Experiences of the First 130 Patients in Gangnam Severance Hospital (강남세브란스병원 토모테라피를 이용한 치료환자의 130예 통계분석 및 경험)

  • Purpose: We are trying to analyze 130 patients' conditions by using our Helical Tomotherapy, which was installed in our center in Oct. 2007. We will be statistically approach this examination and analyze so that we will be able to figure out adaptive plans according to the change in place of the tumor, GTV (gross tumor volume), total amount of time it took, vector (${\upsilon}=\surd$x2+y2+z2) and the change in size of the tumor. Materials and Methods: Objectives were the patients who were medicated with Tomotherapy in our medical center since Oct. 2007 August 2008. The Average age of the patients were 53 years old (Minimum 25 years old, Maximum 83 years old). The parts of the body we operated were could be categorized as Head&neck (n=22), Chest (n=47), Abdomen (n=25), Pelvis (n=11), Bone (n=25). MVCT had acted on 2702 times, and also had acted on our adaptive plan toward patients who showed big difference in the size of tumor. Also, after equalizing our gained MVCT and kv-CT we checked up on the range of possible mistake, using x, y, z, roll and vector. We've also investigated on Set-up, MVCT, average time of operation and target volume. Results: Mean time on table was 22.8 minutes. Mean treatment time was 13.26 minutes. Mean correction (mm) was X=-0.7, Y=-1.4, Z=5.77, roll=0.29, vector=8.66 Head&neck patients had 2.96 mm less vector value in movement than patients of Chest, Abdomen, Bone. In increasing order, Head&neck, Bone, Abdomen, Chest, Pelvis showed the vector value in movement. Also, there were 27 patients for adaptive plan, 39 patients, who had long or multiple tumor. We could know that When medical treatment is one cure plan, it takes 32 minutes, and when medical treatment is two cure plan, it takes 40 minutes that one medical treatment takes 21 minutes, and the other medical treatment takes 19 minutes. Conclusion:With our basic tools, we could bring more accurate IMRT with MVCT. Also, through our daily image, we checked up on the change in tumor so that adaptive plan could work. It was made it possible to take the cure of long or multiple tumor, the cure in a nearby OAR, and the complicated cure that should make changes of gradient dose distribution.

Studies on the Pathogenic Staphylococci aureus from Nasal Cavity (비강(鼻腔)에서 분리(分離)한 황색포도구균(黃色葡萄球菌)의 생화학적(生化學的) 성상(性狀)에 관(關)한 연구(硏究))

  • Staphyylococci are responsible for over 80 per cents of the suppurative diseases encountered in medical practice. They cause most suppurative infections of the skin but may also invade and produce severe infections in any other parts of the body. In order to know the carrier rate of staphylococci between the parahospital workers and in the hospital workers, the author undertook isolation of S aureus from nasal cavity on 68 cases of freshmen, 31 cases of sophomores, and 37 cases of juniors in Busan National University, and 30 cases of nursing students, 30 cases of nurses, 30 cases of nurses in charge of operating room and 30 cases of doctors in Busan National University Hospital. The tested total cases were 256 cases which were 136 Cases in parahospital workers and which were 120 cases in hospital workers. The biochemical characters of S aureus strains isolated were studied on coagulase test, mannitol test, hemolysis test and sensitivity test to antimicrobial agents. The results obtained were as follows: 1. S aureus were isolated 49 cases(29.7%) from nasal cavity parahospital workers and were isolated 67 cases(55.8%) from nasal cavity on hospital workers. 2. Among 40 strains of S aureus isolated from parahospital worker's nasal cavity coagulase positive were 29 cases(72.1%), and coagulase negative were 11 cases(27.5%). And mannitol positive were 29 cases(72.5%), and mannitol negative were 11 cases(27.5%). 3. Among 67 strains of S aureus isolated from hospital worker's nasal cavity coagulase positive were 59 cases(88.1%), and coagulase negative were 8 cases(11.9%). And mannitol positive were 49 cases(73.1%), and mannitol negative were 18 cases(26.9%). 4. The hemolysis test of each erythocytes on coagulase and mannitol positive S aureus isolated were sensitive to rabbit(40 cases: 81.6%), guinea pig(26 cases: 53.6%), sheep(13 case: 26.5%), and were not sensitive to chicken and human erythrocytes, respectively. 5. The hemolysis test of each erythocytes on 10 strains of coagulase and mannitol negative S aureus isolated were not sensitive to all of, erythrocytes. 6. The sensitivity test to the various chemotherapeutic agents was almost sensitive to the strains isolated from parahospital workers, but was almost resistant to the strains isolated from hospital workers.

Risk Assessment of Agricultural Worker's Exposure to Fungicide Thiophanate-methyl during Treatment in Green Pepper, Cucumber and Apple Fields (고추, 오이 및 사과 재배 중 살균제 Thiophanate-methyl 살포 시 농작업자의 노출 및 위해 평가)

  • The present study was carried out to assess exposure and risk to thiophanate-methyl wettable powder for agricultural worker during mixing/loading and application with power sprayer in green pepper, cucumber and apple fields. Dermal exposure was measured with patches, gloves, socks and masks, while inhalation exposure was evaluated with personal air pump and solid sorbent. Those methods were full validated before experiment. During mixing/loading, dermal exposure amount in green pepper, cucumber and apple fields was $24.0{\pm}6.7$, $4.5{\pm}1.5$ and $18.5{\pm}0.6mg$, corresponding to mean 0.007, 0.001 and 0.005% of prepared active ingredient, respectively. The major exposed part for mixer/loader was hands (78-92%). Dermal exposure amount for applicator in green pepper, cucumber and apple fields was $84.9{\pm}14.0$, $34.0{\pm}20.8$ and $30.7{\pm}9.1mg$, corresponding to mean 0.024, 0.016 and 0.013% of applied active ingredient, respectively. The main body parts of exposure in apple field were hands, while thighs and shins in other fields. Inhalation exposure amount in green pepper, cucumber and apple fields was $1.5{\pm}2.2$, $52.7{\pm}48.9$ and $4.0{\pm}4.9{\mu}g$ during mixing/loading and $0.2{\pm}0.1$, $23.2{\pm}12.4$ and $0.4{\pm}0.6{\mu}g$ for applicator, respectively. These results were suggested that main factors affecting dermal exposure were contact frequency to the plants, foliage density, hygienic behavior, work type, and working environment, while inhalation exposure was affected mainly by working environment, especially wind. In risk assessment, margin of safety for thiophanate-methyl in all cases was over 1. However, during application in green pepper field, margin of safety was close to 1.

Combination of Natural Products Removing ROS for Growth Promoting Effects of the Useful Enterobacteria Lacobacillus sp. (ROS 제거기능을 갖는 천연산물로부터 장내 유용세균 Lacobacillus속의 성장을 촉진시키는 조합의 구성)

  • The growth of Enterobacteria, Lactobacillus acidophilus and L. plantarum was promoted by natural products themselves bearing antioxidative capacity and combined two, three and four kinds of them.1. acidophilus was promoted by Paeonia japonica, Acori graminei rhizoma, Aurantii nobilis pericarpium, alone, and two mixed combinations were composed of Acori graminei rhizoma and Cinnamomi cortex, Aurantii nobilis pericarpium and Puerariae radix, and three mixed combinations were formed with Cimicifugae rhi- zoma, Cinnamomi cortex and Glycyrrhizae radix, and four mixed combinations were made up Glycyrrhizae radix, Theae folium, Scutellaria baicalensis george and Cinnamomi cortex. There four combination promoted growth of 1. acidophilus with 1.3 times than that of control, and their antioxidative capacity also higher than that off times, the ratio of elimination of hydroxyl radical was more than 70% in dilution rate of 100 times. In the case of 1. plantarum was premoted by Corm fructus, Nelumbo nuclfera gaertner Sophorae flos, alone, and two mixed combinations were organized with Schizandrae Iructus and Theae folium, Paeonia japonica and Epimedii herba, and three mixed combinations were combined with Corni fructus and Theae folium and Paeonia japonica, Corni fructus and Coptidis rhizoma and Schieandrae fructus, and four mixed combinations were composed of Corm fructus, Theae folium, Coptidis rhizoma, Scutellaria baicalensis george, and Corm fructus, Schizandrae fructus, Paeonia japonica, Theae folium. These four combinations assisted growth of 1. plantarum with 1.5 times than that of control, and their antioxidative capacity also higher than that of 4-5 times, the ratio of elimination of hydroxyl radical was more than 75% in dilution rate of 100 times. As these combinations of natural products could activate some parts of body, they might be applied pharmaceuitcal sources, functional foods, and expected to fermentative beverages.

The Growth Promoting Effect of Bifidobacterium bifidum by Combination of Natural Products Bearing Antioxidative Capacity (항산화 천연산물의 조합구성에 의한 Bifidobacterium bifidum의 성장 촉진 효과)

  • The growth of B. bifidum was promoted by natural products bearing antioxidative capacity and mixed combined two, three and four kinds of them. B. bifidum was expressed a good growth by Nelumbo nuclfera gaertner, Corni fructus, Beiamcanda chinensis, alone, and two mixed combinations were composed of Nelumbo nuclfera gaertner and Corni fructus, Nelumbo nuclfera gaertner and Beiamcanda chinensis, Nefumbo nuclfera gaertner and Theae folium, three mixed combinations were oraganized with Nelumbo nuclfera gaertner, Corni fructus and Theae folium, Nelumbo nuclfera gaertner, Corni fructus and Beiamcanda chinensis, Nelumbo nuclfera gaertner, Theae folium and Beiamcanda chinensis, and four mixed combinations were formed with Nelumbo nuclfera gaertner, Theae folium, Beiamcanda chinensis aud Corni fructus, Theae folium, Beiamcanda chinensis, Corni fructus and Glycyrrhizae radix, Nelumbo nuclfera gaertner, Corni fructus, Teae folium and Glycyrrhizae radix, and Nelumbo nuclfera gaertner, Corni fructus, Beiamcanda chinensis and Glycyrrhizae radix. These few mixed combinations promoted cell growth by 2.6 times than that of control, and its antioxidative capacity was also 5.6 times higher, and the ratio of elimination of hydroxyl radical was more than 80% in each dilution rate. As these combinations of natural products could activate some parts of body, they might be applied to pharmaceuitcal applications, functional foods, also expected to multifunctional fermentative beverages.

A Study on the Demand for Equipent Development in Nursing (간호기기 개발수요 조사연구)

  • The objectives of thes study were to identify the need for equipment development in nursing, and to determine the priorities for that development. The study was descriptive study done between March 2 and May 30, 1995, in which the subjects, including 421 patients, 223 family members, and 198 nurses from neurosurgery, orthopedic, rehabilitation medicine, internal medicine and intensive care units of nine general hospitals in Seoul, completed a questionnarie developed by the research team. The questionnaire consisted of 35 open and closed questions. Data was analyzed using frequencies and percentages. The results ware summarized as follows: 1) The average age of the nurses was 27.9 years, 48% of the patients were between 20 and 40 years of age, and 17% were over 60. The average lingth of experience for the nurse subjects was four years five months with 36.9%. having over five years experience. The most frequent diagnoses of patients were spinal disc(35.9%), internal medicine disease(26.0%), cerebral vascular accident(16.6%) and spinal cord injury(10%) 2) Many of the nurses(96.4%) reported deficiencies with existing equipment and 96.5% of the nurses, but only 79.8% of the patients, nurses' time. Further, 82.3% of the nurses and 75.8% of the patients felt that the development of new equipment would lead to a decrease in the cost of nursing care. 3) Nurses felt that the greatest areas of inconvenience were patient feeding(71.7%), hygiene(71.2%), caring for a patient confined to bed(70.7%), patient clothing(67.2%), mobility transfers(63.5%) and urinary elimination(52.0%). However, patients and family members listed the following as being the most inconvenient: urinary elimination(58.7%), Hygiene(50.5), feeding(48.4%), mobility transfers(47.1%) and bed care(45.2%). 4) Generally the nurses listed more inconveniences and patients and family members listed more demands for the development of equipment. These included utensils with large handles, and regulators for tube feedings; mattresses that provide for automatic position change and massage, which have patient controlled levers and a place for bed pan insertion; automatic lifts or transfer from bed to wheelchair; equipment to facilitate washing and oral hygiene as well as equipment that will allow patients with spinal cord injuries easy access to showers; a bed pan/urinal for women that is comfortable and effective from which urine can be measured and disposed of easily; disposable dressing sets and tracheostomy care sets and a convenient way of measuring changes in wound size; a safe delivery system for oxygen, a variety of mask sizes and better control of humidity, tracheal material than at present, as well as a communication system for patients with tracheostomies; clothing that will allow access to various parts of the body for treament or assessment without patients having to remove all of their clothing; and finally a system that will allow the patient to control lighting, telephones and pagers. Priority areas for equipment development reported by the nurses were, urinary elimination(58. 7%), hygiene(50.5%), feeding(48.4%), mobility transfers(47..1%), bowel elimination(40.8%). Those reported by the patients family members were feeding(71.7%), hygiene(70.0%), bedcare(70.7%), clothing(67.2%), mobility transfers(63.6%), urinary elimination(52.9%) and bowel elimination(50.5%) Altogether, nurses, patients and family members listed the following as priorities; clothing (178), bed care(144), urinary elimination(92), environment(81), hygiene(70). Further, a health professional forum listed urinary elimination, oxygen delivery, medication delivery, mobility transfers, bed care and hygiene in that order as priority areas. From this study it can be concluded that the first need is to develop equipment that will address the problems of urinary elimination. MUSIC THERAPY FOR ADOLESCENTS WITH CONDUCT DISORDER (품행장애 청소년의 음악치료 사례연구)

  • The short-term music therapy was performed for adolescents with conduct disorder admitted to Seoul National Mental Hospital for 3 months from Jun to September, 1998. This case study focused mainly on two female patients who participated regularly in the group music therapy. The music therapy process was divided into three phases;beginning, opening up, and closing. This music therapy session consisted of three parts;hello song as beginning, various musical activities, and sound & movement activity as closing. Free musical improvisation, song discussion, musical monodrama, and sound & movement were the mainly applied techniques. Free improvisation was used to enhance, motivate, identify and contain the adolescents' feelings and ideas. Song discussion was used to convey their thoughts and to support each other. Musical monodrama was used to make them have insights into interpersonal relationships. Sound & movement was used to enhance spontaneity. It made them explore their body and voice as an expressive medium. Throughout three months period of music therapy, patient A's communication skill, socialization, and behavior areas were assessed with improvement. She could use music as a symbolic form and was able to share her feelings about herself and her family. Patient B's self-expression and cognitive areas were assessed with improvement. She became more spontaneous and could verbalize her emotions during the group session. Music as a non-verbal and therefore often a non-threatening medium wherein so much can be expressed provided two female patients an atmosphere where a sense of trust may be regained.

A Web-based Internet Program for Nutritional Counseling and Diet management of Patient with Diabetes Mellitus (당뇨병 환자의 웹기반 식사관리 및 영양상담 프로그램)

  • The purpose of this study was to develop a web-based internet program for nutritional counseling and diet management of patient with diabetes mellitus. The program consisted of four parts according to their functions and contents. The first part explained the metabolism of glucose and mechanism of insulin and insulin receptor expressed by flash 6.0, and defined the diabetes mellitus. The second part is to assess the general health status such as body weight, obesity index, basal metabolic rate and total energy requirement by the input of age, sex, height, weight and degree of activity. This part also provides tlne patient with menu lists and one day menu suitable to his weight and activity, and offers the information for food selection, snacks, convenience foods, dine-out, behavioral modification, cooking methods, food exchange lists, dietary education using buffet, and information on energy and nutrients of foods and drinks, and top 20 foods classified by nutrients. The third part is designed to investigate dietary history of patient, that is, to find out his inappropriate dietary habit and give him some suggestions for appropriate dietary behavior. This part also offers on-line counseling, follow-up management and frequently asked questions. The fourth part is evaluating their energy and nutrients intake by comparing with recommended dietary allowance for Koreans or standardized data for patient with diabetes mellitus. In this part, it is also analyzing energy and nutrients of food consumed by food group and meals, and evaluating the status of nutrient intake. These results are finally displayed as tabular forms and graphical forms on the computer screen. Therefore it is expected that the web-based internet program developed in this study will play a role in their health promotion as widely using by diabetic patients.

A STUDY ON THE EFFECT OF THE CHINCAP BY FINITE ELEMENT ANALYSIS IN JUVENILE SKELETAL CLASS III PATIENTS (유년기 골격성 III급 부정교합자에서 이모장치의 효과에 관한 유한요소분석법적 연구)

  • This study was conducted to investigate the changes in the structural parts of the craniofacial skeleton subsequent to chincap therapy in the juvenile skeletal Class III patients. The subject consisted of 29 Korean children(14 males, 15 females) who had skeletal Class III malocclusion and were undergone chincap therapy from the beginning of the treatment (and an auxilliary upper removable appliance, if necessary). The control group was composed of 21 children(10 males, 11 females) with skeletal Class III malocclusion who had no orthodontic treatment. Cephalometric data at the mean age of 7 and 2 years later were analyized by finite element method, and compared between groups by independent group t-test(p<0.05). The results of the present study were as follows; 1. There were no significant changes in the cranial base, posterior face, upper anterior face, ramus, chin and soft tissues by the chincap therapy. 2. The mandibular body showed significant differences in the minimum extention ratio and the overall shape ratio. This means that the vertical direction of growth was retarded by the chincap therapy. 3. The major direction of the growth in the maxillary basal bone was significantly more horizontal in the experimental group, which suggests that the vertical growth of maxilla was inhibited. 4. There was statistical difference in the major direction of the growth of the anterior face between groups. This may be due to the significant difference in the major direction of growth of the lower anterior face, supposed to be resulted from the mandibular rotation and/or displacement by the chincap therapy. The change in the oral functional space seemed to be caused by the same reason. 5. From the standpoint of these results, the retardation of growth, the changes of the growth direction and the morphological changes could be accepted partly, but the major effect of the chincap seems to be the rotation and the displacement of the mandible.

C. butyricum was showed a good growth by Lycii fructus, Sophorae flos, Chelidonium majus L., Atractylodis rhizoma alba, Paeonia japonica, alone, and two mixed com-binations were composed of Paeonia japonica and Epimedii herba, Paeonia japonica and Aurantii nobilis pericarpium, Paeonia japonica and Puerariae radix, Pneonia japonica and Angelicae gigantis radix, and three mixed combinations were organized with Epimedii herba, Sophorae flos and Nnelumbo nuclfera gaertner, and Epimedii herba, Sophorae flos and Scutellaria haicalensis george, and Epimedii herba, Sophorae flos and theae folium, and Epimedii herba, Paeonia japonica and Angelicae gigantis radix, and four mixed combinations were formed with Epimedii herba, Puerariae radix, Nelumbo nuclfera gaertner and Paeonia japonica, and Epimedii herba, Puerariae radix, Nelumbo nuclfera gaertner and Theae folium, and Epimedii berba, puerariae radix, Nelumbo nuclfera gaertner and Angelicae gigantis radix, and puerariae radix, Nelumbo nuclfera gaertner, paeonia japonica and Theae folium. As these combinations of natural products could activate some parts fo body, they might be applied to pharmaceuitcal applications, functional foods, antiaging tea, also expected to promote useful enterobacterial growth for multifunctional fermentative beverage.
