둔상에 의한 흉골골절 101례의 임상적 분석 (A Clinical Analysis of 101 blunt sternal fractures)

  • 흉부 외상은 경우에 따라서 심근 손상, 대혈관 손상, 척추 손상 등의 심각한 합병 손상을 일으킬 수 있기 때문에 신속한 진단과 적절한 처치를 해야 한다. 이에 순천향 대학교 의과대학 흉부외과학교실에서는 1986 년 1월부터 1995년 12월까지 흉골 골절로 입원, 치료 받았던 101명의 환자를 대상으로 후향적 조사를 하였 다. 흉골골비관통성 흉부 손상 환자 2877명중 3.51%였으며, 남녀비는 1.82 : 1 로 남자에서 많았다. 손상원인은 차량에 의한 추돌사고가 73례로 가장 많았으며, 골절 부위는 흉골 체부 골절이 75례로 가장 많았다. 평균 재 원일수는 26일 이었으며 9주 이상 장기 치료받은 9명을 제외하면 18일이었다. 심전도상 이상소견은 동성 서 맥 7례, 완전 혹은 불완전 우각차단 6례, 동성 빈맥 4례, 의미있는 5-T분절 이상 3례, 1도 차단 2례, 좌심실 비대, 심실 조기 수축, 심전도 저전압이 각 1레씩 있었으며 CPK-b%분획상 비 정상적 증가 소견을 보인 경 우는 32.1%였다. 치료중 사망한 2명을 제외한 99명중 94명이 보존적 치료를 받았고, 5명이 흉골 고정술을 시 행받았다. 수술후의 합병증은 창상감염이 1례 있었다. 입원 치료중 2명이 사망하였는\ulcorner, 원인은 저혈량성 쇼 크, 급성 호흡 부전이 각각 1명이었다. 이상의 결과로 보아 흉골 골절은 그 빈도가 많지 않고 심한 합병증이 유발되는 경우도 적으며, 대부분의 경우 보존적 치료로 회복된다고는 하지만 소홀히 했을 경우 생명에 치명적인 영향을 줄 수 있으므로 보다 더 신속하고 정확한 진단이 이루어져야 하겠다.

  • The incidence and importance of tricuspid valve regurgitation after a blunt chest injury has risen with the increase in the number of automobile accidents and steering wheel traumas. This kind of injury has been reported more frequently in the last decade because of the better diagnostic procedures and understanding of the pathology. However, tricuspid valve regurgitation following a blunt chest injury can still be easily missed because most patients do not show symptoms at the time of the trauma. A 55-year-old male patient presented himself at our facility after suffering a chest injury from an automobile accident. His transthoracic echocardiography (TTE) revealed severe tricuspid valve regurgitation due to the prolapse of his anterior valve leaflet. We report a case of asymptomatic tricuspid regurgitation that developed after a blunt chest injury.

  • The primary and secondary survey was designed to identify all of a patient's injuries and prioritize their management. However 15 to 22.3% of patient with missed injuries had clinically significant missed injuries. To reduce missed injury, special attention should be focused on patients with severe anatomical injury or obtunded. Victims of blunt trauma commonly had multiple system involvement. Some reports indicate that inexperience, breakdown of estalished protocol, clinical error, and restriction of imaging studies may be responsible for presence of missed injury. The best way of reducing clinical significant of missed injuries was repeated clinical assessment. Here we report a case of severe blunt hepatic injury patient and pericardial injury that was missed in primary and secondary survey. After damage control surgery of hepatic injury, she remained hemodynamically unstable. Further investigation found cardiac tamponade during intensive care. This was managed by pericardial window operation through previous abdominal incision and abdominal wound closure was performed.

둔상에 의한 흉부대동맥 손상에서 TEVAR에 관한 임상연구 (Clinical Analysis of TEVAR in Blunt Thoracic Aortic Injury)

  • Purpose: Thoracic aortic injury is a life-threatening injury that has been traditionally treated by using surgical management. Recently, thoracic endovascular aortic repair (TEVAR) has been conducted pervasively as a better alternative treatment method. Therefore, this study will focus on analyzing the outcome of TEVAR in patients suffering from a blunt thoracic aortic injury. Methods: Of the blunt thoracic aortic injury patients admitted to Eulji University Hospital, this research focused on the 11 patients who had received TEVAR during the period from January 2008 to April 2014. Results: Seven of the 11 patients were male. At the time of admission, the mean systolic pressure was $105.64{\pm}24.60mm\;Hg$, and the mean heart rate was $103.64{\pm}20.02per$ minute. The median interval from arrival to repair was 7 (4, 47) hours. The mean stay in the ICU was $21.82{\pm}16.37hours$. In three patients, a chimney graft technique was also performed to save the left subclavian artery. In one patient, a debranching of the aortic arch vessels was performed. In two patients, the left subclavian artery was totally covered. In one patient whose proximal aortic neck length was insufficient, the landing zone was extended by using a prophylactic left subclavian artery to left common carotid artery bypass before TEVAR. There were no operative mortalities, but a patient who was covered of left subclavian artery died from ischemic brain injury. Complications such as migration, endovascular leakage, collapse, infection and thrombus did not occur. Conclusion: Our short-term outcomes of TEVAR for blunt thoracic aorta injury was feasible. Left subclavian artery may be sacrificed if the proximal landing zone is short, but several methods to continue the perfusion should be considered.

비장둔상의 수술적 치료를 대체하는 혈관색전술의 유용 (Use of Angioembolization to Replace Operative Management for Blunt Splenic Injury)

  • Purpose: Over the past few decades, the treatment of traumatic splenic injuries has shifted to nonoperative management from surgical intervention. Although some nonoperative management failure have been reported, in most trauma centers, nonoperative management is now believed to be the treatment of choice in hemodynamically stable patients. Then, in this study, we have retrospectively evaluated our experience with traumatic splenic injury. Methods: From January 2005 to July 2009, 150 patients with blunt splenic injuries were managed in our hospital. Patients' charts were retrospectively reviewed to analyze their treatment, the patients were grouped according to those who had been admitted before October 2006, defined as the "early group", and those who had been admitted after October 2006, defined as the "late group". After the patients had been divided into two group, physiologic parameters and differences between the treatments were compared. Results: 150 patients were admitted to our hospital with blunt splenic trauma. In late group, both the surgical management rate and the nonoperative management failure rate were lower than they were in the early group. Conclusion: We expect angioembolization to effectively replace surgery for the treatment of selected patients with blunt splenic injury and to result in fewer complications.

외상성 횡격막 파열에 대한 임상적 고찰 (Clinical Evaluation of Traumatic Diaphragmatic Ruptures)

  • 연세대학교 원주의과대학 흉부외과학교실에서는 1989년 1월부터 1996년 2월까지 외상성 횡격막 파열로 진단되어 수술을 시행하였던 환자 50례를 대상으로 하였으며, 남녀 성비는 4:1이였다. 연령 분포는 2세부터 80세까지로 평균 연령은 37세 였다. 외상의 종류로는 둔상이 39례였고 관통상이 11례로서, 둔상인 경우 교통사고가 29례, 추락사고 7례, 경운기 사고가 3례였으며, 관통상인 경우에는 자상 9례, 총상 1례, 유리에 찔린 경우가 1례 였다. 가장 빈번한 증상은 호흡곤란, 흉통 및 복통이었고, 파열 부위는 둔상인 경우 좌측이 30례, 우측이 9례였으며, 관통상인 경우에는 좌측이 8례, 우측이 3례였다. 수술 접근 방법은 18례에서 개흉술을 시행하였고 3례는 개흉 및 개복술을, 29례는 개복술을 시행하였다. 수술후 합병증은 15례로서 둔상인 경우에는 창상 감염이 5례, 호흡 부전이 3례 ,수흉 및 장폐쇄가 각각 2례였으며 농흉, 신부전 및 패혈증이 각각 1례였다. 관통상인 경우는 창상 감염이 2례, 농흉이 1례였다. 사망은 3례로서 사망 원인은 패혈성 쇼크, 뇌손상 및 호흡 부전이 각각 1례였다.

Successful minimally invasive management using transcatheter arterial embolization in a hemodynamically stable elderly patient with mesenteric vascular injury in a hybrid emergency room system in Korea: a case report

  • Mesenteric injury occurs rarely in cases associated with blunt abdominal trauma. Despite its low incidence, mesenteric injury can lead to fatal outcomes such as hypovolemic shock due to hemoperitoneum or sepsis due to intestinal ischemia, or perforation-related peritonitis. For mesenteric injuries, especially those involving massive bleeding, intestinal ischemia, and perforation, the standard treatment is surgery. However, in the case of operative management, it should be borne in mind that there is a possibility of complications and mortality during and after surgery. The usefulness of transcatheter arterial embolization (TAE) is well known in solid organs but is controversial for mesenteric injury. We present a 75-year-old man with mesenteric injury due to blunt abdominal trauma. Initial abdominal computed tomography showed no hemoperitoneum, but a mesenteric contusion and pseudoaneurysm with a diameter of 17 mm were observed near the origin of the superior mesenteric artery. Since there were no findings requiring emergency surgery such as free air or intestinal ischemia, it was decided to perform nonoperative management with TAE using microcoils in hybrid emergency room system. TAE was performed successfully, and there were no complications such as bleeding, bowel ischemia, or delayed bowel perforation. He was discharged on the 23rd day after admission with percutaneous catheter drainage for drainage of mesenteric hematoma. The authors believe that treatment with TAE for highly selected elderly patients with mesenteric injuries has the positive aspect of minimally invasive management, considering the burden of general anesthesia and the various avoidable intraoperative and postoperative complications.

  • Budd-Chiari syndrome is an uncommon condition characterized by hepatic outflow obstruction. Direct suture of the injured Inferior vena cava in a patient with blunt hepatic trauma also may cause an equivalent condition. However, early diagnosis is possible with common symptoms and radiologic evaluation. Moreover, a transluminal approach with balloon angioplasty could prevent long-term complications of Budd-Chiari syndrome without repeated abdominal surgery.

Successful endovascular embolization for traumatic subcutaneous abdominal wall hematoma via the superficial inferior epigastric artery: a case report

  • Abdominal wall hematoma (AWH) after blunt trauma is common, and most cases can be treated conservatively. More invasive treatment is required in patients with traumatic AWH if active bleeding is identified or there is no response to medical treatment. Herein, we report a case of endovascular embolization for traumatic subcutaneous AWH. Almost endovascular treatment for AWH is done through the deep inferior epigastric artery. However, in this case, the superficial inferior epigastric artery was the bleeding focus and embolization target. After understanding the vascular system of the abdominal wall, an endovascular approach and embolization is a safe and effective treatment option for AWH.

Successful nonoperative management of a simultaneous high-grade splenic injury and devascularized kidney in Australia: a case report

  • Severe blunt injuries to isolated solid abdominal viscera have been previously managed nonoperatively; however, management algorithms for simultaneous visceral injuries are less well defined. We report a polytrauma case of a 33-year-old man involved in a motorbike collision who presented with left-sided chest and abdominal pain. Initial imaging demonstrated multiple solid organ injuries with American Association for the Surgery of Trauma (AAST) grade V splenic injury and complete devascularization of the left kidney. The patient underwent urgent angioembolic coiling of the distal splenic artery with successful nonoperative management of simultaneous grade V solid organ injuries.
