Oral chemical burns caused by topical application of policresulen: a case report

  • Oral mucosal burns can occur after contact with various chemical agents, and commonly manifest as areas of mucosal sloughing and ulceration. Policresulen (Albothyl, Celltrion Pharm Inc.) is an over-the-counter topical antiseptic that is frequently used to treat stomatitis. Policresulen solution is highly acidic, with an approximate pH of 0.6; it can thus cause mucosal injury when improperly applied in the oral cavity. Here, we present a rare case of an oral mucosal burn resulting from incorrect self-administration of policresulen and emphasize the importance of increasing understanding of this adverse drug event among consumers and health professionals.

산업장 화학 손상 환자에서 중증 화상에 영향을 미치는 요인 (Factors Associating Major Burn in Chemical Injury Patients due to Industrial Place Incident : A Retrospective study)

  • 본 연구는 유해 화학 물질 방출에 의해 손상 받은 환자들을 대상으로 중증 화상에 관하여 연관된 요인들의 강도를 밝힘을 목적으로 하였다. 2010년 1월 1일부터 2014년 12월 31일 까지 화학 손상으로 본원 응급실을 방문했던 총 446명의 환자를 조사 하였다. 중증 화상은 화학 손상의 중증도를 대변하는 종속변수로 사용되었다. 중증 화상과 관련이 있는 독립변수들을 밝혀내기 위한 통계 방법으로 카이 제곱 검정(Chi-square test, CST)과 이분형 로지스틱 회귀분석(Binary logistic regression test, BLRA)이 사용되었다. CST상, 중증 화상에 관하여 여성과 사고 현장에 있던 군, 다발 부위 손상 군이 동시에 연관성을 보였다(p<0.05). BLRA상 사고 현장에 있던 군과 폭발 군에 비하여 누출 군과 입원 군에 비하여 퇴원 군이 동시에 중증 화상과 관련 있었다(p<0.05). 본 연구에서는 사고 현장에 존재 여부가 중증 화상에 관하여 가장 중요한 요인 이었다. 뿐만 아니라 성별과 손상의 개수, 사고 노출 기전(폭발에 비해 누출), 응급실 진료 후 배치(입원에 비해 퇴원)가 또한 중증 화상과 관련이 있었다.

  • Autologous microvascular breast reconstruction is widely accepted as a key component of breast cancer treatment. There are two basic donor sites; the anterior abdominal wall and the thigh/buttock region. Each of these regions provides for a number of flaps that are successfully utilised in breast reconstruction. Refinement of surgical technique and the drive towards minimising donor site morbidity whilst maximising flap vascularity in breast reconstruction has seen an evolution towards perforator based flap reconstructions, however myocutaneous flaps are still commonly practiced. We review herein the current methods of autologous microvascular breast reconstruction.

발열의류로 인한 화재위험성 및 저온화상에 대한 분석 연구 (A Study of the analysis on the risk of ignition and low-temperature burns caused by the use of electrically heated clothes)

  • 연구목적 : 본 연구는 최근 많이 이용하고 있는 발열조끼에 의한 저온화상과 화재위험성 도출을 목적으로 한다. 연구방법 : 사용전압보다 더 높은 전압에 발열조끼를 연결하는 실험을 실시하였다. 연구결과 : 재현실험한 결과 높은 저온화상과 발화위험성을 알 수 있었다. 또한 같은 전압을 인가했을 때 상대적으로 열선의 저항값이 더 낮은 발열조끼 제품이 더 높은 온도에 도달한다는 것도 알 수 있었다. 결론 : 발열조끼는 온도조절장치나 타이머가 존재하지 않아 온도가 급격히 올라갈 경우 안전장치가 없다는 문제점이 있다. 따라서 화재나 저온화상의 위험성을 줄이기 위해서는 열선의 최소저항값 기준을 설정하고, 낮은 저항값을 가지는 열선 사용의 규제가 필요하다.

Early Intervention for Low-Temperature Burns: Comparison between Early and Late Hospital Visit Patients

  • Background Various focal heating devices are popular in Korea under the cultural influence of the traditional ondol under-floor method of home heating. These devices can cause severe burn-like injuries resulting from device malfunction or extended with low heat contact. In addition to injuries under these high heat contact, burns can be occurred by low heat exposure with prolonged periods despite the devices are properly functioning. In order to develop strategies to reduce the duration of periods of illness due to low-temperature burns, we analyzed and compared treatment methods and therapeutic periods for this type of injury. Methods This retrospective study included 43 patients burned under low heat conditions. Patients were divided into an operative group and a conservative group. The patients in the operative group underwent at least one surgical excision, and were further subdivided into early and late visit groups. The conservative group was treated only with dressings. We compared the treatment periods between the operative group and the conservative group, and also compared the preparation periods and treatment periods between the two operative groups. Results The average treatment period was significantly shorter in the operative group (P=0.02). In the early visit operative group, both wound preparation and treatment were briefer than in the late visit group. Conclusions We recommend that early proper burn care and early surgical intervention, including appropriate excision, are feasible ways to reduce the treatment period of lowtemperature burn patients.

Toe Tissue Transfer for Reconstruction of Damaged Digits due to Electrical Burns

  • Background : Electrical burns are one of the most devastating types of injuries, and can be characterized by the conduction of electric current through the deeper soft tissue such as vessels, nerves, muscles, and bones. For that reason, the extent of an electric burn is very frequently underestimated on initial impression. Methods : From July 1999 to June 2006, we performed 15 cases of toe tissue transfer for the reconstruction of finger defects caused by electrical burns. We performed preoperative range of motion exercise, early excision, and coverage of the digital defect with toe tissue transfer. Results : We obtained satisfactory results in both functional and aesthetic aspects in all 15 cases without specific complications. Static two-point discrimination results in the transferred toe cases ranged from 8 to 11 mm, with an average of 9.5 mm. The mean range of motion of the transferred toe was $20^{\circ}$ to $36^{\circ}$ in the distal interphalangeal joint, $16^{\circ}$ to $45^{\circ}$ in the proximal interphalangeal joint, and $15^{\circ}$ to $35^{\circ}$ in the metacarpophalangeal joint. All of the patients were relatively satisfied with the function and appearance of their new digits. Conclusions : The strategic management of electrical injury to the hands can be both challenging and complex. Because the optimal surgical method is free tissue transfer, maintenance of vascular integrity among various physiological changes works as a determining factor for the postoperative outcome following the reconstruction.

흰쥐의 피부화상 후 저강도 레이저 조사가 Substance P의 발현에 미치는 영향 (Effects of Low Power Laser for the Expression of Substance P in the Burned Skin of the Rats)

  • This study was performed to investigate the effect of low power laser irradiation on Substance P(SP) expression in the burned skin of the rats. Burns of about 3cm in diameter were created with $75^{\cric}C$ water on the back of the rats, and the lesion of experimental group were irradiated on days 1, 2, 3 and 4 postwounding. Control leasions were not irradiated. After burns, low power laser irradiation was applied by using 1000Hz, 830nm GaAlAs(Gallium-aluminum-arsenide) semiconductor diode laser. The expression of evaluated Substance P(SP) immunohistochemistry on rabbit anti-SP The results of this study wereas follows 1. The Substance P was expressed in the lamina I and II of dorsal horn of spinal cord. In expression of SP, the lesion of control group made SP to more induce significantly than experimental leasions. 2. SP immunoreactivity in burned leasion of spinal cord were decreased markedly 4 days after burns, and decreased gradually from 1 day to 2 days in burns which is laser irradiation These data suggest that low power laser have a pain release effect in the burned skin of the rats.

대구시내 종합병원에 입원한 화상환자의 화상원인과 현장에서 취한 응급처치 (Causes of Burn and Emergency Care on the Spot for the Patients Admitted to Three Hospitals in Taegu)

  • This study was conducted to investigate the causes of burn and emergency cares taken on the spot for the burn patient. Study population included 161 burn patients admitted to 2 university hospitals and 1 general hospital in Taegu from November 1, 1987 to April 30, 1988. Patients or guardians were interviewed with a structured questionnare. Out of 161 burn patients 111(68.9%)were males and 50(31.1%) females. Preschool children of 1-4 years old accounted for 29.8% of the total patients. Burns of children under 15 years of age took place at home in 91.0%, while 48.3% of burns of adult (15 years and over)males occurred at the working place, and 68.0% of adult females occurred at the home. Out of total burns occurred at home 39.8% took place at kitchen/dining room and 24.1% in the room. The most common cause of burns in children was the boiling water or hot food (74.3%). In adults the common causes were electrical burn(22.4%), hot water or food(19.0%) and explosion(12.1%) for males, and hot water or food(32.0%) and explosion (20.0%) for females. Common emergency cares for the burn taken on the spot were undressing(64.6%), pouring Soju(liquor)(13.7%), and pouring cold water(5.0%). There were a few cases who applied ash, soy or salt. To prevent burn, it is recommended to remodel the traditional kitchen and coal-briquet hole, to strengthen the safety control of LP Gas and LN Gas supply, to educate the public for the handling method for such gases, to strengthen the occupational safety control, to improve the safety device for the electric wire and socket, and to limit the temperature of hot water at home and public baths.
