conflict causes based on the conflict treatment cost and the choice of significant risk factors to be managed to prevent conflict and its treatment cost, which enables reducing the conflict treatment colt on control the risk factors.

Importance of the knowledge of pathological processes for risk-based inspection in pig slaughterhouses (Study of 2002 to 2016)

  • Objective: The objective of this work was to determine the prevalence of the pathologies that caused the condemnation of pig carcasses in an area of intensive pig farming and Mediterranean climatology and to evaluate their influence in a risk-based inspection procedure for slaughterhouses. Methods: A retrospective observational investigation was carried out from 2002 to 2016 into the pathological processes that caused the condemnation of pig carcasses in a slaughterhouse from South-eastern Spain. The seasonal effect on the causes of condemnation carcass was reported. Negative binomial model was used to evaluate the effect of season on the rate of antemortem rejections and post-mortem condemnations. Histopathological examinations were performed to confirm the diagnosis. Results: The risk of antemortem rejections (0.0564%) was significantly greater in summer (risk ratio [RR] = 1.57). Autumn was associated with higher rate (RR = 1.69) of the total postmortem condemnations (0.1046%). Significantly higher rates of pronounced anaemia (0.0111%) were observed in summer (RR = 3.20). The main causes of anaemia were observed gastroesophageal ulcers and haemorrhagic enteropathies. Significantly highest risk of erysipelas (0.0074%) were observed in autumn (RR = 5.485). About other zoonosis, only eight cases (0.0013%) of carcasses were declared unfit due to tuberculosis lesions. Porcine muscular cysticercosis was not detected. Nevertheless, nonspecific causes such as generalized infections and emaciation represented the half of the condemned carcasses (50.90%). Conclusion: The pathologies leading to the condemnation of carcasses in this study can be considered representative of the pathologies that affect the pig population from a region with a high intensive production and Mediterranean climatology because this slaughterhouse receives a lot of animals from many farms of different size in a high intensive pig production zone (Mediterranean region). Increased knowledge of environmental factors that may foment the appearance of the diseases is essential for implementing inspection programs based on risk assessment in pig's slaughterhouses.

A problem, the assessment not being improved to be applied to the construction work site where requires diversity and complexity, causes the assessment to be inefficient to bring quality results. A study on the investigates and compares the surveyed degree of recognitions of workers who works in companies executing the risk assessment By the investigation and comparison, it is expected to bring the better solution for early and efficient application for those companies which are not taking the risk assessment.

  • Risk assessment of a railway system should be periodically conducted managing a large amount of accumulating accident/incident data and scenarios, which generally requires enormous time and efforts. Therefore, special information management system is essential for railway risk assessment, where data needed for decisions on managing the railway safety could be promptly supported. The objective of this study is to develop a railway accident analysis program for risk assessment. The program is application running on the web which links railway accident analysts throughout the railway industry to a central database. Data entered, together with associated code tables. is stored on MS-SQL database. The program uses the concepts of accident, safety events, causes, related factors(vehicle, person, infrastructure, tool/equipment), recommendations to bring together the various elements of railway accidents. The program will be useful in finding hazard conditions, accident scenarios, quantitatively assessing the risk, and providing pertinent risk measures, eventually serving to prevent railway accidents and reduce severities of railway accidents.

  • Purpose: To develop a risk metric for failure cause that can help determine the action priority of each failure cause in FMEA considering time sequence of cause- failure- detection. Methods: Assuming a quadratic loss function the unfulfilled mission period, a risk metric is obtained by deriving the failure time distribution. Results: The proposed risk metric has some reasonable properties for evaluating risk accompanied with a failure cause. Conclusion: The study may be applied to determining action priorities among all the failure causes in the FMEA sheet, requiring further studies for general situation of failure process.

Forest Fire Risk Zonation in Madi Khola Watershed, Nepal

  • Fire, being primarily a natural phenomenon, is impossible to control, although it is feasible to map the forest fire risk zone, minimizing the frequency of fires. The spread of a fire starting in any stand in a forest can be predicted, given the burning conditions. The natural cover of the land and the safety of the population may be threatened by the spread of forest fires; thus, the prevention of fire damage requires early discovery. Satellite data and geographic information system (GIS) can be used effectively to combine different forest-fire-causing factors for mapping the forest fire risk zone. This study mainly focuses on mapping forest fire risk in the Madikhola watershed. The primary causes of forest fires appear to be human negligence, uncontrolled fire in nearby forests and agricultural regions, and fire for pastoral purposes which were used to evaluate and assign risk values to the mapping process. The majority of fires, according to MODIS events, occurred from December to April, with March recording the highest occurrences. The Risk Zonation Map, which was prepared using LULC, Forest Type, Slope, Aspect, Elevation, Road Proximity, and Proximity to Water Bodies, showed that a High Fire Risk Zone comprised 29% of the Total Watershed Area, followed by a Moderate Risk Zone, covering 37% of the total area. The derived map products are helpful to local forest managers to minimize fire risks within the forests and take proper responses when fires break out. This study further recommends including the fuel factor and other fire-contributing factors to derive a higher resolution of the fire risk map.

  • Risk Management does not mean accepting a risk but having a good grasp of, eliminating and controling the exact causes of known hazards. Good safety management plan or system for the safety of any systems need to include the procedures about standing safety goals, related technical information, time schedule, audit programs, etc at least. In this paper the summeries on general risk assessment techniques and the examples of risk assessment system and railway risk management strategies used in UK, Australia and Canada are introduced, and applicable establishment procedures for domestic railway industries are proposed.
