The Effect of Patient-related and Environment-related Characteristics on the Risk of Falling in Inpatient Care Unit - A Case-Control Study to identify Inpatient Fall Risk Factors (환자의 개별 특성 및 병동의 환경 특성이 환자낙상 위험도에 미치는 영향 - 환자낙상 위험인자 파악을 위한 사례-통제 연구)

  • Purpose: The study aims at identifying patient-related and environmental factors associated with an increased risk of falling and, therefore, both caregivers and designers can be aware of fall risk factors and can contribute to prevent inpatient falls in their own areas of expertise. Methods: A case-control study has been conducted, utilizing patient data and physical environmental data in the unit of General Medicine in the United States. The case-control study investigated data about patients who had suffered falls as well as patients with similar characteristics (e.g., age, gender, and diagnosis) who did not suffer falls. Results: The study identified both patient-related and physical environmental factors associated with inpatient falls. Morse fall risk score, patient visibility, and patient accessibilityB were identified as significant predictors to inpatient falls, when controlling for other significant variables. Implications: The findings of the study can provide implications to both caregivers and healthcare and hospital environment designers. Caregivers should give special attention to patients with high Morse Fall Risk Scores to prevent inpatient falls. Designers also need to examine and to fine-tune the unit layout of inpatient care units to maximize each patient room's patient visibility from the rest of the unit and patient accessibilityB from working areas of nurses.

A contact investigation after exposure to a child with disseminated tuberculosis mimicking inflammatory bowel disease

  • Purpose: Tuberculosis (TB) is one of the most important diseases that cause significant mortality and morbidity in young children. Data on TB transmission from an infected child are limited. Herein, we report a case of disseminated TB in a child and conducted a contact investigation among exposed individuals. Methods: A 4-year-old child without Bacille Calmette-$Gu{\acute{e}}rin$ vaccination was diagnosed as having culture-proven disseminated TB. The child initially presented with symptoms of inflammatory bowel disease, and nosocomial and kindergarten exposures were reported. The exposed individuals to the index case were divided into 3 groups, namely household, nosocomial, or kindergarten contacts. Evaluation was performed following the Korean guidelines for TB. Kindergarten contacts were further divided into close or casual contacts. Chest radiography and tuberculin skin test or interferon-gamma-releasing assay were performed for the contacts. Results: We examined 327 individuals (3 household, 10 nosocomial, and 314 kindergarten contacts), of whom 18 (5.5%), the brother of the index patient, and 17 kindergarten children were diagnosed as having latent TB infection (LTBI). LTBI diagnosis was more frequent in the children who had close kindergarten contact with the index case (17.1% vs. 4.4%, P=0.007). None of the cases had active TB. Conclusion: This is the first reported case of TB transmission among young children from a pediatric patient with disseminated TB in Korea. TB should be emphasized as a possible cause of chronic diarrhea and failure to thrive in children. A national TB control policy has been actively applied to identify Korean children with LTBI.

  • In this paper, a new method of the optimal control of the regulator and tracking control problem concerning the dimension of the velocity is derived and applied for the second order plant. The output position, output velocity and the control effort are plottcd with time applying analog computer simulation. And it is compared with the output of the ordinary optimal control law in case the reference input is given by velocity function, and studied about the gain of this method.

  • Using the p chart is not adequate in case that there are lots of data and it is difficult to divide into products conforming or nonconforming because of obscurity of binary classification. So we need to design a new control chart which represents obscure situation efficiently. This study deals with the method to performing arithmetic operation representing fuzzy data into fuzzy set by applying fuzzy set theory and designs a new control chart taking account of a concept of classification on the term set and membership function associated with term set.

  • This work deals with fast-to-compute global control laws for time-optimal motion of strongly nonlinear dynamic systems like resolute robots. the theory of cell-to-cell mappings for dynamical systems offer the possibility of doing the vast majority of the control law computation offline in case of time optimization with constrained inputs. These cells result from a coarse discretization of likely swaths of state space into a set of nonuniform, contiguous volumes of relatively simple shapes. Once the cells have been designed, the bang-bang schedules for the inputs are determined for all likely starting cells and terminating cells. the resulting control law is an open-loop optimal control law with feedback monitoring and correction.

  • Digital control laws are implemented on an active magnetic bearing system with DSP. The results of tests using a experimental apparatus are (1) in a case that conventional PID, PIDD2 controls are employed, implemention of digital control law has similar characteristics to that of analogue control law. (2)The experiments reveal the results that the dynamic compensation based on the observer may be better than that of the other conventional controllers.

  • Generally, for nonlinear control of robotic system, the method of computed torque or inverse dynamics is frequently used. In this case, exact knowledge of the system parameters is required, however. This paper addresses the problem of nonlinear control when the parameter of system is varied. The approach is based upon decoupled model following. As an example, control of a three degree of freedom manipulator arm under mass variation is simulated.

Parameter Estimation for Digital Current Control of PWM Converters

  • From the viewpoint of model-based current control, it is indispensable to use the accurate system parameters for the high control performance. This paper adopts the Least-Squares algorithm as a parameter estimation scheme because it has the fast convergence rate and the low sensitivity to noises. In case of the intelligent current controller with delay compensator, the simulation results show that the adopted estimation scheme can be successfully applied to PWM converters and also show the improved control performance in the estimated parameters.

  • Motion control system using motor generally consists of 2 inertio system, motor inertio and load inertio linked with flexible coupling. In the case of direct drive motor control, the effect of coupling inertio decrease the precision performance. The study is about semiclosed control scheme of the system and experimental setup. To compensate the effect of coupling inertio, semiclosed control system by disturbance observer is employed. The simulation result is shown.
