Multidrug resistance is a principal mechanism by which tumors become resistant to structurally and functionally unrelated anticancer drugs. Resistance to chemotherapy has been correlated with overexpression of p-glycoprotein (p-gp), a member of the ATP-binding cassette (ABC) superfamily of membrane transporters. P-gp mediates resistance to a broad-spectrum of anticancer drugs including doxorubicin, taxol, and vinca alkaloids by actively expelling the drugs from cells. Use of specific inhibitors/blocker of p-gp in combination with clinically important anticancer drugs has emerged as a new paradigm for overcoming multidrug resistance. The aim of this paper is to review p-gp regulation by dietary nutraceuticals and to correlate this dietary nutraceutical induced-modulation of p-gp with activity of anticancer drugs.
Extensive use of antibiotics over recent decades has led to bacterial resistance against antibiotics, including gentamicin, one of the most effective aminoglycosides. The emergence of resistance is problematic for hospitals, since gentamicin is an important broad-spectrum antibiotic for the control of bacterial pathogens in the clinic. Previous study to identify gentamicin resistance genes from environmental samples have been conducted using culture-dependent screening methods. To overcome these limitations, we employed a metagenome-based culture-independent protocol to identify gentamicin resistance genes. Through functional screening of metagenome libraries derived from soil samples, a fosmid clone was selected as it conferred strong gentamicin resistance. To identify a specific functioning gene conferring gentamicin resistance from a selected fosmid clone (35-40 kb), a shot-gun library was constructed and four shot-gun clones (2-3 kb) were selected. Further characterization of these clones revealed that they contained sequences similar to that of the RNA ligase, T4 rnlA that is known as a toxin gene. The overexpression of the rnlA-like gene in Escherichia coli increased gentamicin resistance, indicating that this toxin gene modulates this trait. The results of our metagenome library analysis suggest that the rnlA-like gene may represent a new class of gentamicin resistance genes in pathogenic bacteria. In addition, we demonstrate that the soil metagenome can provide an important resource for the identification of antibiotic resistance genes, which are valuable molecular targets in efforts to overcome antibiotic resistance.
Streptoalloteichus hindustanus ATCC 31219, a nebramycin complex producer, is similar to Streptomyeces tenebrarius in a viewpoint of resistance to a wide range of aminoglycoside antibiotics. S. tenebrarius has resistance mechanisms of 16s rRNA methylation and aminogycoside modification. However, it is not known whether resistance mechanisms of Stall. hindustanus are the same as in S. tenebrarius. Therefore, we have tried to isolate resistance determinants from Stall. hindustanus. Two different types of aminoglycoside resistance determinants were isolated from Stall. hindustanus and expressed in Streptomyces lividans TK24. The apramycin resistance gene (amr) and the tobramycin resistance gene (tmr) isolated from Stall. hindustanus showed broad resistance spectrum against a dozen of aminoglycoside antibiotics. The complete nucleotide sequences of apramycin resistance gene (amr) were determined. The deduced amino acid sequence of the amr gene of Stall hindustanus ATCC 31219 showed extensive sequence homology to the 16s rRNA methylase gene (kamB) of S. tenebrarius.

Polychlorinated biphenyls (PCBs) are persistent environmental contaminants that elicit a broad spectrum of toxic effects in mammals and other vertebrate species. Because of their lipophilicity, chemical stability and resistance to biodegradation, PCBs tend to accumulate in the human body via food chain and environmental matrices including human adipose tissues, blood and milk.(omitted)
Waksman's group discovered SM in 1944, and opened a new field of antibiotcs: i. e. AGs. A large group of antibiotics containing aminosugar and/or aminocyclitol is called the AGs. A majority of AGs are produced by actinomycetes. In the first period, AGs effective against tuberculosis were chiefly examined. Following the studies on NM and KM, AGs active against staphyllococci and gram-negative robs were investigated. The discovery of GM and synthesis of DKB and AMK led to the studies on the third generation AGs, which show a broad antimicrobial spectrum including Pseudomonas aeruginosa and drug-resistant bacteria. Since opportunistic infection caused by drug-resistant bacteria are increasing, the third generation AGs are extensively investigated at present.
Fluoroquinolones have been extensively used in treatment or prevention of serious infections in humans and food-producing animals, especially in poultry, due to their broad-spectrum activity to both Gram-positive and -negative pathogens. However, the use of fluoroquinolones in poultry production selects for bacteria carrying genetic determinants for quinolone resistance. Here, we present the complete genome sequences of two methicillin-resistant Staphylococcus aureus (MRSA) strains displaying quinolone resistance, which were isolated from healthy broilers in Korea.
Background: Antibiotic resistance has been becoming serious challenge to human beings. Overuse of antibiotics, especially, for infants is concerned, but studies are very few for the prescribing pattern of antibiotic use for infants. This study analyzes prescribing patterns of antibiotics in outpatients of preschool children with acute respiratory tract infections in South Korea. Methods: Data are used from 2011 Health Insurance Review & Assessment Services-pediatric patients sample. Inclusion criteria is outpatient children (0 to 5 years) with top five frequent diseases. Prescription rates are analyzed by types of disease, provider, specialty, region, and ages. Binary or multinomial logit models are used to analyze determinants of providers' prescription pattern. Results: The main findings are as follows. First, distributions of prescription rates are shown as L-shape or M-shape depending on the types of disease. Second, the prescription variation is so large among providers, where providers are polarized as a group with low prescription rates and the other group with high prescription rates, though the shapes are shown diversified across types of disease. Third, prescription rates appear to be lower in pediatrics and higher in ENT (ear-nose-throat). Fourth, broad spectrum antibiotics are widely used among children. Finally, the logit analysis shows similar results with descriptive statistics, but partly different results across types of disease. Conclusion: Antibiotics for respiratory tract infections of infants are used excessively with a large variation among providers, and especially broad spectrum antibiotics are used. The prescription guideline for antibiotics should be provided for each specific disease to reduce antibiotic resistance in the future.
This study was conducted to investigate incidence of extended-spectrum ${\beta}$-lactamase (ESBL) producing strains and characteristics of ESBL gene in pathogenic Escherichia coli isolated from poultry during the period from April 2003 to December 2005 in Korea. Among 203 isolates, 4 isolates (3 from broilers and 1 from layer) were confirmed as ESBL producing strains by double disk synergy test, polymerase chain reaction and sequencing for ${\beta}$-lactamase genes. $bla_{CTX-M-15}$ and $bla_{CMY-2}$ were detected in these 4 isolates and were transferred to recipient by conjugation, respectively. Also, these ESBL producing strains were associated with multiple drug resistance. In conclusion, these results exhibit incidence of CTX-M and CMY-2 ${\beta}$-lactamase in pathogenic E coli from poultry in Korea, and clinically important meaning in human. And they also suggest the needs for rapid and broad surveillance to monitor ESBL genes and R plasmid transferring resistant gene in poultry.
Antimicrobial Peptides are natural antibiotics evolved by many plants, invertebrate, and vertebrate to defend against the microbial infection. Antimicrobial peptides show a broad-spectrum antimicrobial activity with little opportunity for the development of resistance since they target microbial membranes that distinguish microbes from enkaryotic cells. The oral cavity is constantly exposed to microbial challenges and antimicrobial peptides play an important role in managing the oral health. With the increase of resistant micro-organisms to conventional antibiotics, antimicrobial peptides are attracting interests as novel antibiotics. In this review, the characteristics of antimicrobial of antimicrobial peptides including the classification, mechanism of action, resistance, and expression in the oral cavity have been discussed in the prospects of application to oral disease.
Inappropriate antibiotic use is the most important factor causing increased bacterial resistance to antibiotics, thus affecting patient outcomes. Multidrug-resistant bacteria have become a serious public health threat, causing significant morbidity and mortality worldwide. In Korea, the burden of antibiotic-resistant bacteria has become an important public health issue. There is increasing evidence of overuse and misuse of antibiotics in Korea, as observed in cohorts with large sample sizes. Antibiotic use among children should receive particular attention because of the frequency of community-associated infections among this population and the elevated risk of transmission. Recent studies from Korea have demonstrated that the use of broad-spectrum antibiotics, either for inpatient or outpatient treatment, has increased among many age groups, especially children. In this review, we aim to describe the patterns of antibiotic prescription and evaluate recent trends in antibiotic use among children. Coordinated efforts toward communication and education in order to address misunderstandings regarding antibiotic use, involving interprofessional antimicrobial stewardship programs, are required in the near future.
