Screening Colonoscopy Participation in Turkish Colorectal Cancer Patients and their First Degree Relatives

  • Background: This study aimed to research the awareness of screening colonoscopy (SC) among patients with colorectal cancer (CRC) and their relatives. Methodology: A questionnaire form including information and behavior about colonoscopic screening for CRCs of patients and their first-degree relatives (FDRs) was prepared. Results: A total of 406 CRC patients were enrolled into the study, with 1534 FDRs (siblings n: 1381 and parents n: 153). Positive family history for CRC was found in 12% of the study population. Previous SC was performed in 11% of patients with CRC. Mean age of the patients whose FDRs underwent SC was lower than the patients whose FDRs did not (52 vs 57 years; p<0,001). The frequency of SC in FDRs was 64% in patients diagnosed CRC under 35 years of age. Persons having a positive family history of CRC had SC more often (51 vs 22%, p<0,001). FDRs of patients having a higher educational level and income had SC more frequently. Conclusions: When screening for CRC is planned, elderly subjects, those with family history for CRC, and those with low educational and lower income should be given esspecial attention in order that they be convinced to undergo screening for CRC.

  • 연구 배경: 자궁경부암은 조기검진을 통해 사망률과 의료비 부담을 감소시킬 수 있는 질환으로, 아직까지 우리나라에서는 자궁경부암이 여성암 중 높은 발생률을 보이고 있으나, Pap test를 통해 자궁경부암을 조기에 발견하고, 치료할 수 있는 것으로 알려져 있다. 연구 목적: 이 연구는 자궁경부암 조기검진 수검행동에 영향을 미치는 인지-행동적 요인을 고려하여 행동변화단계별 교육 프로그램을 개발하였고, 이를 40세 이상의 여성 주민에게 적용해봄으로써 행동변화단계별 맞춤형 교육 프로그램의 효과를 평가하고자 하였다. 연구 방법: 이 연구는 서울시 서초구에 거주하는 40세$\sim$59세 사이의 여성 주민을 대상으로 2003년 9월 1일부터 11월 14일까지 자료를 수집하였으며, 서초구의 5개 지역(동)에서 교육 중재군(162명)과 대조군(121명)을 선정하였다. 또한 행동변화단계별 교육 프로그램은 범이론적모형(TTM)을 활용하여 개발되었고, 교육 중재군의 교육내용 전달방법으로 인터넷 서비스와 우편 서비스가 활용되었다. 연구 결과: 조사대상자중 88.9%가 일생 중 1번 이상, 65.4%가 최근 2년 동안 1번 이상 자궁경부암 조기검진을 수검한 것으로 나타났다. 행동변화단계별 교육 프로그램의 효과를 살펴보면, 교육 후 인지 행동적 요인 중 태도와 변화의 과정이 통계적으로 유의하게 높아졌고, 행동변화단계 중 유지단계가 가장 많이 증가하였다. 교육내용 전달 방법은 인쇄된 교육자료를 활용한 우편 서비스가 인터넷 서비스보다 효과적인 것으로 나타났고, 행동변화단계별 전환 양상에서도 우편 서비스 중재군이 인터넷 서비스 중재 군보다 긍정적인 변화를 나타내었다. 한편 행동변화단계별 교육 프로그램에 관한 만족도 평가에서는 전달방법에 따른 유의한 차이가 없는 것으로 나타났다. 결론: 이 연구는 지역사회 여성 주민의 자궁경부암 조기검진에 관한 수검 행동을 증진시키기 위해 행동변화단계에 따른 맞춤형 교육 프로그램을 개발하여 적용하였고, 그 결과, 행동변화단계별 교육 프로그램이 자궁경부암 조기검진의 수검 행동을 증진시키는데 효과적인 것으로 나타났다.

가임기 여성의 유방암 검진행위 영향요인 (Factors Affecting Breast Cancer Screening Behavior among Women of Childbearing Age)

  • 본 연구는 가임기 여성의 유방암 검진행위 영향요인을 규명하기 위해 시행된 서술적 조사연구이다. 연구대상자는 19-40세의 가임기 여성 2,000명으로, 자료수집은 2020년 9월부터 2021년 8월까지 온라인 및 모바일로 설문조사로 진행되었다. 유방암 검진행위 영향요인을 규명하기 위해 시행된 다중 순서형 로지스틱 회귀분석 결과, 나이가 20-29세 (OR=.2.145, CI=1.219-3.777), 30세 이상(OR=5.663, CI=2.784-11.521), 연소득이 1000만원 미만(OR=1.606, CI=1.070-2.413), 3,000만원 이상(OR=2.422, CI=1.550-3.785), 유방암 가족력(OR=2.421, CI=1.154-5.080), 난소암 가족력(OR=4.321, CI=1.382-13.516), 주관적 건강상태 인식이 '보통'(OR=1.466, CI=1.064-2.020), '건강하지 않음'(OR=1.854, CI=1.188-2.895)인 경우 가임기 여성의 유방암 검진행위가 증가한 것으로 나타났다. 따라서 본 연구를 바탕으로 가임기의 젊은 여성군의 유방암 검진행위를 강화시킬 수 있는 방안이 마련되어야 할 것으로 생각된다.

The Extension of the RISP Model with Behavioral Intention regarding Breast Cancer Screening

  • This study aims to analyze multiple predictors of information seeking behavior, including information seeking intention about breast cancer, using the risk information seeking and processing (RISP) model. Data from 680 female respondents shows that when individuals perceived the risk of breast cancer, they felt more negative emotions, such as anxiety and nervousness, which increased the need for more information about the risk and tendency to seek information. In addition, information subjective norms were positively related with information seeking intention. When individuals felt they lacked relevant information, their seeking intention increased. Lastly, seeking intention led to breast cancer screening intention.

Factors related to the Performance of Mammography Screening among Women with a Family History of Breast Cancer in Korea

  • Purpose: Early diagnosis is the primary method aimed at controlling breast cancer. The purpose of this study was to analyze some factors affecting the performance of mammography screening among women with a family history of breast cancer in Korea. Methods: This study applied a descriptive design method through structured self-report questionnaires. The Care Seeking Behavior Theory provided a theoretical framework for the study. Factors measured in this study represent demographic, clinical, and psychosocial variables including anxiety, barriers, utility, habits, perception, and facilitators. A total of 212 participants, of at least 20 years old, were sampled from April 8, 2010 to March 31, 2011. The data was analyzed by logistic regression method using the Statistical Package for the Social Science 18.0 software. Results: Of the 212 participants, 122 women (57.5%) went through mammography screening. The results of the analysis showed that (a) age (Odds Ratio [OR] =1.10, p<.001), (b) facilitating influences (OR=1.83, p=.008), (c) perception of mammography importance (OR=1.92, p=.011), (d) barriers to mammography (OR=0.60, p=.031), and (e) utility of mammography (OR=2.01, p=.050) significantly affect mammography screening. Conclusion: The results underscore the impact that psychosocial variables in obtaining mammography have on adherence to screening. Women with a family history of breast cancer should be given accurate information and recommendation about mammography by healthcare provider and a regular source of healthcare.

대장암 조기검진행위와 영향요인 (Factors Influencing of Colorectal Cancer Screening Behavior)

  • 본 연구는 건강신념모델(Health Belief Model, HBM)을 이론적 기틀로 대장암 조기검진행위에 영향을 미치는 요인을 규명하기 위한 서술적 조사연구이다. 만 50세 이상 성인 148명을 대상으로 인구사회학적 특성, 건강신념, 자기 효능감 및 행동의 계기 변수를 포함한 구조화된 설문지를 이용하여 조사하였다. 수집된 자료는 SPSS/WIN 25.0을 이용하여 기술통계, t-test, chi-square test 및 다중로지스틱 회귀분석을 시행하였다. 연구결과 대장암 조기검진행위에 영향을 미치는 요인은 지각된 민감성, 배우자 대장암 검진 경험 및 가족력이었다. 따라서 대장암 수검율 향상을 위해서는 대장암 조기검진의 중요성에 대한 체계적인 교육을 통해 지각된 민감성을 높이는 것이 필요하다. 또한 대상자의 가족력 및 가족단위 검진 주기를 사정하고, 부부의 파트너쉽을 활용한 교육 프로그램 개발이 이루어져야 할 것이다.

Mammography Screening Uptake among Female Health Care Workers in Primary Health Care Centers in Palestine - Motivators and Barriers

  • Background: Early detection remains the cornerstone of breast cancer control in terms of outcome and survival. Thus far the only breast cancer screening method proven effective is mammography. The awareness of female health care workers (HCW) about breast cancer prevention is of vital importance, as their beliefs and behavior may have a major impact on other women. This study was designed to assess mammography screening uptake among female healthcare workers at primary healthcare centers, and to identify the primary motivators and barriers that affect uptake results. Materials and Methods: A cross sectional study design was used to assess mammography screening by 299 female healthcare workers who completed a self-administered questionnaire that assessed demographics, screening uptake, motivators and barriers. Results: The mean age was 46 years (within age of risk). The majority (95.1%) demonstrated adequate knowledge about breast cancer and mammography screening and 50% of the participants reported having at least one mammogram; however only 21% of them had regularly scheduled mammograms. The most frequent reported motivator was the perceived benefit that early detection of breast cancer is important for its management (89.6%), followed by the belief that mammography can detect breast cancer before its symptoms appear (84.4%). On the other hand, the most frequent barrier reported was being busy (46.7%), followed by the lack of perceived susceptibility (41.5%). Conclusions: Mammography screening was found to be sub-optimal in a population of HCW's with 50 % stating that they received a mammogram at least once, and a minority reported regular screening. There is a pressing need for educational programs aimed at removing the barriers that limit compliance with recommendations for mammography screening, and to emphasize the importance of early detection in breast cancer treatment. Ensuring the availability and accessibility of screening services, particularly for healthcare workers within their work settings are other important factors that would improve the acceptance and compliance for mammography screening programs.

재미 한인 여성의 암 예방 경험 (Korean-American Women's Experience of Cancer Prevention in the U.S.)

  • Purpose: This study is a qualitative analysis of Korean-American (K-A) women's cancer prevention in the US. Methods: Qualitative research was conducted. Fifteen K-A women in four states were interviewed. Content theme analysis was used to analyze verbatim transcriptions of interviews. Results: Participants experienced difficulties in utilizing cancer screening programs. Factors include unfamiliarity with the US health care system, high health care costs or lack of health insurance, language barriers, and irregular and sporadic cancer screening participation. Participants also actively pursued non-institutional approaches to cancer prevention. They engaged in word-of-mouth informational exchanges in K-A communities, sought cancer screening in hospitals in Korea, conducted internet searches, autonomously decided on their health issues, and adopted healthy practices including better diets, physical exercise, and spiritual practices. Conclusion: It is necessary to implement measures to increase K-A women's utilization of the US cancer screening services and to encourage their active engagement in hands-on cancer prevention practices. K-A women should be empowered through increased familiarity with US cancer screening services and through the establishment of improved K-A community social services.

Factors Affecting Breast Self-examination Behavior Among Female Healthcare Workers in Iran: The Role of Social Support Theory

  • Objectives: In women, breast cancer is the most common cancer and the leading cause of cancer death. Screening tests are the basis for early diagnosis. In Iranian women, the mortality rate of breast cancer is high due to insufficient screening examinations and delayed visits for care. Therefore, this study aimed to determine the factors affecting breast self-examinations among Iranian women employed in medical careers. Methods: This cross-sectional study included 501 women working in the medical professions at Hamadan University of Medical Sciences in western Iran in 2018. The subjects were selected by stratified random sampling. Data were collected using a researcher-developed, self-report questionnaire that contained demographic information and questions based on protection motivation theory and social support theory. Descriptive data analysis was conducted using SPSS version 23 and model fitting with PLS version 2. Results: The mean age of the participants was $37.1{\pm}8.3years$, and most of the women (80.4%) were married. Most women had a bachelor's degree (67.5%). The findings of this study showed that the coping appraisal construct was a predictor of protection motivation (${\beta}=0.380$, p<0.05), and protection motivation (${\beta}=0.604$, p<0.05) was a predictor of breast self-examination behavior. Additionally, social support theory (${\beta}=0.202$, p<0.05) had a significant positive effect on breast self-examination behavior. Conclusions: The frequency of practicing self-examinations among women employees in the medical sector was low; considering the influence of social support as a factor promoting screening, it is necessary to pay attention to influential people in women's lives when designing educational interventions.

Lifestyle Behaviors and Early Diagnosis Practices of Cancer Patients

  • Background: The aim was to determine the lifestyle behaviors and the practices for early diagnosis of cancer of cancer patients. Materials and Methods: A descriptive cross-sectional design was used for this study. The sample consisted of 222 patients with a diagnosis of cancer (non-random sample method). Ethical permission was obtained of the Non-interventional Research Ethics Committee of our Institution. Values of p<0.05 were accepted as statistically significant. Results: It was observed that 54.4% of the patients had never performed breast self-examination, 60.8% had never had a mammography, and 71.2% had never had a Pap smear. Sixty-six point two percent of patients had never had screening for colon cancer within the past ten years. GIS cancers were higher in smokers and ex-smokers (p=0.005), in drinkers and in ex-drinkers (p=0.000). The breast cancer rate was higher in obese people (p=0.019). Conclusions: The results of this study provide information on the healthy lifestyle behavior of cancer patients before their diagnosis, and their use of early diagnosis practices. The important aspect of this study is to extend cancer patients' period of life after the diagnosis and treatment process, to make them conscious of risky lifestyle and nutritional behavior so that they can maintain a high quality of life, and to start initiatives in this direction that would ensure changes in behavior.
