Malignant Bowel Obstruction in Terminal Cancer Patients (말기암 환자의 악성 장 폐색)

  • Purpose: As for the malignant bowel obstruction of terminal cancer patient, a prognosis is relatively bad. Physicians consider palliative procedures or surgery for the quality of life, but sometimes it is hard to decide. After diagnosis of a malignant bowel obstruction in terminal cancer patients, we investigated the clinical characteristics, the prognostic factors and the survival of patients with palliative procedures or surgery. Methods: we retrospectively reviewed the medical records in 40 malignant bowel obstruction patients who had been diagnosed as terminal cancer from May in 2002 to May in 2004. Results: There were 21 males (53%) and 19 females (47%), and median age of patients was $64.1{\pm}1.58$ years. The most common cause of malignant bowel obstruction was colorectal cancer (18 patients, 45%), followed by stomach cancer (11, 28%), pancreatic cancer (4, 10%), others (7, 19%). Metastases were carcinomatosis peritonei (14 patients, 35%), liver (13, 33%). During a bowel obstruction, symptoms were vomiting (15 patients, 38%), abdominal pain (10, 25%), constipation (6, 15%), abdominal distension (5, 13%). Performance status (ECOG) was 2 score (16 patients, 40%), 3 score (20, 50%), 4 score (4, 10%). Palliative procedure group were 30 patients, the others 10. Median survival in palliative procedure group was 142 days, that of no palliation group 30. Median survival time of palliative procedure group from palliative procedures or surgery were significantly higher than that of no palliation group from diagnosis of malignant bowel obstruction. Prognostic factors of palliative procedure group were PS, site of obstruction and primary cancer. Median survival in PS 2, lower GI obstruction and colorectal cancer was higher than PS 3, upper GI obstruction and others, respectively. Conclusion: we recommend aggressively palliative procedures or surgery in malignant bowel obstruction patients diagnosed with terminal cancer if palliative procedures or surgery could be performed effectively.

Analysis and Performance Evaluation of Pattern Condensing Techniques used in Representative Pattern Mining (대표 패턴 마이닝에 활용되는 패턴 압축 기법들에 대한 분석 및 성능 평가)

  • Frequent pattern mining, which is one of the major areas actively studied in data mining, is a method for extracting useful pattern information hidden from large data sets or databases. Moreover, frequent pattern mining approaches have been actively employed in a variety of application fields because the results obtained from them can allow us to analyze various, important characteristics within databases more easily and automatically. However, traditional frequent pattern mining methods, which simply extract all of the possible frequent patterns such that each of their support values is not smaller than a user-given minimum support threshold, have the following problems. First, traditional approaches have to generate a numerous number of patterns according to the features of a given database and the degree of threshold settings, and the number can also increase in geometrical progression. In addition, such works also cause waste of runtime and memory resources. Furthermore, the pattern results excessively generated from the methods also lead to troubles of pattern analysis for the mining results. In order to solve such issues of previous traditional frequent pattern mining approaches, the concept of representative pattern mining and its various related works have been proposed. In contrast to the traditional ones that find all the possible frequent patterns from databases, representative pattern mining approaches selectively extract a smaller number of patterns that represent general frequent patterns. In this paper, we describe details and characteristics of pattern condensing techniques that consider the maximality or closure property of generated frequent patterns, and conduct comparison and analysis for the techniques. Given a frequent pattern, satisfying the maximality for the pattern signifies that all of the possible super sets of the pattern must have smaller support values than a user-specific minimum support threshold; meanwhile, satisfying the closure property for the pattern means that there is no superset of which the support is equal to that of the pattern with respect to all the possible super sets. By mining maximal frequent patterns or closed frequent ones, we can achieve effective pattern compression and also perform mining operations with much smaller time and space resources. In addition, compressed patterns can be converted into the original frequent pattern forms again if necessary; especially, the closed frequent pattern notation has the ability to convert representative patterns into the original ones again without any information loss. That is, we can obtain a complete set of original frequent patterns from closed frequent ones. Although the maximal frequent pattern notation does not guarantee a complete recovery rate in the process of pattern conversion, it has an advantage that can extract a smaller number of representative patterns more quickly compared to the closed frequent pattern notation. In this paper, we show the performance results and characteristics of the aforementioned techniques in terms of pattern generation, runtime, and memory usage by conducting performance evaluation with respect to various real data sets collected from the real world. For more exact comparison, we also employ the algorithms implementing these techniques on the same platform and Implementation level.

A Study on Hygiene and Safety of Sanitary Wet Towel (물수건의 위생실태 및 안전성 연구)

  • The risks of sanitary indicative bacteria, heavy metals and chlorinated derivatives in 94 cases of sanitary wet towels used in food services (39 from sanitary wet towel treatment business, 55 from food services) were assessed in the present study. Lead was detected in the range of N.D.~0.41 mg/kg (75 cases were not detected), N.D.~0.25 mg/kg of arsenic (93 cases were not detected), N.D.~0.01 mg/kg of cadmium (7 cases were lower than limit of quantitation; 87 cases were not detected), 0.003 mg/kg ~ 0.09 mg/kg of mercury. And chromium (VI) was not detected from all samples. The level of lead was the highest among the tested heavy metals, and the highest concentration of lead was 0.41 mg/kg. However, it was only 2.1% of legal limit (less than 20 mg/kg). The average moisture content of the samples was 61.9% (50.0% ~ 77.0%) and it showed no relevance to the detection of bacterial counts. Escherichia coli was not detected. Bacterial counts were detected 43 cases and among them, 24 cases were exceeded the legal limit. It was verified that the packaging conditions of sanitary wet towel (whether it is packed by a piece or not and sealed or not) are critical factors to cause the germ contamination and cross contamination in the wet towels. The chlorinated derivatives (chlorites and chlorates) were detected in 17 (19.3%) out of 88 cases. The results would be used as preliminary information to establish the programs of "Safety education for manufacturers and public policy of safety".

Sanitary Characteristics of Seawater and Sediments in Tongyeong Harbor (통영항의 해수 및 저질의 위생학적 특성)

  • The bacteriological and physiochemical analysis of sea water and sediments in Tongyeong harbor was conducted to evaluate sanitary conditions. The samples were collected at 8 stations established once a month from June, 2008 to May, 2009. During the study period, the range of temperature was from 6.7 to $25.2^{\circ}C$, transparency ranged from 1.2 to 2.6 m, chemical oxygen demand ranged from 1.90 to 2.92 mg/L, dissolved oxygen ranged from 6.2 to 10.5 mg/L, dissolved nitrogen ranged from 0.052 to 0.098 mg/L, phosphate ranged from 0.044 to 0.065 mg/L, respectively. Seafood, if eaten raw, carries the risk of food poisoning. Seafood poisoning is often cause by pathogenic microorganism originating from fecal contamination, such as Salmonella sp., Shigella sp. and norovirus. Fecal coliforms are an important indicator of fecal contamination. Therefore, data on fecal coliform are very important for evaluating the safety of fisheries in coastal areas. So, we investigated the sanitary indicate bacteria. The coliform group and fecal coliform MPN's of sea water in Tongyeong harbor were ranged from < 1.8~22,000/100 mL (GM 164.9 MPN/100 mL) and < 1.8~7,900 MPN/100 mL (GM 33.7 MPN/100 mL), respectively. Total coliform were detected 97.0% in 96 of samples and 68.9% of total coliforms were fecal coliforms. These results similar to another seawater detection ratio of total coloforms and fecal coliforms. The Vibrios was isolated and identified with VITEK system. Four hundred eighty strains that were obtained from sea water samples in Tongyeong harbor Detection ratio Vibrio alginolyticus, 34.2%, Vibrio parahaemolyticus, 13.8%, Vibrio vulnificus 10.0%, and V. mimicus 12.5% respectively. Vibrio cholerae O1, was not detected. During the study period, the ranges of water content, ignition loss, COD, and acid volatile sulfates in sediments in Tongyeoung harbor were 41.0~57.4%, 7.8~10.5%, 6.51~9.30 mg/g, 0.04~0.09 mg/g, respectively. Heavy metals in sediment of Tongyeoung harbor were Cd, $0.10{\pm}0.05$; Cu, $4.79{\pm}8.20$; As, $1.95{\pm}0.17$; Hg, $0.10{\pm}0.07$; $Cr^{6+}$, $0.34{\pm}0.12$; Zn, $125.33{\pm}16.40$; Ni, $16.43{\pm}1.93$ mg/kg.

Antimicrobial Effect of Red Ginseng Against Major Food Poisoning Microorganisms Including Staphylococcus aureus (황색포도상구균을 포함한 식중독 미생물들에 대한 홍삼의 항균효과)

  • This study was carried out to evaluate the antimicrobial effect of red ginseng (Panax ginseng C.A. Meyer) against several foodborne pathogens including Staphylococcus aureus, Escherichia coli, Candida albicans and Aspergillus niger. The antimicrobial effect was determined by agar diffusion method using red ginseng extract, crude saponin and non-water-soluble fractions. Red ginseng extract showed antimicrobial effect against S. aureus, but not C. albicans or A. niger. The extract showed anti-bacterial activity at concentration above 30% against S. aureus, which cause both food poisoning and atophic dermatitis. Crude saponin showed antibacterial activity above 7.5% against the bacterium. However, the ginsenosides purified from crude saponin showed no antimicrobial activities at 100-200 ㎍/mL. To investigate the mode of growth inhibition, red ginseng extract and crude saponin were added to 0.85% NaCl solution containing S. aureus and then incubated at 35℃ for 12 h. The results showed that viable cells were rapidly reduced in above 10% concentration of red ginseng extract and above 2% of crude saponin, respectively. However, the crude saponin and red ginseng extract did not inhibit the bacterial cells completely at those same concentrations. On the other hand, whereas all non-water-soluble fractions showed inhibition zones above 10 mm against S. aureus, they showed no inhibition effects against E. coli, C. albicans or A. niger. The methanol fraction-1 (MF-1) showed the highest antibacterial activity against S. aureus, and the MIC (minimal inhibitory concentration) was 0.625 mg/mL. These results suggest that red ginseng extract, crude saponin and non-water-soluble fractions show selective antibacterial activity against S. aureus, and non-water-soluble fractions might be used as natural antibacterial agents.

Potential use of Bacillus amyloliquefaciens as a probiotic bacterium in abalone culture (북방전복, Haliotis discus hannai 에 대한 Bacillus amyloliquefaciens의 probiotic 효과)

  • In comparison to the numbers of such studies of fish, few studies have been carried out on the immunity, physiology and ecology of abalone, while studies on abalone disease are also extremely rare. Moreover, mass mortality of cultured abalone due to pathogenic bacteria has not been reported in the southern coast of Korea. However, Vibrio-like bacteria have been isolated from dead abalone, which indicates that a review is required in order to determine the cause of abalone mortality. Use of an antimicrobial agent to minimize the damage caused by disease in abalone farms is common, but the therapeutic effects are insignificant. Demand for probiotics has increased, but research on the development of probiotics for use in abalone culture is very rare. Therefore, the present study isolated KC16-2 from fermented kimchi soup and investigated the characteristics of the isolate as a candidate probiotic bacterium in abalone. KC16-2 was identified as Bacillus amyloliquefaciens (B. amyloliquefaciens KC16-2) based on its biochemical properties and 16S rRNA gene sequence. B. amyloliquefaciens KC16-2 showed inhibitory effects against the growth of various vibrios in vitro, and kept the numbers constant until four days after inoculation in marine water at a temperature of $15{\sim}25^{\circ}C$, indicating the possible use of KC16-2 as a probiotic, except in the winter. The growth of KC16-2 was inhibited by bile salt, but the numbers increased over time suggesting the bacteria were still alive in the abalone's digestive tract. Abalone fed with a diet including KC16-2 for 12 weeks showed good growth, but showed no significant differences from the control group. However, the mortality of the abalone supplied the probiotic diet was reduced to half that of the control group in a challenge test with Vibrio tubiashii. Therefore, we suggest that B. amyloliquefaciens KC16-2 could be used as a probiotic bacterium for control of the mortality of abalone caused by opportunistic pathogenic vibrios.

  • Stress-susceptible pigs have been known as the porcine stress syndrome (PSS), swine PSS, also known as malignant hyperthermia (MH), is characterized as sudden death and production of poor meat quality such as PSE (pale, soft and exudative) meat after slaughtering. PSS and PSE meat cause major economic losses in the pig industry. A point mutation in the gene coding for the ryanodine receptor (RYR1) in porcine skeletal muscle, also known calcium (Ca$^{2+}$) release channel, has been associated with swine PSS and halothane sensitivity. We used the PCR-RFLP(restriction fragment length polymorphism) and PCR-SSCP (single strand conformation polymorphism) methods to detect the PSS gene mutation (C1843T) in the RYR1 gene and to estimate genotype frequencies of PSS gene in Korean pig breed populations. In PCR-RFLP and SSCP analyses, three genotypes of homozygous normal (N/M), heterozygous carrier (N/n) and homozygous recessive mutant (n/n) were detected using agarose or polyacrylamide gel electrophoresis, respectively. The proportions of normal, carrier and PSS pigs were 57.1, 35.7 and 7.1% for Landrace, 82.5, 15.8 and 1.7% far L. Yorkshire, 95.2, 4.8 and 0.0% for Duroc and 72.0, 22.7 and 5.3% for Crossbreed. Consequently, DNA-based diagnosis for the identification of stress-susceptible pigs of PSS and pigs producing PSE meat is a powerful technique. Especially, PCR-SSCP method may be useful as a rapid, sensitive and inexpensive test for the large-scale screening of PSS genotypes and pigs with PSE meat in the pork industry.y.

Drug Interaction between Ginseng Extract (GE) and Sorafenib (쏘라페닙과 홍삼추출물간의 약물상호작용)

  • Sorafenib is the only approved systemic, therapeutic agent for hepatocellular carcinoma (HCC). The use of Ginseng Extract (GE) in cancer patients is growing worldwide; however, drug interaction between sorafenib and GE has not been illuminated. Four different human cancer cell lines including HepG2 were used and immunocompetent mice were implanted subcutaneously with a mouse HCC cell line. Treatment with low dose GE stimulated cell growth, while a high dose inhibited growth. pERK (phosphorylation of extracellular signal-regulated kinase) was concomitantly increased and decreased respective of different doses of GE. Antitumoral effect of sorafenib decreased in non-proliferating phase cells but was sensitized after low dose GE (LDG) treatment. PD98059 (ERK phosphorylation inhibitor) efficiently blocked ERK phosphorylation, resulting in loss of sorafenib sensitization even after LDG treatment. In the HCC mouse model, LDG alone slightly increased tumor size while sorafenib alone significantly decreased it. However, a combination of LDG and sorafenib significantly decreased tumor size compared with sorafenib alone. Increase of pERK was observed in some normal mice organs and mild inflammatory change was observed in some of these organs, suggesting pERK activation by LDG may cause unexpected toxicity in normal cells. GE, dose-dependently, induced stimulation or inhibition in some human cancer cell lines. Combinational use of GE and sorafenib possibly potentiated an antitumoral response to sorafenib. pERK level has been provided as a potential predictive marker for sorafenib. Our result may suggest GE's dual effects in relation to pERK level in HCC cancer cell lines, and that certain doses of GE can sensitize sorafenib.

Application of Transposable Elements as Molecular-marker for Cancer Diagnosis (암 진단 분자 마커로서 이동성 유전인자의 응용)

  • Until now, various oncogenic pathways were idenfied. The accumulation of DNA mutation induces genomic instability in the cell, and it makes cancer. The development of bioinformatics and genomics, to find the precise and reliable biomarker is available. This biomarker could be applied the early-dignosis, prediction and convalescence of cancer. Recently, Transposable elements (TEs) have been attracted as the regulator of genes, because they occupy a half of human genome, and the cause of various diseases. TEs induce DNA mutation, as well as the regulation of gene expression, that makes to cancer development. So, we confirmed the relationship between TEs and colon cancer, and provided the clue for colon cancer biomarker. First, we confirmed long interspersed nuclear element-1 (LINE-1), Alu, and long terminal repeats (LTRs) and their relationship to colon cancer. Because these elements have large composition and enormous effect to the human genome. Interestingly, colon cancer specific patterns were detected, such as the hypomethylation of LINE-1, LINE-1 insertion in the APC gene, hypo- or hypermethylation of Alu, and isoform derived from LTR insertion. Moreover, hypomethylation of LINE-1 in proto-oncogene is used as the biomarker of colon cancer metastasis, and MLH1 mutation induced by Alu is detected in familial or hereditary colon cancer. The genes, effected by TEs, were analyzed their expression patterns by in silico analysis. Then, we provided tissue- and gender-specific expression patterns. This information can provide reliable cancer biomarker, and apply to prediction and diagnosis of colon cancer.

A Study for the Shoes Micro-sized Manufacturing Industry and the Development of the Government Policy: Surveyed on Beomcheon-Dong in Busan (신발소공인 산업의 실태분석 및 정책지원 방향: 부산진구 범천동을 중심으로)

  • Korean Economy has been developed by the Korean Government's Support for the Large-sized Firms. This Government Policy causes the Polarization between Large-sized and Micro-sized Firms Aggravated. Micro-sized Firms are distributed over the Whole Industry Area, and can also cause the Economic Crisis If They are crashed down. Therefore Government Policy for the Micro-sized Manufacturing Industry is very Important Issue. This Paper Focused on the Analysis of Current Status for the Shoes Micro Manufacturing Industry. For the Effective Analysis, This Paper uses the Statistical Data Open to the Public and also conducts the Survey for the Micro-sized Firms in Busan. Statistical Program is used for Analyzing the Collected Data and the Major Findings are as Follows. First, Shoes Industry is led by the Micro and Small & Medium sized Firms rather than the Large-sized. And the Micro-sized Firms are getting the High Rate among the Whole Shoes Industry. Busan is heavily populated Area as the Origin of Shoes Industry. Second, even though Most of the Owner of the Micro-sized Firms have the High Technology Skill Level, Worker's Aging Phenomenon gets Worse and causes the Technology Handing down to the Next Generation Difficult. Third, Because the Factory Facility of the Micro-sized Firms is Dirt and Unstable, the Modernized Manufacturing Infrastructure such as the Apartment Factory Facility is Necessary. Forth, as the Micro-sized Firms which have the Intangible Asset such as Patent is Few, the Government Policy for Encouraging the Patent Application is strongly Needed. Fifth, Entrepreneurship and Collaboration Mind between Micro-sized Firms are Lacked, so Establishment of the Cooperative Union is required. Finally, the Effort for the Systemic Planning for the Management is lacked, and the Introduction of the Management Innovation is strongly needed. The Limitation and Future Research Direction is also discussed.
