Effects of a Case-Based Sepsis Education Program for General Ward Nurses on Knowledge, Accuracy of Sepsis Assessment, and Self-efficacy

  • Purposes: Sepsis is a critical condition in which nurses should detect clinical manifestations and provide early intervention to prevent unwanted serious conditions in the patients. The initial occurrence and management of sepsis take place in general units, but there is a lack of knowledge in nurses. The purpose of this study was to examine the effects of a case-based sepsis education program and compare the case-based education program with and without smartphone applications. Methods: A quasi-experimental pre-test-post-test design with a control group was used. We provided a case-based education program with and without smartphone applications to the nurses and tested the effects of the program on knowledge, the accuracy of sepsis assessment, and self-efficacy as outcome variables. A total of 60 nurses in general units participated. To test differences in knowledge, the accuracy of sepsis assessment, and self-efficacy regarding sepsis between the groups over time, a mixed-design ANCOVA was used for parametric analysis, and generalized estimating equations (GEE) were used for nonparametric analysis. Results: There were significant differences in knowledge, the accuracy of sepsis assessment, and self-efficacy between the groups and within the groups over time. The intervention groups treated with the case-based education program showed improved outcome variables compared to the control group. There was no difference between case-based education with the smartphone application or without the application. Conclusions: The case-based education improved knowledge, the accuracy of sepsis assessment, and self-efficacy in the care of sepsis by nurses working in the general wards. The results suggest that the case-based education program for nurses was effective and eventually improved patient health outcomes.

Production of Cellulase from Cellulomonas sp. KL-6 (Cellulomonas sp. KL-6에 의한 섬유소 분해효소의 생산)

  • Among the cellulases by Cellulomonas sp. KL-6. CMCase and filter paperase, which were produced as the out enzymes of cell, had been much produced, but very small amounts of ${\beta}-glucosidase $, the enzyme of which is cell bound form, was produced by this organism. The optimal culture times for CMCase and filter paperase productions were 5 days, while that of ${\beta}-glucosidase$ was 4 days. When this strain was cultured under the optimal medium for enzyme production, CMCase, FPase and ${\beta}-glucosidase$ were $82\;units/m{\ell},\;80\;units/m{\ell}\;and\;1.2\;units/m{\ell}$, respectively. Thus these results were showed to increase enzyme productivities as about $60{\sim}70%$ than those produced in basal medium. $CaCO_3$ injected to the medium as the ratio of 0.1% was not only enhanced cellulase activities but also effective as acid neutralizing agent. The production effects of lignase and lactase by this bacterium in filter paper medium was not appeared.

Detection of Biological Activities of Wild Mushroom Methanol Extracts (야생버섯 메탄올추출물의 생리활성 검색)

  • In this study, the biological activities of 33 kinds of wild mushroom methanol extracts were investigated. Boletellus elatus showed the greatest fibrinolytic activity (1.08 plasmin units/mL) in a fibrin plate assay, and the activity of Heterobasidion insulasis was 0.89 plasmin units. The thrombin inhibitory activities of Boletellus elatus and Heterobasidion insulasis were 93.32% and 93.69%, respectively. In a ${\alpha}$-glucosidase inhibitory activity test, Laccaria amethystina showed the greatest inhibitory activity at 81.25%. The antioxidative activities of Gomphus sp. and Geastrum lageniforme were 91.37% and 90.42%, respectively. Since Boletellus elatus and Heterobasidion insulasis have strong fibrinolytic and thrombin inhibitory and antioxidative activities, the two mushrooms can be used as material for the development of biofunctional foods for cardiovascular diseases.

  • This study was conducted to develop web-based distance learning program of basic nursing physical science which was composed of a multimedia contents such as documents, voices and pictures. This was to advance curriculum of basic nursing physical science based on a proper nursing characteristics related to other major nursing subjects and induce a new and creative method to gain a knowledge. Based on a general learning design model, distance learning program was developed centered on a structure and function of heart physical science in 1999. The stages of development were like this. Firstly, at the analytic stage, specific units and contents for developing multimedia contents were decided and presentation methods for each item were designed and then the evaluation items were developed. Secondly, at the design stage, the most effective design of multimedia construction was selected and interfaces, basic frames, and presentation elements were chosen. Thirdly, at the developmental stage, internet files were designed using Namo web editor 4.0 and this files were modified with Adobe Photoshop.

Analysis of Microsatellite Patterns in the Genome of Red Sea Cucumber (홍해삼 유전체 분석에 의한 microsatellite의 분포도 연구)

  • This study was conducted to analyze genome of red sea cucumber and to use it as basic data for the development of genetic markers for red sea cucumber. Microsatellite marker analysis of Ulleungdo_normal and Ulleungdo_native red sea cucumbers revealed that dinucleotide simple sequence repeats (SSRs) had the highest ratio, at 81.3~81.4%, and the number of the detected SSRs tended to decrease as the number of repeating sequence units in SSRs increased. In general, microsatellites with between 5 and 10 iterations were most common. As the size of the SSR repeating sequence units increased, the SSR iterations gradually decreased. The di-, tri-, and tetra-nucleotides in SSRs were detected in the highest numbers as (AT)5, (AAT)5, and (AAAT)5, respectively. (CG) and (CCG) had very low frequencies compared to the numbers of other repeating SSR units. The numbers of di-and tri-nucleotide repeats were up to 35 and 32, respectively, and then increased discontinuously up to 44 and 43 repeats, respectively. Tetra-, penta-, and hexa-nucleotides in SSRs occurred in numbers up to 25, 21 and 14, respectively. This analysis of red sea cucumber indicated that it maintains its own repetition sequence and repetition number; therefore, we suggest that using it as basic data for molecular marker will be possible in future research.

Heterologous Production of Streptokinase in Secretory Form in Streptomyces lividans and in Nonsecretory Form in Escherichia coli

  • The skc gene encoding streptokinase (SK) with a molecular mass of approximately 47.4 kDa was cloned from Streptococcus equisimilis ATCC 9542 and heterologously overexpressed in Streptomyces lividans TK24 and E. coli using various strong promoters. When the promoter for sprT [Streptomyces griseus trypsin (SGT)] was used in the host S. lividans TK24, a 47.4-kDa protein was detected along with a smaller hydrolyzed protein (44 kDa), suggesting that posttranslational hydrolysis had occurred as has been reported in other expression systems. The casein/plasminogen plate assay revealed that the plasmid construct containing the SGT signal peptide was superior to that containing the SK signal peptide in terms of SK production. Maximal production of SK was calculated to be about 0.25 unit/ml of culture broth, a value that was five times higher than that obtained with other expression systems using ermE and tipA promoters in the same host. When the skc gene was expressed in E. coli BL21(${\Delta}DE3$)pLys under the control of the T7 promoter, a relatively large amount of SK was expressed in soluble form without hydrolysis. SK activity in E. coli/pET28a-$T7_pSK_m$ was more than 2 units/ml of culture broth, even though about half of the expressed protein formed an inactive inclusion body.

Overexpression of the spr D Gene Encoding Strptomyces griseus Protease D Stimulates Actinorhodin Production in Streptomyces lividans

  • The spr D gene encoding Strptomyces griseus protease D(SGPD); a chymotrypsin-like proteae, was cloned from Strptomyces griseus IFO13350 and sequence. Most of the amino-acid sequence deduced from the nucleotide sequence is idential to that Strptomyces griseus IMRU3499 except that one amino acid has been deleted and Trp 369 has been substituted into Cys369 in the SGPD from S. griseus IFO13350 without affecting the protease activity. The spr D gene was overexpressed in Streptomyce liv-idans TK24 as a heterologous host. Various media with different compositions were also used to max-imize the productivity of SGPD inthe heterologous host. The SGPD productivity was best when the transformant S. lividans TK24 was cultivated in R2YE medium. The relative chymotrypsin activity of the culture broth measured with an artificial chromogenic substrate, N-scuccinyl-ala-ala-pro-phe-p-nitroanilide, was 16 units/ml. A high level of SGPD was also produced in YEME and SAAM medial but it was relatively lower that in R2YE medium and negligible amounts of SGPD were produced in GYE, GAE and Benedict media. The growth of S. lividans reacted the maximum level of cell mass at days 3 and 4 of the culture, but SGPD production started in the stationary phase of cell growth and kept increase in till the 10$^{th}$ day of culture in R2YE and YEME medium, but in GYE media the productivity reached maximum level at 8days of cultivation. The introduction of the spr D gene into S. lividans TK24 triggered biosyntheis of the pigmented antibiotic , actinorhodin, which implies some protease may paly a very improtant role in secondary-metabolite formation in sStreptomyces.

Biological activities of Solvent Fractions Isolated from Areca catechu L

  • This study was conducted to investigate the biological activities of Areca catechu L. The antioxidative, fibrinolytic, thrombin inhibitory, and ${\alpha}$-glucosidase inhibitory activities of Areca catechu L extracted with hexane, $CHCl_3$, ethyl acetate, butanol, and water were measured. The water fraction showed the highest extraction yield at 3.65% (w/w). The butanol, $CHCl_3$, water, and ethyl acetate fractions showed strong antioxidative activities at 81.6%, 87.1%, 88.0%, and 89.5%, respectively. The fibrinolytic activity was strong only in the ethyl acetate fraction at 0.84 plasmin units/ml. The 100-fold dilution of the water fraction had the strongest thrombin inhibitory activity at 59.2%. The 100-fold dilution of butanol fraction displayed the strongest ${\alpha}$-glucosidase inhibitory activity at 88.6%. In conclusion, the extracts of Areca catechu L hold promise for use in the development of biofunctional foods to prevent cardiovascular diseases.

Manifestation of Transparent-Scaled Variant Type in Rhodeus notatus (떡납줄갱이(Rhodeus notatus)의 Transparent-Scaled Variant Type 출현 보고)

  • This study evaluated the external form of Rhodeus notatus TSV (transparent-scaled variant) type identified in Korea for the first time and compared it with that of the normal type. The TSV units newly found on Rhodeus notatus are iii10 of dorsal fin and anal fin iii10, which are similar to that in the normal type, considering the observing coefficient of each part's fins. The external form is, transparent enough to see the red gill filament in the operculum, the spiral digestive organ is observed opaquely, and numerous melanophores are stained at the upper part of abdomen. The coloration and scales are different between TSV type and normal type; however, there is no difference in the external form and meristic character.

A Study of Clinical Nurses' Knowledge, Need and Clinical Performance about Pathophysiology (임상간호사가 인지한 병태생리학 지식, 필요도와 임상실무와의 연계성)

  • Purpose: The purpose of this study was to identify knowledge and the Need of pathophysiology, perceived connectivity between knowledge of pathophysiology and clinical practice, and confidence and satisfaction with clinical performance by applying the knowledge to clinical practice, and to analyze the correlation between the knowledge, perceived connectivity, and confidence and satisfaction of clinical nurses. Methods: Subjects consisted of 149 clinical nurses who work at 7 general hospitals and 6 university hospitals located in Seoul, Incheon, and Gyeonggi province in Korea. Those who learned pathophysiology during their undergraduate program were recruited by using a convenient sampling. The data were collected between April 23 and May 24 in 2012. Participants responded to self-administered questionnaires about knowledge and the need of pathophysiology, and the perceived connectivity between knowledge and clinical practice, and confidence and satisfaction with clinical performance by applying knowledge to clinical practice. Results: Mean scores of knowledge and the need of pathophysiology were $2.88{\pm}0.34$ and $2.33{\pm}0.39$ each. Scores of knowledge and the need of 3 units such as fluid and electrolyte imbalance, acid-base imbalance, and alterations in the respiratory system were high among 23 units while that of adolescent disorders was the lowest. The mean score of perceived connectivity between knowledge and clinical practice was $7.32{\pm}1.57$, and those of confidence and satisfaction with clinical performance by applying the knowledge to clinical practice were $7.66{\pm}1.66$ and $7.42{\pm}1.67$ respectively. Knowledge of pathophysiology was positively correlated with the perceived connectivity between knowledge and clinical practice, and confidence, and satisfaction with clinical performance. Conclusion: The results indicate that knowledge and the need of pathophysiology in clinical nurses is above average, and that knowledge of pathophysiology is important in promoting perceived connectivity with clinical practice and increasing confidence and satisfaction with clinical performance by applying knowledge to clinical practice.
