Appropriateness of Selecting the Chest Compression Site by Lay-Persons: Compared to the center of the lower half of the breastbone and above the two fingers on solar plexus

  • This study is to find out the effectiveness by adult CPR method of a single rescuer. For patients with acute cardiac arrest, CPR is the only first aid, and chest compression is the most important of these techniques. The subjects of this study were freshman, 120 subjects who did not learn CPR, and for data analysis, the SPSS 22.0 Version was used. The accuracy of chest compression site selection was significantly higher in the upper part of the two fingers in the pit of the stomach(p<.001), and the ease was also high(p<.001). In the case of the general public, they select the chest compression area easily and quickly, it turns out that it is easier to designate the landmark toward the pit of the stomach. In the future, through various job groups, research to select the chest compression site is needed, and additional studies should be conducted to confirm the maintenance and accuracy during chest compression.

Pathological Studies on the Esophagogastric Ulcers in Swine (돼지의 위궤양(胃潰瘍)에 관한 병리학적(病理學的) 연구(硏究))

  • Stomachs from 3,354 fattening pigs were examined at slaughter during the period from September 1980 to August 1981. Pigs of both sexes and $Landrace{\times}Hampshire$ crossbreds were included in the present studies, and they weighed about 60-120kg. Gross pathologic alterations of the stomach were classified as normal, epitnelial change, erosion, ulcer and scar formation. Representative tissue sections were taken from the stomach lesions at random and fixed in 10% buffered formalin. Cut sections were stained with hematoxylin and eosin, and examined histopathologically. The results obtained in the present studies were as follows. 1. In the seasonal prevalence of gastric ulceration, severe ulceration with erosion was shown during the Autumn and Winter, whereas mild ulceration was mainly shown during the Spring and Summer. 2. Of the 3,354 stomachs of the pigs, 20.8% were found to be normal. Of the rest, 40% had epithelial changes, followed by erosion (24.7%), ulcer (13.6%) and scar formation (0.9%), respectively. 3. In the prevalence of ulcers in the different regions of the stomach, the fundic region had the highest rate (61.6%) of ulceration, followed by cardiac (21.7%), esophageal (15.0%) and pyloric region (1.7%). 4. The principal gross changes were severe epithelial changes with keratotic proliferation in the esophageal region, and in the fundic region severe folding of the stomach wall was covered with bloody mucous exudates. 5. Main histopathological changes were inflammatory cell infiltrations in most cases, hemorrhages in acute ulcers and prominent proliferation of granulation tissues in chronic ulcers.

Incidence, Survival and Prevalence of Esophageal and Gastric Cancer in Linzhou City from 2003 to 2009

  • This study describes recent trends in incidence, survival and prevalence of subgroups of esophageal and gastric cancer in Linzhou city between 2003 and 2009. Data of esophageal and gastric cancer for the period of interest were extracted from the Linzhou Cancer Registry. Using information on tumor morphology or anatomical site, data were divided into six groups; esophageal squamous cell carcinoma, esophageal adenocarcinoma, other and unspecified types of esophageal cancer, and cardia, non-cardia, and unspecified anatomical site of stomach cancer. Incidence, survival and prevalence rates for each of the six cancer groups were calculated. The majority of esophageal cancers were squamous cell carcinomas (82%). Cardiac cancer was the major gastric cancer group (64%). The incidence of esophageal squamous cell carcinoma and gastric cardiac cancer increased between 2003 and 2009. Both esophageal and gastric cancer had a higher incidence in males compared with females. Overall survival was poor in all sub-groups with 1 year survival ranging from 45.9 to 65.6% and 5 year survival ranging from 14.7 to 30.5%. Prevalence of esophageal squamous cell carcinoma and gastric cardiac cancer was high (accounting for 80% overall). An increased focus on prevention and early diagnosis, especially in esophageal squamous cell carcinoma and gastric cardiac cancer, is required.

An immunohistochemical study of the endocrine cells in the stomach of the Korean hedgehog(Erinaceus korean us) (고슴도치 위점막의 내분비세포에 관한 면역조직화학적 연구)

  • The gastric endocrine cells of the Korean hedgehog, Erinaceus korean us were studied immunohistochemically. Seven kinds of endocrine cells-, gastrin-, somatostatin-, 5-HT-, glucagon-, BPP-, motiIin-and GIP-immunoreactive cells- were identified in this study. The chracteristic findings of the regional distribution and relative frequency of them were examined. Gastrin-immunoreactive cells were very numerously detected only in the pyloric region. Somatostatin-immunoreactive cells were more numerous in the pyloric region than in the cardiac and fundic regions. 5-HT-immunoreactive cells were more numerous in the cardiac and pyloric regions than in the tundic one. Glucagon-immunoreactive cells were found few or rarely in the fundic and pyloric regions. BPP-imunoreactive cells were numerously distributed in the pyloric region, moderately in the fundic region and few in the cardiac region. Motilin-immunoreactive cells were found rarely or few in the fundic and pyloric regions. GIP-immnuoreactive Cells were detected onIy in the pyloric region.

Ontogenetic Development of the Digestive System in Chub Mackerel Scomber japonicus Larvae and Juveniles

  • Chub mackerel, Scomber japonicus, larvae and juveniles were reared from hatching to 35 days after hatching (DAH), and the development of their digestive systems was histologically investigated. The larvae were initially fed on rotifers and Artemia nauplii starting around 19 DAH, and thereafter on Artemia nauplii, fish eggs, and a formulated feed mixture. The primitive digestive system differentiated at 3 DAH; the digestive tract was distinctively divided into the buccopharyngeal cavity, esophagus, stomach, air bladder, intestines, and rectum. The gastric gland and pyloric caeca first appeared at 5 and 7 DAH, respectively. The stomach was divided into cardiac, fundic, and pyloric regions in the preflexion phase. The number of gastric glands and pyloric caeca, as well as the volume of the gastric blind sac increased markedly, with development continuing into the juvenile stage. The precocious development of the digestive system during the larval period might be related to the early appearance of piscivory, which is able to support high growth potential. The organogenesis results obtained for this precocial species represent a useful tool to aid our understanding of the physiological requirements of larvae and juveniles to ensure optimal welfare and growth under aquaculture conditions, which will improve current rearing practices of this scombrid species.

Short-term Results of Surgical Treatment in Esophageal Carcinoma (식도암의 외괴적 조기관찰 성적)

  • Twenty nine adult patients underwent surgical esohpagectomy and one, bypass procedure for documented carcinoma of esophagus and cadiac portion of stomach at Chonnam National University Hospital from Jan 1986 to April 1991. There were several kinds of esophagectomies including through transhiatal, left thoracotomy only, laparotomy and thoracotomy, and laparotomy and right thoracotomy and cervical incision. Twenty five and squamous cell carcinoma and 5, adenocarcinoma. The tumor locations were the upper third in 3, middle third in 12, lower third in 10 and cardiac portion of stomach in 5. After operation, 8[27%] patients were classified in Stage IIa, 6[20%] patients in Stage IIb, 15 patients[50%] in Stage III and one patient in Stage IV. Major postoperative complications included anastomotic narrowing in 3, limited suture line leak in 2, wound infection in 2, hoarseness in 2, pseudomembraneous enterocolitis in 1 and herpes zoster in 1. There was no death within 30 days of operation. Ten months survival was 100% for patients with Stage lIa, 67% for patients with Stage IIb, 50% for patients with Stage III. Furthermore, 20 months survival was 75% in IIIa, 33% in IIb, and 40% in III. But there were no significant differences in survivals among the stage. The actuarial survival is 58% at one year and 41% at two years, The periods of average survival is 589 days after operation.

Surgical Treatment of Esophageal Cancer (식도종양의 외과적 치료)

  • Fourty nine patients out of 127 esophageal cancer were managed surgically from January 1986 to December 1991, at the Department of Thoracic and Cardiovascular Surgery, Jeonbuk National University Hospital. Most frequent preoperative symptom was dysphagia and its mean duration was 3.1 months. In histopathologic examination, squamous cell carcinomas were 44 cases [89.8% , and adenocarcinomas 5 [10.2% . The tumor location were the upper esophagus in 6.1%, middle esophagus in 57.2%, lower and cardiac portion of stomach in 36.7%. Involved and metastatic organs, which were detected perioperatively, were celiac lymph nodes in 6 cases, aorta 2, stomach 2, pericardium 2, cervical lymph node 1. The esophagus was resected radically, and the procedures for esophageal replacement were performed with esophagogastrostomy in 45 cases, esophagocologastrostomy 3, and esophagojejunostomy 1. Postoperative complications occurred in 16 cases [hospital morbidity = 32.6% ,anastomotic leak 3, anastomotic stricture 2, respiratory insufficiency 2, hemoperitoneum 1, chylothorax 1, intussusception 1, empyema 1, non-A,non-B hepatitis 1, and mediastinitis 1. Hospital deaths were experienced 3 cases [ hospital mortality = 6.1% . The 6 month, one, two, and five year actuarial survival rates were 85.7%, 71.4%, 57.1%, and 27.9%, respectively. One year survival rates of stages were 100% in stage I, 90.9% in stage IIa, 63.6% in stage IIb, 25.0% in stage III, and 7.2% in stage IV.

The Structure of Digestive Tract and Histological Features of the Larvae in Sevenband Grouper, Epinephelus septemfasciatus (능성어 (Epinephelus septemfasciatus) 자어의 소화기관 구조 및 조직학적 특징)

  • Histological and morphological development of the digestive tract of sevenband grouper were observed from after hatching to 60 days. Fishes were fed with rotifer (Brachionus rotundiformis) and chlorella (Chlorella ellipsoidea) after hatching from 2 to 20 days rotifer and brine shrimp (Artemia salina) in after 20 days rotifer, brine shrimp and semi-dry artificial diet in after 23 days. Histological and morphological development of ten larvae was observed by paraffin embedding method after fixing in 10% neutral buffered formalin. Sevenband grouper RLG showed characteristics of carnivorous fish by average 0.87. Larvae after hatching can't open the mouth and anus digestive tract was observed in a straight line following yolk sac. Larvae was observed feeding activity by opening the mouth and anus. Metamorphosis started 8 days after hatching. Esophagus divided four layer, and goblet cell was observed in esophagus, mid intestine and rectum. Larvae started cannibalism and it was caused by difference of growth. The inside of stomach was differentiated to cardiac orifice, body of stomach, pyloric stomach, and pyloric caeca. Goblet cell was observed all intestine. Gastric gland differentiated after hatching 28 days in stomach. Secretion of gastric juice was found at stomach and mucosal fold pyloric caeca. Even thought the inside of stomach expended and the number of gastric gland increased consistently and goblet cell in intestine and mucosa became longer, histochemical changes follow couldn't be found during transforming juveniles 38 days after hatching.

Validation of the ACS NSQIP Surgical Risk Calculator for Patients with Early Gastric Cancer Treated with Laparoscopic Gastrectomy

  • Purpose: The American College of Surgeons National Surgical Quality Improvement Program (ACS NSQIP) risk calculator is useful in predicting postoperative adverse events. However, its accuracy in specific disorders is unclear. We validated the ACS NSQIP risk calculator in patients with gastric cancer undergoing curative laparoscopic surgery. Materials and Methods: We included 207 consecutive early gastric cancer patients who underwent laparoscopic gastrectomy between January 2018 and January 2019. The preoperative characteristics and risks of the patients were reviewed and entered into the ACS NSQIP calculator. The estimated risks of postoperative outcomes were compared with the observed outcomes using C-statistics and Brier scores. Results: Most of the patients underwent distal gastrectomy with Roux-en-Y reconstruction (74.4%). We did not observe any cases of mortality, venous thromboembolism, urinary tract infection, renal failure, or cardiac complications. The other outcomes assessed were complications such as pneumonia, surgical site infections, any complications requiring re-operation or hospital readmission, the rates of discharge to nursing homes/rehabilitation centers, and the length of stay. All C-statistics were <0 and the highest was for pneumonia (0.65; 95% confidence interval: 0.58-0.71). Brier scores ranged from 0.01 for pneumonia to 0.155 for other complications. Overall, the risk calculator was inconsistent in predicting the outcomes. Conclusions: The ACS NSQIP surgical risk calculator showed low predictive ability for postoperative adverse events after laparoscopic gastrectomy for patients with early gastric cancer. Further research to adjust the risk calculator for these patients may improve its predictive ability.

Heterotopic bone formation in normal gastric cardiac mucosa (정상 위 분문부 점막에서 발견된 이소성 골 형성)

  • Heterotopic bone formation in the gastrointestinal tract is a rare phenomenon. Most reported cases were associated with benign and malignant neoplasms, except for a case in which heterotopic bone formation was found in a patient with Barrett's esophagus. The exact pathogenesis of the disease has not yet been established. However, most heterotopic bones found in the gastrointestinal tract were associated with mucinproducing tumors of the appendix, colon, and rectum. Inflammation may also play a role in osseous metaplasia in a case with bone formation at the base of an ulcer in Barrett's esophagus. Here, we report on a patient with heterotopic bone formation in normal gastric cardiac mucosa. A 50-year-old female visited our hospital for a routine health examination. She had no gastrointestinal symptoms, and her physical examination, blood test, X-ray, urine, and stool examination results were normal. A 0.3 cm sized polypoid lesion located just below the squamocolumnar junction was observed on upper gastrointestinal endoscopy. A piece of biopsy was taken. Histologically, a lamella bone trabecula and chronic inflammatory cells were observed in the gastric cardiac mucosa. The follow-up endoscopy performed one month later showed no residual lesion.
