횡문근융해증 이후 나타난 양측 좌골신경병: 증례보고 (Bilateral Sciatic Neuropathy Following Rhabdomyolysis: A Case Report)

  • Rhabdomyolysis is a syndrome caused by injury to skeletal muscles and involves leakage of large quantities of potentially toxic intracellular contents into the plasma. It is known that rhabdomyolysis results in peripheral nerve injury, however, reports of bilateral sciatic neuropathy following rhabdomyolysis are rare. We report a case involving a 42-year-old female patient with no past medical history, who presented with sudden bilateral calf pain, redness, and burning sensation with weakness of both lower extremities after sleeping on an electric heating pad following alcohol drinking. Lower extremity magnetic resonance angiography (MRA) revealed multifocal edema with enhancement of bilateral lower extremity muscles. Clinical and electrodiagnostic tests were consistent with the diagnosis of bilateral sciatic neuropathy following rhabdomyolysis. This is a rare case of bilateral sciatic neuropathy following rhabdomyolysis.

  • Sjögren syndrome is a chronic autoimmune inflammatory disease characterized by lymphocytic infiltration of exocrine glands, predominantly the parotid and lacrimal glands, thereby resulting in oral and ocular dryness. It has been reported to occur most frequently in women between 40 and 50 years of age. Sjögren syndrome has an insidious onset, is slowly progressive, and presents a wide range of clinical manifestations, leading to delays or challenges in the diagnosis. Early diagnosis of this condition is essential to prevent the associated complications that affect patients' quality of life. This report presents 3 cases of Sjögren syndrome in female patients aged between 40 and 75 years who presented with complaints of persistent dry mouth and burning sensation. The cases highlight the diagnostic value of 3-dimensional cone-beam computed tomographic sialography in the detection of salivary gland pathologies at an early stage.

  • A 74-year-old female presented with a complaint of dry mouth, continuous spontaneous burning sensation in the tongue, and asymptomatic submucosal soft tissue mass on both sides of the lower labial mucosa. She refused to undergo total excision of the mass due to concern about the possibility of complications such as nerve damage because of the large size of the mass. As her clinical features and magnetic resonance imaging indicated the possibility of Sjögren's syndrome, a biopsy of the minor salivary gland of the right lower lip was performed. Consequently, she was diagnosed with mucosa-associated lymphoid tissue (MALT) lymphoma. Although the patient had typical signs and symptoms of Sjögren's syndrome, the histopathological result of MALT lymphoma made it impossible to determine whether the patient had a history of Sjögren's syndrome. For patients with risk factors for MALT lymphoma, such as Sjögren's syndrome, a biopsy of the labial minor salivary gland with immunohistochemical staining can be helpful in the diagnosis of not only Sjögren's syndrome but also MALT lymphoma.

심도자 검사 환자를 위한 구체적 감각 정보의 개발 (Development of Concrete Sensory Information for Patients Undergoing Cardiac Catheterization)

  • Cardiac catheterization is a diagnostic procedure which is intrusive and anxiety provoking. Patient education and information offer have been suggested as effective nursing interventions to reduce patients' anxiety and stress. Main objectives of this study are : 1) to develop concrete objective information for patients undergoing cardiac catheterization : 2) to analyze the types of information included in the newly developed concrete objective information. The subjects of this study are 11 patients who were admitted to the CCU of a general hospital in Seoul. The subjects were provided with preparatory information about cardiac catheterization by using interview and a booklet. After the procedure, the subjects were asked to describe additional information which they suggest to be added into the booklet and the physical sensations felt during the procedure. The results of the study are summarized as follows : Most subjects were satisfied with the preparatory information that was provided by interview and the booklet before the procedure. But patients suggested several problems related to the content of the booklet. First, they reported difficulty to differentiate the terms -coronary angio gram and cardiac catheterization. Also, some patients expressed that they feared after reading the information about the incision of inguinal area. Subjects responded that the information about the direct process of the test did not reduce their anxiety and the information was not detail enough. Next, most subjects would want to know about the monitor and the sound from monitor. They said that they could not hear instructions from doctor or nurse during the procedure due to tension. Considering above response results, the need for more effective way to provide information, like visual and auditory information through video tape for giving information is suggested. Sensations related to the procedure were the smell coming from sterilization of inguinal area, stinging pain in groin when the doctor inserts a needle into artery, and the sensation of pressure and moving of vessels surrounding neck when the catheter was inserted and visualized on fluoroscopy. Besides, subject reported hot sensation and burning feeling in face and chest area, and nausea when dye is injected by hand. In the analysis of information content, there was 79% agreement on the actual units of analysis that were coded. In the analysis of type of information, procedural information was 60.4%. Concrete objective information was 28.1%. and other information was 11.5%. Agreement of the coders in categorizing the units of information was determined by using Cohen's kappa which corrects for chance agreement. Cohen's kappa was .84.

熱氣熏法 및 침, 뜸, 한약 치료가 帶下를 주소로 내원한 환자의 증상변화에 미치는 영향에 대한 연구 (Study on Therapeutic Effect of KMST (Korean Medicine Steam Therapy) -Included Korean Medicine Combination Therapy about Leukorrhea Patients)

  • Objectives This study aimed to investigate therapeutic effect of Korean Medicine Steam Therapy (KMST) for leukorrhea patients. Methods From December 1st 2013 to Nov 31st 2014, 16 leukorrhea outpatients treated with KMST filled out questionnaire before and after treatments. The questionnaire was composed of 8 question items such as sleep, appetite & digestion, urination, defecation, emotion, cold hypersensitivity, menstrual pain, leukorrhea and patients were told to evaluate their symptoms as NRS score. We analyzed mean NRS score of each question item before and after KMST by using Wilcoxon's signed rank test of SPSS ver. 19. Results Mean age of participants was 40.8±13.0 years and mean value of treatment numbers per person was 3.8±1.8 times. 43.75% (n=7) of the patients had history of vaginitis caused by gardnella vaginalis, ureaplasma urealyticum or candida albicans. All patients were treated with acupuncture, moxibustion and Korean medicine. In addition, there was no side effect reported by participants. NRS score of quantity, odor of leukorrhea, perineal unpleasant sensation such as itching, dryness, burning sensation remarkably decreased (p<0.01, p<0.05, p<0.01 each). NRS score of symptoms including sleep, urination, cold hypersensitivity, menstrual pain also remarkably decreased (p<0.05). Conclusions Symptoms of leukorrhea patients including sleep, urination, cold hypersensitivity, menstrual pain, leukorrhea showed significant decrease after treated with concurrent KMST.

구강작열감증후군의 치료를 위한 전신적 클로나제팜의 투여 시 환자의 임상적 특징에 따른 효능의 차이에 관한 연구 (Clinical Features Affecting the Efficacy of Systemic Clonazepam for Management of Burning Mouth Syndrome)

  • 구강작열감증후군은 구강점막의 만성통증성 작열감이다. 구강작열감증후군의 치료로는 약물치료, 심리치료가 있다. 과거의 연구를 통해 캡사이신, 알파-리포산, 도포용 클로나제팜, 전신적 클로나제팜이 구강작열감증후군 증상의 감소에 효과적임이 보고되었다. 이 연구의 목적은 구강작열감증후군 환자에게 전신적 클로나제팜을 투여하여 치료 효과를 평가하고 약제의 효능과 연령, 통증의 강도, 통증의 기간, 과거의 치과병력, 구강점막 상태 등의 다양한 임상적 특징과의 관계를 밝히는 것이다. 이 연구는 2011년 1월과 2012년 8월 사이에 구강작열감증후군으로 진단된 환자를 대상으로 후향적 임상자료 분석을 통해 이루어졌다. 환자들에게 0.5mg 클로나제팜을 하루 2회 처방하였다. 통증은 전신적 클로나제팜의 투여 전과 투여 1-2주 후에 11-점 숫자등급척도(11-point numeric rating scale, NRS)를 통해 측정되었다. 클로나제팜의 효능은 환자의 연령, 초기 통증 강도, 통증 지속기간, 유발인자의 존재유무, 혀의 상태, 의치착용 유무에 따라 평가되었다. 총 50명의 환자(여성 46명, 남성 4명)가 연구에 참여하였다. 환자들은 상기의 임상적 특징에 따라 2-3개의 군으로 나누었다. 강한 초기 통증 환자들의 평균 NRS 감소량은 $3.33{\pm}2.74$인 반면에 경도 혹은 중등도 초기 통증 환자들의 평균 NRS 감소량은 $1.64{\pm}1.54$였다. 70세 이상의 가장 높은 연령의 환자군의 평균 NRS 감소량은 $3.53{\pm}1.94$였으며, 60세 이하 환자군에서는 $2.88{\pm}1.80$, 60세에서 70세 사이 환자군에서는 $1.54{\pm}2.86$의 감소량을 보였다. 결론적으로 높은 연령의 환자들과 강한 초기 통증을 가진 환자들에게서 클로나제팜의 효능이 뛰어난 경향이 있다. 그러나 통증 기간, 유발인자 유무, 열구설 유무, 의치 장착 유무에 의한 클로나제팜의 효능의 차이는 통계학적 유의성이 없었다.

The Use of Locally Applied Vibration to Minimize Pain during Fractional CO2 Laser Therapy in Living Liver-Donor Scar Management

  • Background Fractional $CO_2$ laser is an effective treatment for scars, but most patients complain about sharp burning pain, even after the application of lidocaine ointment. This study analyzed the impact of a vibrating device to nonpharmacologically reduce the acute pain of laser treatment, in accordance with the gate control theory of pain management. Methods This is a prospective study performed from May 2013 through March 2014. Fifty-three patients (mean age, 26.7 years; range, 16-44 years) who had donated livers for liver transplantation were treated with a fractional $CO_2$ laser (10,600 nm; model $eCO_2$, Lutronic Corp) for their abdomen scars. Laser treatment was applied 4 months after surgery. A commercially available, locally applied vibrating device (model UM-30M, Unix Electronics Co. Ltd.) was used, in an on-and-off pattern, together with the $CO_2$ laser. A visual analogue scale (VAS; 0, no pain; 10, most severe pain) of pain sensation was assessed and statistically analyzed using a paired t-test. Results The average VAS score for pain with the vibrating device was 4.60 and the average VAS score without the vibrating device was 6.11. The average difference between scores was 1.51 (P=0.001). Conclusions A locally applied vibrating device was demonstrated to be effective in reducing pain when treating with a fractional $CO_2$ laser. Vibration treatment could be helpful when treating scars with fractional $CO_2$ laser in pain-sensitive patients, particularly children.

Capsaicin이 치주 치료시 통증에 미치는 영향 (ANALGESIC EFFICACY OF CAPSAICIN IN PERIODONTAL THERAPY)

  • The purpose of present study is to assess the effects of capsaicin topically applicated to the chronic periodontal pain suffering area. In the First study, twenty patients with chronic pain caused by mild periodontal disease were selected, and periodontal pack containing capsaicin(PPC) was attached to these patients gingiva around pain suffering area. Then the presence of discomfort had been recorded every ten minutes for the first 1 hour. After 1 hour again, It had been recorded according to the presence of pack and to the existence of pain. In the second study, twenty moderate periodontitis patients were selected. After subgingival curettage of two quadrant area, non-euginol periodontal pack or PPC were attached to the curetted gingival margins of them (Non-euginol pack bearing area and capsaicin pack bearing area is supposed to control group and test group respectively.), and the degrees of pain with time had been recorded eight times with 1 hour interval (at that day) or recorded once in a day (from the next day to the next appointment day). The results are as follows : 1. PPC has caused discomfort accompanied by burning sensation to the mild periodontitis patients with chronic pain. 2. PPC has given little effects to improve the pain after subgingival curettage of moderate periodontitis patients.

산소중독시 가토의 [A-a] gas Gradient 의 변화 (Changes of [A-a] gas Gradient in Rabbits with Oxygen Toxicity)

  • Respiratory care with oxygen inhalation is often a necessity to maintain life, and it is one of the important therapeutic adjuncts in respiratory disease and in intensive care after surgery. However, it has been reported that oxygen toxicity occurs after prolonged exposure to 100% 0, [Smith, 1899; Kistler et al. 1967; Schaffner et al. 1967; Rowland and Newman, 1969. Subjective symptoms of oxygen toxicity include tracheal irritation, frequent cough, some burning sensation in the trachea, tachypnea, severe dyspnea, etc. [Welch, 1963; Fisher et al, 1968; Milier et al, 1970; Clark and Lambertsen, 1971; Sackner, 1975]. Pathologic findings are atelectasis, injuries to the pulmonary capillaries and hemorrhage in the alveoli in gross specimens. There can be inflammation, proliferation of fibrin, thickening of alveolar membranes, degeneration of collagen fibers and interstitial edema in the microscopic findings. [Penrod, 1956; Cedergren, 1959; Bean, 1965; Schaffner, 1967]. Dubois and colleagues [1961] found that the amount of pulmonary surfactant was decreased in oxygen toxicity and atelectasis followed by the decreased pulmonary surfactant. Many authors reported that vital capacity, inspiratory force, pulmonary compliance, pulmonary capillary blood flow and pulmonary elasticity were deceased and arteriovenous shunting increased. [Comroe et al, 1945; Fuson et al, 1965; Kistler et al, 1966; Knowles and Blenner-hassett, 1967; Barber et al, 1978]. Many human volunteers were examined after prolonged exposure in a high oxygenated chamber and there were a few reports on animals with oxygen toxicity, subjects including rabbits. Gas partial pressures of alveoli and arteries were measured in rabbits exposed to 100% $O_2$ and the alveolar-arterial gas gradients were analyzed, which is the basis for the study of oxygen toxicity. These rabbits were divided into two groups; rabbits under natural respiration, and second group under artificial respiration with a respirator. The alveolar $PO_2$ [$P]AO_2$] and $PCO_2$ [$PACO_2$], and the arterial $PO_2$ [$PaO_2$] were measured under varying $O_2$ pressures; 15% $O_2$, 21% $O_2$ and 100% $O_2$.

건강한 성인 지원자를 대상으로 한 필로칼핀 저작정의 타액분비 유도 및 타액중 용출패턴 평가 (Evaluations on Salivary Flow Induction and Dissolution Patterns in Saliva of Pilocarpine Chewing Tablet in Healthy Human Volunteers)

  • Xerostomia is caused by organic or functional changes affecting the salivary system at different levels. Patients suffering from xerostomia may also complain of an oral burning sensation, ulceration or soreness, difficulty in swallowing, and poor denture retention. And pilocarpine is administered orally to induce salivary secretion. In Seoul National University Hospital(SNUH) pharmacy, the pilocarpine chewing tablets are prepared and supplied to patients of xerostomia in request of the dental hospital in SNUH. And we tested the salivary flow induction and the dissolution patterns of these products in saliva by a double-blind, sequential cross-over trials to eight healthy human volunteers with placebo. The pilocarpine chewing tablet contained 5 mg of pilocarpine, and placebo consisted of same materials as test drug, but didn't contain pilocarpine. In vivo experiment, all subjects were instructed to chew as 60-80 times/min. Mixed saliva was collected in the ranges of intervals such as 0-2, 2-5, 5-10, 10-15, 15-20, 20-30, 30-45 and 45-60 min after pilocarpine chewing tablet or placebo administration. Saliva volume was measured in each collecting time interval, and saliva pilocarpine concentrations were determined by reversed phase HPLC. The 82.5 percent $(4.13{\pm}0.69\;mg)$ of pilocarpine was extracted from chewing tablets during mastication of 60-80 times per minute for 60 minutes. Among these dissolved amounts, 90 percent was extracted within 20 minutes. The salivary flow rates were more increased in a group who administered pilocarpine chewing tablet at the interval of 5-10, 10-15, 20-30 and 45-60 min rather than a placebo-group, but only extracted amount of pilocarpine at 45-60 min interval is significanly different between two groups (p<0.05). But total amounts of saliva secreted for 1 hour in two group-pilocarpine and placebo treated- were $46.36{\pm}9.72\;ml\;and\;39.09{\pm}7.81\;ml$, respectively, and were not significantly different between two groups.
