A Clinical Study of Patent Ductus Arteriosus (동맥관개존증의 임상적 고찰)

  • Since the first report of successful ligation of patent ductus arteriosus in 1939, it`s surgical intervention has become a routine and relatively safe procedure. During the past ten years from Aug. 1975 to Aug. 1985, 107 cases were operated on for a patent ductus arteriosus at the Department of thoracic and Cardiovascular Surgery, School of Medicine, Kyungpook National University. Clinical analysis of these cases was performed. Mean age at operation was 9.4 years, ranging from 20 months to 32 years. Sex ratio of female to male was 1.8;1. Most common symptoms were frequent respiratory infection, exertional dyspnea, and palpitation. Diagnosis was made by auscultation, 2 dimensional echocardiography, cardiac catheterization, and cineangiocardiography. A moderate to severe pulmonary hypertension was found in 42 cases [49.4%] in cardiac catheterization. Operative methods were multiple ligation of paten`. ductus arteriosus with or without Dacron or Teflon wrapping in 72 cases [68%], and division and suture in 34 cases [32%]. There were three operative deaths [2.8%]. The causes of death were hemorrhage from tearing of aorta, low cardiac output, and arrhythmia. All of these cases had moderate degree of pulmonary hypertension.

  • The clinical observations on the bronchogenic cancer of one hundred sixty cases treated in the Department of Thoracic Surgery, Seoul National University Hospital are as follows: 1] Most of them,. 90 percent of the cases were over 40 years of age and male was predominated over female with sex ratio of 6 to 1. 2] The average duration of symptoms before the first examination was about 5 months arid the most common complaints were cough, chest pain, breathing difficulty and blood-tinged sputum. 3] In ninety-four cases[59%] thoracotomy was undertaken with resection of the lesion in 52 cases[33% ]. 4] Histopathological diagnoses made in 141 cases were squamous cell carcinoma in 47.5%, undifferentiated cell carcinoma in 33.3%, adenocarcinoma in 9.2,% and alveolar cell carcinoma in a single case.

  • Obstruction of the right ventricular outflow tract occurs in many various positions. Recently, obstruction due to anomalous muscle bundle has been clearly recognized. This anomaly is caused by a large anomalous muscle bundle, which creates an obstruction low in the body of the right ventricle and divides it into two chambers. Three cases of double chambered right ventricle were treated surgically utilizing cardiopulmonary bypass in the Department of Chest Surgery, College of Medicine, Seoul National University. 1] All of the 3 cases were female. 2] In 2 cases, FKG findings were RVH and RAD, but in case 2, normal EKG findings. 3] In case 2, no pressure gradient between the inflow portion and outflow portion of the right ventricle was observed, but in case 1 and 3, pressure gradients were 70 and 64 mmHg, respectively. 4] In 2 of the 3 cases, type II VSD was combined to double chambered right ventricle. 5] All of the 3 cases were discharged with symptomatic improvement after operation.

  • Aneurysmal rupture of sinus Valsalva is known as one of rare cardiac disease and not controlled with medical treatment. We experienced 9 cases which were composed with 7 case of male and 2 cases of female during last 15 years. 7 cases were ruptured into right ventricle from right. coronary sinus and 2 cases were ruptured into right atrium from non-coronary sinus. The diagnosis was made with cardiac catheteriza1icn and cineangiccardicgram but 2 cases were misdiagnosed with only cardiac catheterization. All cases were corrected under cardiopulmonary bypass with means of direct suture with tdlon pledget by transatrial or transventricular approach. All were not confirmed in it's origin because of no history, no evidences of syphilis, TB, or bacterial endocarditis and only fibrosis in pathologic report. In postoperative course, 1 case had postoperative bleeding and 1 case was sufferd from left hemiplegia due to may be air embolism. Follow up study revealed all patient go on their usual life well with good improvement at this present time.

  • This is a case report of traumatic arterial injuries with false aneurysm & arterio-venous fistula treated surgically at National Medical Center. 3 cases were A-V fistula and 2 cases only false aneurysm. Physiological disturbance were produced by only arteriovenous fistula; In one case ulceration of mid. 1/3 tibia due to diminished arterial flow and in 2 cases left ventricular hypertrophy, in which cases Bramhan`s sign were positive. Removing out the fistulous lesions and aneurysm, all of the arterial continuities has been reconstructed by means of end to end anastomosis, Dacron graft and vein graft, veins were managed by ligations of both ends in two cases and end to end anostomosis in one case. Immediate post operative results were good, and two cases were followed for 10 months.

Surgical Treatment of Esophageal Achalasia - Report of 6 cases - (식도 Achalasia 의 외과적 치료 -6례 보고-)

  • Six cases of achalasia were treated by modified Hellers operation at the department of thoracic % cardiovascular surgery of Kosin medical college from April 1984 to July 1988. Among 6 cases, 4 were male and 2 were female. Preoperative main symptoms were dysphagia in 5 cases and regurgitation in 1 case, its duration was variable from 1.5 years to 40 years. Esophagocardiomyotomy was perfomed in all cases, and the results were excellent to good, but reoperation was needed in one case due to incomplete myotomy. One case of advanced achalasia was also experienced and the result was satisfactory. Antireflux procedures were not performed in all cases and there was no reflux esophagitis on follow-up study.

  • Purpose: The purpose of this report was to describe the clinical and microbiological characteristics of two rare cases of necrotizing stomatitis, and the outcomes of a non-invasive treatment protocol applied in both cases. Methods: We report two cases of necrotizing stomatitis in a rare location in the hard palate of a 40-year-old woman and a 28-year-old man. Neither had a relevant medical history and both presented with highly painful ulceration in the palate and gingival margin that was accompanied by suppuration and necrosis. 3% hydrogen peroxide was applied to the lesions using sterile swabs, and antibiotic and anti-inflammatory treatment was prescribed to both patients in addition to two daily oral rinses of 0.2% chlorhexidine. Results: In both cases, radiological examination ruled out bone involvement, and exfoliative cytology revealed a large inflammatory component and the presence of forms compatible with fusobacteria and spirochetes. There was a rapid response to treatment and a major improvement was observed after 48 hours, with almost complete resolution of the ulcerated lesions and detachment of necrotic areas with partial decapitation of gingival papillae. Conclusions: Necrotizing periodontal lesions can hinder periodontal probing and the mechanical removal of plaque in some cases due to the extreme pain suffered by the patients. We present a non-invasive treatment approach that can manage these situations effectively.

Ventricular Septal Defect with Aortic Insufficiency: A Report of 7 Cases (대동맥판막 폐쇄부전증을 동반한 심실중격결손증 -7례 보고-)

  • The development of aortic insufficiency radically alters the physical findings which are generally associated with ventricular septal defect that was not hemodynamically significant, and the combination of the two lesions produces a typical clinical picture, that may be serious and life-threatening when it is left untreated. Therefore, the selection of patients, type and timing of surgical treatment is considered to be important. Among 114 cases of ventricular septal defect treated surgically utilizing cardiopulmonary bypass in the Department of Thoracic Surgery, Seoul National University Hospital, 7 cases were associated with aortic insufficiency. 1. Five cases were male, and 2 cases were female. Ages were from 4 years to 24 years, and mean age was 11.9 years. 2. In all cases, ventricular septal defect was closed with Teflon patch. In a case, a aortic valvuloplasty and in another, a aortic valve replacement with Hancock valve 23 mm., 5 months after the closure of ventricular septal defect were done. 3. Four cases were type I ventricular septal defect by Kirklin`s classification, 3 cases were type II ventricular septal defect, and diameters of ventricular septal defect were from 3.5 cm. to 0.7 cm. A PDA. was combined to a type I ventricular septal defect. 4. In 5 cases, herniation of the aortic cusp through the ventricular septal defect and in a case, annulus dilatation on the aortic valve was noted. 5. Two cases with type I ventricular septal defect and severe pulmonary hypertension expired. A re-opened case with type II ventricular septal defect expired. 6. Four cases were alive, and all of them show decrease of pulse pressure and aortic insufficiency.

Spinal Schwannoma; Analysis of 40 Cases

  • Objective: This study is to report our experience of 40 cases of spinal schwannoma. Methods: From 1995 to 2006, medical records were retrospectively reviewed in 40 cases of spinal schwannoma. Results: We treated 40 spinal schwannomas in 38 (22 male and 16 female) patients. The mean age was 50.2. Four cases were sited in the cervical spine, 11 cases in the thoracic spine, and 25 cases in the lumbar spine. Two patients showed recurrences. Thirty-eight cases were intradural-extramedullary type and 2 cases were extradural. Two cases (5%) including 1 recurred case had no postoperative motor improvement. Ninety-five percents of patients improved on postoperative motor grade. Conclusion: Spinal schwannoma is mostly benign and extramedullary tumor. There were 2 recurred cases (5%) that had history of previous subtotal removal at first operation and had shown worse prognosis compared with the cases without recurrence. To reduce the recurrence of spinal schannoma, total excision of tumor mass should be done.

  • Deep vein thrombosis and pulmonary embolism are serious fatal complication after orthopedic surgery. Most of them occur after large operation such as total knee/hip arthroplasty. Some reports about deep vein thrombosis after simple arthroscopic procedure were on the Western literatures, but there was no report on the Korean literature. So we report two cases of pulmonary embolism after knee arthroscopic procedures.
