Stevens-Johnson syndrome and abuse of anabolic steroids

  • Stevens-Johnson syndrome (SJS) is characterized by mucocutaneous tenderness and typical hemorrhagic erosions, erythema and epidermal detachment presenting as blisters and areas of denuded skin. SJS is often observed after drug use as well as after bacterial or viral infections. Several drugs are at high risk of inducing SJS, but there are no cases in the English literature regarding anabolic steroid use triggering SJS. In our paper, we describe a case in which use of anabolic androgenic steroids (AAS) was associated with SJS. The patient participated in competitive body-building and regularly took variable doses of AAS. Initial symptoms (headache, weakness, pharyngodynia, and fever) were ignored. After a week he presented to the Emergency Department with a burning sensation on the mouth, lips, and eyes. Painful, erythematous, maculopapular, and vesicular lesions appeared all over the body, including on the genitals. During hospitalization, he also developed a cardiac complication. The patient had not taken any drugs except AAS.

A Case of Treating the Paresthesia Suggested from Peripheral Neuropathy (말초신경병증으로 추정되는 비증(痺證) 치험 1례)

  • Peripheral neuropathy is a disease of multiple Peripheral nerves. Tingling, pins-and needles, numbness, burning and raw sensation are symptoms of peripheral neuropathy. These symptoms are frequently complained by patients who suffered from obstructive syndrome of Ki and blood (痺證). This case was reported to evaluate the effects of oriental medicine therapy on a patient with obstructive syndrome of Ki and blood (痺證). The subject was a female patient who had obstructive syndrome of Ki and blood (痺證). She complained of calf pain, cold sense, walking discomfort, insomnia, dyspepsia, anorexia etc. We administered the medicine with dispeling the cold pathogenic factor (寒邪), adjusting the constructive and defensive energy (調營衛), tonifying the Ki and blood (補氣血) by stage. After the treatment, the symptoms improved. This case suggests that oriental medicine therapy can be applicable to improve in symptoms with Peripheral neuropathy.

A Case Report of VulvoVaginal Candidiasis treated by Dahuanghuanglianxiexin-tang based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 대황황련사심탕(大黃黃蓮瀉心湯)을 투여한 칸디다성 질염 치험 1례)

  • Objective : This case report show the effect of Dahuanghuanglianxiexin-tang on VulvoVaginal Candidiasis. Methods : According to disease pattern identification diagnostic system based on shanghanlun provisions, The author diagnosed Taeyang-byung kyeolhyung. The symptoms of VulvoVaginal Candidiasis were worsen when the patient had long sitting position and digestive problems. Therefore the patient treated with Dahuanghuanglianxiexin-tang for 3 months. The severity of Candida Vaginitis was evaluated by patient's subjective statements(Vaginal pruritus, Vaginal secretion, Burning sensation). Results : After the treatment, Level of syptoms decreased from Severe to Trace and Eliminated. Conclusions : Dahuanghuanglianxiexin-tang improved the symptoms of VulvoVaginal Candidiasis on this study. Although Dahuanghuanglianxiexin-tang is not an usual treatment for VulvoVaginal Candidiasis, the author could prescribe the medicine due to shanghanlun six meridian patterns diagnostic system.

The clinical observation of 1 case of the osteochondritis dissecans (박리성 골연골염환자 1예에 대한 증례보고)

  • Objective : The objective of this study is to observe the effect of anti-inflammatory herbal acupuncture, acupuncture, physical theraphy, external treatment, therapeutic exercise on the osteochondritis dissecans. Methods : Anti-inflammatory herbal acupuncture and acupuncture were used to treat knee pain and gait disturbance. We evaluated the patient through VAS(visual analogue scale) daily and symptoms. Results & Conclusions : After 31 days of treatment, right knee pain was decreased from VAS 10 to VAS 4, redness, burning sensation, edemaright of right knee were decreased. And the patient could walk. Oriental medicine treatment is proved to be helpful to imporve the symptoms of the osteochondritis dissecans.

Screening of Antimicrobial Activity among the Therapeutic Herbal Extracts on Dental Pathogens

  • The periodontal diseases and dental caries are major infectious diseases in oral cavity. Many of the preventive and therapeutic dental products contain the antimicrobial and anti-inflammatory components. But some of these antimicrobial have weak points such as coloration, burning sensation and insolubility in water. We have screened the therapeutic herbal extracts of the Plant Extract Bank for the antimicrobial activity on the major dental pathogens by growth inhibition assay. For the Porphyromonas gingivalis, 8 herbal extracts had an antimicrobial activity, 11 herbal extracts for the Prevotella intermedia, 43 herbal extracts for the Haemophilus actinomycetemcomitans and 61 herbal extracts for the Streptococcus mutans. Among these extracts, 6 herbal extracts had an antimicrobial activity for more than 3 species of dental pathogens. These extracts are Araliae Cordatae Radix, Crassirhizomae Rhizoma, Mori Radicis Cortex, Psoraleae Semen, Pini Ramulus and Sieges- beckiae Herba. All of effective extracts were CA group, ethanol extracts. Among these 6 herbal extracts, only Crassirhizomae is known to have a antibacterial effects. Therefore these herbal extracts have a possibility to be a candidate for a major antibacterial components in dental products.

  • Objective : This study was performed to prove the effect of the oriental medical treatments on urticaria and angioedema caused from serum sickness reaction by bee venom. Method : We treated one case of urticaria and angioedema with oriental medical treatments such as 8 constitution acupuncture, herbal medicine, venesection therapy and facial pack therapy. We took pictures through the treatment to evaluate the improvement. Six items, itching, burning sensation, pain, wheal, redness, edema were used to diagnose urticaria and angioedema. Result & Conclusion : After treatments the urticaria and angioedema were improved. The symptom of itching, heating, pain, wheal, redness of whole body and facial edema disappeared.

Management of traumatic neuralgia in a patient with the extracted teeth and alveoloplasty: a case report

  • A majority of patients who sustain injuries to the peripheral sensory nerves of the face and jaws experience a slow but gradual return of sensation that is functional and tolerable, if not the same as before the injuries. However, long-term effects of such injuries are aggravating for many patients, and a few patients experience significant suffering. In some of these patients, posttraumatic symptoms become pathological and are painful. The predominant painful components are (1) numbing anesthesia dolorosa pain, (2) triggered neuralgiaform pain, (3) burning and aching causalgiaform pain, and (4) phantom pain. This is a case report of conservative management of traumatic neuralgia and neuritis as part of posttraumatic pain syndromes in geriatric patients who have undergone the teeth extraction and alveoloplasty.

Successful Treatment of Severe Sympathetically Maintained Pain Following Anterior Spine Surgery

  • Sympathetic dysfunction is one of the possible complications of anterior spine surgery; however, it has been underestimated as a cause of complications. We report two successful experiences of treating severe dysesthetic pain occurring after anterior spine surgery, by performing a sympathetic block. The first patient experienced a burning and stabbing pain in the contralateral upper extremity of approach side used in anterior cervical discectomy and fusion, and underwent a stellate ganglion block with a significant relief of his pain. The second patient complained of a cold sensation and severe unexpected pain in the lower extremity of the contralateral side after anterior lumbar interbody fusion and was treated with lumbar sympathetic block. We aimed to describe sympathetically maintained pain as one of the important causes of early postoperative pain and the treatment option chosen for these cases in detail.

  • Objective : The purpose of this report is to know the effect of Gamisunbangwhalmyung-Eum(加味仙方活命飮) on pain and burning sensation that last for months after Debridement caused by Cellulitis. Methods : We experienced one case of patient underwent Debridement caused by Cellulitis treated with Gamisunbangwhalmyung-Eum. To evaluate the effectiveness of this treatment, we used the Visual Analogue Scale(VAS). Results : After the treatment, the grade of VAS was decreased and clinical symptoms were improved. Conclusion : Gamisunbangwhalmyung-Eum can be used on the treatment of patient underwent Debridement caused by Cellulitis.

Neuropathic cancer pain: prevalence, pathophysiology, and management

  • Neuropathic cancer pain (NCP) is caused by nerve damage attributable to the cancer per se, and/or treatments including chemotherapy, radiotherapy, and surgery; the prevalence is reported to be as high as 40%. The etiologies of NCP include direct nerve invasion or nerve compression by the cancer, neural toxicity, chemotherapy, and radiotherapy. NCP is subdivided into plexopathy, radiculopathy, and peripheral neuropathies, among several other categories. The clinical characteristics of NCP differ from those of nociceptive pain in terms of both the hypersensitivity symptoms (burning, tingling, and an electrical sensation) and the hyposensitivity symptoms (numbness and muscle weakness). Recovery requires several months to years, even after recovery from injury. Management is complex; NCP does not usually respond to opioids, although treatments may feature both opioids and adjuvant drugs including antidepressants, anticonvulsants, and anti-arrhythmic agents, all of which improve the quality-of-life. This review addresses the pathophysiology, clinical characteristics and management of NCP, and factors rendering pain control difficult.
