규칙적인 운동이 sarcopenic 비만여성고령자의 심혈관질환 위험인자에 미치는 영향 (Effects of Exercise on Cardiovascular Disease Risk Factors in Sarcopenic Obesity Elderly Women)

  • 고령기의 생활을 위협하는 obesity 여성고령자를 대상으로 sarcopenia 동반에 따른 CVD 위험의 가중성과 16주간의 운동중재가 CVD 위험인자에 미치는 변화를 알아보고자 한다. 또한, 이러한 운동효과를 검증하여 obesity 여성고령자에게 obesity와 sarcopenia에 대한 운동의 중요성을 부각시켜 삶의 질 향상에 도움을 주기 위한 연구이다. 비만 여성고령자를 sarcopenia 동반유무에 따라 sarcopenic obesity group(SOG)과 non-sarcopenic obesity group(NOG)으로 나누어 16주간, 주 3회, 50-60분간 복합운동 실시 후 심혈관질환 위험인자에 미치는 영향을 분석하여 다음과 같은 결론을 얻었다. 운동 전 두 그룹간 CVD 위험인자 비교에서는 비만 여성고령자에게 sarcopenia가 동반될 경우 CVD 위험이 가중되지 않았으며, sarcopenic obesity 여성고령자가 non-sarcopenic obesity 여성고령자보다 HDL-C는 높게, TG는 더 낮게 나타났다. 운동중재 후에는 혈청지질에서 sarcopenic obesity 여성고령자의 경우 TC, TG는 감소하였고, HDL-C, LDL-C은 차이가 없었다. non-sarcopenic obesity 여성고령자의 경우 TC, TG, LDL-C는 감소하였으며, HDL-C 증가하였다. 따라서, sarcopenia와 CVD 위험인자와의 낮은 관련성을 확인할 수 있었으며, sarcopenic obesity 여성고령자가 non-sarcopenic obesity 여성고령자에 비해 HDL-C, LDL-C, BP, right baPWV에서 운동효과가 더디다는 것을 발견할 수 있었다.

Prevalence of Metabolic Syndrome and Its Predicting Factors among Small-sized Company Workers

  • Purpose: This study was aimed to examine the prevalence of metabolic syndrome (MetS) and cardiovascular risk factors among workers at small-sized companies having fewer than 50employees in Korea. Methods: A descriptive cross-sectional study was conducted using a secondary data analysis on workers' health examination data. Data from 21,784 employed workers were analyzed, including 16,488 males (mean age 38.7years)and 5,296 females (mean age 40.5 years). Participants were newly enrolled in annual health check-ups at a professional health clinic from 2009 to 2011. Logistic regression analysis was performed to identify age-adjusted gender specific predicting factors of MetS incidence. Results: The prevalence of MetS was 13.4% in male and 7.7% in female workers. Logistic regression analysis showed that, when age was adjusted for, family history of cardiovascular disease (CVD) was a predicting factor in both male and female workers. For male workers, heavy alcohol drinking and non-manual occupation (office workers/drivers) predicted MetS. Conclusion: Health care providers should screen for MetS periodically in small work places especially for those with a family history of CVD. Educational counseling needs to be given to male workers with sedentary occupations with an aim to reduce heavy drinking and encourage lifestyle modification.

The Effects of Hazardous Chemical Exposure on Cardiovascular Disease in Chemical Products Manufacturing Workers

  • The purpose of this study was to understand the mechanism of cardiovascular disease (CVD) caused by exposure to hazardous chemicals. We investigated changes in the symptoms of metabolic syndrome, which is strongly related to CVD, and in levels of other CVD risk factors, with a special emphasis on the roles of catecholamines and oxidative stress. The results revealed that neither body mass index (BMI) nor waist and hip circumferences were associated with exposure to hazardous chemicals. Among metabolic syndrome criteria, only HDL-cholesterol level increased on exposure to hazardous chemicals. Levels of epinephrine (EP) and norepinephrine (NEP) were not influenced by exposure to hazardous chemicals; however, the total antioxidative capacity (TAC) reduced because of increased oxidative stress. Both hazardous chemical exposure level and metabolite excretion were related to EP, NEP, and the oxidative stress index (OSI). Logistic regression analysis with these factors as independent variables and metabolic syndrome criteria as dependent variables revealed that EP was associated with blood pressure, and NEP with metabolic syndrome in the chemical-exposed group. In conclusion, the results suggest that reactive oxygen species generated and oxidative stress due to exposure to hazardous chemicals act as mediators and cause changes in the physiological levels of EP and NEP to increase blood pressure. This ultimately leads to the development of CVD through increase in cholesterol, triglyceride, and blood glucose levels by lipid peroxidation.

한국인 제2형 당뇨병 여성환자에서 심혈관질환 위험인자와 과일류 및 생선류 섭취와의 관련성: 제4기와 제5기 국민건강영양조사 자료를 이용하여 (Relationship between fruit and fish intakes and cardiovascular disease risk factors in Korean women with type 2 diabetes mellitus: Based on the 4th and 5th Korea National Health and Nutrition Examination Surveys)

  • 본 연구는 2007~2012년 국민건강영양조사 자료를 이용하여 30세 이상 제2형 당뇨병 여성 환자의 과일류 및 생선류 등의 식품섭취량과 심혈관질환 위험인자와의 관련성을 분석하였다. T2DM군이 NFG군에 비해 과일류 및 생선류의 섭취량이 유의적으로 낮았고 이들의 섭취량이 증가할수록 고중성지방혈증의 위험성이 유의적으로 감소하는 것으로 나타났다. 또한, 과일류와 생선류 섭취량은 각각 중앙값 이상으로 많이 섭취한 군 (FF3)이 각각 중앙값 이하로 섭취한 군 (FF1)에의 비해 TG 농도가 높을 위험이 유의적으로 낮았다. 이러한 결과는 과일류에 김치를 제외한 채소류, 버섯류, 해조류의 섭취량을 포함시켜 분석했을 때에도 같은 결과를 나타내었다. 따라서, 한국인 제2형 당뇨병여성 환자의 과일류 및 생선류의 섭취가 심혈관질환에 긍적적인 영향을 미치는 것으로 보이며, 과일류나 생선류 중 어느 한 종류만 많이 섭취하기보다는 두 종류를 모두 적절히 섭취하는 것이 심혈관 질환 위험성을 낮추는데 효과적일 것으로 사료된다.

Effects of dietary fish oil and trans fat on rat aorta histopathology and cardiovascular risk markers

  • Fish oil and shortening have been suggested to have opposite effects on cardiovascular disease (CVD). This study investigated the effect of shortening and fish oil on CVD risk factors and aorta histopathology, and the association between risk factors and aorta histopathology. Male Wister rats (n=30) were fed an AIN-93G diet containing 20% fat in the form of fish oil, shortening, or soybean oil for 4 weeks. Total cholesterol (TC), triacylglyceride (TG), and C-reactive protein levels were significantly (P<0.001) lower in the fish oil than in soybean oil and shortening groups. HDL-cholesterol concentrations were significantly different (P<0.001) between groups. In addition, LDL-cholesterol levels were significantly (P<0.001) lower in the fish oil and shortening groups than in the soybean oil group. Insulin and glucose concentrations did not differ among groups. Effect of dietary fat on tissue fatty acid composition significantly differed in abdominal fat and brain compared with RBC, heart, kidney and liver. The aortic wall was significantly (P=0.02) thinner in the fish oil group than in the soybean oil and shortening groups. The aortic wall thickness was positively correlated with TG and TC, but negatively with EPA + DHA levels of all tissues. These results suggested that fish oil had protective effects on aorta histopathology by hypolipidemic action in this rat model.

심혈관질환 수술을 받은 환자에서 한식식이조절 영양교육이 심혈관질환 위험요인에 미치는 영향 (Effects of Korean diet control nutrition education on cardiovascular disease risk factors in patients who underwent cardiovascular disease surgery)

  • 본 연구는 심혈관질환으로 심장수술을 받은 환자 15명 대상에게서 12주 동안 한식으로 식이조절 교육을 통해 한식 섭취가 증가하고 이를 통해 CVD 위험인자 및 약물 투여량의 변화, 영양소 섭취량에 미치는 영향과 관련성에 대한 분석 결과는 다음과 같았다. 1. 대상자의 평균 연령은 $60.7{\pm}2.5$세로 최근 3년 이내에 심혈관질환 수술을 받고 지속적으로 심혈관질환 약물을 복용하고 있는 환자였다. 2. 대상자의 한식 식이조절 교육 순응도 점수는 교육 전 $35.8{\pm}0.9$점에서 교육 후 $43.0{\pm}1.2$점으로 유의적으로 증가하였는데 (p < 0.001), 이는 한식 섭취를 적극적으로 실천하였음을 시사한다. 3. 대상자의 비만지표인 체중 및 체질량지수는 교육 후, 감소 경향을 보였으며 허리둘레 (p = 0.002) 및 허리 엉덩이둘레비인 WHR (p < 0.001)는 유의적으로 감소하였다. 4. 대상자들의 혈당조절 지표인 HbA1c는 교육 전 $7.3{\pm}1.0%$에서 교육 후 $7.0{\pm}1.1%$로 유의적으로 감소하였다 (p < 0.05). 5. 대상자들의 항고혈압제약물의 투여량 변화는 교육 전 2.1개에서 교육 후 1.7개로 줄었음에도 불구하고 혈압은 정상범위에서 안정적으로 유지되었음을 확인하였다. 항당뇨병 약제 복용량은 유의적인 변화는 보이지 않았으나, 안정된 상태로 적절한 혈당을 유지함과 동시에 혈당 조절 지표인 당화혈색소가 감소함을 확인하였는데 이는 혈당조절이 개선되고 있는 것으로 나타났다. 6. 한식 식사유형인 음식군별 섭취량의 변화는 교육 전에 비해 교육동안에 전곡류로 지은 밥 중심의 식사량이 유의적으로 증가하였고 (p = 0.018), 채소류인 나물류 (p = 0.007), 김치 (p < 0.001) 및 전통 콩발효 양념 (p < 0.001) 등의 섭취가 유의적으로 증가하였다. 7. 대상자들의 영양소 섭취는 일일 에너지 섭취량은 변화가 없었으나 교육 전보다 교육 동안에 동물성급원식품에서 유래한 동물성 단백질과 지질 및 콜레스테롤의 섭취량은 유의적으로 감소 (p < 0.05)한 반면 나트륨 (p = 0.019), 칼륨 (p = 0.028), 식이섬유 (p < 0.050), 비타민 A (p = 0.028), 비타민 C (p = 0.010) 및 엽산 (p < 0.001)의 섭취는 유의적으로 증가하였다. 이상의 결과를 종합해 보면, 적극적인 한식 섭취를 권장하고 장려하는 식이조절교육 중재는 심혈관질환 수술을 받은 환자에게서 영양소 섭취와 심혈관계 위험인자인 비만지표, 혈당조절 지표에 대하여 긍정적인 영향을 주었다. 본 연구의 한계로는 교육에 참여한 대상자가 15명으로 다소 적었으며 단기 교육에 의한 전후 비교 평가에 의한 것으로 그 결과를 일반화하기에는 제한점이 있다. 따라서 향후 연구 장기간의 교육과 대조군이 있는 비교 연구가 필요할 것으로 사료된다.

일 지역 남성 운전직 근로자들의 심뇌혈관질환 발병위험도, 예방 관련 지식, 변화단계 및 건강행위 (Incidence Risk of Cardiocerebrovascular Disease, Preventive Knowledge, Stage of Change and Health Behavior among Male Bus Drivers)

  • Purpose: This study was designed to identify the incidence risk of cardicerebrovascular disease (CVD) among male bus drivers, and to examine and compare the predictors of their health behavior according to the level of CVD incidence risk. Methods: The convenience sample of 222 male bus drivers were recruited from a bus company located in Jeonnam province. Data were collected from self-reported questionnaires and annual medical examination records from 2010. The CVD incidence risk was calculated based on the risk criteria for industrial workers. Results: The 26.6% and 26.1% of the participants were in the moderate and high risk group, respectively. The 72% of the participants were in the precontemplation stage and reported no intention to change their unhealthy lifestyles. Stepwise multiple regression analyses showed that current smoking, excessive alcohol drinking, physical inactivity and lack of knowledge were negative predictors of good health behavior in the normal/low risk group (Adj $R^2$=.443). Heavy alcohol drinking, current smoking, physical inactivity and dyslipidemia were reported by the moderate/high risk group (Adj $R^2$=.427). Conclusion: This study suggested that targeted education and counseling are needed to modify unhealthy lifestyles such as alcohol consumption, smoking and exercise among middle aged male drivers. Especially, dyslipidemia should be managed among those who are at risk for CVD.

건강한 여성 근로자에서 Apolipoprotein E와 혈중 지질 농도와의 관련성 (The Association Between Apolipoprotein E Genotype and Lipid Profiles in Healthy Woman Workers)

  • Objectives: Plasma lipid profiles and Apolipoprotein E (ApoE) are established risk factors for cardiovascular disease (CVD). The knowledge of lipid profile may estimate the potential victims of cardiovascular disease before its initiation and progression and offers the opportunity for primary prevention. The most common ApoE polymorphism has been found to influence plasma lipid concentrations and its correlation with CVD has been extensively investigated in the last decade. Methods: The ApoE polymorphism and its influence on plasma lipid were investigated in healthy woman workers. The information on confounding factors was obtained through a self-administered questionnaire and ApoE polymorphism was investigated using PCR. Results: The relative frequencies of alleles E2, E3 and E4 for the study population (n = 305) were 0.127, 0.750 and 0.121, respectively. ApoE polymorphism was associated with variations in plasma HDL-cholesterol lipid profile. In order to estimate the independent effects of alleles E2 and E4, as compared with E3, on lipid profile, multiple regression was performed after adjustment for confounding variables such as age, BMI, blood pressure, education status, insulin, fasting glucose, HOMA-IR, menopause. ApoE2 had a negative association with HDL cholesterol and ApoE4 had a positive association with LDL cholesterol. Conclusions: This study identified that the ApoE and CVD risk factors contribute to the lipid profiles, similar to other studies. The analysis including dietary intake and other gene in further studies may help to identify clear effects on lipid profiles as risk factor for CVD.

The association between measurement sites of visceral adipose tissue and cardiovascular risk factors after caloric restriction in obese Korean women

  • Quantities as well as distributions of adipose tissue (AT) are significantly related to cardiovascular disease (CVD) risk factors and can be altered with caloric restriction. This study investigated which cross-sectional slice location of AT is most strongly correlated with changes in CVD risk factors after caloric restriction in obese Korean women. Thirty-three obese pre-menopausal Korean women ($32.4{\pm}8.5$ yrs, BMI $27.1{\pm}2.3\;kg/m^2$) participated in a 12 weeks caloric restriction program. Subcutaneous adipose tissue (SAT) and visceral adipose tissue (VAT) were measured using computed tomography (CT) scans at the sites of L2-L3, L3-L4, and L4-L5. Fasting serum levels of glucose, insulin, triglyceride, total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), leptin and homeostasis model assessment-insulin resistance (HOMA-IR) were observed. Pearson's partial correlation coefficients were used to assess the relationship between AT measurement sites and changes in CVD risk factors after calorie restriction. When calories were reduced by 350 kcal/day for 12 weeks, body weight (-2.7%), body fat mass (-8.2%), and waist circumference (-5.8%) all decreased (P < 0.05). In addition, following caloric restriction, serum levels of glucose (-4.6%), TC (-6.2%), LDL-C (-5.3%), leptin (-17.6%) and HOMA-IR (-18.2%) decreased significantly (P < 0.05) as well. Changes in VAT at the level of L3-L4 were significantly greater than those at other abdominal sites, and these changes were correlated with changes in TC (P < 0.05), LDL-C (P < 0.001), SBP (P < 0.001) and HOMA-IR (P < 0.01). These results show that VAT at L3-L4 had a stronger correlation with CVD risk factors than with other AT measurement sites after caloric restriction.

심뇌혈관질환 위험요인을 가진 남성 사무직근로자를 위한 배우자참여 건강 코칭의 효과 (Effects of Spouse-Participated Health Coaching for Male Office Workers with Cardiocerebrovascular Risk Factors)

  • Purpose: This study aimed to examine the effect of spouses participating in health coaching on stage of the change, health behaviors, and physiological indicators among male office workers with cardiocerebrovascular disease (CVD) risk factors and compare the findings with trainers who provided health coaching only to workers. Methods: A quasi-experimental pretest-posttest design was used. Convenience sampling was used to recruit participants from a manufacturing research and development company in the city of Gyeonggi province. The health coaching program for the experimental group (n=26) included individual counseling sessions according to workers' stage of change, and provision of customized health information materials on CVD prevention to workers and their spouses for 12 weeks through mobile phone and email. Results: After 12 weeks of intervention, the total score for health behavior, and scores on the sub-areas of exercise and health checkups significantly improved in the experimental group, but there were no significant differences in the scores of stage of the change and physical indicators. The results of a paired t-test showed a significant decrease in the body mass index, abdominal circumference, systolic blood pressure, diastolic blood pressure, total cholesterol and triglyceride values, and a significant increase in the high-density lipoprotein cholesterol value in the experimental group after the intervention. Conclusion: To improve the health of male workers with CVD risk factors in the workplace, sharing health information with their spouses has proven to be more effective than health coaching for only workers. Therefore, it is important to develop strategies to encourage spousal participation when planning workplace health education for changing health-related behaviors.
