Suppressing breast cancer by exercise: consideration to animal models and exercise protocols

  • [Purpose] Exercise is thought to have a significant effect on chemotherapy, and previous studies have reported that exercise can increase patient survival. Thus, in this review, we aimed to summarize various animal models to analyze the effects of exercise on breast cancer. [Methods] We summarized types of breast cancer animal models from various reports and analyzed the effects of exercise on anti-cancer factors in breast cancer animal models. [Results] This review aimed to systematically investigate if exercise could aid in suppressing breast cancer. Our study includes (a) increase in survival rate through exercise; (b) the intensity of exercise should be consistent and increased; (c) a mechanism for inhibiting carcinogenesis through exercise; (d) effects of exercise on anti-cancer function. [Conclusion] This review suggested the necessity of a variety of animal models for preclinical studies prior to breast cancer clinical trials. It also provides evidence to support the view that exercise plays an important role in the prevention or treatment of breast cancer by influencing anticancer factors.

Partial Breast Reconstruction Using Various Oncoplastic Techniques for Centrally Located Breast Cancer

  • Background As the breast cancer incidence has increased, breast-conserving surgery has replaced total mastectomy as the predominant procedure. However, centrally located breast cancers pose significant challenges to successful breast-conserving surgeries. Therefore, we performed partial mastectomy and oncoplastic procedures on centrally located breast cancer as a means of partial breast reconstruction. The authors examined and evaluated the functional and aesthetic usefulness of this reconstruction method. Methods From January 2007 to June 2011, 35 patients with centrally located breast cancers who underwent various oncoplastic procedures based on the breast size and resection volume. The oncoplastic procedures performed included volume displacement surgical techniques such as purse-string suture, linear suture, and reduction mammaplasty. Other oncoplastic procedures included volume replacement procedures with an adipofascial, thoracoepigastric, intercostal artery perforator, thoracodorsal artery perforator, or latissimus dorsi flap. Results Mean patient age was 49 years, and mean follow-up period was 11 months. In cases of small to moderate-sized breasts and resection volumes <50 g, volume displacement procedures were performed. In cases of resection volumes >50 g, volume replacement procedures were performed. In cases of larger breasts and smaller resection volumes, glandular reshaping was performed. Finally, in cases of larger breasts and larger resection volumes, reduction mammaplasty was performed. This reconstruction method also elicits a high patient satisfaction rate with no significant complications. Conclusions In centrally located breast cancer, oncoplastic surgery considering breast size and resection volume is safe and provides appropriate aesthetic outcomes. Therefore, our method is advisable for breast cancer patients who elect to conserve their breasts and retain a natural breast shape.

유방확장기만을 사용한 유방재건술 후 C-V 피판을 이용한 유두재건술 (Nipple reconstruction using the C-V flap technique after breast reconstruction with the only breast expander)

  • Purpose: Breast cancer is second most common cancer in women. Almost of patients with breast cancer treated with mastectomy undergoes breast reconstruction. Nipple reconstruction is an important step in breast reconstruction. Many surgeons have investigated of nipple reconstruction using the flap technique after breast reconstruction with the autologous tissue. The objective of this study is to evaluate the results of nipple reconstruction using the C - V flap technique after breast reconstruction with the only breast expander. Methods: From April 2006 to May 2008, the authors treated 17 patients of nipple reconstruction using C - V flap technique, who were received breast reconstruction with the only breast expander. We have predicted decrease of the size of reconstructed nipple and designed flap a little larger than wanted nipple size. Nipple splint was applied for 4 - 6 months for minimizing decrease of the size of reconstructed nipple. The diameter and height of the reconstructed nipple were measured and patient's satisfaction score was assessed. Results: Mean follow-up duration was 12.5 months. Among the 17 patients, the average absorption rate by height of nipple was 47.0%. Partial necrosis was noted in 1 case, and treated well with conservative management. There was no other significant complications noted. Patient's satisfactory score was assessed by the height, design and location of the nipple. The average of satisfaction score was 85%, 68%, 62% and total average was 83%. Conclusion: The authors experienced 17 patients of nipple reconstruction using the C - V flap technique after breast reconstruction with the only breast expander. The absorption rate of the size of the nipple, complications and patient's satisfactory score of this study were similar to those of nipple reconstruction after breast reconstruction with the autologous tissue. It is expected that nipple reconstruction after breast reconstruction with the only breast expander is safe and reliable. It is considered that a long - term study is necessary.

한방치료를 통해 Adriamycin/Cytoxan 및 Taxol 항암제 부작용이 감소한 유방암 환자 증례보고 (A Case of Breast Cancer Patient Experiencing Adriamycin Cytoxan and Taxol Side Effects Managed by Traditional Korean Medicine)

  • Progress : A 33 year old female patient diagnosed with left breast cancer stage II was admitted to EWCC (East-West Cancer Center) in November of 2009. She had planned chemotherapy. She was treated with herbal medicine, acupuncture, moxibustion and physiotherapy for a period of 4 months, from Nov 5th, 2009 to Feb 18th 2010. We evaluated the grade of chief complaints and performed blood tests periodically. Results : TKM alleviates symptoms induced by anticancer chemotherapy. Nausea, headache, dizziness and chemotherapyinduced peripheral neuropathy were reduced. Quality of life was also upward. Conclusions : This case study supports TKM's potential efficacy in treating breast cancer patients suffering from anticancer chemotherapy.

유암에 대한 지식 태도 및 예방 실천행위에 관한 조사연구 (A Survey of Knowledge, Attitudes toward Breast Cancer and behavior related to the prevention of Breast Cancer - of Women outpatients of general hospitals in Seoul -)

  • This study was carried out to collect basic information on the knowledge and attributes and behavior related to the prevention of breast cancer of a Sample Seoul women. Similar studies had dealt with samples of tumor clinic patients and their families and with non-professional hospital welters. The sample for this study was drawn by women out patients from eight centrally located general hospitals, in Seoul where the number of annual out patients exceeded 100,000, and the number of beds exceeded 100. The interview schedule consisted 33 questions related to knowledge, attitude, and behavior related to prevention of breast cancer. It was modified from the previous on cancer studies. The investigator interviewed 665 women over 20 years of age, from those who visited these eight hospitals from 24, June to 20, August 1975. More than half of the respondents were housewives and primarily in the 20 to 29 ages group. They had a mean level of 3.562 pregnancies and 1,986 children. 33.4% were high-school graduates, 32.2% had a monthly family income in the 50,000 to 100,000 won range. 75% of the respondents had heard about breast cancer through mass media or personal media, but the majority of respondents did not have an accurate understanding of it. This varied according to educational, economical income and occupations. The primary reason given for maintaining distance from a patient with breast cancer was not because of cancer but simply because person was a patient. 74.7% of the respondents thought that early cancer of the breast could be cured with early treatment. Only 5.3% of the respondents had ever had a physical examination and only 6.3% practised self examination of the breast.

Lobular Breast Carcinoma Metastasis to the Thyroid Gland: Case Report and Literature Review

  • Metastasis from primary cancer to the thyroid is uncommon in breast cancer. Here we present a case of lobular breast carcinoma that metastasized to the thyroid. A 54-year-old woman without symptoms was admitted to our institution for staging of the lymph node above the left clavicle. An $^{18}F$-fluoro-deoxy-D-glucose positron emission tomography scan was performed for staging, and low uptakes were observed in the left supraclavicular and cervical lymph nodes. High uptake was seen in the posterior and lower left lobe of the thyroid. Histologic findings indicated lobular breast carcinoma (positive GATA3, loss of E-cadherin expression) metastatic to the thyroid with a luminal profile. Immunohistochemical analysis was negative for primary thyroid or parathyroid carcinoma. To our knowledge, this is the first report of a patient presenting a metastatic invasive lobular carcinoma in the thyroid and lymph nodes without a prior diagnosis of breast cancer.

Giant Intrathoracic Meningocele and Breast Cancer in a Neurofibromatosis Type I Patient

  • Intrathoracic meningoceles are relatively rare entities found in patients with neurofibromatosis type I (NF1). Given that both the BRCA1 and NF-1 genes are located on the same long arm of chromosome 17, one would expect concurrence of neurofibromatosis and breast cancer. However, incidence of such co-disorders is very rare in the literature. Here, the authors report a case of a 50-year-old female patient with NF-1 and concurrent cancer of the left breast, who had a huge bilobulated intrathoracic meningocele with thoracic dystrophic scoliosis, treated surgically via a posterior-only approach for the meningocele and spinal deformity in the same setting.

Regulation of post-translational modification in breast cancer treatment

  • The small ubiquitin-related modification molecule (SUMO), one of the post-translational modification molecules, is involved in a variety of cellular functions where it regulates protein activity and stability, transcription, and cell cycling. Modulation of protein SUMOylation or deSUMOylation modification has been associated with regulation of carcinogenesis in breast cancer. In the dynamic processes of SUMOylation and deSUMOylation in a variety of cancers, SUMO proteases (SENPs), reverse SUMOylation by isopeptidase activity and SENPs are mostly elevated, and are related to poor patient prognosis. Although underlying mechanisms have been suggested for how SENPs participate in breast cancer tumorigenesis, such as through regulation of target protein transactivation, cancer cell survival, cell cycle, or other post-translational modification-related machinery recruitment, the effect of SENP isoform-specific inhibitors on the progression of breast cancer have not been well evaluated. This review will introduce the functions of SENP1 and SENP2 and the underlying signaling pathways in breast cancer for use in discovery of new biomarkers for diagnosis or therapeutic targets for treatment.

유방암 수술 후 발생한 림프부종과 말초신경병증에 대한 도침술과 정맥자락술 집중치료 증례보고 (Acupotomy and venesection in Upper Limb Lymphedema and Peripheral neuropathy following Breast Cancer Surgery)

  • Purpose: In order to estimate clinical effects of acupotomy and venesection in a patient with peripheral neuropathy and upper limb lymphedema following breast cancer surgery. Methods: From 17th August, 2009 to 29th August 2009, 1 female patient with peripheral neuropathy and upper limb lymphedema following breast cancer surgery was treated with general oriental medicine therapy(acupuncture, moxibustion, cupping, physical therapy, herbal medication) and acupotomy with venesection. Results: The patient's chief complaints- Lt hand numbness, Lt arm edema, Lt. wrist flexion limitation - were notably improved. Conclusions : This study demonstrates that oriental medical treatment with acupotomy and venesection therapy has significant effect in improving symptoms of peripheral neuropathy and upper limb lymphedema following breast cancer surgery, as though we had not wide experience in this treatment, more research is needed.

Comfort and Anxiety Levels of Women with Early Stage Breast Cancer Who Receive Radiotherapy

  • Background: The aim of this planned research was to determine the comfort and anxiety levels of women with breast cancer receiving radiotherapy. Materials and Methods: This descriptive type study covered patients that applied to the radiation oncology breast polyclinic of our university hospital between January and May 2011. Patient Identification Form, Radiation Therapy Comfort Questionnaire (RTCQ), Spielberger State Trait Anxiety Inventory (STAI) were completed and analysed. Results: The mean age of the women who participated in the study was $51.6{\pm}10.4$ years. Mean scores of women were $3.73{\pm}0.31$ for RTCQ, $29.1{\pm}5.88$ for SAI and $37.8{\pm}6.91$ for TAI. While the comfort levels of the women with breast cancer receiving radiotherapy were moderate, they experienced only low levels of anxiety. Conclusions: By determining the comfort level of the patient before radiotherapy, besides providing comfort in this direction, eliminating/minimizing anxiety and stress will positively affect radiotherapy application. More attention of nurses to this issue is to be recommended.
