이중에너지 방사선흡수 골밀도 장치의 품질관리 현황 (A Status Report on Dual Energy X-ray Absorptiometry Quality Control in Korea)

  • DEXA는 골다공증 진단에 가장 많이 사용되는 표준 측정법이다. 현재 국내에서 운영되고 있는 DEXA 장치는 5,671대이다. 본 연구에서는 국내에서 설치 운영되고 있는 DEXA장비의 품질관리 실태를 조사하였다. 3,000개 기관을 대상으로 한 설문 조사에서 12.6%의 응답을 받아 그 결과를 분석하였다. 결과에서 77.5%가 일일 품질관리를 시행하고 있으나 Shewhart chart는 68.5%, CUSUM chart는 86.0%가 모르거나 사용하지 않는다고 답하여 정확한 품질관리가 되지 않는 것으로 조사되었다. 정확한 골밀도 검사는 환자의 골절의 위험도를 산정하기 위한 자료로 활용된다. 하지만 부적절한 검사는 오진을 유발할 가능성을 높여준다. 따라서 정확한 골밀도 검사를 위해서는 적절한 보조기구를 사용하고 장치의 품질관리를 반드시 시행하여 장치와 자세에 대한 오차를 줄이고 검사 의료진의 교육을 통해 오류 요인을 줄이는 것이 필수적이다.

  • Implant placement is frequently complicated and challenging because of the poor quality and inadequate height of bone. Clinicians should consider various surgical procedures to overcome the problems. We report a case with various surgical procedures used such as inferior alveolar nerve repositioning, sinus bone graft, and autogenous block bone graft using the coronoid process and ramus to overcome severe vertical and horizontal alveolar bone atrophy.

탈단백 우골의 골이식 대체재로서의 특성에 대한 평가;세 종류의 골 대체재의 기본 특성에 대한 비교분석 (Evaluation of deproteinized bovine bone mineral as a bone graft substitute;A comparative analysis of basic characteristics of three commercially available bone substitutes)

  • Bovine bone-derived bone substitutes are widely used for treatment of bone defects in dental and orthopedic regenerative surgery. The purpose of this study was to evaluate the basic characteristics of deproteinized bovine bone mineral as a bone graft substitute. Commercially available products from three different bovine bone minerals-Bio-Oss(GeistlichPharma, Switzerland), BBP(Oscotec. Korea), Osteograf/N-300(Dentsply Friadent Ceramed, USA) - were investigated. They were evaluated by scanning electron microscopy(SEM), energy dispersive X-ray spectrometer(EDS), surface area analysis(BET), and Kjeldahl protein analysis. Cell viability on different products was evaluated by 3-(4, 5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide(MTT) assay. The results of this study indicated that each bone substitute displayed distinct surface properties. Furthermore, Kjeldahl protein analysis indicated that residual crude proteins are present in deproteinized bovine bone mineral. BBP showed relatively large amount of residual protein, which indicated that the possibility of disease transmission can not be safely ruled out. Based on the results of this study, it is suggested that active quality management is strongly needed in operations that involve processing bovine bone tissue for medical use.

한우 뼈 부위별 국물의 품질 특성 및 이화학적 특성 비교연구 (A Comparative Study on Quality and Physicochemical Characteristics of Segmental Bone Korean Beef Broth)

  • The purpose of this study was to perform a functional component analysis to optimize the recipes for Korean beef main bone area. To optimize the recipes for beef leg bone broth, beef feet broth and tail broth, the quality and physiochemical properties of the dishes were comparatived and investigated. Crude protein, crude fat and moisture showed significant differences among the beef leg bone broth, Beef feet broth and tail broth (p<0.001). All free amino acids showed significant differences, with the exception of cystine and tryptophan (p<0.05). All minerals showed significant differences (p<0.01). Na content was highest in the beef leg bone broth (211.77 mg/kg), followed by beef feet broth (254.40 mg/kg), and tail broth (134.93 mg/kg). Collagen was significantly different each dish (p<0.001), but chondroitin sulfate was not. And all nucleic acids showed significant differences.

Assessing the EORTC QLQ-BM22 Module Using Rasch Modeling and Confirmatory Factor Analysis across Countries: a Comprehensive Psychometric Evaluation in Patients with Bone Metastases

  • Background: The European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire Bone Metastases Module (EORTC QLQ-BM22) is a recently designed supplement to EORTC Quality of Life Questionnaire-C30 (EORTC QLQ-C30). Additional psychometric properties, especially using confirmatory factor analysis (CFA) and the Rasch model, are warranted. Materials and Methods: A total of 573 patients with bone metastases were enrolled from eight countries with a mean${\pm}$SD age of $55.8{\pm}13.7years$. Slightly more than two thirds of them were female (n=383; 66.8%). CFA was used to examine the BM22 framework; Rasch models were applied to understand misfit items and differential item functioning (DIF). Results: The fit indices were satisfactory in CFA (comparative fit index=0.972, Tucker-Lewis index=0.964, root mean square error of approximation=0.076, and standardized root mean square residual=0.045). All items fit well in the Rasch models (mean square values were between 0.5 and 1.5), and only one item (number 17) displayed DIF across gender. However, there were six DIF items across Canada and Taiwan, ten across Canada and Iran, and six across Taiwan and Iran. Conclusions: The BM22 has satisfactory psychometric properties, and could assess the QoL of patients with bone metastases specifically focusing on their symptoms. Clinicians may want to use it to capture the underlying QoL for patients with bone metastases. However, the score of item 17 should be interpreted with caution when comparing male and female patients. In addition, researchers should note that variation in DIF items may occur when conducting an international study.

Health-related Quality of Life After Treatment for Malignant Bone Tumors: A Follow-up Study in China

  • Aim: We conducted the present study to assess health-related quality of life (HRQoL) among bone cancer patients after surgical treatment in one large teaching hospitals in China, and assess the risk factors for improving the physical or mental HRQoL. Methods: 344 eligible adult patients who were admitted to the hospital with malignant bone tumors during the period of Jun. 2008 to Dec. 2011, and a reference group with 361 health cases was recruited in the same hospital during the same period. All 344 patients were followed up for one year. The HRQoL before treatment and after one year was evaluated with the Medical Outcome Short Form 36 (SF-36). Results: All 8 domains of HRQoL had the lowest scores greatly improved over the first year after discharge. However, the patients still had significantly lower scores in every domain than the reference group one year after discharge. Age and type of surgery were associated with HRQoL in the mental domain. Conclusion: The HRQoL of patients with malignant bone tumors greatly improved one year after the treatment. This study also highlighted the utility of HRQoL assessment for prognostic evaluation of patients after surgical treatment for bone cancer.

Effects of onion extracts (Allium cepa) inclusion in diets on growth performance, carcass characteristics, and bone morphometric of broiler chickens

  • Objective: Animal feed companies and researchers are currently embarking on quests for feed additives that could combat the pathogens while promoting growth as well as maintaining quality products. The current study aimed to evaluate the effects of onion extracts on growth performance, carcass quality, and bone morphometrics of broiler chickens. Methods: A total number of 200 one-day-old unsexed Ross 308 broiler chicks were assigned to 5 treatment groups, replicated 4 times. Each replicate (pens) held 10 chickens in a completely randomized design. The experimental diets were then randomly allotted to the pens which act as experimental units. The isoenergetic and isonitrogenous diets were formulated by including onion extracts at 0, 5, 10, 15, or 25 g/kg in a complete broiler diet. Feed intake, and body weight, were recorded then used to calculate feed conversion ratio. At the end of the experiment (42 days), four chickens from each pen were randomly selected for slaughter for carcass, bone morphology and sensory evaluation. Results: Results showed that onion extract supplementation did not affect (p>0.05) growth performances and meat sensory evaluation. However, there was a significantly increased (p<0.05) meat shear force in groups receiving onion extracts dietary treatments. Furthermore, onion extracts improved (p<0.05) bone morphology of broiler chickens in terms of weight, diameter, calcium, and phosphorous contents. Conclusion: In conclusion, onion extracts can be safely included in a commercial broiler diet as a growth promoter without causing adverse effects on growth performance traits and carcass quality in chickens. Onion extract supplementation improved tibia bone growth and strength in broiler chickens.

HA, TPS 및 $Al_2O_3$ 인공치근 매식체의 조직반응에 관한 실험적 연구 (EXPERIMENTAL STUDIES ON THE TISSUE RESPONSE OF HA COATED, TPS AND $Al_2O_3$ ARTIFICIAL ROOT IMPLANTS)

  • Since the concept of a direct contact between bone and implants, without interposed soft- tissue layers, was reported by Dr. $Br{\aa}nemark$, there has been increasing necessity for correct under-standing of bone-implant interface and surrounding tissue response. Beside quality of bone, surgical technique, load applied to implants, one must consider implant materials, design and surface characteristics to obtain osseointegration. In this study HA plasma-sprayed implants, TPS implants and $Al_2O_3$ implants were inserted into the alveolar bone of dog and tissue response was observed with radiograph, stereoscope, light microscope, and scanning electron microscope. Results were as follows : 1. There was rapid and active bone formation in the region adjacent to HA plasma-sprayed implants but in the deep supporting bone only slight bone formation was seen. 2. There was considerable lamella bone formation in the region adjacent to TPS implants and the deep supporting bone became more compact. 3. There was some gap and sclerosing bone formation in the adjacent region of $Al_2O_3$ implants, but there was irregular new bone formation in the deep supporting bone. Therefore, it seems that $Al_2O_3$ is not adequate for osseointegrated implants.

  • In this paper, it is fundamental purpose that finding the relationship between quality and structure of bone, by reconstructing the bone structure image, using ultrasound. In this study, longitudinal transmission method was used for experiment as basic measuring method, which is known as ultrasonic diagnosis method for human tibia. And using ultrasonic velocity parameter that can be detected and calculated with the transmitted signal, new estimated parameter, called Bone Area Fraction, is applied to reconstruct image. Through the in-vitro experiment in cattle's tibia bone, basic sectional image of bone which is similar in real bone structure image can be reconstructed.

폐경비만여성의 골밀도와 골대사 지표 및 식사의 질 상관성 조사 (Relationship between Bone Mineral Density and Bone Metabolic Biochemical Markers and Diet Quality Index-International(DQI-I) in Postmenopausal Obese Women)

  • Objectives: This study compared the differences of postmenopausal women's bone mineral density in relation to the degree of obesity, metabolism index and dietary factors that affect bone mineral density. Methods: The subjects included in the study are 39 postmenopausal women of normal weight with body mass index less than $25kg/m^2$ and 32 postmenopausal who are obese. Anthropometry and biochemical analysis were performed and nutrient intakes and DQI-I were assessed. Results: Normal weight women were $56.03{\pm}3.76years$ old and obese women were $58.09{\pm}5.13years$ old and there was no significant difference in age between the two groups. The T-score of bone mineral density was $0.03{\pm}1.06$ in normal weight women and $-0.60{\pm}1.47$ in obese women and this was significantly different between the two groups (p<0.05). Blood Leptin concentration was significantly lower in normal weight women ($6.09{\pm}3.37ng/mL$) compared to obese women in ($9.01{\pm}4.99ng/mL$) (p<0.05). The total score of diet quality index-international was $70.41{\pm}9.34$ in normal weight women and $64.93{\pm}7.82$ in obese women (p<0.05). T-score of bone mineral density showed negative correlations with percentage of body fat (r = -0.233, p=0.05), BMI (r = -0.197, p=0.017), triglyceride (r = -0.281, p=0.020) and leptin (r = -0.308, p=0.011). The results of multiple regression analysis performed as the method of entry showed that with 22.0% of explanation power, percentage of body fat (${\beta}=-0.048$, p<0.05), triglyceride (${\beta}=-0.005$, p<0.05) and HDL-cholesterol (${\beta}=0.034$, p<0.01), moderation of DQI-I (${\beta}=-0.231$, p<0.05) affected T-score significantly. Conclusions: The results of the study showed that obese women have less bone density than those with normal weight women. In addition, the factor analysis result that affect bone mineral density showed that intake of fat is a very important factor. Therefore, postmenopausal women need to maintain normal weight and manage blood lipid levels within normal range. They also need to take various sources of protein and reduce consumption of empty calorie foods that have high calories, fat, cholesterol and sodium.
