제주 지역 병원 간호사의 직장 폭력 경험 실태 (The Experiences of Workplace Violence toward Nurses in Hospitals in Jeju Province, South Korea)

  • Purpose: This study was to investigate the prevalence & types of workplace violence toward nurses in hospitals, and to understand nurse's coping response, cause of violence and prevention strategy. Methods: The data were collected from 254 nurses working in 9 hospitals in Jeju Province by the self-report from June to August 2010. Results: The respondents experienced unpleasant or insulting words (89.8%), verbal threat (38.2%), physical threat (67.7%), physical injury (32.7%), severe physical injury (2.8%), and sexual harassment (26.4%) during the last one year. The frequent offenders were patients, patients' family and physicians in order. The causes of violence which nurses perceived were personality of offenders (76.4%), lack of assessment of aggressive patients or care givers (42.1%), and lack of explanation to patients or caregivers (33.5%). They reported that coping strategies for workplace violence were 'established reporting system (63.4%)', 'building a cooperative circumstances within team members (58.3%)', and 'formulation of hospital policies for violence prevention and coping (54.3%)'. Conclusion: These findings showed nurses are at considerable risk for workplace violence, and they experience various types of violence from patients, caregivers, and physicians. We suggest that hospitals formulate appropriate policies, guidelines and programs to prevent and cope with workplace violence in hospitals.

The Effects of Pitch Increasing Training (PIT) on Voice and Speech of a Patient with Parkinson's Disease: A Pilot Study

  • The primary goal of therapeutic intervention in dysarthric speakers is to increase the speech intelligibility. Decision of critical features to increase the intelligibility is very important in speech therapy. The purpose of this study is to know the effects of pitch increasing training (PIT) on speech of a subject with Parkinson's disease (PD). The PIT program is focused on increasing pitch while a vowel is sustained with the same loudness. The loudness level is somewhat higher than that of the habitual loudness. A 67-year-old female with PD participated in the study. Speech therapy was conducted for 4 sessions (200 minutes) for one week. Before and after the treatment, acoustic, perceptual and speech naturalness evaluation was peformed for data analysis. Speech and voice satisfaction index (SVSI) was obtained after the treatment. Results showed Improvements in voice quality and speech naturalness. In addition, the patient's satisfaction ratings (SVSI) indicated a positive relationship between improved speech production and their (the patient and care-givers) satisfaction.

The Influence of Culture on the Experiences of Korean, Korean American, and Caucasian-American Family Caregivers of Frail Older Adults: A Literature Review

  • Purpose. The purpose of this review is to explore cultural influences on the experiences of Korean, Korean American, and Caucasian American family caregivers caring for frail older adults in terms of the selection of a primary caregiver, caregiving motivation, support/help-seeking, and negative emotional responses (depression and burden). Methods. Seven electronic databases were searched to retrieve studies from 1966 to 2005. Thirty-two studies were identified. Results. This review supported cultural influences on the selection of primary caregiver, caregiving motivation, and support/help-seeking among the three caregiver groups. In Korean caregivers, the major primary caregivers were daughters-in-law while among Korean American and Caucasian American caregivers, the major primary caregivers were daughters or spouses. As a major caregiving motivation, Caucasian American care¬givers reported filial affection while Korean caregivers and Korean American caregivers reported filial obligation. Korean caregivers reported higher extended family support, while Caucasian American caregivers reported higher utilization of formal support. Korean caregivers showed the highest levels of depression followed by Korean American caregivers and Caucasian American caregivers. Conclusion. In order to develop culturally appropriate interventions and policies, more research is needed to further explain these differences among the three groups, especially regarding support/help-seeking and negative emotional responses.

  • 삶의 질을 높이기 위하여 무선 애드혹망 기술은 컴퓨터과학과 건강관리 응용산업에서 가장 중요한 분야의 하나로 알려져 있다. 유비쿼터스 건강시스템은 또한 실시간으로 비상상태의 경고기능을 제공해준다. 이러한 기능은 봉사자들의 수를 줄여줄 뿐 아니라 만성적인 병자와 노인들을 살아갈 수 있도록 돕는다. 시스템의 응용을 위하여 효과적이며 적절한 네트워크 시스템은 필수적이다. 따라서, 본 논문에서는 환자 노드가 지속적으로 뇌파를 측정하여 병원에 위치한 서버로 전송하는 다양한 네트워크 환경을 제안한다. 그리고, 이동노드들의 다양한 이동성과 토폴로지로 구성된 네트워크 시스템들을 옵넷 시뮬레이터로 시뮬레이션을 하고 평가할 것이다.

광주광역시 광산구 보육시설 유아의 식행동과 식품알레르기 발생 현황 (Dietary Behaviors of Preschool Children and Food Allergy Prevalence of Preschool Childcare Facilities in Gwangsan-gu, Gwangju Metropolitan City)

  • This study aimed to evaluate dietary behaviors and food allergy status of preschool children in Gwangsan-gu, Gwangju Metropolitan city. The survey included 592 preschool children aged 1-5 years old in 31 childcare facilities. General characteristics, feeding type, complementary feeding, food allergy, dietary behaviors based on the NQ-P questionnaire, and dietary habits of their family were considered. Exclusively breast-fed children was 32.2% of the subjects and the breast feeding duration was 6.5 months. Subjects who had complementary feeding within 6 months was 77.2% and starting time of complementary feeding was 6.1 months. Fifteen percent of the subjects had food allergies and foods that induced allergy were instant foods, eggs, milk and dairy products, nuts, seafood. Food allergy was not related to breast feeding nor complementary feeding. The NQ-P score and its 3 factors including 'balance', 'moderation', and 'environment' were 59.9, 61.1, 56.0, 62.6, respectively. There were positive relations between children's dietary behaviors and family dietary habits such as breakfast eating frequency and meal regularity. As age of children increased, instant food intake increased and breakfast eating frequency decreased. Proper nutrition education is needed to children, their parents, and their care givers at childcare facilities to improve children's dietary behavior and health.

와상환자복 하의 디자인 개발 (A Design Development of Hospitalized Patients' Pants for Bed-ridden Patients)

  • This study aims to develop the functional pants for the hospitalized bed-ridden patient. The procedure of this study consisted of three parts. First, the interview survey of nursing care givers was conducted to inquire into the conditions of bed-ridden patients' clothing. Second, the bed-ridden patients' pants design and sample making were accompanied. Then, the wearing tests and design development were completed. The results are as follows; The conditions of bed-ridden patients' clothing were the convenience of clothing change, the partial opening for diaper change and medical treatment and the ventilation for bedsore prevention. The design development of the bed ridden patients' pants was accomplished which had the side seams with two way opening zippers, the wraparound pattern of abdomen and the opening under crotch applied the advantage of korean traditional underwear sokkot. The samples' wearing tests were performed three times and those subjects were the hospitalized bed-ridden patients. Consequently, we suggested the appropriate hospitalized patients' pants for bed-ridden patient.

아토피피부염의 한의학 진료 지침 개발을 위한 사전 연구 (A study on the Development of Traditional Korean Medicine Clinical Practice Guideline for Atopic Dermatitis)

  • Background : The atopic dermatitis patient and their families are looking for safer herb-medicine treatments that possess therapeutic effects, but without the recurrence of symptoms and long-term harmful consequences that can result from other treatment. However, for many reasons, including methodological difficulties and lack of high quality study like randomized clinical study and systematic review, there are no consistent clinical guide line for atopic dermatitis in Traditional Korean Medicine. Objectives : Evidence-based clinical practice guidelines support clinical decision-making by making recommendations to guide clinical practice. The purpose of this study was to develop Traditional Korean Medicine clinical practice guideline for atopic dermatitis Result : The future guide should be based on scientific evidence and include the followings: (1) diagnosis of atopic dermatitis (2) the pattern identification of atopic dermatitis (3) classification of Sasang Constitution (4) efficacy assessment (5) treatment guideline (6) education for patients and care givers Conclusion : The development of Traditional Korean Medicine clinical practice guideline for atopic dermatitis is needed.

일 대학 병원의 가정간호시범사업 서비스 내용 및 만족도에 대한 조사연구 (A study on Hospital based Home Health Care Service and the Level of Client Satisfaction)

  • The purpose of this study was to assess the provided home health care services and to evaluate the patient's satisfaction level of received home health care services. Well trained two home health care nurses interviewed with 138 respondents who received home health care by Keimyung University Hospital from January 1st to August 31st 1999. The results were summarized as follows : 1) Among 138 respondents, $55.8\%$ were mail and $44.2\%$ were female and $70.3\%$ of them were over sixty years old. Respondents main family care givers were spouse$(53.6\%)$, daughters and sons$(36.2\%)$ and parents$(7.2\%)$. 2) $60.2\%$ of cancer patients received home health care services, $23.3\%$ of cerebral­cardiovascular patients, $7.5\%$ of endocrine disorder patients, $2.3\%$ of those who have indwelling foley catheter patients, $1.5\%$ of those who have respiratory problems and others$(5.2\%)$. 3) $88.1\%$ of respondents were satisfied with the number of home visits they received. $50.5\%$ of respondents' were received 1 to 3 times of home visits by home health care nurse per month. $48.6\%$ of respondents answered they were introduced by attending doctors or nurses to home health care services. $55.8\%$ of respondents answered registration to home health care services was simple and easy. $97.4\%$ of respondents answered home health care payment system was adequate. $64.9\%$ of respondents answered the cost of home health care per visit was adequate and comfortable. 4) Health education, counselling, physical assessment was provided to most of the patients. Those who suffered with cerebral-cardiovascular disease was needed hands on direct care most of all. The least home health care service provided was medication. 5) The satisfaction measurement tool was composed with 13 items and 3 score scale. The mean score of satisfaction on provided home health care services was 2.67 out of 3. Among 13 items. 'home health care service was kind enough' was highest(2.84). 'nurse use precise word to understand and communicate'. 'nurse gave home visiting notice ahead of time and kept the home visiting promise on time' was 2.83. 'whenever I need home health care nurse I can give a call and meet the nurse' was lowest 2.41. Special Home Health care programs such as comprehensive hospice care programs for elders over sixty years old should be organized. Adequate and standardized home health care payment system should be developed as soon as possible. In korean family situation. when family members are getting sick and stay at home. family members were taking care of the patients. special program such as counselling family members are needed.

소아암 환아 어머니의 희망, 자기효능감이 외상 후 성장에 미치는 영향 (Effects of Hope and Self-Efficacy on Posttraumatic Growth in Mothers of Children with Cancers)

  • Purpose: The aim of this study was to identify the effects of hope and Self-efficacy on posttraumatic growth in mothers of children with cancers. Methods: A descriptive research design was used and 102 mothers participated in the study. They were primary care givers of children with cancers who were being treated at a tertiary care hospital in Seoul. A structured questionnaires which measured the degree of hope, self-efficacy, and posttraumatic growth was used for data collection. Data analysis was conducted using descriptive statistics, independent t-test, one-way analysis of variance, Pearson correlation coefficient, and multiple regression. Results: There were positive correlations between posttraumatic growth and hope (r=.44, p<.001) as well as self-efficacy (r=.33, p=.003). The major predictors of posttraumatic growth were religion (${\beta}=0.29$, p=.001), only child or first child (${\beta}=-0.25$, p=.015), the number of children in the family (${\beta}=0.25$, p=.016), and hope (${\beta}=0.38$, p=.001). This model explained about 30.4% of the total variables found in posttraumatic growth (F=9.84, p<.001). Conclusion: The findings from this study show that posttraumatic growth in mothers of children with cancers is largely predicted by hope indicating a need to develop nursing intervention programs to enhance hope in these mothers.

Low doses of amitriptyline, pregabalin, and gabapentin are preferred for management of neuropathic pain in India: is there a need for revisiting dosing recommendations?

  • Background: Current therapy for the treatment of neuropathic pain is often unsatisfactory. Considerable variation in treatment pattern still exists in spite of availability of sufficient literature from various guidelines. Recent Indian market data suggested that the utilization (sale) of drugs such as amitriptyline, pregabalin, and gabapentin was more for low-dose unit packs than that of the high-dose unit packs, raising the belief that these drugs are prescribed at a lower dose than is actually recommended in the guidelines. To test this hypothesis, a survey was conducted across speciality throughout the country to observe the prescription pattern of these drugs amongst the health care providers in India. Methods: Three hundred fifty survey forms were distributed of which 281 forms were included for analysis. Results: It was observed that the commonly used initiation and maintenance dose for amitriptyline, pregabalin, and gabapentin was 5-10 mg/day, 50-75 mg/day, and 100-300 mg/day, respectively. The reason to select the lower dosages was to have a balancing effect to achieve good efficacy with minimum side effects. Care-givers reported no side effects/not many side effects as a reason in 22.2%, 16.88%, and 23.86% patients with amitriptyline, pregabalin, and gabapentin, respectively. Sedation and giddiness were commonly reported with all three drugs. Conclusions: Commonly prescribed drugs for management of neuropathic pain, such as amitriptyline, pregabalin, and gabapentin are preferred at lower doses in Indian clinical settings. Acceptable efficacy and low tolerance to the standard dosage is believed to be the reason behind the prescribed dose.
