Usefulness of Image Registration in Brain Perfusion SPECT (Brain Perfusion SPECT에서 Image Registration의 유용성)

  • Purpose: The brain perfusion SPECT is the examination which is able to know adversity information related brain disorder. But brain perfusion SPECT has also high failure rates by patient's motions. In this case, we have to use two days method and patients put up with many disadvantages. We think that we don't use two days method in brain perfusion SPECT, if we can use registration method. So this study has led to look over registration method applications in brain perfusion SPECT. Materials and Methods: Jaszczak, Hoffman and cylindrical phantoms were used for acquiring SPECT image data on varying degree in x, y, z axes. The phantoms were filled with $^{99m}Tc$ solution that consisted of a radioactive concentration of 111 MBq/mL. Phantom images were acquired through scanning for 5 sec long per frame by using Triad XLT9 triple head gamma camera (TRIONIX, USA). We painted the ROI of registration image in brain data. So we calculated the ROIratio which was different original image counts and registration image counts. Results: When carring out the experiments under the same condition, total counts differential was from 3.5% to 5.7% (mean counts was from 3.4% to 6.8%) in phantom and patients data. In addition, we also run the experiments in the double activity condition. Total counts differential was from 2.6% to 4.9% (mean counts was from 4.1% to 4.9%) in phantom and patients data. Conclusion: We can know that original and registration data are little different in image analysis. If we use the image registration method, we can improve disadvantage of two days method in brain perfusion SPECT. But we must consider image registration about the distance differences in x, y, z axes.

Effect of Community-based Rehabilitation on Patients with Brain Injury: Meta-analysis Based on the ICF Model (뇌손상 환자의 지역사회재활(CBR) 프로그램의 효과: ICF 모델에 근거한 메타분석)

  • In this study, a meta-analysis was conducted to investigate the effect of community-based rehabilitation on patients with brain injury by ICF factor in previous studies and examine the application of recent community-based rehabilitation program. Total 15 articles that satisfied the selection criteria were divided by ICF factors to obtain the effect size. Homogeneity and publication bias were tested for those selected papers. As a result, the overall effect size of community-based rehabilitation was 0.22; the effect size by physical function and structure among the ICF factors came to be 0.21; and the effect size of activity and participation was 0.20, indicating a small effect size. The studies involved in a meta-analysis were considered as the absence of heterogeneity and publication bias, which indicates that the results of this study are subjectively reliable. These results well explained the level of change on ICF factors, especially physical function and structure, activity and participation, in other words, the intervention effect of community-based rehabilitation on patients with brain injury.

Evaluation of Some Biochemical Parameters and Brain Oxidative Stress in Experimental Rats Exposed Chronically to Silver Nitrate and the Protective Role of Vitamin E and Selenium

  • Due to undesirable hazardous interactions with biological systems, this investigation was undertaken to evaluate the effect of chronic exposure to silver on certain biochemical and some oxidative stress parameters with histopathological examination of brain, as well as the possible protective role of selenium and/or vitamin E as nutritional supplements. Thirty six male rats were divided into six groups of six each: the first group used as a control group. Group II given both vitamin E (400 mg/kg) of diet and selenium (Se) (1 mg/L) in their drinking water. Group III given silver as silver nitrate ($AgNO_3$) (20 mg/L). Group IV given vitamin E and $AgNO_3$. Group V given both $AgNO_3$ and selenium. Group VI given $AgNO_3$, vitamin E and Se. The animals were in the same exposure conditions for 3 months. According to the results which have been obtained; there was an increase in serum lactate dehydrogenase (LDH), lipase activities and cholesterol level, a decrease in serum total protein, calcium and alkaline phosphatase (ALP) activity in Ag-intoxicated rats. Moreover, the findings showed that $Ag^+$ ions affected antioxidant defense system by decreasing superoxide dismutase (SOD) activity and increasing vitamin E concentration with a high level of malondialdehyde (MDA) in brain tissue. The histological examination also exhibited some nervous tissue alterations including hemorrhage and cytoplasm vacuolization. However, the co-administration of selenium and/or vitamin E ameliorated the biochemical parameters and restored the histological alterations. In conclusion, this study indicated that silver could cause harmful effects in animal body and these effects can be more toxic in high concentrations or prolonged time exposure to this metal. However, selenium and vitamin E act as powerful antioxidants which may exercise adverse effect against the toxicity of this metal.

A Study about Peritumoral Brain Edema in Meningiomas using Angiographic Pattern and MIB-1

  • Objective: Peritumoral brain edema(PTBE) accounts for approximately 60% of meningiomas. It has not been identified why vasogenic edema, frequently shown in intra-axial tumors is also developed in extra-axial tumor such as meningiomas. Therefore, the authors assess the peritumoral brain edema of meningiomas with a focus on the angiographic pattern and expression of MIB-1 to clarify their correlation. Methods: A total 32 cases of meningioma was studied. The authors attempted to identify 1) the location of PTBE and the edema index (EI), 2) the location and dominancy of pial supply compared with meningeal supply, 3) the biological activity of meningiomas indicated by the MIB-1 LI (labeling index), 4) their interaction. Results: No PTBE was observed in the meningiomas without pial arterial supplement from internal carotid artery (ICA) and vertebral artery (VA). The PTBE of meningiomas with pial supply was developed intensely along the pial arterial supplement, and increased statistically in proportion to the extent of pial supply from ICA or VA rather than meningeal supply. Also, the MIB-1 LI in meningiomas tended to be larger in the tumors of the larger EI and the dominancy of pial supply. Conclusion: A strong correlation is found between the extent of PTBE in meningiomas and the dominancy of pial supply. The MIB-1 LI also tend to be associated with the PTBE. Therefore, the MIB-1 LI in benign meningiomas may represent not only the proliferative potential of the tumor, but also the biological activity like angiogenesis.

Comparison of Electromyography Activity in Accordance with Contact Pad Type during the Use of a Standing Table for Pediatric Patients with Brain Lesions (뇌병변 소아환자의 기립보조기 사용 시 접촉 패드 유형에 따른 근활성도 비교)

  • This study conducted a comparative analysis of the contraction pattern of trunk and lower limbs muscles of handicapped children with brain lesions during the use of foam pad and air pad for fixing a standing table to the body for pediatric patients with brain lesions using a standing table product applied with a pneumatic air fitting system. There was a difference in the measurement of muscle strength activity by muscle in accordance with pad type (foam pad vs. air pad). A large difference was found with big movements (movements of eating), while a small difference was discovered with small movements (writing and reading movements). This was found to furnish a little more comfort to users compared with the existing foam pad during the use of a standing table product applied with a pneumatic air fitting system.

Study on the Relationship Between EEG of Brain Laterality and Personality Traits (좌·우뇌 비대칭 뇌파와 성격특성요인의 관계에 대한 연구)

  • The purpose of this study is to find out the relationships of brain laterality, active EEG over all brain regions and personality traits by measuring EEG signals on the basis of the counseling psychology personality theories. For this study, the EEG of ninety-six college students as measured by an eight channel EEG device and analyzed through the computer and the data of their Big Five Personality Test were analyzed by statistical analysis. The result was that when theta's laterality at the prefrontal lobe is bigger, neuroticism is higher in the personality factors. On each of the brain regions, theta's activity on the left of the prefrontal lobe makes higher neuroticism but lower conscientiousness, and beta's activity on the left of the frontal lobe makes lower extroversion and openness to experience. These results showed that there are statistically meaningful relationships between the brain region activated specific EEG and individual personality or psychological traits. This study branched out into theta band while most previous studies measured in alpha and beta band. Also from these results it suggested the counseling strategy with the brain and follow-up studies.

Palliative Radiotherapy for Brain Metastases (전이성 뇌종양의 고식적 방사선치료)

  • Purpose : Radiotherapy has been the mainstay of the treatment of brain metastases. We evaluated the response rate, survival and prognostic factors of patients with brain metastases treated with radiotherapy for palliative purpose. Materials and Methods : From January 1994 through April 1997, in all 42 patients, a retrospective analysis was undertaken. Of these, 33 patients received whole brain irradiation with 30Gy in 10 daily fractions with or without a boost of 10Gy in 5 daily fractions to the site of solitary lesion. Nine patients failed to complete the planned treatment Results : Of 33 patients who finished radiotherapy, complete and partial response were observed in $4(12\%)$ patients and $22(67\%)$ ones, respectively. Overall response rate was $79\%$ and median survival was 4 months. In univariate analysis, prognostic factors affecting survival were initial neurologic function class(p=0.0136), extracranial tumor activity(p=0.042), and response after radiotherapy(p=0.001). Conclusion : We confirmed that whole brain irradiation is the effective means for treating the patient with brain metastases. initial neurologic function class, extracranial tumor activity, and response alter radiotherapy were identified as prognostic factors affecting survival.

Determination of Brain Death by $^{99m}Tc$ DTPA and $^{99m}Tc$ HMPAO Images ($^{99m}Tc$ DTPA와 $^{99m}Tc$ HMPAO를 이용한 뇌사결정)

  • To evaluate availability of cerebral radionuclide imaging for diagnosis of brain death, we examined 25 patients with a suspected clinical diagnosis of brain death. 8 patients were studied by $^{99m}Tc$ DTPA and 15 patients were studied by $^{99m}Tc$ HMPAO (Hexamethyl propyleneamine oxime). Seven patients with $^{99m}Tc$ DTPA studies revealed absence of cerebral blood flow and sagittal sinus activity. All of 15 patients with $^{99m}Tc$ HMPAO studies revealed complete absence of cerebral perfusion. The results of the cerebral radionuclide studies of brain death correlated with other clinical conditions, such as intracranial pressure(ICP), EEG, transcranial doppler sonography(TCDS), and neurologic examination. The ICP of 8 patients, who are confirmed by brain death with $^{99m}Tc$ HMPAO study are elevated in all cases. In conclusion, cerebral radionuclide imaging for diagnosis of brain death is available. $^{99m}Tc$ HMPAO imaging is unequivocal, easily interpreted, well reflect the physiologic state of increased ICP, and provides adequate assessment of posterior fossa activity. In addition, the SPECT imaging with $^{99m}Tc$ HMPAO produces more accurate results due to it's superiority of image contrast and proper localization of radiopharmaceutical distribution than conventional planar imaging.
