Experimental Studies on the Protective Effect of Kamicheungkantang(KCKT) on Hepatic Damage Induced by CCl4 (가미청간탕(加味淸肝湯)의 보간작용(補肝作用)에 대(對)한 실험적(實驗的) 연구(硏究))

  • In order to evaluate the protective effect of Kamicheungkantang(KCKT) on hepatic damage induced by $CCl_4$, the study was done. The blood chemistry and histological study were done following oral administration with materials. The results were obtained as follows. 1. KCKT extracts didn't show cytotoxicity against BALB/C mouse lung fibroblast cell. 2. In the hepatotoxicity with $CCl_4$, serum alanine aminotransferase(ALT) was significantly decreased in KCKT treated group as compared with control group. 3. In the hepatotoxicity with $CCl_4$, serum aspartate aminotransferase (AST) was significantly decreased in KCKT treated group as compared with control group. 4. In the hepatotoxicity with $CCl_4$, serum alkaline phosphatase(ALP) was significantly decreased in KCKT treated group as compared with control group. 5. In the hepatotoxicity with $CCl_4$, serum lactate dehydrogenase(LDH> was insignificantly decreased in KCKT but insignificantly as compared with control group. 6. In the hepatotoxicity with $CCl_4$, serum cholestorol was significantly decreased in KCKT treated group as compared with control group. 7. In the hepatotoxicity with $CCl_4$, serum triglyceride was insignificantly decreased in KCKT treated groups as compared with data of control. 8. In the hepatotoxicity with $CCl_4$, serum total bilirubin, direct bilirubin, ${\gamma}$-GTP were not changed in KCKT treated groups as compared with data of control. 9. In histopathological changes, fatty changes, vacuole, nucleotic changes and fibrosis were observed in control group and degree of changes was increased over time. Whereas no differences were observed in KCKT treated group These results suggested that KCKT extracts might be usefully applied for treatment of hapatic disease and also it was necessary to do more studies about its mechanisms.

Comparative Study on the Effects of Herbal Medicine and Western Medicine on the Liver Function Based on Cross-Sectional Design

  • Objective : The purpose of this study is to examine safety of herbal medicine on liver function and compare with western medicine. Method : 303 subjects of skin disease(vitiligo and psoriasis) were chosen at a local oriental medical clinic, and tested with a spot liver function test from Sept 1, 2006 to Dec 31, 2006. Subjects were grouped into untreated, herbal medicine, western medicine, and combination group by treatment record. Total bilirubin, AST, ALT and LDH was compared in each group, odds ratio and regression coefficient was calculated. Results : Subjects comprised of 116 individuals receiving western medication(38.28%), 54 receiving herbal medication(17.82%), 107 receiving combined forms of medication(35.31%), and 26 individuals without any types of medication(8.58%). With the mean age of 37.0 yr. 204 were male(67.3%), 99 were female(32.7%). Comparing variables of liver functions, there was no significant difference between the control and experiment groups. After adjusting potential confounders, monthly $\beta$(SE) of multiple regression -0.009~0.000 for the herbal group, -0.005~0.000 for the western group, and -0.001~0.001 for the combination group. No significant difference between the groups. OR of T-bilirubin were 1.02, 1.05, and 1.04. AST were 0.92, 0.94, and 0.98. ALT were 0.85, 0.99, and 0.97. LDH were 0.96, 1.06, 1.00 for the herbal, western, and combination group, respectively. Conclusion : Comparing with western medication, herbal medication did not cause special ill-effect on the liver function based on liver function tests.

Pharmacological effects of Artemisia messes-schmidiana var viridis on 1-naphthylisothiocyanate-induced intrahepatic cholestasis in rat (1-naphthylisothiocyanate에 기인된 랫드의 간내성 담즙분비 정지에 대한 인진호(Artemisia messes-schmidiana var viridis)의 약리학적 효과)

  • In oriental folk medicine, Artemisia messes-schmidiana var viridis(Compositae) has been used for jaundice, hepatitis, diuretic and liver cirrhosis etc. 1-naphthylisothiocyanate(ANIT) has been used for more than 20 years as a model compound to study mechanisms of intrahepatic cholestasis in laboratory animals as rat and mouse. Various biochemical and morphological changes including biliary epithelial and parenchymal cell necrosis occur in the liver of animals treated with ANIT. The purposes of present study are to examine pharmacological effects of Artemisia messes-schmidiana var viridis water extract(AMWE) on alterations of secretion volume and total bile acids level in bile juice, and that of serum AST, ALT, ALP, bilirubin, and glucose levels in rat. AMWE stimulated bile secretion and recovered ANIT-induced cholestasis. Bile acid concentrations increased to more than 60% compared with normal by ANIT, which were returned toward normal value with AMWE treatment. Serum AST and ALT activities were increased by ANIT and yet which were significantly decreased with AMWE treatment. In addition, this effect was apparent in AMWE pretreatment group. Serum glucose levels were increased with AMWE and ANIT, while were decreased compared with control in AMWE posttreatment group. Increased serum total bilirubin contents and ALP activities by ANIT were significantly decreased with AMWE posttreatment. In conclusion, AMWE exerted bile acid-independent choleresis effect and then improved to normal conditions ANIT-induced cholestatic syndromes. Also, AMWE have protective and regenerative effect of hepatocytes in rat.

Effect of Corydalidis Tuber.Carthami Flos.Paeoniae Radix Rubra on Anti-Inflamation and function of the Liver at Arthritis Rats (현호색(玄胡索).홍화(紅花).적작약(赤芍藥)이 관절염 흰쥐의 염증과 간(肝)에 미치는 영향)

  • This study is designed to find out the effect of Corydalidis Tuber Carthami Flos Paeoniae Radix Rubra mixed water extract on adjuvant arthritis, and to check the stability for Corydalidis Tuber Carthami Flos Paeoniae Radix Rubra mixed water extract about the liver. At first, rats were devided into four groups; Normal(Non-treated group with FCA), Control(Group administrated saline everyday for 1 week after treatment of FCA), Sample A(group administrated Corydalidis Tuber Paeoniae Radix Rubra Carthami Flos mixed water extract $9mg/200{\sim}220g$ everyday 1 week after treatment of FCA) and Sample B(Group administrated Corydalidis Tuber Paeoniae Radix Rubra Carthami Flos mixed water extract $18mg/200{\sim}220g$ everyday 1 week after treatment of FCA). After then, separated searches were made on the change of edema rate, the number of WBC, the contented quantities of total protein, total bilirubin, GOT, GPT in the serum. Result : In the right planter edema, both of Sample groups showed a decrease with statistical significance in comparison with control group. In the WBC count, Sample B group showed a decrease with statistical significance in comparison with control group and also sample A group showed a decrease, but did not show statistical significance. In the serum total protein, both of samples did not show statistically significant decrease in comparison with the control group. In the serum total bilirubin, GOT and GPT, both of samples did not show any harmful change about the liver in comparison with the control group. According to the above results, it is concluded that Corydalidis Tuber Paeoniae Radix Rubra Carthami Flos mixed water extract had the therapeutic effect of anti-inflammation in adjuvant arthritis with no injury to liver.

Lamivudine Therapy Exacerbates Bilirubinemia in Patients Underlying Severely Advanced Hepatitis

  • Lamivudine belongs to the set of antiviral agents effective against hepatitis B virus infection. Given case reports on liver injuries after certain antiviral agent treatments, this study examined the effects of lamivudine on alanine aminotransferase (ALT) and total bilirubin (TB) using a medical system database. A total of 1,321 patients taking lamivudine alone or with others were evaluated using laboratory hits in an electronic medical system at Seoul National University Hospital from 2005 through 2011. The patients were grouped according to prior ALT results: G#1, ALT < 40 IU/L; G#2, 40 IU/L ${\leq}$ ALT < 120 IU/L; G#3, 120 IU/L ${\leq}$ ALT < 240 IU/L; and G#4, ALT ${\geq}$ 240 IU/L. In G#1 and G#2 patients, lamivudine or adefovir treatment decreased ALT and TB compared to prior values. In G#3 and G#4 patients with three times the upper limit of normal (ULN) ${\leq}$ ALT < 15 times the ULN, both ALT and TB were decreased after treatment with lamivudine alone, or adefovir following lamivudine therapy, indicating that lamivudine therapy ameliorated liver functions. However, in G#4 patients who experienced severely advanced hepatitis (ALT ${\geq}$ 15 times the ULN, or ${\geq}$ 600 IU/L), lamivudine augmented TBmax ($6.3{\rightarrow}13.3mg/dL$) despite a slight improvement in ALT ($839{\rightarrow}783IU/L$), indicative of exacerbation of bilirubinemia. Patients who used adefovir after lamivudine also showed a high incidence of hyperbilirubinemia when they experienced severely advanced hepatitis. Treatment with adefovir alone did not show the effect. In conclusion, lamivudine may increase the risk of hyperbilirubinemia in patients with severely advanced hepatitis, implying that caution should be exercised when using lamivudine therapy in certain patient populations.

Effects of Fly Maggot Extracts on the Liver and Plasma Lipid in Rat Fed High-Fat Diets (고지방식이 유도된 흰쥐의 혈액지질 및 간에 관한 파리유충 추출물의 효과)

  • The bioactive effects of ethanol extracts from fly maggot (ME) on reduction of plasma lipids levels in rats fed high-fat diets (Expt. Ⅰ), and on liver function recovery of hepatotoxicity rats by intraperitoneal injection of carbon tetrachloride ($CCl_4$) or by orally administration of alcohol (Expt. II) were investigated. In expt. I, twenty seven, male rat SDS(sprague dawley strain) were randomly assigned to three treated groups, including normal control group, HF (group with high fat diets which have no extracts) and HFE (HF plus orally administered doses of ME extract at 5.0 mg/100g of body weight). In expt. II, forty five, male rats (SDS) were randomly assigned to each of the five groups: T1 (control), T2 (intraperitoneal injection of $CCl_4$), T3 (intraperitoneal injection of $CCl_4$ after orally administered with ME), T4 (orally administered with combination of ME and alcohol), T5 (orally administration of ME after orally administered with alcohol). There were significant decreases in plasma (TAG), (TC), (LDL-C) in the HFE group with orally administered doses of ME at 5.0 mg/100g of body weight, respectively, however, the (HDL-C) were significantly increased in HFE group as compared to HF group with high fat diets which have no extracts (p<0.05). The levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), ${\gamma}$-glutamyl transferse(${\gamma}$-GTP) and bilirubin were highest in T2 or T3, and high in order T4 or T5, and lowest in T1 except for bilirubin which has same with T4, T5 (p<0.05). The high recovery of liver damage by $CCl_4$ from the light microscopic appearance was observed in rats (T3) with extracts, and also high in T4 than T5 by orally administrated with alcohol. In conclusion, the ethanol extracts from fly maggot may have a bioactive effects to prevent for human lipids disorder and alcoholic disease.

A Case Report of Alcoholic Liver Disease with Jaundice Treated with Korean Medicine (황달을 동반한 알코올성 간질환 환자의 한방치료 치험 1례)

  • Objective: This study aimed to report a case of alcoholic liver disease with jaundice that was improved with Korean medicine treatment. Methods: A patient who developed jaundice due to continuous drinking was treated with herbal medicine, acupuncture, moxibustion, and cupping. Blood tests were performed to measure the levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), gamma glutamyl transpeptidase (γ-GTP), total bilirubin, albumin, and total protein; AST/ALT ratio; and mean corpuscular volume (MCV). Jaundice parameters were subjectively recorded at 3-day intervals. Results: After treatment, the AST, ALT, ALP, γ-GTP, and total bilirubin levels; MCV; and jaundice were decreased. Conclusion: Korean medicine treatment appeared to be an effective method for alcoholic liver disease with jaundice.

Effect of Extreme Long-Distance Running on Hepatic Metabolism and Renal Function in Middle-Aged Men

  • The aim of this study was to investigate the impact of participation in the 622 km hyper-ultra-marathon on hepatic metabolism and renal function in middle-aged men. Healthy middle-aged male amateur ultra-marathoners between the ages of 40 and 60. Blood was collected at the pre-race, immediately after 300 km, 622 km hyper-ultra marathon race, 72 hours (3 day) and 144 hours (6 day) after the race, AST (aspartate aminotransferase), ALT (alanine aminotransferase), ALP (alkaline phosphatase), ${\gamma}$-GTP (gamma glutamyl transferase), T-Bil (total bilirubin), D-Bil (direct bilirubin), T-protein (total protein), albumin, uric acid, BUN (blood urea nitrogen), creatinne were analyzed. ALP was significantly increased at 300 km, 622 km, day 3 and day 6 than the pre-race. ${\gamma}$-GTP, T-protein, albumin, uric acid, BUN and creatinine were not significantly different between the distances and the recovery period respectively. AST and ALT were significantly increased at 300 km, 622 km, day 3 and day 6 than the pre-race, respectively (P<0.05) at day 3 and day 6 they showed significant decrease from 300 km and 622 km, respectively (P<0.05). T-Bil and D-Bil increased significantly at 300 km and 622 km, respectively (P<.05) and significantly decreased at day 3 (P<0.05) compared to the pre-race, at day 3 and day 6 they were decreased significantly than 300 km and 622 km, respectively (P<0.05). In conclusion, no disturbance of renal function was observed according to the distances and between the recovery period of 622 km hyper-ultra marathon race, but reversible hepatocyte function could be degraded and some hemolysis of blood vessels was induced.

Effect of Synbiotic on the Treatment of Jaundice in Full Term Neonates: A Randomized Clinical Trial

  • Purpose: Jaundice accounts for most hospital admissions in the neonatal period. Nowadays, in addition to phototherapy, other auxiliary methods are used to reduce jaundice and the length of hospitalization. This study aimed to investigate the effect of probiotics on the treatment of hyper-bilirubinemia in full-term neonates. Methods: In this randomized clinical trial, 83 full-term neonates, who were admitted to the hospital to receive phototherapy in the first 6 months of 2015, were randomly divided into two groups: synbiotic (SG, n=40) and control (CG, n=43). Both groups received phototherapy but the SG also received 5 drops/day of synbiotics. Serum bilirubin, urine, stool, feeding frequency, and weight were measured daily until hospital discharge. A p-value<0.05 was considered statistically significant. Results: The mean total serum bilirubin in the SG was lower than that in the CG ($9.38{\pm}2.37$ and $11.17{\pm}2.60mg/dL$, respectively). The urine and stool frequency in the SG was significantly higher than that in the CG (p<0.05). The duration of hospitalization in the SG was shorter than that in the CG. Conclusion: Use of synbiotics as an adjuvant therapy had a significant treatment effect on jaundice in full-term neonates. Further studies including larger samples with long follow-up periods are essential to confirm the benefits of routine use of synbiotics in neonatal patients with jaundice.

Safety of delayed umbilical cord clamping in preterm neonates of less than 34 weeks of gestation: a randomized controlled trial

  • Objective There is concern regarding the safety of delayed cord clamping (DCC) in babies born at less than 34 weeks' gestation. Therefore, the primary objective of this study was to compare the rates of hyperbilirubinemia and polycythemia during initial 7 days in infants born at less than 34 weeks' gestation and randomized to receive DCC by 120 seconds or early cord clamping (ECC) within less than 30 seconds. Methods One hundred pregnant women were randomly subjected to DCC or ECC at the time of birth in a tertiary referral hospital setting. Blood samples were obtained from each newborn at 48 hours and 7 days for hematocrit measurement. Serum bilirubin levels were estimated once the infant had clinically significant jaundice or at 72 hours. For the statistical analysis, the ${\chi}^2$ test, Student's t-test, or Wilcoxon rank sum test was used. Results The hematocrit was significantly higher in the DCC group than in the ECC group (P<0.001). None of the babies had polycythemia. Mean total serum bilirubin level was 6.6 mg/dL in the DCC group and 8.7 mg/dL in the ECC group (P<0.001). There was no increased risk of hyperbilirubinemia in the DCC group. Conclusion DCC benefits preterm neonates with no significant adverse effects.
