Development of Adverse Drug Event Surveillance System using BI Technology (BI기술을 적용한 약물부작용감시시스템 개발)

  • In this study, we are analysing adverse drug events and proposing a technical structure of "adverse drug event surveillance system" using business intelligence technology, hoping that we can use the system commonly and actively. It is the recent trend to adopt both of electronic review and manual review process to surveil adverse drug events and this study construct CDW applying ETL in BI Technology. As the result of analysis, the data pool included 701 doctors who prescribed and 3059 patients(1528 male, 1531 female), of total 318,222 cases, 2,086cases(0.6%) were suspected as having adverse drug events. And the single type of T.bilirubin> 3mg/dL(ADE type-LabR0005) was the most common(548 among 2085 cases) within the framework of signals.

Effects of Herbal Acupuncture(Raphanus Sativus) at $BL_{20}$ on the Hyperlipemia Rats Induced by High Fat Diet (비유에 대한 래복자 약침(藥鍼)이 고지방 식이로 유발된 비만(肥滿) 백서(白鼠)에 미치는 영향)

  • Objectives : This research was performed to investigate the effect of herbal acupuncture(Raphanus sativus) at Bisu($BL_{20}$) on weight gain, food intake, food efficiency, serum of lipid concentrations, liver function and HDL to total cholesterol ratio of rats fed high fat diet for 5weeks. Methods : Experimental groups were divided into high fat diet group(Control), high fat diet and saline in $BL_{20}$ treated group(Saline), high fat diet and Raphanus sativus-herbal acupuncture in $BL_{20}$ treated group(RHA-1, 0.058mg/ml/g), high fat diet and Raphanus sativus-herbal acupuncture in $BL_{20}$ treated group(RHA-2, 0.029mg/ml/g), high fat diet and Raphanus sativus-herbal acupuncture in $BL_{20}$ treated group(RHA-3, 0.011mg/ml/g), Herbal acupuncture was bilaterally treated at 0.2cc per 4days, total 14 times in 56days. Results : Body weight and food efficiency were decreased in saline, RHA-1, RHA-2. Feed intake was increased in saline, RHA-1, RHA- 2, RHA- 3. The level of serum total cholesterol was decreased in saline, RHA-1. The level of serum HDL-cholesterol, LDL-cholesterol were not significant. The level of serum triglyceride, Total Bilirubin were decreased in saline, RHA-1, RHA-2, RHA-3.The level of serum AST was decreased in RHA-1. But the level of serum ALT was increased in RHA-1. The level of atherogenic index was decreased in saline, RHA-1. The level of HTR was increased in saline, RHA-1. Conclusions : Raphanus sativus-herbal acupuncture in Bisu($BL_{20}$) can control body weight, feed intake and feed efficency ratio, total cholesterol, total bilirubin, and prevention cardiovascular risk.

A Review of the Treatment to Prevent Neonatal Jaundice - Based on Traditional Chinese Medicine (신생아 황달의 예방적 치료에 대한 연구동향)

  • Introduction Jaundice is one of the most common conditions found in neonatal period. Phototherapy is one of the main treatments for neonatal jaundice. However, several adverse effects of the phototherapy have been reported, including DNA damage recently. Therefore, a variety of treatments have been conducted to shorten the duration of phototherapy. Meanwhile, it has been hardly tried to prevent neonatal jaundice, but diverse approaches have been tried in traditional Chinese medicine (TCM) for many years. Therefore, this study aims to analyze the studies for the treatments to prevent neonatal jaundice based on TCM. Materials and Methods Various literatures have been searched via CNKI, and PubMed using the terms "neonatal jaundice" (新生儿黄疸, 胎黃, 胎疸) in category of 'Traditional Chinese Medicine', 'Traditional Chinese Medicinal Herbs' and 'Combination of Traditional Chinese Medicine With Western Medicine'. The search range included randomized controlled trials (RCTs), controlled clinical trials (CCTs), case reports, reviews and animal experiments published from 2013 to 2015. Results A total of 104 studies were found. 93 articles were excluded by reviewing the titles and abstracts. Out of the remaining 11 studies, 9 articles were RCTs, 2 articles were CCTs. The treatment used in neonatal babies in the studies were, using herbal remedies (meditation 5, bath 3 and retention enema 1) and the acupressure. All treatments were initiated within the first 24-48 hours after their birth. Then, there was a study that the subjects taken the herbal medicine were women during pregnancy. The outcome assessments used were the serum total bilirubin (TB), transcutaneous bilirubin measurement (TCB), the incidence of the hyperbilirubinemia, the duration time of the jaundice. Especially, TB, TCB and the incidence of the hyperbilirubinemia have decreased significantly more than that of the control group in the most of the results. No severe adverse events were reported in all articles reviewed. Conclusions Conventional treatment such as, herbal medicine in TCM, seems to be the effective way to prevent neonatal jaundice or hyperbilirubinemia. Therefore, the conventional treatment may be favorable choice for preventive treatment for neonatal jaundice.

Preoperative Levels of Hematological and Biochemical Indices Affect Perioperative Variables in Adult Patients with Coronary Artery Bypass Graft Surgery

  • The objective of this research was to evaluate the relationships of preoperative (Pre-OP) levels of hematological and biochemical indices to perioperative variables in patients that underwent coronary artery bypass graft surgery (CABG). Pre-OP levels of hematological factors [total white blood cells (T-WBC), erythrocytes, hemoglobin, hematocrit, glycohemoglobin A1c (HbA1c), or platelet] were negatively or positively related with biochemical indices [alanine aminotransferase (ALT), bilirubin, glucose, fructosamine, triglyceride, and high density lipoprotein cholesterol (HDL)]. Pre-OP levels of hematological factors and biochemical indices were negatively or positively correlated with echocardiographic variables. Pre-OP level of HbA1c had a relationship with C-reactive protein. Pre-OP levels of aspartate aminotransferase (AST), ALT, HDL, glucose, fructosamine, or blood urea nitrogen (BUN) were positively or negatively associated with Pre-OP levels of cardiac markers (brain natriuretic peptide, troponin-I, creatine kinase isoenzyme 2, or CRP). Pre-OP levels of hematological factors (excepting T-WBC) related with operation time (OPT), postoperative mechanical ventilation time (POMVT), intensive care unit-period (ICU-period) or hospitalization. Pre-OP levels of AST, ALT, bilirubin, triglyceride, HDL, low dwensity lipoprotein, fructosamine, or BUN were positively or negatively correlated with OPT, graft numbers, POMVT, ICU-period or hospitalization. Retrospective this study reveals that Pre-OP levels of hematological and biochemical markers are associated with echocardiographic variables, several cardiac markers and postoperative outcomes, suggesting that Pre-OP levels of hematological and biochemical markers may be useful predictors for the diagnosis and prognosis of coronary artery disease.

Whole-liver Radiotherapy Concurrent with Chemotherapy as a Palliative Treatment for Colorectal Patients with Massive and Multiple Liver Metastases: a Retrospective Study

  • The purpose of this study was to investigate whether whole-liver radiotherapy plus a tumor-boost dose with concurrent chemotherapy is beneficial for colorectal cancer patients with massive and multiple liver metastases. From January 2007 to December 2012, 19 patients who exhibited massive (with a longest diameter > 5 cm) and invasive liver metastases and multiple metastases were treated with radiotherapy and concurrent chemotherapy. The total radiation dose was 53.4 Gy (range 38.8 Gy-66.3 Gy). All of the patients received a continuous intravenous dose of 5 fluorouracil (5-FU) 225 mg/m2 concurrently with radiation. The median survival time was 19 months. The 1- and 2- year overall survival rates were 78.3% and 14.3%, respectively. Of all of the patients who presented with abdominal pain, 100% experienced a decrease in pain. Decreases in the rates of ascites and jaundice were confirmed by ultrasound and bilirubin levels. No cases of Grade 4 or 5 acute or late toxicity were recorded. There were only two cases of Grade 3 toxicity (elevated bilirubin). These data provide evidence that whole-liver radiotherapy plus a tumor-boost dose with concurrent chemotherapy is beneficial for colorectal cancer patients with massive and multiple liver metastases.

Effect of Taraxacum Mongolicum Extract on Serum Heavy Metal Levels and Blood Parameters of Albino Rats Exposed to Lead and Mercury (포공영 추출물이 납 및 수은에 노출된 랫드 혈청의 중금속 및 혈액지표에 미치는 영향)

  • BACKGROUND: The purpose of this study was that serum metabolic and hematological variables were measured to investigate the detoxication effect of Taraxacum mongolicum extract on male albino rat exposed with lead and mercury. METHODS AND RESULTS: For this study, Pb and Hg-exposed( 50 ppm) albino rats was used, and the treatments were carried out in three doses of 100 mg/kg, 200 mg/kg and 500 mg/kg as dry weight of T. mongolicum extract. T. mongolicum extract could improved the body weight gain and feed efficiency ratio, except to food intake. The levels of biochemical factors elevated by Pb-Hg mix exposure, which are Bilirubin, Alkaline phosphatase(ALP), glutamic oxaloacetate transaminase(GOT), glutamic pyruvate transaminase (GPT), lactate dehydrogenase(LDH), ceatinine and blood urea nitrogen(BUN), were significantly reduced in all treated groups as compared to Pb-Hg mix exposure alone. T. mongolicum extract was shown to suppress the accumulation of Hg and Pb in serum by dose dependent manner. CONCLUSION: Therefore, this study suggest that T. mongolicum extract might have the potential effect to minimize the toxic effects of Pb and Hg.

A Case of Hemolytic Disease of a Newborn by an Anti-$Di^a$ Antibody Treated with Intravenous Immunoglobulin (정맥용 면역글로불린 투여로 호전된 항-$Di^a$ 항체에 의한 신생아 용혈성 질환 1예)

  • Hemolytic disease in a newborn that causes early jaundice is common. It is often due to the Rh (D) and ABO incompatibility, but rarely due to unexpected antibodies. Among these unexpected antibodies, the anti-$Di^a$Dia antibody rarely occurs. The anti-$Di^a$ antibody was observed in the serum and red-cell eluate of an infant, and in the serum of his mother. The frequency of the appearance of the $Di^a$ antigen in the Korean population is estimated to be 6.4-14.5%. This paper reports a case of hemolytic disease in a newborn associated with the anti-$Di^a$ antibody. A full-term male infant was transferred to the authors' hospital due to hyperbilirubinemia the day after his birth. The laboratory data indicated a hemoglobin value of 11.6 g/dL, a reticulocyte count of 10.6%, a total bilirubin count of 14.4 mg/dL, a direct bilirubin count of 0.6 mg/dL, and a positive result in the direct Coombs' test. Due to the identification of an irregular antibody from the maternal serum, an anti-$Di^a$ antibody was detected, which was also found in the eluate made from the infant's blood. The infant had been treated with phototherapy and intravenous immunoglobulin since the second day after his birth and was discharged due to an improved condition without exchange transfusion. Therefore, in cases of iso-immune hemolytic disease in a newborn within 24 hours from birth who had a negative result in an antibody screening test, the conduct of an anti-$Di^a$ antibody identification test is recommended due to the suspicion of an anti-$Di^a$ antigen, followed by early administration of intravenous immunoglobulin.

A Comparison in the Value of the Clinical Assays of Dementia with Normal Geriatrics (정상 노인과 치매 환자간 임상화학적 검사 수치의 비교)

  • The measurement of many biochemical changes that could be appeared as aging will be resulted in the very useful and basal marker to maintain healthy life and to predict the health condition and the retardation of the aging. We classified the geriatrics into the normal and same aged dementia on the based of clinical diagnostic as well as MMSE evaluation. And then we were collected blood samples from both groups and examined 13 biochemical assays to screen any valuable biochemical index for dementia. Among 13 assays, several assay data showed the statistically significant differences between two groups. The values of serum albumin, APase, and GOT were significantly decreased in demented group compared to control. On the other hand, the values of glucose and LDH were increased in dementia compared to control. Additionally, the value of Pi was increased but the values of calcium and GPT were decreased, without significant difference (p>0.05). The values of other assays such as bilirubin, BUN, cholesterol, creatinine and total protein did not show any statistically significant differences. Conclusively it may be suggested that dementia might be resulted in the variable biochemical changes which could be affected by many environmental factors as well as dementia itself.

Analysis of Different Ways of Drainage for Obstructive Jaundice Caused by Hilar Cholangiocarcinoma

  • Objective: To evaluate the prognosis of different ways of drainage for patients with obstructive jaundice caused by hilar cholangiocarcinoma. Materials and Methods: During the period of January 2006- March 2012, percutaneous transhepatic catheter drainage (PTCD)/ percutaneous transhepatic biliary stenting (PTBS) were performed for 89 patients. According to percutaneous transhepatic cholangiography (PTC), external drainage was selected if the region of obstruction could not be passed by guide wire or a metallic stent was inserted if it could. External drainage was the first choice if infection was diagnosed before the procedure, and a metallic stent was inserted in one week after the infection was under control. Selection by new infections, the degree of bilirubin decrease, the change of ALT, the time of recurrence of obstruction, and the survival time of patients as the parameters was conducted to evaluate the methods of different interventional treatments regarding prognosis of patients with hilar obstruction caused by hilar cholangiocarcinoma. Results: PTCD was conducted in 6 patients and PTBS in 7 (p<0.05). Reduction of bilirubin levels and ALT levels was obvious after the procedures (p<0.05). The average survival time with PTCD was 161 days and with PTBS was 243 days (p<0.05). Conclusions: With both drainage procedures for obstructive jaundice caused by hilar cholangiocarcinoma improvement in liver function was obvious. PTBS was found to be better than PTCD for prolonging the patient survival.

Effects of Propolis oral administration according to mixture with Hovenia dulcis Thunb. and Artemisia capillaris Thunb. on D-galactosamine-induced liver injury in rats (지기자(枳期子) 및 인진호(茵蔯蒿) 배합(配合) Propolis의 구강투여(口腔投與)가 D-Galactosamine으로 유발(誘發)된 간손상(肝損傷)에 미치는 영향(影響))

  • Objectives : Propolis and Hovenia dulcis Thunb. has been used as treatment of diseases in the Korean medicine. In this study, we investigated that the hepatoprotective effects of Propolis oral administration according to mixture with Hovenia dulcis Thunb. on ${\gamma}-GTP$, GOT, GPT, Total bilirubin, LDH, ALP, Total cholesterol, Triglyceride, SOD, activity of catalase and Glutathione Peroxidase in galactosamine (GalN)-induced liver in rats. Methods : The animals were divided into 5 groups. Control, the liver injury-induced and not treated group. Pro1, liver injury and administrated propolis. Pro2, liver injury and administrated propolis capsule. Pro3, liver injury and administrated mixture of propolis capsule with Hovenia dulcis Thunb. Pro4, liver injury and administrated mixture of Propolis capsule with Hovenia dulcis Thunb. and Artemisia capillaris Thunb.. Animals were treated by Oral administration of Propolis, Hovenia dulcis Thunb., and Artemisia capillaris Thunb. mixture ltime 2 days for 14 days. Results : The Pro1 group was significantly increased on ${\gamma}-GTP$ and activity of Glutathione peroxidase but decreased on GOT in serum as compared with the control group. The Pro2 group was significantly increased on WBC, RBC, Hct, HGB in serum and activity of CuZnSOD as compared with the control group. The Pro 3 group was decreased on Total bilirubin, increased on LDH, WBC, RBC, Hct and HGB in serum as compared with the control group. The Pro 4 group was decreased on GOT in serum as compared with the control group. Conclusion : By evaluating the liver function and lipid metabolism, Pro3 had a hepatoprotective effect on the prevention of hepatotoxity.
