Association of total dietary antioxidant capacity with oxidative stress and metabolic markers among patients with metabolic syndrome (대사증후군 환자 및 위험군의 식사 내 총 항산화능과 산화스트레스 및 대사 지표의 연관성)

  • Purpose: This study aimed to investigate the association of total dietary antioxidant capacity (TAC) with oxidative stress and metabolic markers among patients with metabolic syndrome according to gender. Methods: A total of 346 subjects aged 30~59 years with two or more risk factors of metabolic syndrome were recruited from a general hospital near Seoul in South Korea between 2010 and 2012 based on data from the medical checkup. Biochemical indices for oxidative stress and metabolic markers were measured. Food consumption data from 3-day food records were linked with the antioxidant capacity database for commonly consumed Korean foods to estimate individual's TAC. Results: Average dietary TAC of the study subjects was 132.0 mg VCE/d/1,000 kcal in men and 196.4 mg VCE/d/1,000 kcal in women. Levels of ${\gamma}$-glutamyltransferase (GGT), systolic blood pressure, diastolic blood pressure, and blood triglycerides were reduced significantly according to increasing TAC in men, but there was no significant trend in women. Intakes of total flavonoids and carotenoids were significantly negatively correlated with GGT (p < 0.05) and d-ROMs (p < 0.01) in men, whereas those of ${\alpha}$-tocopherol (p < 0.05) and ${\gamma}$-tocopherol (p < 0.05) were positively correlated with biological antioxidant potential (BAP) in women. The odds ratio of high oxidative stress indices and abnormal metabolic markers according to TAC level were not significant in either men or women. Conclusion: The results show that dietary TAC was partially associated with oxidative stress and metabolic markers among patients with metabolic syndrome. Further research is required for elucidating the association between dietary TAC and incidence of metabolic syndrome and chronic diseases within a large population in prospective studies.

Clinical Significance of Immature Reticulocyte Fraction and Reticulocyte Cellular Indices in Pediatric Anemia Patients (망상적혈구 지수 및 미성숙망상적혈구 분획의 소아 빈혈에서의 임상적 의의)

  • Purpose : Flow cytometric automated reticulocyte analysis is a superior method to manual reticulocyte counting, with respect to precision and sensitivity. Furthermore, flow cytometric analysis is able to measure immature reticulocyte fraction(IRF) and reticulocyte cellular indices(RCI : cell hemoglobin content : CHr, mean cell volume : MCVr, cell hemoglobin concentration mean : CHCMr, distribytion width : RDWr, HDWr, CHDWr). In this study, we investigated the mean values and clinical significances of IRF and RCI in healthy children and pediatric anemia patients. Methods : IRF and RCI were measured with an automated blood cell analyzer, ADVIA 120(Bayer, USA) using oxazine 750 dye, in 57 healthy children and 61 children with anemia. The anemia group consisted of 27 iron deficiency anemia(IDA) patients and 34 patients with anemia associated with acute infection(AAI). We compared the mean values of IRF and RCI in the control group classified according to age, between anemia groups and the control group, and between the IDA group and the AAI group. Results : For the normal control group, the mean values of IRF, CHr, MCVr and HDWr were higher in neonates when compared to older children. The mean values of IRF and RDWr were significantly higher, and the mean values of CHr and CHCMr were significantly lower in the IDA group when compared to the control group. The mean value of IRF was significantly higher, and the mean value of CHDWr was significantly lower in the AAI group when compared to the control group. The mean values of IRF, CHr and CHCMr were significantly lower in the IDA group when compared to the AAI group. Conclusion : We could determine the normal mean values of IRF and RCI in healthy children classified according to age for understanding of hematopoietic response differences according to age. The evaluation of IRF and RCI by automated reticulocyte analyzer seemed to be accurate and clinically useful for the early diagnosis of anemia and the differentiation of IDA from AAI.

The Associations of Percent Body Fat with Dietary Intake, Plasma Lipids, Lipoprotein(a), and PAI-1 in Middle Aged Korean Adults

  • This study was designed to investigate the associations of the percent body fat dietary intake, plasma lipoprotein profile, lipoprotein(a), and plasminogen activator inhibitor-1(PAI-1) concentrations of 1982 Korean subjects(men : 1000 and women : 982) between the ages of 40 and 59 years. The dietary assessment consisted of twenty-four hour dietary records and food frequency questionnaires. The subjects were identified into one of the five rating groups of % body fat : lean, underweight, normal, overweight and obese groups. The biochemical assessment included measurements of plasma total cholesterol(TC), HDL-cholesterol(HDL-C), LDL-cholesterol(LDL-C), triglyceride(TG), lipoprotein(a)(Lp(a)), and PAI-1. With respect to the ratio of percent energy intake of carbohydrate : protein : fat of the normal group of the women was 62% : 17% : 20%, respectively. Women apparently had a higher intake of carbohydrates than men(52% : 17 : 20%) did. There was a linear relationship between energy intake and % body fat in both mean and women(with the exception of the underweight group of women). The relationship of % body fat of men to the protein and fat intake was higher than that of the carbohydrate intake. Of the men in the study, intakes of energy, protein and alcohol were positively correlated to % body fat. In women, energy, carbohydrate and protein intake were positively correlated to % body fat, however, the fat, cholesterol and alcohol intake did not show any correlation to the % body fat in women. This study showed that % body fat was positively correlated with plasma TC, LDL-C, PAI-1 levels, and TG, but the % body fat was negatively correlated with plasma HDL-C level in both men and women. These results indicated that the high energy intake of obese or overweight subjects might contribute to several of the biochemical indices fo coronary heart disease(CHD) risk. In conclusion, increased energy intake is associated with overweight or obesity in middle aged Korean people. There was no relationship between % energy intake of fat and % body fat in the study, in middle-aged Korean men and women. The plasma lipid profile and PAI-1 level thought to be the risk factors of CHD were positively associated with percent body fat in middle aged Korean people.

Protective Effects of Houttuynia cordata Thunb. on Gentamicin-induced Oxidative Stress and Nephrotoxicity in Rats

  • Development of a therapy providing protection from, or reversing gentamicin-sulfate (GS)-induced oxidative stress and nephrotoxicity would be of great clinical significance. The present study was designed to investigate the protective effects of Houttuynia cordata Thunb. (HC) against gentamicin sulfate-induced renal damage in rats. Twenty-eight Sprague-Dawley rats were divided into 4 equal groups as follows: group 1, control; group 2, GS 100 mg/kg/d, intraperitoneal (i.p.) injection; group 3, GS 100 mg/kg/d, i.p. + HC 500 mg/kg/d, oral; and group 4, GS 100 mg/kg/d i.p. + HC 1000 mg/kg/d, oral administration). Treatments were administered once daily for 12 d. After 12 d, biochemical and histopathological analyses were conducted to evaluate oxidative stress and renal nephrotoxicity. Serum levels of creatinine, malondialdehyde (MDA), and blood urea nitrogen (BUN), together with renal levels of MDA, glutathione (GSH), superoxide dismutase (SOD), and catalase (CAT) were quantified to evaluate antioxidant activity. Animals treated with GS alone showed a significant increase in serum levels of creatinine, BUN, and MDA, with decreased renal levels of GSH, SOD, and CAT. Treatment of rats with HC showed significant improvement in renal function, presumably as a result of decreased biochemical indices and oxidative stress parameters associated with GS-induced nephrotoxicity. Histopathological examination of the rat kidneys confirmed these observations. Therefore, the novel natural antioxidant HC may protect against GSinduced nephrotoxicity and oxidative stress in rats.

  • To develop the Korean Standard Differentiation of the symptoms and signs for the Stroke(KSDS), the committee on Stroke Diagnosis Standardization of Korean Traditional Medicine was organized by nineteen experts in College of Korean Medicine, The Consensus of the second Consultation Meeting was as follows : First is the definition of the stroke on the Korean Traditional Medicine. Second is the five categories to the Differentiation of the symptoms and signs for the Stroke - fire and heat, dampness and phlegm, blood stasis, qi deficiency, yin deficiency. Third is the indices of the Differentiation of the symptoms and signs for the Stroke respectively. KSDS-1 will be applied to the clinical practice and revised. The Consensus of the third Consultation Meeting had agreed 81 symptoms indexes of KSDS.

Effect of Smoking and Carotenoid-rich Food Consumption on Lipid Peroxidation and Antioxidant Status (흡연과 Carotenoid 함유 식품 섭취빈도에 따른 지질과산화 및 항산화능 비교)

  • This study was performed to determine the effect of carotenoid-rich food consumption and smoking on the lipid peroxidation and antioxidant status in human. The subjects consisted of the health 210 middle-aged adults who visited health care center in the Inha University hospital. The blood and urine samples of the subjects were taken to analyze serum lipid profiles, plasma TBARS, total antioxidant status (TAS) and urinary 8-isoprostanes concentration. The anthropometric indices of the subjects were measured. The nutritional intake and the frequency of carotenoid-rich food consumption was determined by semi-quantitative food frequency questionnaire survey. HDL-cholesterol level of the smoking subjects was significantly lower than that of the non-smoking subjects in men. In the analysis of the carotenoid-rich food consumption, the frequency of pepper and tomato consumption of the non-smoking subjects was significantly higher than that of the smoking subjects in men. In women, the Sequency of pear and peach consumption of the non-smoking subjects was significantly higher than that of the smoking subjects. HDL-cholesterol level of the high carotenoid rich food consumption group was significantly higher than that of medium and low group in women. TAS of the high carotenoid rich food consumption group was significantly higher than that of medium and low group. In conclusion, carotenoid intakes seemed to be effective to reduce lipid peroxidation and increase antioxidant status in the body. The frequency of the carotenoid-rich food consumption seemed to be lower in the smokers than in the non-smokers. However, further researches consisted of large-scaled and randomized clinical trials are required to determine whether carotenoids have any other beneficial effect in human. Changes in bioimpedance analysis components before and after hemodialysis

  • Background: This study compared nutritional parameters in hemodialysis (HD) subjects and controls using bioimpedance analysis (BIA) and investigated how BIA components changed before and after HD. Methods: This cross-sectional study included 147 subjects on maintenance HD from two hospitals and 298 propensity score-matched controls from one healthcare center. BIA was performed pre- and post-HD at mid-week dialysis sessions. Results: Extracellular water/total body water (ECW/TBW) and waist-hip ratio were higher in the HD patients; the other variables were higher in the control group. The cardiothoracic ratio correlated best with overhydration (r = 0.425, P < 0.01) in HD subjects. Blood pressure, hemoglobin, creatinine, and uric acid positively correlated with the lean tissue index in controls; however, most of these nutritional markers did not show significant correlations in HD subjects. Normal hydrated weight was predicted to be higher in the pre-HD than post-HD measurements. Predicted ultrafiltration (UF) volume difference based on pre- and post-HD ECW/TBW and measured UF volume difference showed a close correlation ($r^2=0.924$, P < 0.01). Remarkably, the leg phase angle increased in the post-HD period. Conclusion: The estimated normal hydrated weight using ECW/TBW can be a good marker for determining dry weight. HD subjects had higher ECW/TBW but most nutritional indices were inferior to those of controls. It was possible to predict UF volume differences using BIA, but the post-HD increase in leg phase angle, a nutritional marker, must be interpreted with caution.

A Review of the Study Trends on the Relationship between Primary Dysmenorrhea and Doppler Indicies of Uterine Artery (일차성 월경통과 자궁동맥의 혈류역학적 측정값의 관련성에 대한 최신 연구 동향)

  • Objectives: This study was performed to review the research trends in the relationship between primary dysmenorrhea and doppler indicies of uterine artery. Methods: The search for related papers used 'Pubmed', a reserch engine in the America National Library of Medicine and Korean studies Information Service System (KISS). Used searching terms were 'primary dysmenorrhea', 'menstrual pain' in all cases. And among these studies, we searched by using key word 'uterine artery', 'doppler indices', 'doppler parameters', 'pulsatile index', 'resistance index'. Results: Overall 49 studies, 8 studies were finally selected to this study. There were 6 controlled studies and 2 randomised-controlled studies. In all 8 studies, transvaginal ultrasound was used to measure the resistance of uterine blood vessels. All of these studies reported that in patients with primary dysmenorrhea, hemodynamic values of uterine arteries measured by Doppler ultrasound were significantly higher than in normal subjects. Conclusions: According to the results, it was found that there was a positive correlation between the pain level of primary dysmenorrhea and the pulsation index and resistance index of the uterine artery. Based on these results, it can be seen that the doppler indicies of uterine artery have the potential to be used as an evaluation scale for Korean traditional medicine for primary dysmenorrhea.

Assessment of Nutritional Status in Elderly Dialysis Patients (노인 투석환자의 영양상태 평가)

  • It is important to evaluate nutritional status of elderly patients receiving dialysis, since wasting and malnutrition are their common problems. This study aims at assessing their nutritional status by the type and duration of dialysis. The nutritional status such as somatic fat and protein storage was evaluated with anthropometric measure including weight/height ratio, triceps skinfold thickness and midarm muscle circumference. It was also measured with albumin, transferrin, C3 and IGF-1 and calorie and protein intakes. The general clinical condition of patients was evaluated with the severity of uremia and metabolic acidosis. which were measured through the levels of serum urea, creatinine and bicarbonateion. The data were analyzed by using t-test, ANOVA, Wilcoxon-rank sums test, Scheffe test, Kruskal-Wallis test and Pearson correlation coefficients. The results are following : 1. There was no significant difference in the calorie and protein intakes by the type and duration of dialysis received. 2. As for the anthropometric measures, no significant difference was found by the type of dialysis in body mass index triceps skinfold thickness and midarm muscle circumference. Yet these anthropometric measures differed significantly by the duration of dialysis in those elderly patients receiving hemodialysis(HD group), but this finding was not found in those receiving continuous ambulatory peritoneal dialysis(CAPD). 3. Regarding the indicators of uremia and metabolic acidosis, blood urea nitrogen(BUN) and creatinine were lower in the CAPD group than in the HD group, whereas bicarbonate ion was higher in the CAPD group than in the HD group, with no statistical significance. In the HD group, creatinine increased significantly with the increase of the duration of dialysis. 4. Serum trasferrin and C3 were significantly higher in the CAPD group than in the HD group. However. each of biochemical indices did not show statistical significance by the duration of dialysis in both HD and CAPD groups. 5. Anthropometric measures were significantly associated with dietary intake. Significant correlations were observed between biocarbonate ion, BUN and creatinine. In addition, the correlations between serum protein and albumin and between transferrin and C3 were statistically significant. Yet, IGF-1 revealed no significant correlation with other nutritional indices. The above findings indicate that there were no difference in nutritional status measured with protein and calorie intakes between the type and duration of dialysis, but CAPD seems to benefit correcting uremia and metabolic acidosis than HD. Studies of dietary management for dialysis patients need to be pursued in order to improved the quality of aged patients receiving dialysis.

Comparison of the Usefulness of Lipid Ratio Indicators for Prediction of Metabolic Syndrome in the Elderly Aged 65 Years or Older (65세 이상 고령자에서 대사증후군 예측을 위한 지질비율 지표의 유용성 비교)

  • The purpose of this study was to compare the usefulness of the lipid ratio indicators for the diagnosis of metabolic syndrome in the elderly aged 65 years or older. From January 2018 to December 2020, 1,464 people aged 65 years or older who underwent a health checkup at a general hospital in Seoul were included. Lipid ratio indicators were measured through blood tests. The prevalence of metabolic syndrome according to the quartiles of the lipid ratio index was confirmed by logistic regression analysis. In addition, the metabolic syndrome predictive ability and cutoff value of the lipid ratio indices were estimated with the receiver operating characteristic(ROC) curve. The correlation between atherogenic index of plasma(AIP) and waist circumference was the highest in both men and women(r=0.278, p<0.001 vs r=0.252, p<0.001). As for the lipid ratio indices, the incidence of metabolic syndrome was higher in the fourth quartile than in the first quartile. The area under the ROC curve(AUC) value of AIP was higher at 0.826(95% CI=0.799-0.850) and 0.852(95% CI=0.820-0.881) for men and women, respectively, compared to other lipid ratio indicators, and the optimal cutoff values for both men and women was 0.44(p<0.001). Therefore, the AIP among the lipid ratio indicators was found to be the most useful index for diagnosing metabolic syndrome in the elderly aged 65 years or older.
