외상성 기관-식도파열에 대한 외과적 고찰-2례보고- (Surgical Evaluation of Traumatic Tracheo-Esophageal Rupture-Two Cases Report-)

  • 흉부 둔상에 의한 기도와 식도의 동시적 파열상은 매우 드물게 발생되고 있다. 그러나 최그들어 고속 교통사고의 증가와 환자에 대한 응급체계의 발전으로 점차 치험 보고례가 증가되고 있다. 저자들은 흉부의 둔상에 의하여 기도와 식도에 종적으로 긴 파열상을 입은 환자 2례의 치험을 보고한다. 1례는 이전 병원에서 시행한 기관식도 재건술이 실패한 상태에서 전원되어와 2차적으로 식도절제 및 기관재건술을 실시하였으나 사망하였고. 다른 1례는 식도절제 및 기도재건술로 합병증이 없이 생존하였다.

  • Trauma, congenital malformation and aging process can be a cause of the deviation of laryngeal prominence in the thyroid cartilage. Among these, the senility is the most common cause. Usually, ossification in the thyroid cartilage has occurred symmetrically, but the asymmetrical event leads to the shift of laryngeal prominence. Also, such deformity can provoke protrusion of false vocal fold. A 75-year-old man with hoarseness and globus sense in throat visited our clinic. Five years ago, he experienced a blunt trauma on left midline neck and had a concave deformity in the left thyroid cartilage lamina. Laryngoscopic findings revealed a marked protrusion in the left false vocal fold. We performed the laryngeal microsurgery to discriminate the tumorous condition. The pathology revealed non-pathologic mucosa. We report a unique and didactic case with a brief literature review.

  • 외상으로 인한 횡격막 파열과 그로 인한 신장 이탈은 드문 현상이며, 더욱이 신장 이탈시 신장 다리(renal pedicle)가 완전히 찢어지는 현상은 매우 드물게 발생하며 응급 수술을 시행하지 않으면 생명이 위험할 수 있다. 이에 저자들은 외상성 횡격막 파열과 함께 흉강 내 혈관과 요관이 완전히 찢어진 상태로 신장이 탈출한 1예를 경험하였기에 간단한 문헌 고찰과 증례를 보고하는 바이다.

  • Fourteen cases of diaphragmatic hernias surgically treated at the Dept. of Thoracic & Cardiovascular Surgery in Kosin Medical Center from Jan. 1979 to Feb. 1991, were reviewed in this study. This report includ 9 cases of traumatic diaphragmatic hernia, 5 cases of congenital diaphragmatic hernia. Among the traumatic hernias, 5 cases were blunt trauma and 4 cases were penetrating trauma. Five cases of congenital diaphragmatic hernias were presented, including 2 cases of esophageal hiatal hernia, 2 cases of Bochdalek's hernia and a case of Morgagni's hernia. Operation was performed in all patients. Results of all treated cases were excellent.

외상성 허리 탈장: 증례 보고 (Traumatic Lumbar Hernia: Report of a Case)

  • The occurrence of traumatic hernia is rare. However, traumatic lumbar hernias are the most frequently occurring traumatic hernias. Superior lumbar hernias occur more frequently than inferior lumbar hernias, but for anatomical reasons, among traumatic lumbar hernias, inferior lumbar hernias occur more frequently than superior lumbar hernias. Repair of a lumbar hernia is very difficult. Mesh fixation to the bony part and general weakness of surrounding tissue make repair of a lumbar hernia tricky. A traumatic lumbar hernia was repaired using transfascial fixation around the defect to secure the mesh. This technique is another choice for a lumbar hernia technique.

  • Traumatic chylothorax is the presence of lymphatic fluid in the pleural space resulting from thoracic duct rupture, This condition is a serious and often life-threatening clinical entity. Operative trauma is most common mechanism of injury, but we experienced two case of chylothorax due to blunt trauma. One case was treated conservatively, another case was treated surgically.

  • Rupture of the main bronchus following closed injury to the chest is a comparatively rare accident. The late recognition of this injury is attended by difficulties in management which may endanger life or expose the patient to distressing months and years of arduous therapy. This case was a 17 year old female who was a high school student. The patient had sustained a crushing injury to her right hemithorax and had been taken to an emergency hospital where right closed thoracostomy had been performed for a tension pneumothorax. She improved following this procedure but massive atelectasis of the right lung developed on the 13th day after trauma and transferred to our hospital. Bronchoscopy disclosed granulation tissue in the right main stem bronchus and end to end anastomosis of the bronchus was performed. Postoperative course was uneventful.

Endovascular Salvage for Traumatic Midthoracic Aortic Rupture with Left Diaphragmatic Injury

  • Patients with traumatic aortic rupture rarely reach the hospital alive. Even among those who arrive at the hospital alive, traumatic aortic rupture after high-speed motor vehicle accidents leads to a high in-hospital mortality rate and is associated with other major injuries. Here, we report a rare case of descending midthoracic aortic rupture with blunt diaphragmatic rupture. Successful management with emergency laparotomy after an immediate endovascular procedure resulted in a favorable prognosis in this case.

  • A Morel-Lavallée lesion is a post-traumatic closed degloving soft tissue injury after blunt trauma. Infection and skin necrosis frequently occur if it is not treated properly in the early stages. However, there is no clearly established treatment algorithm. In the acute stage, it is mainly treated with aspiration, simple compression, and incisional drainage. In the chronic stage, sclerotherapy is usually performed. If skin necrosis develops, the necrotic tissue is resected and a skin graft is needed. We describe a case of acute Morel-Lavallée lesion in the buttock region that was treated with limited incisional drainage and negative-pressure wound therapy, and also present a review of the literature.

Experience with the emergency vascular repair of upper limb arterial transection with concurrent acute compartment syndrome: two case reports

  • Upper extremity vascular injuries occurring with acute compartment syndrome are very challenging to manage in an emergency context in resource-poor settings. The need to always recognize the likelihood of coexisting compartment syndrome guides surgeons to perform concomitant fasciotomy to ensure a better outcome. We managed three vascular injuries in the upper extremities in two patients with concomitant imminent compartment syndrome observed intraoperatively. The first injury was complete brachial artery disruption following blunt trauma, while the second and third injuries were radial and ulnar artery transection caused by sharp glass cuts. Both patients were treated with vascular repair and fasciotomy. Secondary wound coverage was applied with split-thickness skin grafting, and the outcomes were satisfactory. Concomitant fasciotomy potentially improves the outcomes of vascular repair in emergency vascular surgery and should be considered for all injuries with the potential for acute compartment syndrome.
