Development of a Cardiovascular Index That Estimates Cardiovascular Health Degree by Survey and Noninvasive Examination (설문과 비침습적 검사를 통하여 심혈관계 건강 정도를 알려주는 심혈관지수의 개발)

  • An index was developed that estimates cardiovascular health degree with easily available physiological information such as survey and noninvasive measurement. The survey score was calculated by utilizing questions related to personal disease history, self-feeling, and management state. The measurement score was calculated using physiological parameters such as blood pressure, accelerated plethysmograph(APG), and heart rate variability(HRV), and augmentation index(AI). In order to evaluate effectiveness of the cardiovascular index and modify weighting factors used in each item, a clinical trial was done in a general hospital. The cardiovascular index showed a clear correlation of 0.685 with the doctor's score on the cardiovascular health degree. The correlation between the self-estimated score and doctor's score was as low as 0.217. The large gap between these two scores demonstrated necessity of more objective tools like the cardiovascular index. The cardiovascular score showed a significant difference between normal persons and patients suffering hypertension or diabetes. (p=0.000).

  • Objective : Stroke is the most common disabling neurological disease of adult life. And some part of the clinical symptoms and signs are correlated to the post stroke complication. This study was to analyze the therapeutic effect of the East-West collaborative treatment, especially on the motor power and the clinical symptoms and signs improvement of ischemic stroke patients. Method : For 2 weeks as possible, we observed the patients with acute ischemic stroke receiving East-West collaborative treatment. We assessed the patients using Motricity Index (MI) and the changes of clinical symptoms and signs three times at seven-day intervals. At the end of the study, we compared the results. Result : Motricity Index of patients changed from 71.55 to 76.03 (upper limb) and 73.05 to 79.42 (lower limb), respectively. Some items of clinical symptoms and signs showed the statistically significant result. Especially, the urination and defecation symptoms improved obviously. Conclusion : The East-West collaborative treatment has an effect on motor power and clinical symptoms and signs improvement of acute ischemic stroke patients.

The Effect of Exercise Type on Cardiovascular Disease Risk Index Factors in Male Workers (일부 직장 남성들의 운동형태가 심혈관질환의 위험 예측인자에 미치는 영향)

  • Objectives : The purpose of this study was to gain an understanding of the effects of three exercise types on anthropometric and serum lipids and physiological index factors, which are known to be the three risk factors of cardiovascular disease in male workers. Methods : The experimental study period was 12 weeks. In this study, 30-40's males (N=31) were assigned to 3 experimental groups: regular aerobic(treadmill walking) exercise group, regular anaerobic(muscular endurance) exercise group, irregular aerobic & anaerobic exercise group and a control group using a stratified random assignment method. Results : In relation to anthropometric factors, the regular aerobic & anaerobic exercise groups showed significant decreases in Weight, Broca's index, WC, BMI, WHtR, WHpR and HRrest. With regard to the serum lipid factors, the TC was decreased, but the HDL-c increased among the regular aerobic & anaerobic exercise groups. However, no significant difference was found between the other groups in respect to the LDL-c and TG. Considering the physiological factors, the TC/HD-c, TC-HDL/HDL-c, LDL-C/HDL-c and NON-HDL-c ratios were decreased, but the HDL-c/TC ratio increased among the regular aerobic & anaerobic exercise groups. The TG/HDL-c and HDL-c/LDL-c ratios showed no significant differences between the groups. These results indicated that the positive change for each factor is much larger in the regular exercise groups, especially in the anaerobic exercise group. Conclusions : The results indicate that not only regular aerobic exercise, but also regular anaerobic(muscular endurance) exercise could be utilized in lessening the deleterious effects of the risk index factors for cardiovascular disease.

A Study of Relationship between Blood Chemical Risk Factor of Cardiovascular Disease and Sasang Constitution, and Effect of Oxygen Free Radical on the Relationship (심혈관계 질환의 위험인자와 사상체질 사이의 연관성 및 활성산소가 이에 미치는 영향에 대한 임상적 연구)

Objectives The purpose of this study was to prove the correlation between risk factor of cardiovascular disease and Sasang Constitution and to investigate the effect of oxygen free radical on this correlation. 2. Methods We analyzed 129 patients who visited Cheonan oriental medical hospital from Apr 1, 2007 until May 31, 2007, for medical examination, which Includes Sasang Constitution examination, blood chemistry test and physical examination. And we classified their results into 4 groups based on with blood lipoprotein level and body mass index. 3. Results and Conclusions Mean of lipoprotein in blood was not significantly different among constitutions but with body mass index, was significantly higher in Taeumin than others. And count of dyslipoproteinemia was significantly higher in Taeumin group than soeumin. Therefore, according to our results, risk of cardiovascular disease was higher in Taeumin than other constitutions. Oxygen free radical did not have much effect on correlation between risk factor of cardiovascular disease and Sasang Constitution. However, there was some evidence in soeumin group, that oxygen free radical may have effect on origination of hyperlipidemia.

Socioeconomic Differentials in Stroke and Cardiovascular Disease Mortality in Korea (소득계층에 따른 뇌심혈관질환 사망률 차이)

  • Objectives: A number of studies in economically developed countries have shown occurrence of stroke and cardiovascular disease to be inversely related to socioeconomic class. The purpose of this study is to investigate socioeconomic differentials in stroke and cardiovascular disease mortality in Korea. Methods: Two data from two sources, registry data from National Health Insurance Corporation and death certification data from National Statistics Office, were used to calculate mortality rate for five socioeconomic classes. Poisson regression analysis was used to calculate relative indices of inequality as a measure of mortality differentials between socioeconomic classes. Results: For males, graded socioeconomic differentials in mortality were observed with higher mortality rates related to lower socioeconomic class for intracerebral hemorrhage, cerebral infarct, hypertension, ischemic heart disease, myocardial infarct, and arrhythmia. The relative index of inequality for stroke and cardiovascular disease was 1.61(95% CI=1.54-1.68). For females, these differentials were observed for arrhythmia and intracerebral hemorrhage. The relative index of inequality was 1.06(95% CI=1.02-1.11). Conclusions: This socioeconomic differential in mortality, consistent with the results of other studies performed in economically developed countries suggest that Socioeconomic class can influence mortality regardless of the developmental stage of the country.

  • Objectives : This study aimed to clarify the relationship between the Oriental medical diagnosis and arteriosclerosis by measuring carotid-ankle vascular index(CAVI) in acute cerebral infarction patients. Method : One hundred thirty-one subjects were recruited from the patients admitted to the Internal Medical Department at Kyunghee University Oriental Medical Center from April 2007 to August 2008. We sorted cerebral infarction patients and assessed one hundred fourteen patients' CAVI data. We diagnosed dampness-phlegm by Oriental medical diagnosis and evaluated stroke type by single or multiple infarctions. then, we analyzed their characteristics with type of stroke, risk factor, lifestyle, metabolic syndrome and dampness-phlegm diagnosis. Result : 1. On the demographic variables of the patients, age, hypertension, hyperlipidemia, multiple infarction group and metabolic syndrome and dampness-phlegm group were significantly higher in the high CAVI score group than in the control. 2. According to the significant difference in the dampness-phlegm group, we analyzed dampness-phlegm related index for pattern identifications by CAVI score. As a result, dark circles, insomnia, headache, white coating tongue. slippery pulse, and rough pulse were significantly higher in the high CAVI score group then in the control. 3. In multivariate analysis, age, hypertension, multiple infarction and dampness-phlegm groups showed a close relationship with the high CAVI score group. Conclusions : According to the analysis, significance between dampness-phlegm diagnosed patients group and high CAVI score were clarified. Moreover, multiple location infarctions also have a relationship with high CAVI score in cerebral infarction patients. These results can be utilized in the future as a basis material.

The association of blood pressure with body mass index and waist circumference in normal weight and overweight adolescents

  • Purpose: Overweight can be defined by the body mass index (BMI) and is likely associated with an increased cardiovascular disease risk. However, waist circumference (WC), a central adiposity index, may be a better indicator of cardiovascular disease risk. Studies comparing the effects of BMI and WC on cardiovascular risk factors, such as high blood pressure (BP), are rare in adolescents. Methods: We analyzed the correlations of BMI and WC with BP in 3,363 Korean adolescents (aged 10-19 years), using data from the Korean National Health and Nutrition Examination Surveys (2009-2011). Results: Systolic BP (SBP) in both sexes and diastolic BP (DBP) in boys were higher in the high BMI (>85th percentile) and high WC (>90th percentile) groups. High BMI and high WC were positively correlated with high SBP (>90th percentile) in both sexes, and high DBP (>90th percentile), in boys. BMI maintained its positive associations with SBP, DBP, high SBP, and high DBP in the normal weight ($BMI{\leq}85th$ percentile) and overweight (BMI>85th percentile) groups in both boys and girls, as well as in all subjects of both sexes, even after adjustment for WC. However, WC did not correlate with SBP, DBP, high SBP, or high DBP after adjustment for BMI in any group in either sex. Conclusion: In Korean adolescents, BMI correlated better with BP and high BP levels than WC. Further, BMI was positively associated with BP and high BP in the normal weight group as well as in the overweight group.

Age-related Circulating Inflammatory Markers and Cardiovascular Disease Risk Factors in Korean Women (한국 성인 여성에서 연령에 따른 혈중 염증 표지자와 심혈관계 질환 위험 요인에 대한 연구)

  • The purpose of this study was to investigate the age-related changes of cardiovascular disease risk factors and inflammatory markers in non-obese Korean women. Subjects were 112 women over 20 years old with body mass index (BMI) less than $30 kg/m^2$ and were divided into 3 groups (< 40 years, $40{\sim}59$ years, ${\ge}60$ years). Mean weight and BMI in the oldest group were significantly higher than those in the other 2 younger groups (p < 0.05). Mean total cholesterol, triglyceride, LDL-cholesterol and apolipoprotein B/apolipoprotein A1 ratio (BAR) in the oldest group were significantly higher than those in the youngest group (p < 0.05), and mean HDL-cholesterol of the oldest group was significantly lower than that of the youngest group (p < 0.05). The older-aged group showed significantly higher mean values of atherogenic index (AI) and LDL/HDL ratio (p < 0.05) than the respective younger-aged group, and AI was significantly correlated with age, nitric oxide and thiobarbituric acid reactive substances (p < 0.01). In addition, mean vascular cell adhesion molecule-l (VCAM-1) tended to be higher in the older-aged group than the younger group. Tumor necrosis factor-${\alpha}$, a proinflammatory maker, was significantly positively correlated with serum homocysteine, a cardiovascular disease risk factor (p < 0.01). In addition, a significantly positive correlation was observed between C-reactive protein and BAR (p < 0.01). Overall results suggested that the aging might affect the increase of cardiovascular disease risk factors including the serum lipid profiles, weight and BMI, and age-related increases of weight and BMI might play a role in changes in certain biomarkers of inflammation. Effect of High Carbohydrate Intakes on the Obesity Index, Blood Pressure, and Blood Lipid Levels in Patients with Cardiovascular Disease

  • This study was designed to investigate the effect of the ratio of energy from carbohydrate to total calories on dietary intake, obesity index, blood pressure, and blood lipid content in cardiovascular disease patients over 35 years old. A total of 552(227 male, 325 female) subjects were divided into three groups according to carbohydrate/total energy ratio : carbohydrate ratios below 25 percent were in the low carbohydrate group( <61.1%), between 25 and 75 percent carbohydrate were medium($\geq$61.1-<74.7%), and higher than 75 percent were in the high carbohydrate group($\geq$74.7%). The anthropometric data, nutrient intake, serum lipid levels, and blood pressure of each group were compared with one another. For men and women with high carbohydrate intakes, Inadequate nutritional intake was observed. Abdominal fat accumulation and blood TC level for men in the high carbohydrate group were higher than in medium or low carbohydrate groups. Therefore, it seems that high carbohydrate intake may produce adverse effects on abdominal fat accumulation and blood lipid patterns. Blood pressure, however, was significantly higher for women in low and high carbohydrate groups than in medium carbohydrate group. These results suggest that extremely high and low carbohydrate intake may raise the risk of cardiovascular disease and that it is necessary to consume nutritionally balanced meals. This can be done by controlling the ratio of dietary carbohydrate at a medium level in order to prevent and/or to reduce the risk.

The relation of Dampness-Phlegm Diagnosis and interanl carotid artery stenosis by carotid artery sonography in cerebral infarction patients (경동맥 초음파를 이용한 뇌경색 환자의 내경동맥 협착도와 습담변증(濕痰辨證)의 관련성 연구)

  • Objectives : This study was aimed to clarify the relationship between the dampness-phlegm diagnosis and internal carotid artery stenosis by measuring carotid artery sonography in cerebral infarction patients. Methods : One hundred eighty subjects were recruited from the patients admitted to the Department of Internal Medicine at Kyunghee university oriental medical center from September 2008 to July 2010. We assessed one hundred eighty patients' carotid artery sonography data and diagnosed dampness-phlegm by oriental medical diagnosis. then, analyzed their characteristics, risk factor, lifestyle, metabolic syndrome, body mass index, Waist/Hip ratio(W/H ratio) and dampness-phlegm diagnosis. Results : On the demographic variables of the patients, age, smoking, W/H ratio and dampness-phlegm group were significantly higher in severe internal carotid artery(ICA) stenosis group than in the control group. According to the significant difference in dampness-phlegm group, we analyzed dampness-phlegm related index for pattern identifications by ICA stenosis. As a result, sputum, bowel sound, chest discomfort, slippery pulse were significantly higher in the severe ICA stenosis group than in the control group. In multivariate analysis, dampness-phlegm group showed close relationship with severe ICA stenosis group. Conclusion : According to the analysis, significance between dampness-phlegm diagnosed patients group and severe ICA stenosis were clarified. These results can be utilized in the future as a basis material.
