Resection and reconstruction of distal trachea or carina have posed tremendous technical challenges for surgeons. Successful outcome depends on thorough preoperative evaluation, careful anesthetic management,strict attention of surgical technique and postoperative care. We report a successful case of revision of tracheal stenosis using femoro-femoral bypass on a 13~year-old boy. The patient complained severe dyspnea about I month following right sleeve pneumonectomy. Preoperative CT scan and intraoperative bronchoscopy showed pin-point tracheal stenosis at a tracheo-bronchial anastomosis site about 1.2cm in length.At operation the lesion was severely adhesed and the lumen was nearly obstructed. The stenotic segment was resected and direct end-to-end anastomosis was done under femoro-femoral bypass for adequate oxygenation. The patient was discharged at postop. 16 days without specific complications and has continued to do well.
Improving intraoperative and postoperative myocardial protection and better construction and design of valvular prosthesis has reduced the mortality of MVR. But, ventricular rupture after MVR occurred occasionally and represented a potentially lethal complication. Transverse midventricular disruption presented as refractory myocardial failure immediately on termination of bypass or later often on initial period of good hemadynamics. From Jan., 1985 through Dec., 1991 131 MVRs were performed as isolated or combined procedures. Rupture of the posterior wall of left ventricle was observed in 2 patients. There were 2 type III ruptures Prevention is of utmost importance, and by taking certain precautions, the chance of ventricular rupture can be reduced. Repair should always be done by patch technique in the aid of the use of cardiopulmonary bypass with cardioplegic arrest. When the laceration is in the middle of the posterior left ventricle, external repair with the pericardial patch can be attempted first.
Autologous blood transfusion is a common method of reducing the need for homologous blood transfusion during cardiac operations. Between June 1991 and May 1992, 12 cases [Group I] were experienced autologous blood transfusion using Cell Saver undergoing double valve replacement or redo-valve replacement. Control group [N=12, Group II] was selected to above similar operation during same period. The Cell Saver system [Haemonetics Corp.] was employed for autologous blood transfusion. The blood shed in the operative field before and after cardiopulmonary bypass and remained cardiotomy reservior was aspirated by means of a locally heparinized collecting system. After centrifused salvaged blood, the resulting red cell concentrate reinfused subsequently. The patient receiving autologous blood transfusion required significantly less homologous blood transfusion than their control group. [Group I; 3519 $\pm$ 869, Group II; 4622 $\pm$ 856, Respectively; P=0.005] There were no clinical infections in the autotransfusion group. And there was no apparent intergroup difference of the clinical findings, hematologio datas and coagulation parameters. We conclude the autotransfusion using Cell Saver is effective for reducing of the hom-ologlous blood transfusion in cardiac surgery.
This study examines and analyzes EMTs works in hospital with 48 subjects working for hospitals at partial areas. It aims to provide basic materials for practice-centered learning to students at Dept. of Emergency Care and information of roles of EMTs. As a result of study, it was found that EMTs works included classification of severity of patients when they came to hosptial, supervision and move of serious case, move of patients and a member of cardiopulmonary resuscitation team as well as filling empty of medical staff and playing medical assistant under direct control of doctors and they played important roles in medical examination. These results showed that EMTs could extend their areas of service within hospital and they could have a positive influence on their seniors' employment.
Objectives: The purpose of this study is to investigate the knowledge and self-efficacy of dental hygiene students after cardiopulmonary resuscitation (CPR) education. Methods: A self-reported questionnaire was filled out by 260 dental hygiene students in the period between 5 November 2015 and 30 December 2015. The data analysis was performed by SPSS win 22.0 program for frequency analysis, chi-square analysis, and dichotomies multiple response analysis. Results: Self-efficacy and knowledge increased after CPR education. Most of the dental hygiene students are statistically significant difference in self-efficacy depending on CPR knowledge. Conclusions: Based on the results of this study, we suggest that the theory and practice of CPR, if taught to dental hygiene students, will boost their self-efficacy, and enable them to perform it correctly when faced with and emergency situation.
A 25-month-old patient with complete atrioventricular septal defect and Tetralogy of Fallot underwent repair of both anomalies. The diagnosis was established preoperatively by 2D-echocardiography, cardiac catheterization and cardioangiogram, Repair was accomplished using cardiopulmonary bypass and profound hypothermia to 18C, Closing of the atrioventricular septal defect was achieved with the use of two Dacron patchs by an atrial approach alone. Infundibulectomy and outflow tract reconstruction with the transannular pericadial patch containing a monocusp were performed. Upon the postoperative evaluation by 2D-echocardiography, mitral regurgitation was absent, but a tiny dehiscence of ventricular patch and minimal tricuspid regurgitation were noticed.
Obstruction of the right ventricular outflow tract occurs in many various positions. Recently, obstruction due to anomalous muscle bundle has been clearly recognized. This anomaly is caused by a large anomalous muscle bundle, which creates an obstruction low in the body of the right ventricle and divides it into two chambers. Three cases of double chambered right ventricle were treated surgically utilizing cardiopulmonary bypass in the Department of Chest Surgery, College of Medicine, Seoul National University. 1] All of the 3 cases were female. 2] In 2 cases, FKG findings were RVH and RAD, but in case 2, normal EKG findings. 3] In case 2, no pressure gradient between the inflow portion and outflow portion of the right ventricle was observed, but in case 1 and 3, pressure gradients were 70 and 64 mmHg, respectively. 4] In 2 of the 3 cases, type II VSD was combined to double chambered right ventricle. 5] All of the 3 cases were discharged with symptomatic improvement after operation.
Massive air embolism during cardiopulmonary bypass is uncommon but serious and often lethal complication. Following this catastrophic event, the immediate institution of retrograde arterial blood perfusion via superior vena cava was made to remove air emboli from cerebral circulation. This method was performed by removing the arterial perfusion line from aortic cannula and connecting it to superior vena caval cannula. Then, retrograde perfusion at a flow rate of 2Umin via superior vena cava was carried out for 3 minutes. After air returning from the aortic cannula was identified, each line was connected to the cannulae primarily. In 2 cases who had massive air emboli due to air pumping into arterial line, the postoperative complete recovery resulted from this technique, which was used in conjunction with other therapy postoperatively.
A direct communication between the left ventricle and right atrium is a relatively uncommon defect. Familiarity with this anomaly has become increasingly important, however, since the preoperative findings may be identical with those of an atrial septal defect. Aneurysmal rupture of sinus Valsalva is known as one of rare cardiac disease and not controlled with medical treatment. We experienced 9 cases which were composed with 7 case of male and 2 cases of female during last 15 years. 7 cases were ruptured into right ventricle from right. coronary sinus and 2 cases were ruptured into right atrium from non-coronary sinus. The diagnosis was made with cardiac catheteriza1icn and cineangiccardicgram but 2 cases were misdiagnosed with only cardiac catheterization. All cases were corrected under cardiopulmonary bypass with means of direct suture with tdlon pledget by transatrial or transventricular approach. All were not confirmed in it's origin because of no history, no evidences of syphilis, TB, or bacterial endocarditis and only fibrosis in pathologic report. In postoperative course, 1 case had postoperative bleeding and 1 case was sufferd from left hemiplegia due to may be air embolism. Follow up study revealed all patient go on their usual life well with good improvement at this present time.
