A CASE OF NARCOLEPSY IN A 11 YEAR-OLD BOY (소아 기면증 1예)

  • Narcolepsy's age at onset is reported to be relatively homogeneous, occuring usually after the onset of puberty, although most cases are diagnosed when the patients are in their late teens to late 20s. It is very unusual for a patient to develop narcolepsy before 15 years of age or after 30 years of age. A 11-year old boy who has developed excessive daytime sleepiness since age of 7 and has all the four major features of narcolepsy by the time of evaluation is presented. On polysomnographic examination, the patient showed two sleep onset REM periods in the three latency test of the multiple sleep latency test and the nocturnal polysomnogram. In addition, the findings of typing HLA class I and II of the patient's family are presented. Reports of pediatric narcolepsy previously reported are reviewed.

The Influence of Demographical Variables and Alienation on Adolescent's Negative Emotion-Induced Eating Motivation (인구학적 변인과 심리적 소외감이 청소년의 부정적 정서로 인한 섭식행동동기에 미치는 영향)

  • The purpose of this study was to investigate the influence of demographical variables(gender and age) and alienation on adolescent's negative emotion-induced eating motivation. The subjects were 389 middle and high school students who lived in Keoungbok. The data were analyzed by t-test, one-way ANOVA and stepwise multiple regression(using SPSS 12.1). Major findings were as follows: 1) There was difference in adolescent's negative emotion-induced eating motivation according to gender, age, and level of alienation. Adolescent's negative emotion-induced eating motivation tended to be higher when adolescent were girl students, or high school students or in higher level of alienation. 2) Alienation was the stronger predictor of adolescent's negative emotion-induced eating motivation than gender and age. 3) There was difference in boy adolescent's negative emotion-induced eating motivation according to age and level of alienation. But there was difference in girl adolescent's negative emotion-induced eating motivation only according to level of alienation. 4) For boy, alienation was the stronger predictor of adolescent's negative emotion-induced eating motivation than age. But for girl alienation was only significant predictor of adolescent's negative emotion-induced eating motivation.

Morgagni Hernia in a 3-year Old Boy - a Case Report - (3세 남아에서 발생한 Morgagni 탈장 - 1예 보고 -)

  • A 3-year-old boy with purulent otitis media received a chest radiograph as the part of a routine work up. The patient was normal appearing, in no acute distress. The patient's lung and heart sounds were clear and normal. The patient's abdomen was soft, non-distended, and non-tender. An anterior cardiophrenic mass was incidentally identified on the lateral chest radiograph. A computed tomography scan demonstrated a diaphragmatic hernia with bowel loops in the retrosternal space. An exploratory operation revealed a diaphragmatic defect (4 cm in diameter) on the left side of the falciform ligament, through which transverse colon was protruded. There was no hernia sac, and the defect was closed with interrupted No. 2 silk sutures. The child was discharged on the 8th postoperative day without any complications. During 6 months of follow-up period, recurrence was not noticed.

Children's Sensation Seeking, Maternal Childrearing Behavior and Children's Problem Behavior (아동의 감각추구성향 및 어머니 양육행동과 남녀 아동의 문제행동과의 관계)

  • This study investigated the relationship among children's sensation seeking, maternal childrearing behavior and children's problem behavior. The subjects were 470 5th and 6th grade elementary school students in Pusan, Korea. Zuckerman's questionnaire on sensation seeking(1994), Park, Young Yae's questionnaire on maternal childrearing behaviors(1995) and Kim Hye Ryun's questionnaire on children's maladjustment behavior(1993) were used. The results were as follows : (1) According to sex, there were significant differences in sensation seeking stores, the aggressive behavior scores and distracted behavior scores. The boys got higher scores than girls in sensation seeking scores, and also in aggressive behavior scores and distracted behavior scores. (2) There were significant positive correlations between sensation seeking scores and the aggressive behavior scores for both boys and girls. And there were significant positive correlations between sensation seeking scores and the distracted behavior scores for both boys and girls. (3) Both sensation seeking and maternal child rearing behavior predicted aggressive behavior and distracted behavior for both boys and girls. For boy's aggressive behavior, sensation seeking and maternal child rearing behavior explained 8%, 8%, respectively. For boy's distracted behavior, sensation seeking explained 5%. And for girl's aggressive behavior, sensation seeking and maternal child rearing behavior explained 27%, 9%, respectively. For girl's distracted behavior, sensation seeking and maternal child rearing behavior explained 28%, 7%, respectively. The sensation seeking variable was the important predictor influencing on children's aggressive behavior and distracted behavior. Implications of this study were discussed in terms of sensation seeking and socialization.

Relationships among Snacks, Unbalanced Diet, and Eating Behaivor of Middle School Students (중학생의 간식섭취, 편식, 식사태도간의 상호관계)

  • The purpose of this study was to find out the relationships among snacks, unbalanced diet, and eating behaviors of middle school students. Subjects of this study were randomly selected from middle school students living in ChungBuk and data were collected by questionaires. Major findings were as follows: First, majority of respondents felt snacks necessary for them. They were influenced by T. V advertisement when they chose snacks. Most students had snacks habitually, and girl students had snacks more freqently than boy students. The students who had more pocketmoney had more snack than those who had less pocktmoney. Second, girl students were more fastidious than boy students about foods. The students who took instant noodles an snacks more frequently got lower scores of eating behaviors than those who took instant noodles and snacks more frequently got lower scores of eating behaviors than those who took instant noodles and snacks less frequently. Finally frequent snacks and unbalanced diet had strong relationships with lower scores of eating behaviors. In conclusion, the findings implied that nutrition-education through mass media like. T.V may be effective and nutrition education also should be practiced both at school and at home for the students’good eating behaviors and health.

Colorectal Cancer in Children - 2 Cases Report - (소아에서 발생한 결장암)

  • A 12-year-old boy with severe periumbilical pain visited the emergency room. Physical examination, abdominal ultrasonography, colonoscopy and CT, identified a lesion of sigmoid colon. Endoscopic biopsy showed a signet ring cell carcinoma of the sigmoid colon. On explorative laparotomy, cancer invasions of the adjacent structures and metastases on peritoneal wall were noticed. We performed palliative loop-ileostomy. He underwent chemotherapy and radiotherapy for 3 months. The second case was a 16-year-old boy with abdominal pain and hematochezia, transferred to our hospital with the diagnosis of acute appendicitis with periappendiceal abscess. Although he underwent appendectomy, the abdominal pain persisted. Digital rectal examination revealed a lumen-obstructing fungating mass in the rectum. Endoscopic biopsy revealed a adenocarcinoma. Cancer invasion of the adjacent structures and metastases involving the mesentery of the small intestine were found at laparotomy. A palliative procedure, a Hartmann's operation and end-colostomy at the sigmoid colon were performed. The patient died 8 month later due to pneumonia and sepsis. Chemotherapy was not applied.

A Study on the Fitting Size of Tween Generation' Garments (트윈세대의 의복 치수적합성에 관한 연구)

  • The purpose of this study was to examine how the garment sizes by junior brand manufactures reflect Tween Generation's (ages from 5th grade to 9th grade) actual body sizes. They have diverse clothing preferences in styles and fitting levels as well as diverse body sizes and shapes. The subjects of this study were 520 students (the 5th and 6th grades in elementary schools, the 1st, 2nd and 3rd grades in middle schools) who lived in Jeonju. A questionnaire was used in this study. The data were analyzed by frequency, means, t-test and $\chi^2$. The results were as follows. In the selection of garment size, both the girl and boy students most preferred selecting garments after trying them on by themselves. Boy students tended to select garment size according to their mothers' advice. On the other hand, elementary school students showed a tendency to select garment size with their mothers. In fitting sizes, middle school girls wanted the size to fit tightly rather than loosely on their bodies much more than elementary school girls. Junior brand manufacturers produced sizes that well reflected actual body sizes of elementary and middle school girls. Specifically, the pants best covered their body sizes.

Transient asymptomatic white matter lesions following Epstein-Barr virus encephalitis

  • We present the case of a patient with Epstein-Barr virus (EBV) encephalitis who developed abnormal white matter lesions during the chronic phases of the infection. A 2-year-old-boy was admitted for a 2 day history of decreased activity with ataxic gait. The results of the physical examination were unremarkable except for generalized lethargy and enlarged tonsils with exudates. Brain magnetic resonance imaging (MRI) at admission showed multiple high signal intensities in both basal ganglia and thalami. The result of EBV polymerase chain reaction (PCR) of the cerebral spinal fluid was positive, and a serological test showed acute EBV infection. The patient was diagnosed with EBV encephalitis and recovered fully without any residual neurologic complications. Subsequently, follow-up MRI at 5 weeks revealed extensive periventricular white matter lesions. Since the patient remained clinically stable and asymptomatic during the follow-up period, no additional studies were performed and no additional treatments were provided. At the 1-year follow-up, cranial MRI showed complete disappearance of the abnormal high signal intensities previously seen in the white matter. The patient continued to remain healthy with no focal neurologic deficits on examination. This is the first case of asymptomatic self-limited white matter lesions seen in serial MRI studies in a Korean boy with EBV encephalitis.

A Case of Subacute Necrotizing Lymphadenitis with Hepatic Complication in an 11-year-old Boy (11세 소아에서 아급성 괴사성 림프절염에 합병된 간염 1례)

  • Subacute necrotizing lymphadenitis was first reported by Kikuchi and Fujimoto in 1972. Young females no more than 30 years of age are mainly affected. It usually manifests as fever and cervical lymphadenopathy. We experienced one case of subacute necrotizing lymphadenitis with hepatic complication in an 11-year-old boy. Symptoms presented were URI signs, diarrhea, headache, and weight loss along with fever and cervical lymphadenopathy. Elevated serum AST/ALT levels were also noted up to 682/1560 (IU/L) and were normalized within one month. We performed aspiration biopsy of the liver twice (at admission and 5 months thereafter). The hepatic histopathologic findings were nonspecific.

TREATMENT OF 4 CASES WITH TEST ANXIETY (시험불안증의 치료 : 증례보고)

  • The author reported treatment experiences of 4 cases with test anxiety. The first one was a 15 year old boy with problem of mild attention deficit which caused test anxiety and academic underachievement around the 8th grade in middle school. The second and the third cases were brother and sister. The test anxiety was caused basically due to the problem of mother child relationship. their mother expected too much of her son and always urged him to study and never left him alone. The mothers practically gave up her whole life to devote to oversee her son's academic achievement and her daughter experienced affectional deprivation. The fourth case, a 16 year old boy, was a borderline personality disorder with extreme anger and hostility toward his parents who controled him too much. Different therapeutic approaches appropriate for different cases were described and the relationship between test anxiety and various psychopathology was discussed.
