Assessment of Xenogenic Bone Plate and Screw using Finite Element Analysis

  • The aim of this study was to evaluate the biomechanical behavior of xenogenic bone plate system (equine bone) using a three-dimensional finite element ulna fracture model. The model was used to calculate the Von Mises stress (VMS) and stress distribution in fracture healing periods with metallic bone plate and xenogenic bone plate systems, which are installed while the canine patient is standing. Bone healing rate (BHR) (0%) and maximum VMS of the xenogenic plate was similar to the yield strength of equine bone (125 MPa). VMS at the ulna and fracture zones were higher with the xenogenic bone plate than with the metallic bone plate at BHRs of 0% and 1%. Stress distributions in fracture zone were higher with the xenogenic bone plate than the metallic bone plate. This study results indicate that the xenogenic bone plate may be considered more beneficial for callus formation and bone healing than the metallic bon plate. Xeonogenic bone plate and screw applied in clinical treatment of canines may provide reduced stress shielding of fractures during healing.

A comparison of fixation methods using three-dimensional finite element analysis following anterior segmental osteotomy

  • Objectives: This study sought to evaluate fixation methods and determine the best method for the postoperative stabilization of maxillary osteotomy. For our analysis we performed a three-dimensional finite element analysis of stress distribution on the plate, screw, and surrounding bone, as well as displacement onto the plate. Materials and Methods: We generated a model using synthetic skull scan data; an initital surface model was changed to a solid model using software. Modified anterior segmental osteotomy (using Park's method) was made using the program, and four different types of fixation methods were used. An anterior load of 100 N was applied on the palatal surface of two central incisors. Results: The Type 1 (L-shaped) fixation method gave stresses of 187.8 MPa at the plate, 45.8 MPa at the screw, and 15.4 MPa at the bone around the plate. The Type 2 (I-shaped) fixation method gave stresses of 186.6 MPa at the plate, 75.7 MPa at the screw, and 13.8 MPa at the bone around the plate. The Type 3 (inverted L-shaped) fixation method gave stresses of 28.6 MPa at the plate, 29.9 MPa at the screw, and 15.3 MPa at the bone around the plate. The Type 4 (I-shaped) fixation method gave stresses of 34.8 MPa at the plate, 36.9 MPa at the screw, and 14.9 MPa at the bone around the plate. The deflection of the plates for the four fixation methods was 0.014 mm, 0.022 mm, 0.017 mm, and 0.018 mm, respectively. Conclusion: The Type 3 (inverted L-shaped) fixation method offers more stability than the other fixation methods. We therefore recommend this method for the postoperative stabilization of maxillary osteotomy.

잠김 금속판(LCP-DF)을 이용한 대퇴골 원위부의 관절외 복합골절 치료시 나사못 배열에 따른 생체역학적 안정성 분석 (Effects of Screw Configuration on Biomechanical Stability during Extra-articular Complex Fracture Fixation of the Distal Femur Treated with Locking Compression Plate)

  • The locking compression plates-distal femur(LCP-DF) are being widely used for surgical management of the extra-articular complex fractures of the distal femur. They feature locking mechanism between the screws and the screw holes of the plate to provide stronger fixation force with less number of screws than conventional compression bone plate. However, their biomechanical efficacies are not fully understood, especially regarding the number of the screws inserted and their optimal configurations. In this study, we investigated effects of various screw configurations in the shaft and the condylar regions of the femur in relation to structural stability of LCP-DF system. For this purpose, a baseline 3-D finite element (FE) model of the femur was constructed from CT-scan images of a normal healthy male and was validated. The extra-articular complex fracture of the distal femur was made with a 4-cm defect. Surgical reduction with LCP-DF and bone screws were added laterally. To simulate various cases of post-op screw configurations, screws were inserted in the shaft (3~5 screws) and the condylar (4~6 screws) regions. Particular attention was paid at the shaft region where screws were inserted either in clustered or evenly-spaced fashion. Tied-contact conditions were assigned at the bone screws-plate whereas general contact condition was assumed at the interfaces between LCP-DF and bone screws. Axial compressive load of 1,610N(2.3 BW) was applied on the femoral head to reflect joint reaction force. An average of 5% increase in stiffness was found with increase in screw numbers (from 4 to 6) in the condylar region, as compared to negligible increase (less than 1%) at the shaft regardless of the number of screws inserted or its distribution, whether clustered or evenly-spaced. At the condylar region, screw insertion at the holes near the fracture interface and posterior locations contributed greater increase in stiffness (9~13%) than any other locations. Our results suggested that the screw insertion at the condylar region can be more effective than at the shaft during surgical treatment of fracture of the distal femur with LCP-DF. In addition, screw insertion at the holes close to the fracture interface should be accompanied to ensure better fracture healing.

생체흡수성 재료를 이용한 하악골절 치료의 결과 (The Result of Mandible Fracture Fixations with Biodegradable Materials)

  • Purpose: Traditionally, titanium miniplate has been used for rigid fixation of mandible fractures. However, the limitations of metal plate have been reported such as hypersensitivity, interference with the cranio-facial growth of growing child, secondary bone resorption around the plate, foreign body reaction, declination of primary callus formation, and bone atrophy, and so forth. Recently, biodegradable miniplate has been introduced and used as an alternative to the metal plate despite of its lower strength. This study evaluated the usefulness and stability of biodegradable plate and screw for treatment of mandible fractures. Methods: In this study, 61 patients(92 areas) diagnosed as mandible fracture in the last 2 years have been reviewed. We used titanium plate and screw in 32 patients, and biodegradable plate and screw($INION^{(R)}$) in 29 patients. Stability of plates and screws, bony healing process and its side effects were observed by clinical and radiographic assessment. Results: In the titanium material group, one of malocclusion, two of mouth opening limitation, three of pain, three of palpation were shown. The plate of six patients involved in these complications were removed. In the biodegradable group, two of mouth opening limitation, two of pain, one of localized wound infection were shown and one plate was removed secondarily. Conclusion: There was no statistical difference between two groups in bony healing and complication rates. Biodegradable implants show efficient stability during initial bone healing and low side effects in long-term follow up periods.

  • First case, a one-year-old female Shih Tzu weighing 3 kg with closed transverse fractures of bilateral mandibular body between 2nd and 3rd premolar teeth were treated surgically with bone plates and screws, and cranial luxation of temporomandibular joints were reduced to closed method. Second case, a 8.4 years old male Yorkshire terrier weighing 2.6 kg with closed transverse/oblique fractures of bilateral mandibular body between premolar and molar teeth were treated surgically with bone plates and screws. Radiographic examination revealed that the fractures in two cases fixed well and temporomandibular joint was stabilized. One month after operation, jaw activity was excellent and malocclusion wasn't observed.

악안면 골절 치료시 흡수성 고정판의 사용에 관한 임상 연구 (CLINICAL STUDY OF RESORBABLE PLATE AND SCREW FOR TREATMENT OF MAXILLOFACIAL FRACTURES)

  • Purpose : This study evaluated the usefulness of resorbable plate and screw for treatment of maxillofacial bone fractures. Patients and methods : From july, 2000 to july, 2002, we used resorbable plates and screws($Biosorb^{(R)}$ FX, Bionix Inc, Finland) on 126 patients for treatment of maxillofacial bone fractures. Among them, 80 patients were capable of periodic following up to present season. We evaluated these patients with clinical, radiographic findings and subjective satisfaction. Results : 80 patients composed of 63 men and 17 women. Complication rates are 7.5%(6/80) recorded. 1 of bulging sensation, 3 of infection sign, 2 of the plate detachment were shown. The plates involved in such complications were removed. The other patients(92.5%) were not shown any problems. We could not find complete resorption of screw holes in the mandibular symphysis area even though two years later after surgery. Conclusion : Bioresorbable plates and screws are useful as fixation material at maxillofacial fractures although strength is thought to be weaker than titanium plate. But to reduce the possible complicaions, need to careful clinical and radiographical evaluations.

Additional fixation using a metal plate with bioresorbable screws and wires for robinson type 2B clavicle fracture

  • Simple clavicle fractures can achieve satisfactory results through conservative treatment, and the less frequency of nonunion. Non-union or malunion can occur in displaced clavicle fractures or comminuted shaft fractures. Treatment of displaced comminuted clavicle shaft fractures is performed by holding together the free fragments with interfragmentary screws or wires and fixing them to the clavicle with a plate. Therefore, we performed interfragmentary fixation using open reduction and internal fixation with bioresorbable screws (Mg-Ca alloy, Resomet bioresorbable bone screw; U&I Corp.) and bioresorbable wires (Mg-Ca alloy, Resomet bioresorbable K-wire and pin, U&I Corp.) for displaced comminuted clavicle fractures (Robinson type 2B) and additionally used a metal plate. We expected decreased irritation and infection due to absorption after surgery. We report four cases that were treated in this way.

  • 본 논문에서는 골절 치료에 사용되고 있는 기존의 금속판과 나사고정 장치를 개선하여, 뼈에 점진적인 생리하중을 가할 수 있도록 고안된 내고정 장치를 살펴보았다. 이 장치는 골절판과 나사를 결합할 때, 그 사이에 생분해성 재질이나 기타 댐핑 특성이 우수한 재질의 와셔를 삽입하는 것을 그 특징으로 한다. 본 논문에서는 주로 이 장치의 충격 흡수 특성을 파악하기 위해, 돼지 대퇴골과 소 경골에대해 충격 망치를 이용하여 얻은 진동 실험 결과를 보여주며, 특히 와셔의 유무에 따른 충격 흡수 특성을 실험적으로 비교하였다.

  • Conventional compression bone fracture plates sometimes cause osteoporosis under the plate due to their high rigidity which in turn transfer physiological load mostly through the plates and screws. In order to prevent the osteopenia we have designed a system which have a viscoelastic washer between plate and screw head. The washer is made of a biocompatible ploymer (untra high molecular weight polyethylene, UHMWPE). Three-dimensional finite element meshes of the human femur with the conventional and new concept bone plate ere generated and the comparative stress analyses are performed with static half-stance loading condition. The results of analyses showed that could reduce the stress shielding effect compared with the conventional plate.
