Store-operated Ca2+ entry in muscle physiology and diseases

  • $Ca^{2+}$ release from intracellular stores and influx from extracellular reservoir regulate a wide range of physiological functions including muscle contraction and rhythmic heartbeat. One of the most ubiquitous pathways involved in controlled $Ca^{2+}$ influx into cells is store-operated $Ca^{2+}$ entry (SOCE), which is activated by the reduction of $Ca^{2+}$ concentration in the lumen of endoplasmic or sarcoplasmic reticulum (ER/SR). Although SOCE is pronounced in non-excitable cells, accumulating evidences highlight its presence and important roles in skeletal muscle and heart. Recent discovery of STIM proteins as ER/SR $Ca^{2+}$ sensors and Orai proteins as $Ca^{2+}$ channel pore forming unit expedited the mechanistic understanding of this pathway. This review focuses on current advances of SOCE components, regulation and physiologic and pathophysiologic roles in muscles. The specific property and the dysfunction of this pathway in muscle diseases, and new directions for future research in this rapidly growing field are discussed.

In-vivo Measurements of Blood Flow Characteristics in the Arterial Bifurcation Cascade Networks of Chicken Embryo (유정란 태아외부혈관의 단계적으로 분기되는 동맥 분지관 내부 혈액 유동특성의 in-vivo 계측)

  • The arteries are very important in cardiovascular system and easily adapt to varying flow and pressure conditions by enlarging or shrinking to meet the given hemodynamic demands. The blood flow in arteries is dominated by unsteady flow phenomena due to heart beating. In certain circumstances, however, unusual hemodynamic conditions cause an abnormal biological response and often induce circulatory diseases such as atherosclerosis, thrombosis and inflammation. Therefore quantitative analysis of the unsteady pulsatile flow characteristics in the arterial blood vessels plays important roles in diagnosing these circulatory diseases. In order to verify the hemodynamic characteristics, in-vivo measurements of blood flow inside the extraembryonic arterial bifurcation cascade of chicken embryo were carried out using a micro-PIV technique. To analyze the unsteady pulsatile flow temporally, the (low images of RBCs were obtained using a high-speed CMOS camera at 250fps with a spatial resolution of $30{\mu}m\times30{\mu}m$ in the whole blood vessels. In this study, the unusual flow conditions such as flow separation or secondary flow were not observed in the arterial bifurcations. However, the vorticity has large values in the inner side of curvature of vessels. In addition, the mean velocity in the arterial blood vessel was decreased and pulsating frequency obtained by FFT analysis of velocity data extracted in front of the each bifurcation was also decreased as the bifurcation cascaded.

Microglial activation induced by LPS mediates excitation of neurons in the hypothalamic paraventricular nucleus projecting to the rostral ventrolateral medulla

  • Microglia are known to be activated in the hypothalamic paraventricular nucleus (PVN) of rats with cardiovascular diseases. However, the exact role of microglial activation in the plasticity of presympathetic PVN neurons associated with the modulation of sympathetic outflow remains poorly investigated. In this study, we analyzed the direct link between microglial activation and spontaneous firing rate along with the underlying synaptic mechanisms in PVN neurons projecting to the rostral ventrolateral medulla (RVLM). Systemic injection of LPS induced microglial activation in the PVN, increased the frequency of spontaneous firing activity of PVN-RVLM neurons, reduced GABAergic inputs into these neurons, and increased plasma NE levels and heart rate. Systemic minocycline injection blocked all the observed LPS-induced effects. Our results indicate that LPS increases the firing rate and decreases GABAergic transmission in PVN-RVLM neurons associated with sympathetic outflow and the alteration is largely attributed to the activation of microglia. Our findings provide some insights into the role of microglial activation in regulating the activity of PVN-RVLM neurons associated with modulation of sympathetic outflow in cardiovascular diseases.

  • Cardiovascular calcifications can occur in various cardiovascular diseases and can serve as a biomarker for cardiovascular event prediction. Advances in CT have enabled evaluation of calcifications in cardiovascular structures not only on ECG-gated CT but also on non-ECG-gated CT. Therefore, many studies have been conducted on the clinical relevance of cardiovascular calcifications in patients. In this study, we divided cardiovascular calcifications into three classes, i.e., coronary artery, thoracic aorta, and cardiac valve calcifications, which are closely associated with cardiovascular events. Further, we briefly described pericardial calcifications, which can be found incidentally. Since the start of lung cancer screening in Korea in the second half of 2019, the number of non-enhanced, non-ECG-gated, low-dose chest CT has been increasing, and the number of incidentally found cardiovascular calcifications has also been increasing. Therefore, understanding the relevance of cardiovascular calcifications on non-enhanced, non-ECG-gated, low-dose chest CT and their proper reporting are important for radiologists.

  • Echocardiography is one of the most frequently used techniques for diagnosing cardiovascular diseases. Over the last twenty years, technological advances have enabled the application of high-quality imaging. Important recent developments have occurred in echocardiography that are already being used clinically. Equipment and hardware is now available to produce reai time three-dimensional and contrast enhanced imaging.. Tissue Doppler and stress echocardiography have provided potential benefit to analyze hemodynamic information of heart. This review discusses each of these new developments and their potential impact on the practice of echocardioaraphy and cardiology in general.

Coronary arteriovenous fistula with VSD: Report of 1 case (심실중격결손증을 동반한 관상동정맥루 -치험 1예-)

  • Congenital coronary arteriovenous fistula is uncommon disease, and was first described by Krause in 1865. About 20% of the cases, it associates additional congenital heart diseases. A 5-year-old female patient was diagnosed as coronary AV fistula with VSD, and was taken surgical correction under cardiopulmonary bypass. VSD was small and subarterial in type, and the fistula was dilated as adult thumb tip size at its distal portion. VSD was closed directly through the pulmonary arteriotomy and the aneurysmal dilation was opened vertically, then it was obliterated using 5-0, 6-0 prolene continuous suture fashioning into a long slender tube. Postoperative course was uneventful.

A Report of Fallot's Tetralogy in Siblings (형제에서 발생한 활로4증)

  • Tetralogy of Fallot has shown its familial aggregation in several familial studies. This reported case is another example which occurred in a family in two brothers. They revealed no cytogenetic abnormalities, but the anatomical cardiac malformation of them showed much similarity, total conal defect in ventricular septum and .one had patent foramen ovale, the other atrial septal defect. The familial recurrence tendency of Tetralogy of Fallot as well as other congenital heart diseases could be explained on multifactorial inheritance as shown in many reports. In spite that we couldn`t find out any environmental trigger or teratogens, our case may be accepted on the base of multifactorial mechanism.

Extra-anatomic bypass for Treatment of Leg Ischemia (하지동맥폐쇄환자에서의 비해부학적 우회술의 임상적 고찰)

  • Extra-anatomic bypass is indicated for the limb salvage of patients with threatened limb loss especially those who have high risk of infection and death after general anesthesia. We did extra-anatomic bypass procedure nineteen times over a seven year, two month period on sixteen patients. Their mean follow up period was 41.3 months. Male to female ratio was 7:1. Major complaints were resting pain[36.8%], claudication[31.6%], cyanosis[15.8%] and ulcerative change[15.8%]. Associated diseases were hypertension, tuberculosis, chronic renal failure, atrial fibrillation, complete heart block and laryngeal cancer. Postoperative patency rates were 76.9% at 1 year, 72.7% at 2 years and 70% at 3 years. Limb salvage rate was 78.9%. Postoperative mortality rate was 10.5%.

  • Chylothorax is a rare complication following cardiac surgery for congenital heart diseases. Although conservative management is successful in the majority of cases, surgical intervention is required in a refractory one. Recently, subcutaneous or intravenous infusion of octreotide has been used as a safe treatment that helps avoiding surgical intervention. Herein, we report two cases of postoperative chylothorax treated with parenteral octreotide and conservative therapy.

Surgery of the Infected Patent Ductus Arteriosus with Teflon Wrapping (세균성 동맥내막염을 동반한 개방성 동맥관의 치험예)

  • Patent ductus arteriosus is one of the most common congenital heart diseases, which treated operatively has excellent results. Treatment by surgical means is common and invariably successful. However,infected PDA cases present problems and surgeons operating must face the possibility of adhesions,aneurysmal dilatation and friabiilty of tissue with tearing of the pulmonary arterial end, especially, which causes a fatal hemorrhagic ccmplication. In the earlier days of cardiac surgery, many surgeons deferred operative treatment infected PDA because of frequent complications, high postoperative morbidity and mortality. This continued until Touroff et al. successfully divided the infected PDA in 1940. In 1944, Harper et aI. have used Cellophane for the wrapping of the infected PDA. This surgical procedure has become a single, simple and safe method for treating infected PDA, since that time. In the Teflon wrapping technique, a cardiothoracic team of Yonsei University Severance Hospital used Teflon felt instead of Cellophane in one case of infected PDA and this method proved intractable to antibiotic treatment for 40 days.
