  • This is case report of total anomalous pulmonary venous return with atrial septal defect which were corrected surgically by intracardiac procedure under total cardiopulmonary bypass. Two patients were supracardiac type, cardiac and mixed type was each one.The mixed type was three years old female patient. She was diagnosed as atrial septal defect with partial anomalous pulmonary venous return[right pulmonary vein drains into superior vena cava and right atrium] and corrected as usual. After operation, she underwent exertional dyspnea and frequent tachycardia.Chest x-ray film showed pulmonary congestion.Follow up cardiac cineangiogram revealed that left pulmonary vein also anomalously drained into left innominate vein through vertical vein.Through left thoracotomy, anastomosis was successfully carried between left atrium and vertical vein without cardiopulmonary bypass and there was no sign of pulmonary artery obstruction for two years follow up.The other three patient were corrected successfully without complication and got good result.

  • Purpose : The comparative effectiveness of without dispatcher-assisted telephone instruction CPR but received lectured and training(NDCPR) and without received lectured and training CPR but dispatcher-assisted telephone instruction(DCPR) in CPR. Methods : The CPR instruction to 774 students and faculties in universities and colleges. There selected without dispatcher-assisted telephone instruction CPR but received lectured and training(NDCRP) and without received lectured and training CPR but dispatcher-assisted telephone instruction(DCPR). Compare the effectiveness to sills performance of DCPR and NDCPR students and faculties of according to the CPR. Results : 397(51%) students and faculties was DCPR and 377(49%) students and faculties was NDCPR. There was no difference in the compression depth, hand position, adequacy of recoil, volume of ventilation, self-confidence, and willingness to do CPR between the two instructional methods. Conclusion : Under the guide of dispatcher-assisted CPR instruction integrated into mobil phone, novice could perform more effective CPR. This method could be used as a supplement to CPR practice and skill retention.

  • Three cases of multiple cardiac valve replacement were done in March 1983 at the department of thoracic and cardiovascular surgery, Chosun University College of Medicine. The patients were moderately symptomatic in all cases, and belonged to the class II and III of the NYHA functional criteria. The diseased valved were replaced with Bjork--Shiley, Ionescu-shiley valve prosthesis under cardiopulmonary bypass using hemodilution technique. The kind of cardioplegic solution used in our institute were Young and GIKs solution with core surface cooling. The average cardiopulmonary bypass time was 251.6 minutes and the average aortic cross clamping time was 223 minutes for aortic and mitral valve replacement. There was no operative morality.

  • Cardiac myxomas are most common benign tumor and comprise approximately 50% of all primary cardiac neoplasms. They are intracavitary tumors occurring within any of the cardiac chambers, but they have a predilection for the atria and particularly the left atrium. Its are usually arise from the region of the limbus of the fossa ovalis. Clinically, they present with various manifestations due to obstruction to blood flow, embolization, and constitutional changes. Excision with the aid of cardiopulmonary bypass has been established as the treatment of choice for these histologically benign, but potentially malignant tumors and has generally produced good results [17]. We have experienced two cases atrial myxomas, one is left, the other is right and resected under established cardiopulmonary bypass, so we report these cases with the review of the literature.

  • Penetrating chest trauma by stab injury may result in massive hemothorax from damage to single or multiple intrathoracic organs such as heart, aorta, internal mammary artery, intercostal artery or pulmonary parenchyme. Prognosis of massive hemothorax necessitating emergency thoracotomy is fatal especially so if there exists concomitant underlying compromise of cardiopulmonary function. A 56 year old man with destroyed left lung due to old pulmonary tuberculosis was stabbed in right parasternal lesion through third intercostal space. Intubation with cardiopulmonary resuscitation and closed thoracostomy were performed to resuscitate from cardiac asystole from hemorrhagic shock and acute respiratory distress. Midsternotomy was made to expose active bleeding foci in right mammary artery, subclavian vein, intercostal artery and anterior segment of right upper lung showing severe bullous change and pleural adhesion. Postoperative care included ventilator support, inotropic instillation and cautious, balance fluid therapy ; successful extubation was done on third postoperative day and patient was discharged on tenth postoperative day without any complication.

  • PURPOSE: The purpose of this study was to investigate the differences and correlation between body composition, exercise performance, and behavior based on playing football in childhood. METHODS: 16 subjects who played football in childhood participated in the study. Body composition and exercise performance were measured, and problem behavior was assessed for each of them. All subjects were asked to play football 50 min/day, one day/week for 8-weeks. RESULTS: Muscle mass, muscular strength, balance, and cardiopulmonary endurance, anxiety depression, atrophy depression, attention problems, rule violations, DSM somatization problems, DSM rebellious behavior problems, and sociality significantly increased after 8-weeks. There was a negative (-) correlation between anxiety depression and atrophy depression, and DSM somatization problem and muscular strength, attention problem and balance, and rule violation and cardiopulmonary endurance, after playing football. CONCLUSION: These results confirmed that playing football in childhood had a positive effect on body composition, and that exercise performance and problem behavior were related.

  • Innominate artery aneurysms are challenging for surgeons to treat because of the requirement for brain protection during surgery. In innominate artery aneurysms, the endovascular approach does not require cardiopulmonary bypass, but patients who can be treated using this approach are limited in number, and the long-term results of endovascular treatment are unclear. Here, we report our experience of successfully treating a patient with an innominate artery aneurysm using near-infrared spectroscopy without cardiopulmonary bypass support or hypothermic circulatory arrest.

  • Occupational lung diseases (OLD) are a group of preventable conditions caused by noxious inhalation exposure in the workplace. Workers in various industries are at a higher risk of developing OLD. Despite regulations contributing to a decreased incidence, OLD remain among the most frequently diagnosed work-related conditions, contributing to significant morbidity and mortality. A multidisciplinary discussion (MDD) is necessary for a timely diagnosis. Imaging, particularly computed tomography, plays a central role in diagnosing OLD and excluding other inhalational lung diseases. OLD can be broadly classified into fibrotic and non-fibrotic forms. Imaging reflects variable degrees of inflammation and fibrosis involving the airways, parenchyma, and pleura. Common manifestations include classical pneumoconioses, chronic granulomatous diseases (CGD), and small and large airway diseases. Imaging is influenced by the type of inciting exposure. The findings of airway disease may be subtle or solely uncovered upon expiration. High-resolution chest CT, including expiratory-phase imaging, should be performed in all patients with suspected OLD. Radiologists should familiarize themselves with these imaging features to improve diagnostic accuracy.
