Objectives: This study aimed to investigate the efficacy of ionic and non-ionic-based contrast media (in vitro study) and the combinatorial effect of chitosan-based endo-radiopaque solution (CERS) (in vivo study) for visualization of the root canal anatomy. Materials and Methods: In vitro study (120 teeth): The root canal of maxillary premolars and molars (in vitro group 1 and 2 respectively, n = 60 each) were analyzed using 4 different contrast media (subgroups: Omnipaque 350, Iopamidol, Xenetix 350, and Urografin 76; n = 15 each) in combination with 5.25% sodium hypochlorite (NaOCl). Based on the results of the in vitro study, in vivo study (80 teeth) was done to compare Xenetix 350 + 5.25% NaOCl with CERS (in vivo group 1 and 2 respectively, n = 40 each) on maxillary and mandibular premolars and molars. Two endodontists used radiovisiography to assess the depth of ingress and identify the aberrant root anatomy after access cavity preparation, and after initial cleaning and shaping of canals. Kruskal-Wallis test was used for in vitro comparison (p < 0.05), and Wilcoxon signed-rank test and Mann-Whitney U test for in vivo analysis (p < 0.01). Results: In vitro study, Xenetix 350 + 5.25% NaOCl facilitated a significant higher visualization (p < 0.05). For in vivo study, CERS had a statistically significant depth of ingress (p < 0.01), and was efficient in identifying the aberrant root canal anatomy of premolars and molars. Conclusions: CERS facilitates better visualization of the root canal anatomy of human premolars and molars.
Microorganism survived in the root canal after root canal cleaning and shaping procedure is a main cause of root canal treatment failure. There are several mechanisms for the bacteria to survive in the root canal after chemomechanical preparation and root canal irrigation. Bacteria organized as biofilm has been suggested as an etiology of persistent periapical lesion. Recent studies were focus on removal of Enterococcus faecalis biofilm due to the report that the persistence of this bacteria after root canal treatment may be associated with its ability to form biofilm. Several investigations demonstrated that current root canal treatment protocol including use of NaOCl, EDTA and Chlorhexidine as irrigants is quite effective in eliminating E. faecalis biofilm. However, this microorganism still can survive in inaccessible areas of root canal system and evade host immune response, suppress immune activity and produce biofilm. Up to date, there is no possible clinical method to completely get rid of bacteria from the root canal. Once the root canal treatment failure occurred, and conventional treatment incorporating current therapeutic protocol has failed, periapical surgery or extraction should be considered rather than prolong the in effected retreatment procedure.
The purpose of this study was to evaluate the canal configuration after shaping by ProFile. ProTaper and K-Flexofile in simulated resin canals with different angles of curvature. Three types of instruments were used: ProFile. ProTaper. K-Flexofile. Simulated root canals. which were made of epoxy resin. were prepared by ProFile. ProTaper with rotary instrument using a crown-down pressureless technique. and hand instrumentation was performed by K-Flexofile using a step-back technique. All simulated. canals were prepared up to size 25 file at end-point of preparation. Pre and post instrumentation images were recorded with Scanner. Assessment of canal shape was completed with Image Analysis program. Measurements were made at 1. 2. 3. 4. 5. 6. 7. 8. 9 and 10mm from the apex. At each level. outer canal width. inner canal width. total canal width. and amount of transportation from original axis were recorded. Instrument deformation and fracture were recorded. Data were analyzed by means of one-way ANOVA analysis of variance and the Sheffe's test. The result was that ProFile and ProTaper maintain original canal shape regardless of the increase of angle of curvature than K-Flexofile. ProFile show significantly less canal transportation and maintained original canal shape better than ProTaper.
Objectives: This study was conducted to evaluate the effects of traditional and contracted endodontic cavity (TEC and CEC) preparation with the use of Reciproc Blue (RPC B) and One Curve (OC) single-file systems on the amount of apical debris extrusion in mandibular first molar root canals. Materials and Methods: Eighty extracted mandibular first molar teeth were randomly assigned to 4 groups (n = 20) according to the endodontic access cavity shape and the single file system used for root canal preparation (reciprocating motion with the RCP B and rotary motion with the OC): TEC-RPC B, TEC-OC, CEC-RPC B, and CEC-OC. The apically extruded debris during preparation was collected in Eppendorf tubes. The amount of extruded debris was quantified by subtracting the weight of the empty tubes from the weight of the Eppendorf tubes containing the debris. Data were analyzed using 1-way analysis of variance with the Tukey post hoc test. The level of significance was set at p < 0.05. Results: The CEC-RPC B group showed more apical debris extrusion than the TEC-OC and CEC-OC groups (p < 0.05). There were no statistically significant differences in the amount of apical debris extrusion among the TEC-OC, CEC-OC, and TEC-RPC B groups. Conclusions: RPC B caused more apical debris extrusion in the CEC groups than did the OC single-file system. Therefore, it is suggested that the RPC B file should be used carefully in teeth with a CEC.
The final preparation (MAF) size in infected root canals is still controversial. Nonetheless, recent studies demonstrated that larger apical preparation sizes produces a greater reduction in remaining bacteria and dentinal debris as compared to smaller apical preparation sizes. Therefore, clinicians should be practiced with treatment strategies guided by evidence-based information, especially in infected/failed root canals.
The purpose of this study was to compare and evaluate the shaping abilities of various hybrid instrumentation method using constant tapered file systems with $ProTaper\^{(R)}$ S1 and the difference between experts and inexperienced clinicians in use of NiTi file. Three hybrid methods used in this study were composed of $ProTaper\^{(R)}\;S1\;and\;K-Flexofile\^{(R)}\;(group S),\;ProTaper\^{(R)}\;S1\;and\;HeroShaper\^{(R)}\;(group\;H),\;and\;ProTaper\^{(R)}\;S1\;and\;ProFile\^{(R)}\;(group\;P)$respectively. The $ProTaper\^{(R)}$-alone method (group C) was introduced as a control group. After canal preparation, the lapse of time was recorded. The images of pre- and post-operative canal were scanned and superimposed. Amounts of instrumented canal widths and centering ratio were measured at apical 1, 2 and 3 mm levels and statistical analysis was performed In this study. both of the group C and S took more time to prepare canals than other groups, Inexperienced operators required more time for the entire preparation with the groups C and H than the experienced (p<0.05). And the centering ratio of group P were preferable to $ProTaper\^{(R)}$-alone method or the hybrid technique using stainless steel files. As such, within experienced operators, group H also showed better results in addition to the group P. Under these condition, the hybrid methods of each the $ProFile^{(R)}$ system and $HeroShaper^{(R)}$ with ProTaper are recommendable comparative to $ProTaper\^{(R)}$-alone method. According to the results, the hybrid instrumentation method is a more appropriate method of canal preparation than single file system for narrow or curved canals.

The purpose of this study was to evaluate the influence of glide path on canal centering ratio after instrumentation with different single file systems; WaveOne and Reciproc. Reciproc R25 (VDW), WaveOne Primary (Dentsply Maillefer) and PathFile #13, 16, 19 (Dentsply Maillefer) were used in this study. In no glide path groups, Reciproc files and WaveOne files used for canal preparation without glide path. In glide path groups, the PathFile were used before canal preparation. Methylene blue dye was introduced into the canal to obtain a clear pre-instrumentation image. Pre-instrumentation images and post-instrumentation images were scanned using Epson Perfection V700 Photo scanner (Epson, Nagano, Japan). Transparencies of post-instrumentation images were changed and superimposed on pre-instrumentation images using Adobe Photoshop CS 3 (Adobe Systems Incorporated, San Jose, CA, USA). The centering ratio was calculated for each instrumented canal using the following formula: CR=|X1-X2|/Y. It was statistically analyzed using two-way ANOVA at 95% confidential level. The centering ratio in glide path groups were significant less than it in no glide path groups at 3, 4, 5 and 6 mm level. Except 1 and 6 mm level, WaveOne groups had significant less centering ration than Reciproc groups. At 6 mm level, there was no significant difference between WaveOne and Reciproc. In the limitation of this study, creation of a previous glide path before reciprocating motion instrumentation in curved canal appears to be appropriate and WaveOne system can be used for preparation of curved canal without severe aberrations.

For the successful endodontic treatment, root canal should be cleaned thoroughly by accurate mechanical and chemical canal preparation and sealed completely with canal filling material without damaging the periapical tissues. The accuracy of the root canal length measurement is a prerequisite for the success of the endodontic treatment, and the root canal length is often determined by the standard periapical radiographs and digital tactile sense. In this study, the accuracy and the clinical usefulness of Digora/sup (R)/, an intraoral digital imaging processor and the conventional standard radiographs were compared by measuring the length from the top of the file to the root apex. 30 single rooted premolars were invested in a uniformly sized blocks and No.25 K-file was inserted into and fixed in each canal. Each block was placed in equal distance and position to satisfy the principle of the bisecting angle and paralleling techniques and Digora/sup (R)/ system's image and standard periapical radiographs were taken. Each radiograph was examined by 3 different observers by measuring the length from top of the file to the root apex and each data was compared and analyzed. The results were as follows; 1. In the bisecting angle technique, the average difference between the Digora/sup (R)/ system and standard periapical radiograph was 0.002 mm and the standard deviation was 0.341 mm which showed no statistically significant difference between the two systems(p>0.05). Also, in the paralleling technique, the average difference between these two system was 0.007 mm and the standard deviation was 0.323 mm which showed no statistically significant difference between the two systems(p>0.05). 2. In Digora/sup (R)/ system, the average difference between the bisecting angle and paralleling technique was -0.336 mm and the standard deviation was 0.472 mm which showed a statistically significant difference between the two techniques(p<0.05). Also, in the standard periapical radiographs, the average difference between the bisecting angle and paralleling technique was 0.328 mm and the standard deviation was 0.517 mm which showed a statistically significant difference between these two techniques(p<0.05). 3. In Digora/sup (R)/ system and the standard periapical radiographs. there was a statistically significant difference between the measurement using the bisecting angle technique and the actual length(p<0.05), But there was no statistically significant difference between the measurement using the paralleling technique and the actuallength(p>0.05). In conclusion. the determination of the root canal length by using the Digora/sup (R)/ system can give us as good an image as the standard periapical radiograph and using the paralleling technique instead of the bisecting angle technique can give a measurement closer to the actual canal length. thereby contributing to a successful result. Also. considering the advantages of the digital imaging processor such as decreasing the amount of exposure to the patient. immediate use of the image. magnification of image size. control of the contrast and brightness and the ability of storing the image can give us good reason to replace the standard periapical radiographs.
The purpose of this study was to compare the canal configuration after shaping by ProTaper rotary files and ProTaper hand files in resin simulated canals. Forty resin simulated canals with a curvature of J-shape and S-shape were divided into four groups by 10 blocks each Simulated root canals in resin block were prepared by ProTaper rotary files and ProTaper hand files using a crown-down pressureless technique All simulated canals were prepared up to size $\#25$ file at end-point of preparation. Pre- and post-instrumentation images were recorded with color scanner. Assessment of canal shape was completed with an image analysis program. Measurements were made at 0, 1, 2, 3, 4, 5, 6 and 7 mm from the apex. At each level, outer canal width, inner canal width, total canal width, and amount of transportation from original axis were recorded. Instrumentation time was recorded. The data were analyzed statistically using independent t-test. The result was that ProTaper hand files cause significantly less canal transportation from original axis of canal body and maintain original canal configuration better than ProTaper rotary files, however ProTaper hand files take more shaping time.
Er: YAG laser irradiation utilizing the newly developed RCLase side-firing spiral tip was used for the cleansing of root canals following their bio-mechanical preparation with ProTaper Ni-Ti files. The distal and palatal roots of 20 freshly extracted molar teeth were instrumented to size F3 with ProTaper files. In the experimental group (10 teeth) the pulp chamber and the root canals were filled with EDTA 17% and the root canals were lased for 30 s using the Er: YAG laser irradiation at 600 mJ per pulse and a frequency of 12 Hz. In the control group (10 teeth) the root canals were not lased. Scanning electron microscope analysis showed clean wall surfaces of the lased root canals with open dentinal tubules, free of smear layer and debris. In the scanning electron microscope photographs of the walls of the non-lased root canals a considerable amount of debris could be detected. It appears that an efficient cleansing of the root canal system can be achieved by using the Er: YAG laser irradiation with the RCLase Side-firing Spiral Tip following bio-mechanical preparation of the root canal with Ni-Ti Taper files.
