유-헬스케어 기반 실시간 혈압, 혈당 측정치 전송의 간호기록 시간 단축 (Shortening of Nursing Record Time about Real Time Transmission Effect of Blood Pressure, Blood Glucose Value Based on U-Healthcare)

  • Purpose: The aim was to measure the real-time trans-mission effect of blood-pressure and blood-glucose value based on u-healthcare for saving the time and effort of nursing recording time. Methods: This study used a u-healthcare system based on the international standards for the exchange of health information. In order to verify the effectiveness of the u-healthcare, a clinical trial for the system regarding blood-pressure and blood-glucose targeting of patients with endocrine disorders at KNUH from February 7 to 9, 2012 was performed. Results: According to the analyzed results, of the 86 times the 11 patients were tested, measuring blood-pressure and blood-glucose using the u-healthcare system, we found the time differences between the real-time transfer recording method and existing hospital records that were used in the hospital. Based on the average time interval, there was a difference of 1,090.45 seconds (18.17 minutes). Conclusion: Therefore, it's cumbersome that nurses in the hospital have to record the numerical values of the measured blood-pressure and blood-glucose manually and input the recorded values directly into the electronic nursing record system. However, it was found in terms of the newly designed system, that it could save time and effort for nurses, since measured information is sent to the hospital information system on a real-time basis.

당뇨병 성인 대상 보건소 영양교육 프로그램 개발 및 효과 분석 - 개인별 맞춤 하루 필요 에너지 및 식품군 단위수 교육을 중심으로 - (Development and Effects' Analysis of Nutrition Education Program for Diabetes Mellitus at Community Health Center - Focused on Individual Daily Energy Requirements and Food Exchange Units -)

  • The purpose of this study was to investigate effects of the developed nutrition education program focused on individual daily energy requirements and food exchange units using Food Exchange System for diabetes mellitus at a community health center. Developed the nutrition education program, four weeks' nutrition education including provided twice individual meal as diet therapy (2 hour/lesson/week, 4 week), was provided to 20 diabetic elderly (12 male, 8 female, 50-75 yrs): 1st lesson "Introduction: management of diabetes mellitus", 2nd lesson "6 Food groups and sources of 6 food groups", 3rd lesson "Individual daily energy requirements and food exchange units", and 4th lesson "Food choice for diabetes mellitus". For effects' analysis of the developed program, we assessed the changes in anthropometric characteristics; biochemical characteristics and nutrient intakes using 24 hr recall method. Effects of the developed nutrition education program were as follows: weight was significantly decreased, blood urea nitrogen (BUN) and glycosylated hemoglobin (HbA1c) were significantly decreased, and distribution of subjects in BUN and HbA1c was significantly changed. In protein : fat : carbohydrate (PFC) ratio, it was significantly changed from 15.98 : 16.30 : 66.69 to 17.51 : 18.94 : 64.10. In evaluation of nutrient intakes by Dietary Reference Intakes for Koreans (KDRI), protein, fiber, fat, vitamin E, niacin, folic acid, calcium and zinc were shown significantly positive changes in distribution of subjects according to intake level. The index of nutrition quality (INQ), nutrition adequacy ratio (NAR) and mean nutrition adequacy ratio (MAR) were significantly increased. In conclusion, the developed 4 weeks' nutrition education program focused on individual daily energy requirements and food exchange units using Food Exchange System for diabetes mellitus at community health center may improve the symptom of diabetes mellitus.

  • Gel filtration과 ion exchange chromatography, reversed-phase 및 ion exchange FPLC를 이용하여 고려인삼 뿌리에서 subunit 분자량 약 25 kDa의 주요단백질(이하 GMP)을 분리하였다. PAS 염색을 통하여 GMP는 carbohydrate moiety를 갖는 당단백질일 가능성을 보여주었고, 이는 glycosidase 처리후 protein band shift 실험으로 확인되었다. GMP는 native polyacrylamide 전기영동 및 gel permeation FPLC를 통해 native form은 약 63 kDa의 분자량을 갖는 dimer로 판단된다. GMP의 생리활성 측정결과, inflammation mediator의 기능탐색에 쓰이는 anticomplementary activity를 보여주었다.

Galangin의 유전독성 억제효과와 작용기전 (Antigenotoxicity of Galangin and its Action Mechanism)

  • In order to compare the suppressive effect of galangin on the genotoxicity by N-methyl-N-nitrosourea (MNU) or benzo[a]pyrene B(a)P, in vivo micronycleus test using mouse peripheral blood and in vitro sister chromatid exchange(SCE) test using mouse spleen lymphocytes were performed. MNU or B(a)P-induced micronucleated reticulocytes in vivo was decreased by the simultaneous treatment of galangin. MNU or B(a)P-induced SCEs in vitro was also decreased by the simultaneous treatment of galangin. On the other hand, the determinations of [$^3$H]MNU-induced total DNA binding and methylated DNA were performed to find out the mechanism of action. [$^3$H]MNU-induced total DNA binding was inhibited by the treatment of galangin in calf thymus DNA. HPLC analysis of DNA hydrolysates showed that galangin caused a decrease of 7-methyl guanine and $O^{6}$-methyl guanine in calf thymus DNA. To elucidate the action mechanism of galangin against B(a)P, alteration of B(a)P metabolism was studied. Galangin inhibited B(a)P metabolism in the presence of S-9 mix and decreased B(a)P-DNA binding in calf thymus DNA with S-9 mix.

당뇨병역에 있는 아동의 영양 교육 사례 (Nutritional Education for the Diabetic Children (A case study))

  • Since diet is an essential component of the treatment for diabetic mellitus, a progressive educational plan was designed to educate diabetic children for the proper selection of their foods. Seven suspicious children were chosen according to the previous oral glucose tolerance test and present blood glucose levels (fasting and postprandial 2 hours). The education program includes the basic nutrition study, learning of the five basic food groups. familiarization with the food exchange lists. calorie calculation and menu planning, and follow-up evaluation by checking every day -food intake. The duration of the education was five weeks. The levels of fasting blood glucose and postprandial -2 hours blood sugar of the seven children were significantly reduced after the 5-weeks education:FBS:from 92-125mg% to 67-80mg%, pp-2hours BS: from 130-169mg% to 69-90mg%. The children have felt much better with profound self-confidence after to program . Their oral glucose tolerance test levels were also significantly decreased after the 5-weeks education in comparison with those of one year ago.

난황 항체를 이용한 탄수화물의 체내 소화흡수 저해 (Inhibition of carbohydrate digestion using egg yolk antibody)

  • The dietary carbohydrates are mainly digested and adsorbed at small intestine. We developed a new food additive as an egg yolk antibody(1gY) against maltase, sucrase and sodium dependent g1ucose cotransporter(SGLT) for the regulation of blood glucose level and weight control. The maltase, sucrase and SGLT were purified from porcine small intestine which is very similar to that of human in physiological characteristics. The purification step contained an ultracentrifugation, ion exchange chromatography and hydrophobic chromatography. The hens were immunized by purified protein and the IgY activities against immunized antigens were determined. This antibody obtained from the immunized hen's egg yolks directly inhibited the activities of maltase and sucrase in vitro. And the IgY delayed and decreased the increment of blood g1ucose level after administration of maltose, sucrose and glucose in rat about 30 to 60%. The results of this study suggest that the IgY inhibiting the carbohydrate digestion could be used as functional food materials for weight control and regulation of blood glucose level in diabetes.

미세 후두현미경술시 윤상갑상막천자를 통한 고빈도 제트환기법 (High Frequency Jet Ventilation via Cricothyroid Membrane Puncture under Microlaryngoscopic Surgery)

  • High frequency Jet ventilation (HFJV) via cricothyroid membrane puncture contols frequency of ventilation as 20-200/min and persuits adequate gas exchange. HFJV was known to have advantages such as improvement of PaO$_2$, lesser barotrauma, stable hemodynamic effects, good operative field and lesser movement of head. The purpose of this study was to clarify the advantages of HFJV in cases of microlaryngeal surgery which operating time was expected even within 30 minutes. Twenty-eight patients were divided two groups : 1)control group : general endotracheal Intubation anesthesia. 2)experimental group : HFJV via cricothyroid membrane puncture with intravenous anesthesia, frequency 40/min, I/E ratio 40%, driving pressure 40 psi. We analyzed blood pressure, arterial blood gas, score of general condition and recovery time after operation. In conclusion, HFJV via cricothyroid membrane puncture had a good score of general condition and rapid recovery of consciousness, although some accumulation of P$CO_2$and elevation of blood pressure.

흰쥐에서 Gentamicin 투여가 심혈관계에 미치는 영향 (Cardiovascular Effects of Gentamicin Administration in Rats)

  • Aminoglycosidic antibiotics have multiple effects on muscle. For example, they have been shown to block L-type $Ca^{2+}$ channels in vascular smooth muscle, cardiac muscle and skeletal muscle. Possibly as a consequence of this effect on $Ca^{2+}$ influx, they have been shown to decrease the contractility of cardiac muscle (gentamicin). The present study evaluated the effects of gentamicin on blood pressure, vasorelaxation and left ventricular pressure. Gentamicin(10, 20, 40mg/kg) produced dose-dependent blood pressure lowering in rat. The pretreatment of MgSO$_4$ and imipramine (Na$^{+}$-Mg$^{2+}$ exchange inhibitor) had no effect in gentamicin-induced hypotension. However, the gentamicin-induced hypotension was significantly potentiated in the preincubation of verapamil or nifedipine (L-type $Ca^{2+}$ channel blockers), and was significantly attenuated by CaCl$_2$ and was slightly attenuated by caffeine (phosphodiesterase inhibitor). Gentamicin (10, 30, 100$\mu$g/m1) did not have an effect on relaxation of phenylephrine-precontracted aortic rings but high concentration of gentamicin(100, 300$\mu$g/ml) relaxed KCl-precontracted aortic rings, which relaxation was potentiated by treatment of nifedipine. Whereas gentamicin markedly decreased left ventricular developed pressure (LVDP) in perfused heart. These data suggest that gentamicin has significant blood pressure lowering of the rat, which seems to be mediated by calcium channel-sensitive pathway and blood $Ca^{2+}$ level may be important role in this response.

자매염색분체교환을 통한 글라이포세이트 유전독성 (Genotoxicity of low-dose Glyphosate by Sister Chromatid Exchange)

  • Purpose: Glyphosate (N-phosphonomethyl glycine) is widely used as an herbicide for weed control in rural areas. It is also readily available for suicide attempts. Glyphosate has high toxicity and negatively affects the human body. The aim of this investigation was to study the genotoxicity of a low-concentration of glyphosate through sister chromatid exchange (SCE) in human blood lymphocytes in vitro. Methods: Primary lymphocyte cultures were obtained from blood samples of 11 males and seven females who had been exposed to glyphosate (0, 100, 200, and 300 ng/mL). The frequency of SCEs was examined and statistical analysis was performed. Results: All doses of glyphosate induced a significant dose-dependent increase in SCE frequency compared with the control group (P<0.001). In particular, the SCE frequency for exposure to low-dose glyphosate was significantly higher in females than in males. Conclusion: According to the result of this study, even a low-dose of glyphosate may damage DNA and females are more vulnerable to glyphosate.

글라이포세이트 노출로 인한 DNA손상에 대한 녹차의 예방적 효과 (In vitro Effects of Epigallocatechin Gallate on Sister Chromatid Exchange in the Lymphocytes Exposed to Glyphosate)

  • Purpose: Green tea is known as a potent anti-oxidant, anti-carcinogen, and genetic protector. Glyphosate (N-phosphonomethyl glycine) is a widely used non-selective herbicide that causes DNA damage. The present study was conducted to investigate the protective effects of green tea in human blood lymphocytes exposed to glyphosate using the Sister Chromatid Exchange (SCE) frequency method. Methods: Peripheral blood was obtained from 10 volunteers and cultured through four different conditions. Four groups were divided into control, glyphosate only (300 ng/mL), glyphosate and low ($20{\mu}m$) concentrations of epigallocatechin gallate (EGCG) and glyphosate and high ($100{\mu}m$) concentrations of EGCG. Results: The glyphosate exposed groups had a higher mean SCE frequency ($10.33{\pm}2.50$) than the control group ($6.38{\pm}2.28$, p<0.001). The low concentrations of EGCG groups had a lower mean SCE frequency ($9.91{\pm}1.93$) than the glyphosate-only group, although this difference was not significant (p=0.219). However, the high concentration group ($9.49{\pm}1.85$) had a significantly lower SCE frequency than the glyphosate-only group (p=0.001). Conclusion: EGCG has a gene protective effect in human lymphocytes exposed to the genotoxicity of glyphosate in the case of high concentrations.
