Twenty-nine cases of unicameral bone cyst developed in long bone of children have been treated and followed up for 4.5 years in average form Department of Orthopaedic Surgery, Guro Hospital, Korea University, College of Medicine since September, 1983, Treatment for those lesions differed to form largely two groups, one of which consised of insillation of Methyl-prednisolone for non-weight bearing bones(12 humeri) and the other of curettage and autogenous bone graft for weight-bearing bones(7 femur). Methl-prednisolone group required repetition of instillation for 3.5 time in average spanning over 4 years until cloudy obliteration occurs. Curettage and bone graft had healed in 3 year 6 months' time in average. There were neither recurrence nor pathologic fractures of the lesions with the latter group. Immobilization period was virtually non with Methyl-prednisolone group and 4-6 weeks by hip spica with curettage and bone graft group. As conclusions, It seems confirmed that treatment strategy of unicameral bone cyst consisted of Methyl-prednisolone instillation for humerus lesions and early curettage and bone graft for femur lesions is applicable as guideline having solid ground in clinical experiences.
Aneurysmal bone cyst is a benign, highly vascular bony lesion of unknown origin that may present difficult diagnostic and therapeutic problems. It is usually located on long bones, but rarely on the spine. The lesion is usually treated successfully by curettage and bone grafting when it involves bones of the extremities, but there are special considerations in the management of the lesion when located on the spine. We report a case of aneurysmal bone cyst in on the 8th thoracic spine treated with surgical treatment.
Purpose: Parathyroid hormone (PTH) therapy has drawn attention, as an alternative to anti-resorptive drugs since PTH accelerates bone density by anabolic action. The purpose of this study was to identify the effect of intermittent PTH administration on jaw bones of rat undergone bilateral ovariectomy. Materials and Methods: Nine female Sprague-Dawley rats were divided into three groups. PTH group was ovariectomized (OVX) to induce osteoporosis and PTH $30{\mu}g/kg$ was administered 1 week after the surgery. In OVX group, ovariectomy was performed and only vehicle was administered by subcutaneous injection 3 times per week. Control group was subjected to sham surgery. The animals were sacrificed 8 weeks after the surgery and specimens were obtained from ilium and upper and lower jaw bones. Histological investigation was carried out by using an optical microscope and micro-computed tomography was taken to examine structural property changes in each bone sample. Result: In the ilium, the bone volume ratio (bone volume/total volume, BV/TV) of PTH, OVX and control groups was $53.75%{\pm}7.57%$, $50.61%{\pm}12.89%$, $76.20%{\pm}5.92%$ (P=0.061) and bone mineral density (BMD) was $1.12{\pm}0.09$, $0.88{\pm}0.48$, $1.38{\pm}0.07g/cm^3$ (P=0.061). In the mandible, BV/TV of PTH, OVX and control groups was $64.60%{\pm}12.17%$, $58.26%{\pm}9.63%$, $67.54%{\pm}14.74%$(P=0.670) and BMD was $1.21{\pm}0.17$, $1.19{\pm}0.13$, $1.27{\pm}0.18g/cm^3$ (P=0.587). In the maxilla, BV/TV of PTH, OVX and control groups was $61.19%{\pm}8.92%$, $52.50%{\pm}11.22%$, $64.60%{\pm}12.17%$ (P=0.430) and BMD was $1.20{\pm}0.11$, $1.11{\pm}0.16$, $1.21{\pm}0.17g/cm^3$ (P=0.561). No statistically significant difference was found in any variables in all groups. Histological observation revealed that the ilium in OVX group demonstrated sparsely formed trabecular bones compared with other groups. However, upper and lower trabecular bones did not present significant differences. Conclusion: Intermittent administration of PTH appears to affect the microstructure of rat jaw bones, but statistical significance was not found. However, the measurements in this study partly implicated the possible anabolic effect of PTH in vivo.
The classification of bony pieces which were excavated from Jongdali archaeological site in Jeju was studied. The total number of bone remains were 81 pieces, in which 31 pieces were classified into animal bones. The animal species consisted of Cervus spp., Sus scrofa, Bos taurus and Equus caballus. This finding suggests that the major fauna in this peroid(B.C. 100 - A.D. 100) is wild boar, deer, horse and cattle.
Highly porous composite bioceramic bone scaffolds were developed using sintered gnotobiotic pig bones. These scaffolds consisted of poly-D,L-lactic acid (P(D,L)LA) and bioceramic materials of pig bone powder. The bone scaffolds were able to promote biocompatibility and possess interconnected pores that would support cell adhesion and proliferation adequately. The composite scaffolds were tested with dental pulp stem cells for cytotoxicity test. Cells seeded on the composite scaffolds were readily attached, well proliferated, as confirmed by cytotoxicity test, and cell adhesion assessment. The composite bone scaffold had no toxicity in cytotoxicity test on the extract of 0.013 g scaffold to 2 ml culture medium. The cells on the composite bone scaffold proliferated better than cells on the P(D,L)LA scaffolds.
Purpose: To evaluate the effect of the systemic osteoporosis on radiographic density of alveolar bone and cortical thickness of mandible. Materials and Methods: The bone mineral density values of lumbar and femur were measured by dual-energy X-ray absorptiometry and T scores of lumbar, femur were obtained respectively. Radiographic densities of alveolar bones and panorama mandibular index (PMI, represents as cortical thickness) were analysed statistically according to age and T score variables. Results: The radiographic density of alveolar bone of maxillary molar showed significant difference by age and femur T group. That of mandibular molar showed significant difference between femur T group. Panorama mandibular index showed significant difference between age groups. Conclusion: The radiographic density of alvealar bones was more dependent on age and femur T than lumbar T. Cortical thickness of mandible was correlated with increasing age.
Several workers reported the relationship between osteoporosis and age-related reductions in the BV/TV (bone volume fraction) of vertebral trabecular bones. However, there were few micro finite element (micro-FE) models to account for the treatments of the osteoporotic trabecular bone. In the present study, micro-FE models of osteoporotic and hormone-treated bone models were constructed to analyze the effect of specimen location and boundary condition on mechanical characteristics of hormone treatment model for osteoporotic trabecular bone. Top and bottom sections of specimens were also investigated individually to study the effect of specimen location. Hormone-treated models were allowed to have the same relative BV/TV (13.4%) as that used in models of previous researchers. The present study reported the elastic and plastic characteristics of the osteoporosis and hormone-treated bone models. In the present study, in-situ boundary condition was applied to the simulated compression tests for in-vivo condition of vertebral trabecular bone. The present study indicated that the hormone therapy was likely to improve the mechanical characteristics of osteoporotic bones and the mechanical characteristics of vertebral trabecular bone specimen were dependent on the captured location and boundary condition.
Segmenting vessels in lower extremity CT images is very difficult because of gray level variation, connection to bones, and their small sizes. Instead of segmenting vessels, we propose an approach that segments bones and subtracts them from the original CT images. The subtracted images can contain not only connected vessel structures but also isolated vessels, which are very difficult to detect using conventional vessel segmentation methods. The proposed method initially grows a 3-dimensional (3D) volume with a seeded region growing (SRG) using an adaptive threshold and then detects junctions and forked branches. The forked branches are classified into either bone branches or vessel branches based on appearance, shape, size change, and moving velocity of the branch. The final volume is re-grown by collecting connected bone branches. The algorithm has produced promising results for segmenting bone structures in several tens of vessel-enhanced CT image data sets of lower extremities.

This study analyzed the regional morphological and mechanical characteristics of vertebrae by using micro-computed tomography (micro-CT) and micro finite element analysis (FEA). For the present study, the $12^{th}$ human thoracic vertebral bones (an 85-years female and a 48-years male) were used. These were scanned by using micro-CT. Structural parameters were evaluated from the acquired 20 image data for fifteen $4{\times}4mm^2$ regions (five regions in respective layers of superior, middle and inferior part) in the thoracic vertebral trabecular bones. $4{\times}4{\times}4mm^3$ cubic finite element models of each regions were created at $70{\mu}m$ voxel resolution to investigate effective modulus ($E^+$). The present study indicated that there were significant differences in morphological and elastic mechanical characteristics of each region. There are close relationship between effective modulus and structural model index (SMI) in the bone of the 48-years male and between effective modulus and bone volume fraction (BV/TV) in the bone of the 85-years female. In addition, the effective modulus of central regions is about 80% stiffer than that of lateral regions at transverse plane. These findings may be likely to explain the previous result that a change of loading distribution of the vertebral trabecular bones is caused by spinal curvature and nucleus pulpous degeneration of the intervertebral disc.
