윤상 교약성 심낭염의 외과적 치료 -2예 보고- (Annular Constrictive Pericarditis: Simulating Vavular Heart Disease : Case Report)

  • This is report of two cases of annular constrictive pericarditis. Since January 1959 to December 1979 the authors experienced 48 cases of chronic constrictive pericarditis treated surgically at the Department of Thoracic and Cardiovascular Surgery, the National Medical Center in Seoul. These cases simulated valvular heart disease. One case, as mitral stenosis, revealed rumbling apical diastolic murmur [II/VI], atrial fibrillation and right ventricular hypertrophy pattern on E.C.G., the other, as infundibular pulmonic stenosis, presented pressure gradient between right ventricle and main pulmonary artery at infundibular level of 76 mmHg in systole. Both patients underwent operation successfully and one of them was assisted by E.C.C. during pericardiectomy and result was excellent. It is difficult to make the diagnosis of these conditions preoperatively so consideration about these might be important to make the diagnosis accurately.

Minimally Invasive Approach to Esophageal Perforation after Endoscopic Ultrasound-Guided Fine-Needle Aspiration: A Report of 2 Cases

  • Esophageal perforation after endoscopic ultrasound-guided fine-needle aspiration for mediastinal staging is a rare but severe complication. We report 2 cases of patients with esophageal perforation who were treated using video-assisted thoracoscopic surgery in combination with esophageal stenting. Through these cases, the feasibility of minimally invasive thoracic surgery was evaluated.

  • Objectives: The purpose of these cases is to report the effects of Korean medicine on two women with recurrent miscarriage.Methods: One women who had recurrent miscarriage with myoma uteri was 39 years old. She was treated with herbal medicine and acupuncture until intrauterine pregnancy (IUP) 14 wks. the other who had recurrent miscarriage with adenomyosis was 40 years old. she was also treated with these until IUP 11 wks.Results: After treatment, one women delivered successfully by normal full term spontaneous delivery. The other succeeded in normal pregnancy and she is 15 weeks pregnant.Conclusions: Two cases show that Korean Medicine has effects on recurrent miscarriage with old age and underlying diseases.

The treatment of malocclusion after open reduction of maxillofacial fracture: a report of three cases

  • The posttraumatic complications of jaw fractures related to jaw function and facial deformity include nonunion, malunion, malocclusion, temporomandibular joint dysfunction and facial asymmetry. This report presents cases referred to our department for revision of malunion and malocclusion following inadequate reduction of jaw fractures. Three patients with posttraumatic malocclusions caused by malunion were treated with a LeFort I osteotomy in one case and re-fracture in two cases. All of the patients exhibited stable results without further complications (e.g., malunion or malocclusion). Accurate preoperative diagnosis and proper anatomical reduction of the fracture segments are essential to preventing post-surgical malunion and malocclusion.

  • Ganglion cysts causing suprascapular nerve compression are uncommon cause of suprascapular nerve entrapment. The advent of magnetic resonance imaging and its application in patients with shoulder pain has improved the ability to diagnose cystic lesions causing extrinsic compression of the suprascapular nerve. We present two cases of suprascapular nerve compression by ganglion cyst which was decompressed by surgical excision. Suprascapular nerve compression was also revealed by magnetic resonance imaging(MRI), electromyogram(EMG) and clinical present of muscle atrophy and motor weakness. In each case, the symptom was resolved after sugical excision of the cystic lesion. We experienced two cases of suprascapular nerve entrapment by ganglion cyst and report the clinical, radiological and pathological findings in detail.

가감청심연자음으로 치료한 질건조증 환자 치험 3예에 대한 임상보고 (Clinical Report on Three Cases of Vaginal Dryness Treated with Gagam-chungsimyeonjaeum)

  • Vaginal dryness is not a rare complaint of postmenopausal women. But the concerns and treatments about this were negative in patients and doctors. So we treated with Gagam-chungsimyeonjaeum(加減淸心蓮子飮) on three cases complaining vaginal dryness. As a result vaginal dryness was improved in all cases. Through this case report, we expect that more interests and participations in vaginal dryness will be got more qualities in life of postmenopausal women.

인공심장판막의 재치환수술: 7례 보 (Re-replacement of prosthetic heart valves: report of 7 cases)

  • Prosthetic valve replacement has resulted in marked improvement in the functional status of many patients with valvular heart disease. But valve failure from various causes has necessitated re-replacement of prosthetic valve in some of these patients. This selective group of patients has many inherent problems. This report presents our experience with seven patients who received re-replacement of prosthetic valve in the Seoul National University Hospital from 1981, to 1983. The patients included one woman and six men in the range of 9 and 49 years old. As the first valve operations, there were five MVR using tissue valves, one DVR [Bjork-Shiley and Ionescu-Shiley valve] and one case of modified Bentall operation with composite graft [Ionescu-Shiley valve]. Reoperations on prosthetic valve failure were performed 17 to 54 months after the first operation [mean 34 months]. Amon8 seven patients, there were two cases of prosthetic valve endocarditis and five cases of primary tissue failure. Gross calcification of the xenograft was found in two children with Ionescu-Shiley valve. All except one had relatively successful operative results. The unsuccessful one died intraoperatively from low output syndrome.

경동맥 협착을 동반한 관상동맥 질환의 수술적 치료 -2례 보고- (Surgical Management of Coronary Artery Disease Combined with Carotid Artery Stenosis -A Report of Two Cases-)

  • The optimal surgical approach to the patients with coronary artery disease combined with carotid artery stenosis is controversial. We report two cases of successful surgical management of the patients with combined obstructive coronary and carotid artery disease. The first case was a 69-year-old female who had unstable angina pectoris and a past medical history of left carotid endarterectomy. She was revealed to have triple vessel coronary disease and nearly total occlusion of right internal carotid artery. She was undergone staged right carotid endarterectomy 10 days before coronary bypass surgery. The second case, a 54-year-old male with a past medical history of left hemiparesis and dysarthria, was admitted due to unstable angina pectoris. He was revealed to have triple-vessel coronary disease and more than 90% stenosis of left internal carotid artery and 50% stenosis of right internal carotid artery. In the latter case, a combined coronary bypass surgery and left carotid endarterectomy was done. In both cases, postoperative neurologic complications were not observed.

족무지 종자골에 발생한 무혈성 골괴사증(3예 보고) (Avascular Necrosis of the Hallucal Sesamoid (Three Cases Report))

  • Although avascular necrosis of the hallucal sesamoid has not been frequently addressed in the literature, it should be considered in the differential diagnosis of persistent forefoot pain. We experienced 3 cases of avascular necrosis of the hallucal sesamoid with sclerosis of the sesamoid bone on radiograghs and computed tomograghy images. T1 and T2-weighted MRI images in 2 patients showed low signal intensity in the sesamoid bone, which suggested osteonecrosis and confirmed by histology. We report 3 cases of avascular necrosis of sesamoid with a review of the literature.

  • Althogh spinal epidural lipomatosis is relatively rare, it is a widely recognized complication of excess exogenous glucocorticoids. We report 4 cases of epidural lipomatosis. None of the patients had definitive history of steroid treatment, obesity, or diabetes mellitus, but all had sciatica and radiating pain. In all cases, the authors removed excess amount of epidural fat with surgery after which significant improvement of symptoms was noted.
