Perceptions of Quality Nursing care of Patients and Families (질적 간호에 대한 환자와 가족의 지각)

  • The purpose of this study was to offer the results of content analysis and qualitative study that explored the perceptions about quality nursing care of patients and families as consumers and to identify the implications of this study for quality nursing care management and research. The data was collected from 12 adult patients and 9 families who were admmitted at medical and surgical nursing unit of one university hospital in Seoul from October, 1996 to January, 1997. Research participants were asked to response "what do you think quality nursing care?" and similar questions during the interviews was performed. Data were analyzed using open coding and content analysis with frequencies and percents of attributes of quality nursing care. Attributes of quality nursing care and meaning of quality nursing care that patients and families perceived were explored. 1. The attributes of quality nursing care that patient and families perceived were categorized into 56 attributes. The highest response rate among the attributes was 'one's heart at ease' (76.2%), and the next high response rates were ranked in order 'consideration' , 'care about' (each 61.9% 'expert skill' (57.1%), 'deal with problem promptly' , 'information offer' (42.9%), 'intimate feeling' (38.1%), 'smile' 'service spirit' , 'do one's best' (each 33.3%), 'frequent visit' (23.8%), 'observe the time' (23.8%), 'direct nursing care' , 'speaking warmly' , give a hope' , 'address kindly' , 'a sense of duty' , 'good facilities' (each 19.0%), 'inquire after a patient health' , 'patient-centered nursing care' , 'showing an example' , 'professional knowledge' , 'careless moraly patient' , 'give encourage to patients' , 'good answer a question' (each 14.3%), 'do not imprudently' , 'do not disregard' , 'broad knowledge' , 'emergency treatment skill' , 'dependability' ,'consolation' giving a sense of security' , 'a self sacrificing spirit' , 'a sense of responsibility' 'hard - working', 'enough disposition of nursing staff (each 9.5%), 'improve patient's pride' and the rest attributes exhibited 4.7%, respectively. 2. The attributes that were identified in patients' data only were 8 categories, 'service sprit' (58.3 %) 'expert knowledge' , 'good answer a question' (each 25.0%), 'hard working' (16.7%), 'a warm character', 'professional attainments', 'do without reserve', 'satisfaction' (each 8.3%), 3. The attributes were identified to families' data only were 31 categories, 'speaking warmly' , 'direct nursing care', 'adress kindly', 'patientcentered nursing care', 'showing an example' (each 33.3%). 'do not imprudently' , 'do not disregard' , 'consolation', 'giving a sense of security', 'broad knowledge' , 'emergency treatment skill', 'dependability' ,'a self - sacrificing spirit', 'a sense of responsibility' (each 22.2%), 'improve patient's pride' , 'without discrimination' , 'show kindness' , 'individual nursing care', 'being with patient' , 'helping' , 'accuracy' , 'without any mistake' , 'love' , 'self - confidence', 'self possession', 'a self - denying spirit' , 'a sense of duty' , 'tighten discipline' , 'disposed room with similar patient to diagnosis', 'compensatory relationship between me dical team' , 'role of connection' (each 11.1 %). 4. The attributes of quality nursing care were integrated into 11 categories that they were 'patientcentered nursing care' (25.1%), 'expertise' (22.1%), 'caring'(18.1%), 'kindness'(11.1%L 'nurse attainments(10.1%), 'sincerity' (7.5%), 'good environment' (2.0%), 'effective organizational management', 'coordination', 'enough nursing staff' ( each 1.0%), 'satisfaction' (0.5%) were showed in the order of the highest rate. 5. The concept of quality nursing care were defined as 'give a satisfaction to patients by patientcentered care based on professional skill and caring with kindness and sincerity'. The description of the meaning of quality nursing care provided by this research participants, patients and families can provide important information for quality nursing care management, medical marketing, education and researches of this field. On the basis of the above findings the following recommendations are made: to suggest to utilize this results for patient care in practice setting, development of quality assessment tool in nursing care, repeat study by the same subjects and method, and to a comparative study by the same method to nurse.

  • Objective : The objective of this study was understanding the procedure of self-care training in occupational therapy and exploring experienced occupational therapists' perspectives in self-care training Method : A phenomenological study design was used to collect information regarding use of self-care training among occupational therapists working in a different settings. The data collection process was conducted by using a structured interview and survey. Result : The self-care training strategies used by occupational therapists were summarized to four main themes; 1. Different strategies for different age group, 2. Design therapeutic strategy rely on client's natural environment, 3. Use of theoretical background: occupation-based or client-centered, or both. 4. Use of self-awareness stragety; using different method to reflect self-awareness. Conclusion : The self-care training is not just simply conduct ADL training but understanding client's age, environment, theoretical background, and self-awareness of the client. Through eatablishing understandable self-care training strategy according to client's age and environment, the more effective self-care training would be possible.

  • Purpose: This study was conducted to develop and evaluate an oral care protocol for cancer patients receiving chemotherapy. Methods: The participants in this study were cancer patients in Severance hospital in Seoul. A total of 31 patients were assigned to the experimental group, and 29 patients to the control group. Data were collected from August 1 to October 5, 2010. The protocol included definition and symptom of oral mucositis, self-check method of oral status, prophylactic method of oral mucositis (oral care, eating habits, and gargling) and management of oral mucositis. Oral Assessment Guide (OAG) was used as the measurement tool of oral mucositis in this study, Oral Care Performance Scale was used as the measurement tool of oral care performance status. Data were analyzed with a $x^2-test$ and t-test, and repeated measures ANOVA, using SPSS/WIN 18.0 program. Results: Patients in the experimental group receiving the oral care protocol had a significant difference (t=-2.938, p=.005) in the oral care performance compared to the control group. However, there was no significant difference (F=1.255, p=.274) in the oral mucositis status between two groups. Conclusion: This study showed that the oral care protocol could improve oral care performance status for the patients under chemotherapy.

Estimating the Willingness-To-Accept for Cash Benefit of Long-Term Care Insurance (노인장기요양보험제도의 현금급여 도입 필요성 - WTA를 통한 적정 현금급여액 추정 -)

  • Korea government has launched long-term care insurance from 2008. However, one of the most important issues, whether or not providing cash benefit, is still unresolved. In this paper, in order to provide policy guidelines for the long-term care insurance, I attempt to estimate the Willingness-To-Accept (WTA) of the cash subsidy for informal care by using Double Bounded Dichotomous Choice method, a branch of Contingent Valuation Method (CVM). In doing so, I also estimated the determinants of the preference for cash benefit. Data were obtained from face-to-face survey interviews with 300 informal care-givers at three major general hospitals in Seoul, Korea. The questionnaire was constructed with two scenarios (mild/severe symptom). The results from logistic regression analyses and the estimation of WTA indicate that informal care-givers are willing to accept the cash benefit as low as 628 thousands won for mild fragile elderly and 1,072 thousands won for severe fragile elderly. The strength of this paper is that I estimated the WTA of the cash benefit by reflecting the changes in preferences of informal care-givers. The analytic results from the this paper suggest that the cash benefit in long-term care insurance is indispensible in achieving the goal of the long-term care system.

A Survey on Pain and Self-Care Behavior of Patients with Chronic Arthritis (만성 관절염 환자의 통증과 자기간호행위 관련요인)

  • Purpose: This study was designed to explore pain and self-care behaviors and identify related variables in patients with chronic arthritis. Method: One hundred fifty patients with arthritis were recruited from two university based arthritis centers according to selection criteria. Collected data were analyzed using the SAS program to analyze the responses to the structured questionnaires of the study. Result: Most of the participants expressed pain and the intensity of the pain was moderate. There were significant differences according to age, educational level, diagnosis, duration of illness, number of affected joint, and use of complementary therapy. Self-care behavior scores were moderately high. The highest practice was for 'regular visits to the hospital', and the lowest for 'applying physical therapy at home'. The mean self-care behavior scores showed significant differences according to economic status and educational level. Pain scores showed no correlation with self-care behavior. Conclusion: Developing self-management programs for patients with chronic arthritis should focus on self-care skills which are applicable in the relief of pain and enhancement of knowledge. The skills are recommended not only for better health practices but also for enhancing the level of well-being and life satisfaction.

Development and Evaluation of a Self Care Education Program for Elderly Patients with Percutaneous Coronary Intervention (경피적 관상동맥 중재술을 받은 노인환자의 자가간호 교육프로그램 개발 및 평가)

  • Purpose: The purpose of this study was to identify the effects of the education program to promote self care for elderly patients with percutaneous coronary intervention. Method: The research was a quasi-experimental study using a nonequivalent control group non-synchronized design. The participants was 56 patients, 28 in the experimental group and 28 in the control group. A pretest and a posttest were conducted to measure main variables. The self care education program, consisting of flash animation, leaflet and telephone counseling, was given for the experimental group. Data were analyzed using ${\chi}^2$, Fisher's exact test, and independent t-test with SAS statistics program. Results: There were statistically significant increase in knowledge and self care behavior, and decrease in anxiety by difference between a pretest and a posttest in the experimental group compared to that in the control group. Conclusion: The result of this study indicate that the self care education program is effective in increasing knowledge and self care behavior, and in decreasing anxiety for elderly patients with percutaneous coronary intervention. Therefore, it can be usefully utilized in the field of nursing for elderly patients with percutaneous coronary intervention.

Patient Satisfaction with Nursing Care in the Center for Emergency Medicine (응급 간호 서비스에 대한 환자만족도)

  • Purpose: To examine patient satisfaction with nursing care and the difference of patient satisfaction by the characteristics of emergency care service in the center for emergency medicine. Method: We surveyed 145 patients who visited Pusan-Wide Emergency Medical Center from Nov 01, 2003 to Nov 30, 2003. Patient satisfaction was assessed by asking patients and medical records reviewed to obtain the characteristics of emergency care service. Results: The mean score of patient satisfaction with nursing care was 3.12. For patient satisfaction, 'The nurse seemed earnestly concerned about my pain, fear, and anxiety' represented highest score (3.58). Patient satisfaction according to the general characteristics of patients was revealed as significantly high for the higher educated and the older. Patient satisfaction by the characteristics of emergency care service was significantly higher in no-wait group. Conclusion: Also, the fundamental of nursing, like respect for human, was maintained in emergency setting, the hospital's administration has implemented service for a high level of satisfaction with nursing care up to the expectations of the patients for with the staff and medical equipment.

  • Purpose: The purpose of this study was to identify manpower acquisition strategies of nurse and nurse aide in long-term care Method: The study was proceed using following three stages: to explore workforce status and labor environment with database analysis, literature reviews, to explore the best practices, practical experiences with expert interviews, with focus group interview, to identify problems and present acquisition strategies Results: Acquisition strategies were increasing of pay, improvement of working condition, development of practical educational program, diversification of entry paths, establishing clear roles for nursing staffs, diversification of training program. Conclusion: We found that the acquisition strategies of nurse and nurse aide can be as an useful methods to increase manpower in long-term care for higher quality of services and care.

A Study of Care Burden, Burnout, and Quality of Life among Family Caregivers for the Elderly (노인 주 부양자의 부양부담감, 소진감 및 삶의 질에 대한 연구)

  • Purpose: This study was a survey to provide basic data about nursing interventions for improving the quality of life among family caregivers for the elderly by examining their care burden, burnout, and quality of life, and by confirming the correlation between each of them. Method: The subjects were 215 people in Seoul and Gyeonggi Province who understood the purpose of this study and participated voluntarily from April 1 to June 4, 2007. Data was analyzed by the SAS program. Result: 1. Concerning primary caregivers of the elderly, it was found that their care burden was slightly high, burnout was high and quality of life was good on the whole. 2. When it comes to the correlation among care burden, burnout, and quality of life among family caregivers, it was found that burnout increases in proportion to care burden, quality of life decreases as care burden increases, and bigger burnout leads to a lower quality of life. Conclusion: The quality of life among primary caregivers should be improved by reducing their care burden and burnout. Thereby, a priority might be considered for admission to nearby, comfortable nursing homes or failing that, home visiting services, rather than family support in the home.

Fairness of Health care financing: Progressivity and Retstributive Effect (가구 소득과 보건의료비 지출의 형평성 : 누진성과 소득재분배 효과)

  • The present study attempts to examine the progressivity of health care financial sources based on the income approach, for which it decomposes redistributive effects into vertical, horizontal, and re-ranking components. The study data include Korean Household Expenditure Survey (2000) conducted every 5 year by Korea National Statistical Office. The data were sampled from the national population by the multistage probabilistic sampling method, and amounts to 23,270 households. For the better application of the income approach, the study employs household total expenditure in Korea instead of total income, because the former data source is more reliable and less fluctuated over time. Progressivity of health care financing was measured by Kakwani index. Aronson's decomposition equation was used in case of the analysis where differential treatment of health care expenditure needs to be considered. Despite the progressivity of Korea's governmental contributions, total expenditure of health care showed regressive pattern, which may largely be attributable to the higher regressivity in out-of-pocket money. With the result of negative Kakwani index, differential treatment increased income redistribution biased for better-off. It is worth to note that social insurance displays not only negative Kakwani index, but also horizontal inequality, suggesting that the first step of health care financing reform should be the revision of social insurance premium rates toward effective and equable way.
