Tuberculosis-associated hemophagocytic lymphohistiocytosis in adolescent diagnosed by polymerase chain reaction

  • We present a case of tuberculosis-associated hemophagocytic lymphohistiocytosis in a 14-year-old girl. The patient presented with weight loss, malaise, fatigue, prolonged fever, and generalized lymphadenopathy. Laboratory investigation revealed pancytopenia (white blood cells, $2,020cells/{\mu}L$; hemoglobin, 10.2 g/dL; platelets, $52,000cells/{\mu}L$), hypertriglyceridemia (229 mg/dL), and hyperferritinemia (1,420 ng/mL). Bone marrow biopsy showed a hypocellular bone marrow with a large numbers of histiocytes and marked hemophagocytosis; based on these findings, she was diagnosed with hemophagocytic lymphohistiocytosis. Polymerase chain reaction (PCR) with both the bone marrow aspiration and sputum samples revealed the presence of Mycobacterium tuberculosis. Antitubercular therapy with immune modulation therapy including dexamethasone and intravenous immunoglobulin was initiated. The results of all laboratory tests including bone marrow biopsy and PCR with both the bone marrow aspiration and sputum samples were normalized after treatment. Thus, early bone marrow biopsy and the use of techniques such as PCR can avoid delays in diagnosis and improve the survival rates of patients with tuberculosis-associated hemophagocytic lymphohistiocytosis.

  • Headache is a term used to describe aching or pain that occurs in one or more areas of the head, face, mouth or neck. Headache can be chronic, recurrent or occasional. The pain can be mild or severe enough to disrupt daily activities. Headache involves the network of nerve fibers in the tissues, muscles and blood vessels located in the head and at the base of the skull. Headache also can result from syndromes involving the eyes, ears, neck, teeth or sinuses. In these cases, the underlying condition must be diagnosed and treated. Recently one patient was come with headache. The patient is a 35-year-old lady with complaints of very severe headache that strike as often as several times a week. She has the throbbing pain on the right side of her head with nausea, vomiting, fatigue, dizziness, coldness of hand and leg, or ringing in the ears sometimes etc. After treatment through Banhabaekchulcheonma-tang, most of the symptoms improved. Therefore, this application of oriental medicine is reported with a plea for further investigation.

  • Purpose: The purpose of this study was to investigate influencing factors of health problems which the vinyl house farmers had experienced. Method: Based on PRECEDE model, this study was done with 110 respondents(50 to 69 years old), working in vinyl house and living in the remote area where the primary health post located. Results: Quality of life was low in economical status, emotional status, and physical function. Health problems were founded as pain and chronic fatigue that were caused by physical symptoms, weakened physical strength, and increased blood pressure and cholesterol. Behavioral and environmental factors were related with lack of exercise, irregular eating habit, curtailed sleep, overwork, wrong working postures, stress, and non-fulfillment of safety standard during spraying pesticide. Self efficacy was low in excercise practice, working way, and stress management. Conclusion: The Health Promotion Program for vinyl house farmers should be developed to improve health promoting behavior and self efficacy, to reduce stress, and to strengthen physical function.

  • Objectives This study was designed to report the effect of Samultang-gamibang(四物湯加味方) for insomnia caused by blood deficiency (syndrome) after excessive weight control treatment with herbal medication Methods This is a retrospective case report. We reviewed the patient's medical record after taking the patient's informed consent. Results After treatment, a patient feels calm and satisfaction emotionally as sleeping time has increased from 2hours to 6hours, the fatigue after sleeping has been reduced, and the quality of sleep has become better. Conclusions This case report suggests that Samultang-gamibang can be effective in improving sleep duration and quality if the patient suffers insomnia due to hyul-heo syndrome. Also the woman who have symptom of hyul-heo(血虛) after weight control through decreased food consumption, should bo-hyul(補血).

Adult Primary Myelodysplastic Syndrome: Experience from a Tertiary Care Center in Pakistan

  • Background: Primary myelodysplastic syndrome (MDS) is an acquired clonal disorder of myeloid progenitor cells, characterized by peripheral cytopenias in the presence of hypercellular marrow with dysplastic features. Our aim was to study the demographical and clinicopathological features of adult Pakistani patients with MDS at disease presentation. Materials and Methods: This single centre study was conducted at Liaquat National Hospital and Medical College, extending from January 2010 to December 2014. Data were retrieved from the patient archives. Results: Overall 45 patients were diagnosed at our institution with de novo MDS during the study period. There were 28 males and 17 females. Age ranged between 18 and 95 years with a mean age of $57.6{\pm}17.4years$ and median of 64 years. The male to female ratio was 1.7:1. The main presenting complaints were generalized fatigue (60%), fever (33.3%), dyspnea (15.5%), bleeding (13.3%) and weight loss (11.1%). Examination was unremarkable in 42.2% of patients. Physical examination revealed pallor in 37.7%, followed by petechial and purpuric rashes in 20%. The commonest laboratory finding was anemia (hemoglobin < 10 g/dl in 41 (91.1%) patients. Out of these, 27 (60%) patients had normocytic anemia, followed by macrocytic (22.2%) and microcytic (8.8%). Conclusions: Primary MDS in Pakistani patients demonstrates a male preponderance. The proportion of anemic patients was high in our series with predominance of normocytic anemia. However, other clinico-hematological features appear comparable to published data.

  • Objectives : Sipjundaebo-tang is prepared by ten medical herbs that tone the blood and vital energy, and strengthen health. This prescription has long been used traditionally against anemia, anorexia, extreme exhausion and fatigue. The purpose of this thesis was to review the effects of Sipjundaebo-tang that are about immunopotentiation effects, antitumor activities and potentiation, detoxification of antitumor drugs Methods : We studied the research methods and results of experiments which were selected from Korean, Japaness and American theses were on the topic of immunopotentiation effects, antitumor activities and potentiation, detoxification of antitumor drugs from Sipjundaebo-tang Results and conclusions : Sipjundaebo-tang not only potentiates the effects of the combined use of chemotherapy, but also reduces and elimimate the immunotoxicity of antitumor drugs and radiotherapy. And it strengthen immunity and improve QOL, S/OH of cancer patients.

  • We report a 10-year-old girl with the blue rubber bleb nevus syndrome (BRBNS) who had chronic severe anemia caused by chronic occult bleeding in the gastrointestinal (GI) tract. The patient was admitted to the hospital frequently for recurrent pallor and fatigue since the age of 7 years. Gastroduodenoscopy and capsule endoscopy revealed multiple venous malformations with blood oozing in the stomach, small bowel and colon. The patient was treated by aggressive surgical resection of the 23 vascular malformations in the GI tract. The patient is well without anemia 15 months post surgery.

  • This study was carried out to improve the method of tapping sap from Betula platyphylla Sukatschev, Betula costata Trautv., Betula schmidtii Regel, Betula davurica Pallas, Betula ermanii Cham, Acer mono Maxim and Acer pseudo-sieboldianum Kom and to use it to natural drinks. The mal or results are as follows: 1. It is no difference of the amount of sap by tapping methods and species of trees. The larger D.B.H. is, the more amount of sap can be gotten. So we thought that the best tapping method is hole-drilling to prevent trees from damage. 2. Sap contains sugars and mineral materials. Birches have glucose and fructose, much more mineral materials than maples. Sugar of maples is mainly sucrose. 3. We assume that mineral materials of birches are effective to urination, to make a stong dody and sucrose of maples is effective to recover from one's fatigue by controlling the blood sugar. 4. In vacuum packing after sterilizing the sap in low temperature, it could be stored more 1 year. So, it is possible that sap will be commercialized.

  • We present the case of a 38-year-old woman admitted to our outpatient clinic with accelerating back pain and fatigue following a kick to her back by her husband. Upon arrival, we detected ST segment elevation in the D1, aVL, and V2 leads and accelerated idioventricular rhythm. She had pallor and hypotension consistent with cardiogenic shock. We immediately performed coronary angiography and found a long dissection starting from the mid-left main coronary artery and progressing into the mid-left anterior descending (LAD) and circumflex arteries. She was then transferred to the operating room for surgery. A saphenous vein was grafted to the distal LAD. Since the patient was hypotensive under noradrenaline and dopamine infusions, she was transferred to the cardiovascular surgery intensive care unit on an extracorporeal membrane oxygenator and intra-aortic balloon pump. During follow-up, her blood pressure remained low, at approximately 60/40 mmHg, despite aggressive inotropic and mechanical support. On the second postoperative day, asystole and cardiovascular arrest quickly developed, and despite aggressive cardiopulmonary resuscitation, she died.

  • Alcohol has extensive effects on sleep and daytime sleepiness. Alcohol has a sleep inducing effect and the effect of increased non-REM sleep and suppressed REM sleep during the first half portion of night sleep, but alcohol induces the effect of decreased non-REM sleep and increased light sleep and frequent awakenings and REM rebound during the second half portion of night sleep. Alcohol provokes chronobiological change such as the changes of amplitude or the phase shifts of hormones or core body temperature. The sleep disruption resulting from alcohol drinking may lead to daytime fatigue and sleepiness. The elderly are at particular in the increased risk of alcohol-related sleep disorders because they achieve higher levels of alcohol in the blood and brain than do younger adults after consuming an equivalent dose. Bedtime alcohol consumption among older adults may lead to unsteadiness if walking is attempted during the night, with increased risk of falls and injuries. Continued alcohol use for sleep induction often induces aggravation of insomnia, alcoholism or sleep related breathing disorders such as obstructive sleep apnea. Alcohol should not be used as substitution of sleep pill because of the dependence and tolerance for sleep inducing effect, and the sleep disruption produced by alcohol withdrawal.
