본 연구는 아동양육시설 보육사가 경험한 통합보육을 통해 통합보육의 다면성과 성공적 통합보육 수행 방향을 제시하고자 했다. 아동양육시설에서 통합보육을 경험한 10명의 보육사를 연구 참여자로 선정하여 질적 사례연구방법에 따라 전사 자료를 분석했다. 보육사들은 통합보육 수행에 거부감을 나타내고 있었으며, 통합보육 적응과정에서 장애아동과 비장애아동 모두에게 충분한 돌봄을 제공할 수 없어 죄책감을 느끼고 있었다. 그들은 아동에게 필요한 지원을 제때에 받을 수 있도록 행정기관과의 즉각적 소통의 창구 마련, 장애아동 케어와 관련한 보육사 교육지원 및 특수교사 파견 등을 원하고 있으며, 3교대 정책에 대해서는 모두 반대의견으로 나타났다. 또한 아동양육시설에서의 통합보육 다면성에 따른 지원방안으로서 보육사의 처우와 급여 문제, 시설보호아동들의 교육 기회와 양육환경 문제에 대한 사회 전반의 관심을 환기 하는데 있어 향후 연구 방향을 추가로 모색할 수 있을 것으로 기대해본다.
Purpose: The purpose of this study was to identify attributes of the concept of spiritual care. Method: Walker & Avant's concept analysis framework was employed to review the clinical guidelines, nursing text books, and nursing research articles which were related to spiritual care and published from 1985 to 2005. Result: The attributes of the concept of spiritual care were a three stage process such as spiritual assessment, spiritual intervention, and spiritual evaluation. Spiritual care included three dimensions of relationships such as transpersonal, interpersonal, and intrapersonal. The quality of spiritual care was dependent upon characteristics of care-givers such as perception and knowledge of spiritual care, and the clinical environment. The antecedents of spiritual care was spiritual needs due to the prompt events. The consequence of spiritual care was spiritual well-being. Conclusion: This concept analysis of spiritual care contributed to promote performance of spiritual care in clinical fields by removing conceptual ambiguity and confirming the true meaning of spiritual care.
The present study investigated differences in children's school achievement and emotional and social development by type of after-school care: self-care and mother-care. It also examined relationships between children's developmental outcomes and ecological variables, including individual, familial, and environmental variables by care methods. The data of 330 mother-care and 161 self-care children was provided by teachers, mothers, and the children themselves. The analyses of the data were conducted by t-test, correlation, and regression. Findings were that mother-care children had higher scores in grades, school adjustment and teacher relationship and lower depression and withdrawn behaviors than self-care children; after-school activities and peer support predicted the developmental outcomes of mother-care children; gender, autonomous ability, and behavior control predicted the development of self-care children.
The purpose of this study is to ethical review on the development and service with care assistance robot. An integrative review concept analysis method was used. We analyzed the classification and role of service robots, the concept of the robot ethic and the care ethic. And there were derived the development and service about care assistance robot in ethical viewpoint. For improving current care problem, government had support to developing four types care assistance robots. But there were provided carefully care service due to the limitations of robot technology and lack of overall social awareness with care robot. In addition, in order to be successfully application in the field, care assistance robots were developed to provide high-quality care service that can consider to personal culture and living environment with the development of artificial intelligence and robot technology, as well as ethical care service.
Purpose: The purpose of this studγ is to understand relations among perceived stress, stress coping method, self-rare behavior and Hemoglobin $A_1c$ of elderly diabetic mellitus patients. Method: Participants were 128 patients who were under medical treatment in outpatient department at C university hospital. The collected data were analyzed using technical statistics, frequency, percentage, T-test, ANOVA and person's correlation coefficient by SPSS 12.0. Result: There were positive correlation between self-care behavior and coping method based on emotions under the lower category of stress coping method. Hemoglobin $A_1c$ of patients had a negative correlation with physical exorcise of self coping method under the lower category. Perceived stress had also a negative correlation with emotions as coping method in the lower category and medication of self-care behavior. Conclusion: It was noted that elderly diabetic mellitus patients didn't much use a coping method based mainly problems and another method based emotions in case of perceived stress. The findings of this study will be useful for developing an nursing intervention program focused on elderly diabetic mellitus patients.
Purpose: The purpose of this study was to explore the subjective experience of life-sustaining treatment care among nurses in intensive care units. Method: A phenomenology was used for the study. Data were collected from October to December, 2015 using open-ended questions during in-depth interviews. Participants were nurses working in intensive care units and were contacted through purposive techniques. Eight nurses participated in this study. Results: Four categories emerged from the analysis using Colaizzi's method: (a) difficulties due to life-sustaining treatment care, (b) dilemma of extension or cessation of life-sustaining treatment, (c) repressed feelings and emotional exhaustion, and (d) forming values for life-sustaining treatment from nursing experience. Conclusion: Provision of clearer guidelines on life-sustaining treatment which reflect a family-oriented culture is important for nurses in ICU and will promote nurses involvement in the decision-making process of life-sustaining treatment of patients.
Purpose : This study compares experiences of ethical dilemma between nurses working in intensive care units and those in general units under specific situations of treatment decisions and confidentiality. Method : This cross-sectional descriptive study utilizes the self-report survey method. The survey questionnaires were completed by 50 and 52 nurses working in intensive care units and general units, respectively. The instrument, which consisted of 16 items of ethical dilemma situations about treatment decision and confidentiality, was used. The mean scores for each item were compared between the two groups. Results : The study found no differences in terms of age, gender, education level, clinical experience in years, and being educated on healthcare ethics. For 9 out of 16 items, the mean scores of nurses in intensive care units were significantly higher than those of nurses in general units. Conclusions : Nurses in intensive care units experienced ethical dilemmas regarding treatment decisions and confidentiality more often than those in general units. This study emphasizes the need to establish strategies for improving the ethical competence of critical care nurses.
Purpose: At the moment, There is a facility standard for intensive care units on the Korea medical services Act. However, the contents of regulation are so limited and difficult to apply. Therefore, it is necessary to draw up the guideline to complement the regulation for efficient operation of intensive care units. This study aims to suggest basic data for guideline of intensive care units. Methods: 2 Methods have been used in this paper. 1) Comparative analysis between the national regulation of Korea with the design standard of the United States America, the United Kingdom and Australia. 2) A literature survey about operation method for intensive care unit, focused on materials published in intensive care medicine society of each country. Results: In comparison with guideline of abroad cases, additional supplement are required, with regard to item such as operation environment, size, nursing manpower ratio, bed space area, space program of intensive care units for healing environment. Implications: This study is a basic research for guideline of intensive care unit, and need to be followed by further study using various perspectives and methods.
현재 우리나라 요양병원은 노인인구 증가와 함께 지속적으로 증가하고 있다. 요양병원에 입원하는 환자는 주로 만성질환을 가진 노인들로서 장기요양을 목적으로 하기 때문에 요양병원에서 임종하는 경우가 많아 요양병원에서의 임종간호 중요성이 점점 커지고 있다. 이러한 특징에도 불구하고 요양병원 간호사를 대상으로 임종간호에 대한 연구로는 주로 양적연구들로서 임종간호 경험에 대해 심층적으로 파악하기에 제한이 있다. 따라서 본 연구의 목적은 현상학적 방법을 통해 요양병원 간호사의 임종환자 간호경험의 의미가 무엇인지 이해하고 그 현상의 의미를 심층적으로 기술하고 이해하고자 함이다. 본 연구의 대상자는 C 시에 위치한 요양병원에서 6개월 이상 근무하고 있는 간호사 7명을 선정하였다. 연구방법은 심층 인터뷰로 2021년 12월부터 2022년 3월까지 자료를 수집하였다. 인터뷰 자료는 Giorgi의 현상학적 방법으로 분석하였다. 연구 결과 '임종간호에 대한 간호사의 태도', '연명에 대한 양가 감정', '요양병원의 임종간호에 대한 현실', '삶과 죽음에 대한 성찰'으로 나타났다. 따라서 요양병원 간호사의 임종간호 태도를 변화시키고 대처능력을 향상시킬 수 있는 임종간호 보수교육 및 중재 교육프로그램이 개발되어야 할 것으로 사료된다.
Purpose: This study was explored the meaning of hospice volunteer's experience in caring for cancer patients and tried to understand their experiences. Method: The data was collected from 7 participants living in Seoul and Gyeonggi province from Jan. to Apr. 2005. Collection of data was by means of in-depth interviews. The analysis of the data was made the phenomenological analytic method suggested by Colaizzi(1978). Result: The result is consisted of nine theme-cluster; experienced a fear, limitation of activity, experienced social care, experienced physical care, good death, experienced necessity of recharging, experienced bereavement care. Conclusion: The result above indicated that health professionals must develop the management and education of volunteer of hospice care for various hospice care. Also, We should to encouraged the continuous education and efficient management. And support system should be developed.
