The aim of this review was to understand the effects of ${\beta}$-adrenergic stimulation on oxidative stress, structural remodeling, and functional alterations in the heart and cerebral artery. Diverse stimuli activate the sympathetic nervous system, leading to increased levels of catecholamines. Long-term overstimulation of the ${\beta}$-adrenergic receptor (${\beta}AR$) in response to catecholamines causes cardiovascular diseases, including cardiac hypertrophy, stroke, coronary artery disease, and heartfailure. Although catecholamines have identical sites of action in the heart and cerebral artery, the structural and functional modifications differentially activate intracellular signaling cascades. ${\beta}AR$-stimulation can increase oxidative stress in the heart and cerebral artery, but has also been shown to induce different cytoskeletal and functional modifications by modulating various components of the ${\beta}AR$ signal transduction pathways. Stimulation of ${\beta}AR$ leads to cardiac dysfunction due to an overload of intracellular $Ca^{2+}$ in cardiomyocytes. However, this stimulation induces vascular dysfunction through disruption of actin cytoskeleton in vascular smooth muscle cells. Many studies have shown that excessive concentrations of catecholamines during stressful conditions can produce coronary spasms or arrhythmias by inducing $Ca^{2+}$-handling abnormalities and impairing energy production in mitochondria, In this article, we highlight the different fates caused by excessive oxidative stress and disruptions in the cytoskeletal proteome network in the heart and the cerebral artery in responsed to prolonged ${\beta}AR$-stimulation.
Between March, 1988 and July, 1992, 114 patients underwent Open Heart Surgery under hypothermic cardiopulmonary bypass. There were 29 cases of congenital heart anomalies (25%), and 85 cases of acquired heart diseases (75%) consisting of 53 cases of valvular heart disease, 31 cases of ischemic heart disease, and a case of left atrial myxoma. The age distribution of 114 cases was 4 to 73 years, and mean age was 43 years old consisting of mean age of congenital heart disease 23, valvular heart disease 47, and ischemic heart disease 57 years old. Overall operative mortality was about 7.9%.
Although left Left ventricular hypertrophy (LVH) is not only an adaptive response of the heart to increased cardiac workload in hypertension, it surelybut also is the most potent risk factor of overt cardiovascular complications such as coronary heart disease, heart failure, arrhythmia and stroke in the hypertensive population. Also it has become generally accepted that subclinical cardiovascular disease begins in childhood and LVH is the most readily assessed marker for that. As LVH can be seen in children and adolescents with even mild blood pressure elevation with the reported prevalence of 10 to 47%, aggressive antihypertensive treatment is critical in preventing the development of hypertensive heart disease in that those cases.
Objectives: Sahyangsohapwon (SS) is a herbal medication that has been widely used with circulatory and neural diseases. This study was conducted to assess the effect of SS on the autonomic nervous system by using heart rate variability measurement. Methods: The eligible subjects were enrolled from the healthy male group of ages 20 to 35 years. They were divided into two groups, the SS group (n=26) and the control (n=24). We monitored the ECG of subjects from the time period 14:00 to 18:00. In the SS group, subjects were administered with a dose of SS at the time 15:00, whereas the control group had none. For each hour HRV measurement was monitored every 15 minutes for 512 seconds from the time period 14:00 to 18:00. The mean value, which was calculated using the 4 values during each hour (i.e. 14:00, 14:15, 14:30, 14:45), was used as the representative value for each individual hour. For the measurement values, RR-interval and SDNN (standard deviation of the NN intervals) were used as time domain analysis, and HF (high frequency), LF (low frequency), and LF/HF ratio were used as frequency domain analysis. Results: The LF value showed an increase after one hour of SS administration and showed gradual diminution for each and every hour. The repeated measures of ANOVA for the comparison of the LF value between the SS group and the control group showed significant differences. While, RR interval, SDNN, HF, and LF/HF ratio values showed no significant differences between the two groups. Conclusions: We suggest that the SS might be useful for stabilizing autonomic nervous system by inhibiting sympathetic nerve activation in healthy people.

Background: Overweight, obesity, and cardiovascular disease risk factors are prevalent among firefighters in some developed countries. It is unclear whether physical activity and cardiopulmonary fitness reduce cardiovascular disease risk and the cardiovascular workload at work in firefighters. The present study investigated the relationship between leisure-time physical activity, cardiopulmonary fitness, cardiovascular disease risk factors, and cardiovascular workload at work in firefighters in Hong Kong. Methods: Male firefighters (n = 387) were randomly selected from serving firefighters in Hong Kong (n = 5,370) for the assessment of cardiovascular disease risk factors (obesity, hypertension, diabetes mellitus, dyslipidemia, smoking, known cardiovascular diseases). One-third (Target Group) were randomly selected for the assessment of off-duty leisure-time physical activity using the short version of the International Physical Activity Questionnaire. Maximal oxygen uptake was assessed, as well as cardiovascular workload using heart rate monitoring for each firefighter for four "normal" 24-hour working shifts and during real-situation simulated scenarios. Results: Overall, 33.9% of the firefighters had at least two cardiovascular disease risk factors. In the Target Group, firefighters who had higher leisure-time physical activity had a lower resting heart rate and a lower average working heart rate, and spent a smaller proportion of time working at a moderate-intensity cardiovascular workload. Firefighters who had moderate aerobic fitness and high leisure-time physical activity had a lower peak working heart rate during the mountain rescue scenario compared with firefighters who had low leisure-time physical activities. Conclusion: Leisure-time physical activity conferred significant benefits during job tasks of moderate cardiovascular workload in firefighters in Hong Kong.
From May 1977 to June 1986, 500 cases of open heart surgery were performed under the cardiopulmonary bypass. There were 278 male and 222 female patients ranging from 6 months to 69 years. 363 cases[73.6%] were congenital heart diseases, and 137 cases[27.4%] were acquired heart diseases, which were 33 valvular diseases, 3 myxomas and 1 IVC obstruction. There were 363 congenital heart anomaly with 35 operative deaths[9.6%], consisting of 279 acyanotic cases with 12 deaths [4.3%] and 85 cases of cyanotic cases with 23 deaths[28.4%]. In 133 patients of acquired valvular disease, 124 valves were implanted and operative death of valvular disease was 12%. Finally the operative mortality was 9.6% in congenital anomaly, and 12.4% in acquired heart disease, overall mortality rate was 10.4%.

Cocos nucifera (C. nucifera) oil is indigenously used to treat cardiovascular diseases. However, coconut husk fibre (which is rich in polyphenols) has not been screened for this property. Based on the ethnomedicinal use of polyphenols in treating cardiovascular diseases, this study was carried out to evaluate the effects of polyphenols of C. nucifera husk fibre on selected cardiovascular disease indices in mice. Fifty adult male Swiss albino mice were assigned randomly into five groups (A-E). Mice in groups B, C, D and E were administered 31.25, 62.5, 125, and 250 mg/kg body weight polyphenols of ethyl acetate extract of C. nucifera husk fibre respectively while the control group (A) mice received 5% DMSO for seven days. The mice were sacrificed twenty four hours after the last administration of polyphenols. Heart and plasma lactate dehydrogenase (LDH), alanine aminotransferase (ALT), aspartate aminotransferase (AST) and alkaline phosphatase (ALP) activities and plasma lipid profile were determined. Results revealed significant reduction (*p< 0.05) in plasma levels of total cholesterol and LDL-cholesterol with no significant change (*p> 0.05) in HDL-cholesterol, triglyceride and VLDL levels in the plasma at all doses of polyphenols administered compared to controls. There was significant reduction (*p< 0.05) in the activities of heart AST and LDH while plasma ALT, AST, and ALP activities were not significantly altered (*p> 0.05) at all doses of polyphenols administered compared to controls. These results suggest that the polyphenols of C. nucifera husk fibre possess cardio-protective properties and also indicate their possible use in the treatment of cardiovascular diseases.
Fifty cases of open heart surgery were done in the Department of Thoracic and Cardiovascular Surgery, Busan National University Hospital during 16 months from July, 1981 to October, 1982. The clinical data were analyzed and summerized as follows. 1. There were 34 cases (68%) of congenital anomalies and 16 cases (32%) of acquired heart diseases. Among the congenital cases, 27 were acyanotic and 7 were cyanotic. All of the acquired heat diseases, 16 cases were valvular diseases and they had valvular replacement surgery. 2. The age distribution of the congenital anomalies ranged from 6 to 27 years with mean age of 14.2 years, and the acquired heart diseases from 18 to 44 years mean age of 27.5 years. The difference of sex distribution was no significance. 3. The clinical minifestations in acyanotic congenital anomalies were exertional dyspnea (81.5%), recurrent respiratory infection (55.6%) and palpitation (22.2%), and in cyanotic congenital anomalies were exertional dyspnea (100%), syncope(57.1%) and growth retardation(57.1%), and in acquired heart diseases were dyspnea(100%), edema (62.5%) and general weakness (62.5%) 4. During the cardiopulmonary bypass, mild to moderate core cooling was performed and added topical cooling for more accurate myocardial preservation. 5. Two kinds of cardioplegic solution used in our institute were Bretschneider solution for the first 7 cases and mixed Harmann's solution 1 L with glucose 5gm, potassium chloride 26 mEq and sodium bicarbonate 24 mEq, making 376 mosmol/L and pH 8.3 at $25^{\circ}C$, for the rest 43 cases. 6. Various kinds of postoperative complications occurred in 14 cases (28%) and showed overall mortality 12%. The mortality along with each disease was 7.4% in congenital acyanotic cases, 42.9% in congenital cyanotic cases and 6.3% in acquired valvular diseases. 7. Pre-and postoperative diagnostic incompatibility was seen in 6 cases (12%). 8. The artificial valves used in the replacement surgery were lonescu-Shiley bovine xenograft in 6 cases and Carpentier-Edwards porcine xenograft in 10 cases.
Seventy cases of open heart surgery were performed in the department of Thoracic and Cardiovascular Surgery, Pusan Paik Hospital, Inje College, from Oct. 1985 to Oct. 1986. And the results were summarized as follows. 1. Among the 70 cases, there were 48 cases of congenital heart anomalies and 22 cases of acquired rheumatic valvular heart diseases. Age range of the congenital patients was 7 months to 31 years with the mean age of 10 years, and the acquired patients was 18 to 62 years with the mean age of 40 years. 2. The heart-lung machine used for cardiopulmonary bypass was Sarns 7000, 5-head roller pump, and the number and type of oxygenators were 5 of membrane type and 65 of bubble type. For all cases GIK [glucose-insulin-potassium] solution was used as cardioplegic solution for myocardial protection during operation. 3. Among the 48 congenital anomalies, there were 12 cases of ASD group, 29 of VSD group, 3 of ECD, 3 of TOF and one of PDA + MR, and to all of which the appropriate radical operations were applied. 4. Among the 22 acquired valvular diseases, there were 11 cases of mitral valve diseases [MS; 4, MSr; 3, MRs; 4], 3 cases of aortic valve diseases [AR:1, ARs;1, ASr;1], 4 cases of double valve diseases [MRs+TR; 3, MRs+ARs; 1] and 4 cases of triple valve diseases [MSr+ASr+TR; 3, MSr+Ar+TR; 1]. To all the diseased mitral and aortic valves, artificial valve replacement was applied except one [As], in which valve plication was applied. And to all the diseased tricuspid valve, DeVega annuloplasty was applied. 5. The number of replaced artificial valves were 29 in 25 patients [congenital; 3, acquire; 22]. In MVR, 6 of mechanical valves [St. Jude Medical valve; 6] and 15 of tissue valves [Carpentier-Edward valve; 11, lonescu-Shiley valve; 4] were used. In AVR, 6 of mechanical valves [St. Jude Medical valve; 6] and 2 of tissue valves [Carpentier-Edward valve; 2] were used. 6. Postoperative complications were occurred in 12 cases. Among them 11 cases were recovered with intensive cares, but one patient [VSD + Fistula of Valsalva sinus] was expired with low cardiac out put syndrome.

Objectives : Metabolic syndrome (MetS) is associated with increasing risk of cardiovascular morbidity and mortality but the risk of stroke associated with the MetS is less well established. The objective of this study was to evaluate the prevalence of MetS in stroke patients. Methods : We analyzed the data of 806 patients with acute ischemic stroke from Hanbang Stroke Registry supported by the Ministry of Science and Technology of Korea. Subjects' enrollment was done in the oriental medical hospitals of 4 universities located in the metropolitan region from April 2007 to August 2008. We analyzed all registered data and evaluated the prevalence of MetS and specific components of MetS. Results : The prevalence of MetS in stroke patients according to American Heart Association/National Heart Lung and Blood Institute (AHA/NHLBI) criteria was 59.6%. The prevalence according to the International Diabetes Federation (IDF) criteria was 41.6%. The prevalence of female patients was higher than that of male patients (71.8% vs 51% by AHA/NHLBI criteria; 61.7% vs 27.7% by IDF criteria). The prevalence of central obesity, low HDL cholesterol and hypertension was higher in female patients. There was no significant relationship between MetS and stroke subtype. Conclusions : This work gives us the basic data on the association between MetS and risk of stroke. There should be the prospective cohort studies to evaluate MetS and the risk of stroke.
