Economical Gas Chamber for In-situ Gas Measurement and Analysis of Gas Response Characteristics according to Sensor Voltage (인시투 가스 측정이 가능한 경제적 가스 챔버 구현 및 센서 전압에 따른 가스 응답 특성 분석)

  • Breath analysis using a portable device is better than the classical breath analysis system in terms of installation and operation. There is an increasing need to develop cost-effective equipment for testing gas sensors from the viewpoint of functionalities that can be applied applicable to portable devices. In the present study, an economical gas chamber for in-situ gas measurement is implemented with a single gas chamber without using expensive gas storage and control equipment; the gas response characteristics are analyzed using the above-described chamber. The main features of the implemented gas chamber are simple injection procedure, improved gas diffusion, easy measurement and cleaning, support for low-power mode measurement function for portable devices, and open source platform. Moreover, an analysis of gas response characteristics based on changes in sensor voltage show that the sensitivity and 90% response time are affected by the sensor voltage. Furthermore, the sensitivity graph has an inflection point in a specific range. The gas sensor applied in this study showed fast response speed and high sensitivity for sensor voltages of 3.0-3.5 V, regardless of the concentration of acetone gas, the target gas used in this study.

Reproducibility Evaluation of Deep inspiration breath-hold(DIBH) technique by respiration data and heart position analysis during radiation therapy for Left Breast cancer patients (좌측 유방암 환자의 방사선치료 중 환자의 호흡과 심장 위치 분석을 통한 Deep inspiration breath-hold(DIBH) 기법의 재현성 평가)

  • Purpose : The purpose of this study is reproducibility evaluation of deep inspiration breath-hold(DIBH) technique by respiration data and heart position analysis in radiation therapy for Left Breast cancer patients. Materials and Methods : Free breathing(FB) Computed Tomography(CT) images and DIBH CT images of three left breast cancer patients were used to evaluate the heart volume and dose during treatment planing system( Eclipse version 10.0, Varian, USA ). The signal of RPM (Real-time Position Management) Respiratory Gating System (version 1.7.5, Varian, USA) was used to evaluate respiration stability of DIBH during breast radiation therapy. The images for measurement of heart position were acquired by the Electronic portal imaging device(EPID) cine acquisition mode. The distance of heart at the three measuring points(A, B, C) on each image was measured by Offline Review (ARIA 10, Varian, USA). Results : Significant differences were found between the FB and DIBH plans for mean heart dose (6.82 vs. 1.91 Gy), heart $V_{30}$ (68.57 vs. $8.26cm^3$), $V_{20}$ (76.43 vs. $11.34cm^3$). The standard deviation of DIBH signal of each patient was ${\pm}0.07cm$, ${\pm}0.04cm$, ${\pm}0.13cm$, respectively. The Maximum and Minimum heart distance on EPID images were measured as 0.32 cm and 0.00 cm. Conclusion : Consequently, using the DIBH technique with radiation therapy for left breast cancer patients is very useful to establish the treatment plan and to reduce the heart dose. In addition, it is beneficial to using the Cine acquisition mode of EPID for the reproducibility evaluation of DIBH.

  • Purpose: On the left side, breast cancer patients have more side effects than those on the right side because of unnecessary doses in normal organs such as heart and lung. DIBH is performed to reduce this. To evaluate the dose of peripheral organs in the left breast cancer including supraclavicular lymph nodes and internal mammary lymph nodes according to the treatment planning method of Conventional Radiation Therapy, Intensity Modulated Radiation Therapy and Volumetric Modulated Arc Therapy. Materials and Methods: We performed CT-simulation using free breathing and deep inspiration breath-hold technique for 8 patients including left supraclavicular lymph nodes and internal mammary lymph nodes. Based on the acquired CT images, the contour of the body is drawn and the convention is performed so that $95%{\leftarrow}PTV$, $Dmax{\leftarrow}110%$. Conventional Radiation Therapy used a one portal technique on the supraclavicular lymph node and used a field in field technique tangential beam on the breast. Intensity Modulated Radiation Therapy was composed of 7 static fields. Volumetric Modulated Arc Therapy was planned using 2 ARC with a turning radius of $290^{\circ}$ to $179^{\circ}$. The peripheral normal organs dose was analyzed by referring to the dose volume of Eclipse. Results: By applying the deep inspiration breath-hold technique, the mean interval between the heart and chest wall increased $1.6{\pm}0.6cm$. The mean dose of lung was $19.2{\pm}1.0Gy$, which was the smallest value in Intensity Modulated Radiation Therapy. The V30 (%) of the heart was $2.0{\pm}1.9$, which was the smallest value in Intensity Modulated Radiation Therapy. In the left anterior descending coronary artery, the dose was $25.4{\pm}5.4Gy$, which was the smallest in Intensity Modulated Radiation Therapy. The maximum dose value of the Right breast was $29.7{\pm}4.3Gy$ at Intensity Modulated Radiation Therapy. Conclusion: When comparing the values of surrounding normal organs, Intensity Modulated Radiation Therapy and Volumetric Modulated Arc Therapy were applicable values for treatment. Among them, Intensity Modulated Radiation Therapy is considered to be a suitable treatment planning method.

An Exploratory Study of Fatigue Related Factors among School Personnelin Seoul by Data mining (데이터 마이닝을 이용한 서울시교직원의 피로요인 탐색연구)

  • Purpose : To identify general characteristics of school personnel with recent fatigue which was the most frequent symptom among subjective symptoms and to explore fatigue-related factors by evaluating physical and perceived health status, life style, and symptoms through data mining techniques. Methods : We collected a data of the 1,147(male 545, female 602) who were elementary, middle, or high school personnel, answered a questionnaire, and received physical examination in Seoul School Health Center from September to November in 2000. And we investigated the differences between fatigue group and non-fatigue group for demographic characteristics, physical health status, perceived health status, symptoms, and laboratory values by frequency, chi-square test, t-test, or simple logistic regression analysis by SAS package 8.1, and then selected significant variables as input variables of a decision tree analysis of CART model by SAS E-miner. Results : In general characteristics, the fatigue consisted of 41.1%(male 35.2%, female 46.4%) among 1,147 school personnel. In classical statistics, factors related with fatigue were female, lower means of systolic and diastolic pressure, young age, personnel in middle school, irregular eating habit, no exercise a week or less than 30minutes exercise a day, perception of unhealthy status, and subjective symptoms including short of breath at exercise. In simple logistic regression to examine the relationship between selected independent variables and fatigue as a dependent variable, the odds ratio of gender (female vs male) was 1.58 times, and young age ( 20s vs 60s) 20.67 times, and middle vs high school personnel 1.86 times. However, we mined combined several characteristics by SAS-E miner. In CART model, if health perception was healthy, and age was >= 37.5 years, the proportion of the fatigue was only 19.3%. but if health perception was not healthy and symptom was severe 'short of breath' during exercise and age was < 53.5 years, and BMI was >= 22.69, the proportion of the fatigue was up to 84.8%. Conclusions : The fatigue consisted of 41.1%(male 35.2%, female 46.4%). Effects of Sahyangsohap-won on Cerebral Hemodynamics in Healthy Subjects (사향소합원(麝香蘇合元)이 정상인의 뇌혈류역학에 미치는 영향)

  • Background and Purpose : Transcranial doppler ultrasonography(TCD) is a noninvasive and nonradioactive technique for evaluation of the hemodynamics in large cerebral vessels. Sahyangsohap-won(SS) has been considered to be effective for the treatment of various disease, especially cerebrovascular, cardiovascular, and psychosomatoform disorders. But, there is no study about the effect of SS on the cerebral hemodynamics in humans. The aim of this study was to assess the effect of SS on the changes in cerebral hemodynamics and the dose-dependant effect by using TCD. Subjects and Methods : 30 healthy subjects were randomly divided into three group: group 1 took no drug, group 2 took SS one pill, and group 3 took SS 2 pills. Changes in the mean blood flow velocity(MBFV) and pulsatility index(PI) in the middle cerebral artery were evaluated by means of TCD. We obtained hypercapnia with breath-holding and evaluated cerebrovascular reactivity with the breath-holding index(BHI). Systolic blood pressure(SBP), diastolic blood pressure(DBP), and heart rate(HR) were measured by means of ambulatory blood pressure monitoring. In group 2 and group 3, the evaluations were performed during the baseline and were repeated at 20, 40, and 60 minutes after SS administration. In group 1, the evaluation was performed at corresponding time intervals. Results : In mean values of MSFV, PI, SSP, DBP, and HR, no stastically significant differences were found between the 3 groups. However, BHI values were significantly lower in groups 2 and 3 than in group 1 at 40 minutes after SS administration(P<0.05, group 1 vs group 2, group 1 vs group 3 by post-hoc analysis: Scheffe's test) but in dose-dependant effect, there was no difference between group 2 and group 3. Conclusion : These results suggest that SS can decrease vascular resistance in cerebral small arteries or arterioles and enhance their distensibility. Further studies on larger numbers of subjects are needed to confirm these effects and the dose-dependant effects.

Characteristics of Vibration Response Imaging in Healthy Koreans

  • Background: Vibration response imaging (VRI) is a new technology that records energy generated by airflow during the respiration cycle. Analysis of lung sound using VRI may overcome the limitations of auscultation. Objectives: To set a VRI standard for healthy Koreans, we conducted a clinical assessment to evaluate breath sound images and quantification in healthy subjects and compared the findings with reported breath sound characteristics. Methods: Recordings were performed using the VRIxp. Eighty subjects took a deep breath four times during a 12-second interval while sitting upright. The quantitative aspect was analyzed using the VRI quantitative lung data (QLD) for total left lung, total right lung and for six lung regions: left upper lung (LUL), left middle lung (LML), left lower lung (LLL), right upper lung (RUL), right middle lung (RML), right lower lung (RLL). The qualitative aspect was provided through image assessments by three reviewers. Results: In all regions the left lung had significantly higher QLD than the right lung (P<0.005, paired t-test). The inter-rater agreement was 0.78. 84% of the images were found normal by the final assessment. Among the 16% (n=13) of images with abnormal final assessment, the most common flawed features were dynamic image (77%, n=10) and maximum energy frame (MEF) shape (77%, n=10). No significant differences were found between males and females for QLD but there were significant differences in qualitative aspects including dynamic images, MEF shape, and missing LLL. Conclusion: The characteristics of healthy Koreans are similar to those of Western subjects reported previously. VRI is easy to use and objective, and so is helpful to diagnose patients with respiratory diseases and to monitor the progress of diseases after medical treatments.

  • Treatise on the Pathogenesis and Manifestations of All Diseases(諸病源候論)was written by Chao Yuanfang that was most active during the Sui Dynasty at A.D. 610. It classified clinical medicine by some departments of internal medicine, surgery, obstetrics and gynecology, pediatrics, and otorhinolaryngology and within each specific department, categorizes etiology and pathology according to characteristics of various diseases. It was the total disease classification book that based on the bibles of the Oriental medicine, (Huangdi''s) Internal Classic(黃帝內經), Classic of Difficult Issues(難經) and Treatise on Cold Damage and Miscellaneous Diseases(傷寒雜病論), A-B Classic of Acupuncture and Moxibustion(鍼灸甲乙經), Essential Prescriptions Worth a Thousand Gold for Emergencies(備急千金要方) and Medical Secrets of an Official(外臺秘要). It was arranged tussive causes, classes, diagnosis, prognosis and stretching for treatment by Volume 13, all symptoms of tussiculation. Dialectic part was divided into cough(咳嗽), cough with dyspnea(咳逆上氣), cough with pus and blood(咳嗽吐膿血), cough with duck crying sound(?嗽), sudden cough(暴氣咳嗽), cough with dyspnea(咳逆), cough with dyspnea and vomiting(咳逆上氣嘔吐). Disease situation part was divided into the new cough(新咳) and old cough(舊咳), deficiency syndrome(虛證) and excess syndrome(實證), visceral cough(藏府咳), etc. Out of these, cough with counterflow(咳嗽上氣), cough with dyspnea(咳逆上氣), cough with dyspnea and vomiting(咳逆上氣嘔吐), cough with shortness of breath(咳逆短氣) have a close connection with dyspnea(上氣), counterflow of qi(逆氣), dyspnea and vomiting(上氣嘔吐) and shortness of breath(短氣) in the Pathogenesis and Manifestations of All qi(氣病諸候) of vol 13. So two parts may be refer to each other. However, the content on the original book has been addition and subtraction on the original context along with many reprints. Therefore, this paper, with regard to the prints of former editions, tried to help in better comprehension of the original context through readings and Korean translation.

  • Voice is one of the most efficient communication media and it includes several kinds of factors about speaker, context emotion and so on. Human emotion is expressed in the speech, the gesture, the physiological phenomena (the breath, the beating of the pulse, etc). In this paper, the method to have cognizance of emotion from anyone's voice signals is presented and simulated by using neuro-fuzzy model.

Surgery of the Trachea

  • Surgical procedures on the trachea have only been undertaken within the past 50 years. Knowing the unique blood supply of the trachea and how to reduce tension on any anastomosis are key to a successful outcome. Tracheal conditions requiring surgery usually present with shortness of breath on exertion, and preoperative evaluation involves computed tomography and rigid bronchoscopy. Tracheal resection and reconstruction can be safely performed with excellent outcomes by following a well-described technique.

  • In this paper we developed a respiration monitor by an impedane pneumography. This technique, measuring the electrical impedance change of the lungs due to breath, provides the accurate data for the respiration trend and rates. To measure the small impedance difference we designed a special current source which was considered the electrical safety and the interference with the ECG signal.
