Magnetic Resonance Findings of a Canine Benign Uveal Melanocytoma

  • A 13-year-old spayed female Beagle dog was referred with high intraocular pressure, hyperemia, and exophthalmos of the left eye and underwent ultrasound, which revealed a mass in the ciliary body of the left eye. Magnetic resonance (MR) imaging was ordered to evaluate invasion of surrounding structures and metastasis to the brain via the optic nerve. On MR imaging, a single, well-defined, smoothly marginated, triangular-to-oval-shaped mass was found. The mass was hyperintense on T1-weighted images and hypointense on T2-weighted images, similar to a previous case of ocular melanoma. The mass originated from the ciliary body and extended from the anterior chamber to the posterior chamber. Slight enhancement was observed in the mass. There was no evidence of invasion into surrounding structures or the optic nerve, and no sign of metastasis to the brain. The mass was histopathologically diagnosed as benign uveal melanocytoma.

Comparison of bone subtraction CT angiography with standard CT angiography for evaluating circle of Willis in normal dogs

  • Background: Bone subtraction computed tomography angiography (BSCTA) is a useful alternative technique for improving visualization of vessels surrounded by skull bone. However, no studies have compared computed tomography angiography (CTA) and BSCTA for improving the visibility of canine cerebral blood vessels. Objectives: To evaluate the potential benefit of BSCTA for better delineation of brain arteries of the circle of Willis (CoW) in dogs by comparing BSCTA with non-subtraction computed tomography angiography (NSCTA). Methods: Brain CTA was performed for nine healthy beagle dogs using a bolus tracking method with saline flushing. A total dose of 600 mgI/kg of contrast agent with an iodine content of 370 mgI/mL was injected at a rate of 4 ml/s. Bone removal was achieved automatically by subtracting non-enhanced computed tomography (CT) data from contrast CT data. Five main intracranial arteries of the CoW were analyzed and graded on a scale of five for qualitative evaluation. Results: Scores of basilar artery, middle cerebral artery, and rostral cerebral artery in the BSCTA group were significantly higher than those in the NSCTA group (p = 0.001, p = 0.020, and p < 0.0001, respectively). Scores of rostral cerebellar artery (RcA) and caudal cerebral artery (CCA) did not differ significantly between the two groups. However, scores of RcA and CCA in the BSCTA group were higher than those in the NSCTA group. Conclusions: BSCTA improved visualization of intracranial arteries of the CoW with close contact to bone. Thus, it should be recommended as a routine scan method in dogs suspected of having brain vessel disease.

Effect of clomipramine in a dog with cataplexy

  • A 5-year-old, castrated male, Maltese was presented with history of acute flaccid paralysis. The dog was presented with sudden loss of muscle tone and involuntary movements of hind limbs. Neurologic examination revealed reduced postural reaction in the bilateral hind limbs. MRI of brain showed moderate hydrocephalus, but other examination results were normal. Based on the characteristic episodes and examination results, canine cataplexy was suspected. Treatment was initiated with clomipramine as cataplexy control. Clinical signs resolved with 3-month medication. This case demonstrates therapeutic diagnosis of cataplexy. To the author's knowledge, this is the first report of cataplexy treating with clomipramine.

  • Lesch-Nyhan syndrome (LNS) is a rare X-linked recessive disorder caused by a mutation in the hypoxanthine phosphoribosyltransferase 1 (HPRT1) gene. This syndrome is characterized by excessive production of uric acid, mental retardation, self-mutilation, choreoathetosis, and spasticity. The most distinctive symptom is compulsive self-mutilation. For patients with LNS, different methods have been tried to reduce self-biting behaviors including restraints, behavioral treatment, medications, deep brain stimulation, tooth extraction and botulinum toxin A injection. In this report, we present a case of LNS undergoing cheiloplasty due to self-mutilation and tooth extraction of the left deciduous maxillary canine.

개에서 컴퓨터단층촬영을 이용한 두부 외상의 평가 3례 (Computed Tomographic Evaluation of Three Canine Patients with Head Trauma)

  • This report describes the use of conventional computed tomography(CT) for the diagnosis of head trauma in three canine patients. According to physical and neurologic examinations, survey radiography and computed tomography, these patients were diagnosed as traumatic brain injury. Especially, CT is the imaging modality of first choice for head trauma patients. It provides rapid acquisition of images, superior bone detail, and better visualization of acute hemorrhage than magnetic resonance imaging. It is also less expensive and more readily available. Pre-contrast computed tomography was used to image the head. Then, post-contrast CT was performed using the same technique. The Modified Glasgow Coma Scale(MGCS) score was used to predict their probability of survival rate after head trauma in these dogs. Computed tomogram showed fluid filled tympanic bulla, fracture of the left temporal bone and cerebral parenchymal hemorrhage with post contrast ring enhancement. However, in one case, computed tomographic examination didn't delineate cerebellar parenchymal hemorrhage, which was found at postmortem examination. Treatments for patients placed in intensive care were focused to maintain cerebral perfusion pressure and to normalize intracranial pressure. In these cases, diagnostic computed tomography was a useful procedure. It revealed accurate location of the hemorrhage lesion.

닥스훈트견의 바이러스성 뇌수막염에서 1.5T와 7T 자기공명영상을 이용한 진단 증례 (Meningoencephalitis in Dachshund Dog with Canine Distemper Virus Infection: 1.5 T and 7.0 T MRI Findings)

  • 6년령의 암컷 닥스훈트견이 2일전 발생한 발작증상으로 내원하였다. 기본 신체검사, 실험실 검사 및 신경검사를 통해, 뇌 질환에 의한 발작이 의심되었다. 환자는 1.5T와 7T 자기공명영상을 통한 진단이 시도 되었다. T2 영상에서 간뇌 에서 연수 부위까지 광범위한 고신호성 뇌병변이 관찰되었다. 뇌척수액의 중합효소연쇠반응(RT-PCR)을 통하여, 개 디스템퍼 바이러스가 검출되었으며, 이를 근거로, 개 디스템퍼 바이러스에 의한 뇌수막염이 최종 진단 되었다. 지속적인 상태 악화에 의해, 환자는 진단 7일 뒤 보호자의 요청에 의해 안락사가 실시되었다. 사후 부검을 통한 조직검사에서 자기공명영상과 같은 부위의 뇌병변이 확인 되었다. 결론적으로 본 증례는 바이러스에의한 뇌병변의 1.5T와 7T 자기공명영상을 통한 첫 번째 진단 증례 보고 이다.

초급성 뇌경색을 일으킨 개에서 Gd-조영제의 주입이 뇌의 확산에 미치는 영향 (Effect of Gd-DTPA on Diffusion in Canine Brain with Hyperacute Stroke)

  • 목적: 체내에 주입된 Gd-DTPA가 뇌의 확산강조 자기공명영상 신호강도 및 현성확산계수에 미치는 영향을 알아보았다. 대상 및 방법 : 성숙한 잡견 5마리에 대하여 동맥내 도관삽입에 의한 좌측 내경동맥 색전방법을 이용하여 초급성 뇌경색 동물모델을 만들었다. 색전 후 1시간째 확산강조영상을 시행하고, Gd-DTPA를 주입한 다음 다시 90분까지 11회의 추가 확산강조영상을 얻었다. 관심영역을 설정하여 측정한 초급성 뇌경색부위와 반대측 정상부위의 확산강조영상 신호강도 및 현성확산계수를 분석하였다. 결과: 뇌경색은 색전 후 1시간에 시행한 확산강조 자기공명영상에서 잡견 5마리 모두에서 발견되었다. 확산강조영상에서 초급성 뇌경색부위의 신호강도는 Gd-DTPA 주입 여부와 관계없이 시간이 경과함에 따라 증가하였으나, 관류가 유지된 정상부위의 신호강 도는 Gd-DTPA 주입 후 2분에 시행한 첫 검사에서 오히려 저하된 후, 시간경과에 따라 다시 증가하였다. 현성확산계수는 초급성 뇌경색부위에서 Gd-DTPA주입여부에 관계없이 시간 이 경과함에 따라 지속적으로 감소되었으나, 관류가 유지된 반대측 정상부위에서는 변화하지 않았다. 결론: 체내에 주입된 Gd-DTPA는 초급성 뇌경색부위 및 정상부위의 현성확산계수에 영향을 미치지 않으나, 정상부위에서는 조영제 주입 직후 초기의 자화율효과에 의해 확산강조영상의 신호 강도를 저하시켰다. 조영제 주입 후 시행한 확산강조영상 신호 강도의 정량적인 측정이 필요한 연구 혹은 임상 증례에 대하여는 현성확산계수를 측정함으로써 Gd-DTPA의 자화율효과에 의한 영향을 배제하여야 할 것이다.

개의 육아종성 뇌수막척수염 증례 보고 (A case of granulomatous meningoencephalomyelitis in a dog)

  • Granulomatous meningoencephalomyelitis (GME) is a sporadic, idiopathic, non suppurative inflammatory disease of the canine central nervous system. GME appears to have a worldwide distribution and to occur mostly in young to middle-age dogs of small breeds. A 6-year-old female mixed dog with wry neck, ataxia and rolling was submitted to the Cheju National University for diagnosis. Grossly, in the brain, cerebrospinal fluid was mildly increased and dilatation of the subarachnoid blood vessels was observed. Histopathologically, the lesions were characterized by perivascular cuffs of lymphocytes, various numbers of macrophages and plasma cells in the brainstem and cerebral white matter. Numerous granuloma composed of lymphocytes and histiocytes were scattered throughout the brainstem. Two malacic foci characterized by axonal swelling and gitter cell infiltration with hemorrhage were noted in the medulla oblongata and cerebellum. Special stains failed to demonstrate any infectious agents. Immunohistochemically, the infiltrated cells demonstrated strong positive reactions for CD3, a marker for T lymphocytes origin. Based on the clinical signs, histopathology, and immunohistochemistry, this case was diagnosed as GME in a mixed dog.

3T 능동차폐형 자기공명영상 장비로부터 얻어진 개의 자기공명영상 (Canine MR Images from 3T Active-Shield MRI System)

  • 목적: 수의 영상진단을 위하여 3T 능동차폐형 자석을 장착한 전신용 자기공명영상장비를 이용하여 고해상도의 개 두뇌, 척추, 복부 및 골반 자기공명영상을 획득하였다. 대상 및 방법: 128 MHz의 공명주파수를 갖는 RF코일을 사용하여 정상 개 및 환축으로부터 스핀에코와 고속 스핀에코 펄스시퀀스를 적용하였다. 전형적인 펄스시퀀스의 매개변수는 512$\times$512 matrix, 20 cm FOV, 3 mm 절편두께, 1 NEX를 사용하였다. 특히 T1 강조영상을 위하여 TR=500 ms, TE=10 혹은 17.4 ms을 사용하였으며, T2 강도영상을 위하여 TR=4000 ms, TE=108 ms을 사용하였다. 결과: 3T의 신호대잡음비는 기존 1.5T에 비하여 2.7배 정도 향상되었다. 본 연구에서 획득한 고해상도의 자기공명영상은 기존의 20cm FOV, 5m의 절편두께와 256$\times$256 해상도의 영상에 비하여 4배이상 증가하였다 3T 자기공명영상은 매우 미세한 혈관 구조물을 표출하는데 도움을 주며, 또한 백질과 회질의 상당한 대조도를 제공하여 주었다. 결론: 본 연구결과에서 3T로부터 얻은 자기공명영상은 기존 1.5T 영상에서 얻은 영상에 비하여 더 높은 해상도와 민감도를 제공하여 주었다. 3T 고자장 자기공명영상에 나타난 증가된 신호대잡음비는 생체 조직단위의 영상을 획득하는데 유용하였다. 이러한 고해상도의 자기공명영상은 비침습적인 방법으로서 미세조직의 이상유무를 진단하는데 있어서 향후 더욱 임상에 도움을 주리라 예상한다. 향후 자기공명영상은 수의 진단방사선분야에 새로운 장을 열어줄 수 있으리라 기대한다.
