공통원인 고장분석을 위한 전산 코드 개발 (Development of a Computer Code for Common Cause Failure Analysis)

  • 원자력 발전소에서 발생하는 공통원인 고장을 분석하기 위한 컴퓨터 코드 COMCAF를 개발하였다. 공통원인 고장을 다룰 때, 먼저 계통의 최소 단절집합들을 공통원인 기본사상들이 고려되지 않은 고장수목으로부터 구한다. 그리고, 공통원인 고장들이 같은 최소 단절집합내의 부품들간에 있는지 또는 서로 다른 최소 단절집합들의 부품들간에 있는지를 고려하여 이들 최소 단절집합들의 발생 확률값을 계산한다. 유사하거나 동일한 부품들간에 공통원인 고장이 있을때는 Basic Para-meter 모델을 사용한다. 그러나, 서로 다른 부품들간에 공통원인 고장이 있을때는 Basic Para-meter모델에 쓰인 Symmetry Assumption을 두개 이상의 부품에 영향을 주는 기본사상들에만 적용한다. Inclusion-Exclusion방법을 사용하여 정점사상확률간을 구한다. 이 경우 같은 최소 단절 집합들에 있는 부품들의 공통원인 고장뿐만아니라 서로 다른 최소 단절집합들에 있는 부품들의 공통원인 고장도 쉽게 고려될 수 있다. 본 연구에서는 이러한 공통원인 고장분석을 가압경수로의 보조 급수계통에 적용하였다. 이들 정점사상의 확률값들을 공통원인 고장이 없는 경우와 비교하였다.

보강토옹벽의 사고사례에 관한 연구 (Reinforced Earth Retaining Wall of The Collapsed-A Case Study.)

  • This paper deal with cause and analysis about case of collapsed reinforced-soil retaining wall. The analysis of the cause was carried through experimentation, slop stability analysis and literature study. The experimentation treated the large direct shear test, the hydraulic conductivity test and the other basic test through backfill extracted from collapsed reinforced-soil retaining wall. The ultimate tensile strength was established by rib tensile strength test of geogrid. The analysis of internal and external stability of reinforced-soil retaining wall was performed on the basis of parameters. The result of analysis, reinforced-soil retaining wall and the slope at the dry season are stable. However, the factors that fine-grained soil at hydrometer test exceed the standard of the design, rainfall duration is too long at the time of collapse and monthly pricipitation is heavy are cause of the collapse.

정보산업 생태계 문제의 근본원인 분석 (Root Cause Analysis of the Information Industry Ecosystem Problems)

  • Finding a root cause is an essential step to solving a complex problem. Some previous studies have used the Delphi method for gathering opinions about root causes from geographically dispersed experts. However, we assert that complicated problems such as an industry ecosystem would make a general type of the Delphi method less practical because of too much psychological burden on study participants. In this study we present a preliminary list-based Delphi study method for identifying a root cause. This method was used to identify a root cause and draw a causal map for the information industry ecosystem problems.

  • Modular Construction is regarded as having enhanced safety compared to traditional construction since most of modular manufacturing process in plants. Unlike general consideration for safety in modular construction, several industrial accident data and studies have pointed out that the accident rate of modular construction is not enough less as much as the practitioners have expected. It means that there is a clear need for improvement of safety management in modular construction. To enhance safety, it is necessary to identify the type and cause of accident through accident cases in order to prevent safety accident in advance. In this consideration, this study analyzed the types and causes of accidents through root cause analysis procedure with accident cases of U.S. OSHA. The classification was carried out in the order of process type, accident type and cause of accident. By following the classification criteria in this study, the causal factor was derived and the root cause map was created. Based on the analysis results, cross-analysis was conducted and it is shown that activity characteristics of modular construction are related to safety accidents. In addition, prevention methods to reduce safety accident by major activity are presented in terms of organizational, educational and technical aspects. This study contributes that the result can be used as the basic safety management in the manufacturing and construction process of modular construction.

  • This paper dives the current status of lightning damages in apartments obtained by questionnaire and analysis the cause of ligntning damages. As a result, the apartments experienced lightning damages are assumed about 5[%] lot total apartments and the lightning surges invaded on power line are identified as the main cause of lightning damages in apartments.

  • Although it is widely accepted that a fire can occur due to electrical causes even when an engine stops, there is little introduction of detailed case analysis. This study analyzed a fire case caused by an electrical cause during engine stopping at parking lot in detail. Moreover, it was revealed that the fire was mainly caused by design defect.

  • 국내 대학 또는 기업 부설 연구소 실험실에서 유해화학 물질 사용 증가에 따라 가스누출 등으로 인한 폭발 화재 등 각종 대형 사고가 발생하여 사회적 이슈로 관심이 집중되었고, 실험실에서 안전관리에 대한 중요성이 강조되어 국내 대학 또는 기업 부설 연구소에서 연구 실험하는 연구 활동종사자의 생명과 신체를 보호하고, 쾌적한 실험 환경 조성을 위하여 연구실 안전환경조성에 관한 법률이 제정되었다. 국내 대학 또는 기업 부설 연구소에서는 이러한 사고 방지를 위하여 실험실 안전관리체계를 구축하고 안전점검과 정밀안전진단 등을 실시하고 있다. 이러한 조치에도 불구하고 국내 실험실에서는 사고가 지속적으로 발생하여 사고에 대한 근본원인분석이 필요한 실정이다. 이 논문에서는 국내 대학 또는 기업 부설 연구소 실험실에서 발생한 사건 사고에 대해 근본원인분석(Root Cause Analysis) 기법을 활용하여 사고 원인에 대해 분석한 결과를 보여주고, 국내 실험실의 안전관리방향을 제시 하고자 한다.

개인 및 사고원인 특성이 재난안전인식에 미치는 영향분석 (Analysis of the Effect of Individual and Accident Cause Characteristics on Disaster Safety Perception)

  • The purpose of this study is to diagnose the level of disaster safety perception (disaster safety importance, accident possibility, personal safety, and regional safety) of local residents, and to analyze the impact of individual and accident cause characteristics. The analysis method used multiple regression analysis, and the main analysis results are as follows. First, disaster safety importance and accident possibility were higher as residents who had experience in safety education and were willing to visit the safety experience center in the province. Second, disaster safety importance was higher as the cause of the accident was the lack of response 119 and police. And accident possibility was higher as the cause of the accident was the lack of prior treatment by the state and local governments. Third, personal safety and regional safety were higher for men, and especially personal safety was lower as residents with children. The results of this study can be used as basic data for establishing regional customized disaster safety policies based on the perception of local residents, who are policy consumers.

항전계통 간헐결함 사례 분석을 통한 군의 해외 정비품 국내정비능력 개발제도 개선방안 고찰 (A Study on the Improvement of the Domestic Maintenance Capability Development System of Military for Overseas Maintenance Items through the Case Study of Intermittent Failure of an Avionics)

  • A flare from an Air Force fighter was abnormally dispensed during the landing. To determine the cause of the abnormal dispensing, fracture analysis, signal analysis and reproduction experiments based on physics of failures were performed. The primary cause of the failure was analyzed to be due to an intermittent fault of an internal circuit card in the AN/ALE-40 chaff/flare dispenser by a broken lid of a capacitor, and the root cause which had derived the primary cause was considered to be an improper handling during the domestic maintenance which were changed from the overseas maintenance due to the DMSMS problem. Therefore, the overall process of the maintenance capability development system was reviewed and alternative ways that considers maintenance error decision aid(MEDA) for system improvement were suggested to prevent further failures.

원심펌프 벌류트 혀의 균열 원인분석 및 건전성 평가 (Integrity Evaluation and Root Cause Analysis of Cracks at the Volute Tongue of Centrifugal Pump)

  • This paper provides integrity evaluation and root cause analysis for defects observed at volute tongue, or cutwater, of the operating centrifugal pump in power plant. The cause of the cracks are analyzed and reviewed from the viewpoint of the operation and maintenance of the pumps, and the sample obtained from the cracked volute tongue of the pump are examined. At first, in-situ hardness test and microstructure examination were performed to understand the cause of cracking at volute tongue. The evaluation of structural integrity and the possibility of the crack propagation is also evaluated. Cracks were typical intergranular cracking and propagated along with prior austenite grain boundary. At easing volute tongue, the hardness was higher than ASTM requirement and a large amount of intergranular Cr carbide was precipitated. These were due to high C content in material. P content was also higher than ASTM requirement. Therefore, Cr carbide precipitation and P segregation at grain boundary, caused by higher C and P content in material, resulted in intergranular cracking of casing volute tongue. This procedure for integrity evaluation and root cause analysis is used to guide, and support the pump designer and manufacturer's material selection and process design to avoid a costly, unplanned outage of plant.
