형제에서 발생한 Rotor 증후군 2례 (Two Cases of Rotor Syndrome in Siblings)

  • We experienced two cases of Rotor syndrome in brothers who were a 13 year-old boy and an 11 year-old boy, respectively. They presented with icteric scleras for a few months. Their common laboratory characteristics were as follows: Direct bilirubin was more increased than indirect bilirubin, but aminotransferases were normal. Plasma indocyanine green (ICG) test revealed hepatic excretory defect: plasma ICG concentrations 15 minutes after intravenous injection were 80.45% and 78.28%, respectively. 99mTc-DISIDA Hepatobiliary scan showed that severely decreased hepatic extraction with mild cardiac blood pool, markedly delayed biliary excretion in both intra- & extra- hepatic bile ducts, delayed visualization of gall bladder, and markedly delayed intestinal biliary passage. Needle liver biopsy showed normal hepatic histology without pigmentation.

Effects of a breastfeeding coaching program on growth and neonatal jaundice in late preterm infants in South Korea

  • Purpose: This study examined the effects of a breastfeeding coaching program for mothers on growth and neonatal jaundice in late preterm infants (LPIs). Methods: This was a quasi-experimental study (non-randomized intervention) with a time-series design. The study was conducted among 40 LPIs who were admitted to the neonatal intensive care unit of a university hospital in Daegu, South Korea. In the order of admission, the first 21 infants were assigned to the experimental group, and 19 were assigned to the control group. The intervention program consisted of home- based and web-based practical breastfeeding support education for mothers across a total of 5 sessions. Infant growth was measured using body weight, length, and head circumference, and neonatal jaundice was assessed using transcutaneous bilirubin levels. Results: The likelihood of breastfeeding for infants in the experimental group at 4 weeks after discharge was the same as on the day of discharge, whereas it steadily decreased in the control group. There were significant differences in head circumference between the groups. However, weight, length, and transcutaneous bilirubin levels did not show a significant group-time interaction. Conclusion: A formal breastfeeding coaching program should be considered in clinical settings and at home within the first few weeks postpartum.

Examination of the Fragmentation Behavior of Hemin and Bilin Tetrapyrroles by Electrospray Ionization and Collision-induced Dissociation

  • Bilin tetrapyrroles are metabolic products of the breakdown of porphyrins within a species. In the case of mammals, these bilins are formed by the catabolism of heme and can be utilized as either biomarkers in disease or as an indicator of human waste contamination. Although a small subset of bilin tandem mass spectrometry reports exist, limited data is available in online databases for their fragmentation. The use of fragmentation data is important for metabolomics analyses to determine the identity of compounds detected within a sample. Therefore, in this study, the fragmentation of bilins generated by positive ion mode electrospray ionization is examined by collision-induced dissociation (CID) as a function of collision energy on an FT-ICR MS. The use of the FT-ICR MS allows for high mass accuracy measurements, and thus the formulas of resultant product ions can be ascertained. Based on our observations, fragmentation behavior for hemin, biliverdin and its dimethyl ester, phycocyanobilin, bilirubin, bilirubin conjugate, mesobilirubin, urobilin, and stercobilin are discussed in the context of the molecular structure and collision energy. This report provides insight into the identification of structures within this class of molecules for untargeted analyses.

Blood test results from simultaneous infection of other respiratory viruses in COVID-19 patients

  • Since 2019, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has spread rapidly, infecting millions of people worldwide. On March 11, 2020, the World Health Organization declared coronavirus disease (COVID-19) a pandemic owing to the worldwide spread of SARS-CoV-2, which created an unprecedented burden on the global healthcare system. In this context, there are increasing concerns regarding co-infections with other respiratory viruses, such as the influenza virus. In this study, clinical data of patients infected with SARS-CoV-2 and other respiratory viruses were compared with patients infected with SARS-CoV-2 alone. The hematology and blood biochemistry results of 178 patients infected with SARS-CoV-2 , who were tested on admission, were retrospectively reviewed. In patients with SARS-CoV-2 and adenovirus co-infection, C-reactive protein levels were elevated on admission, whereas lactate dehydrogenase (LDH), prothrombin time, international normalized ratio, activated partial thromboplastin clotting time, and bilirubin values were all within the normal range. Moreover, patients with SARS-CoV-2 and human bocavirus co-infection had low LDH and high bilirubin levels on admission. These findings reveal the clinical features of respiratory virus and SARS-CoV-2 co-infections and support the development of appropriate approaches for treating patients with SARS-CoV-2 and other respiratory virus co-infections.

신생아 특발성 비용혈성 고빌리루빈혈증의 위험 요인에 따른 임상 양상 (The Clinical Characteristics According to the Risk Factors of Idiopathic Nonhemolytic Hyperbilirubinemia)

  • 목적: 최근 출생 후 조기 퇴원과 모유수유 증가로 인해 신생아 고빌리루빈혈증에 의한 재입원 비율이 증가하고 있다. 신생아 고빌리루빈혈증은 적절한 시기에 치료하지 않으면 빌리루빈 뇌증을 유발하여 치명적인 신경학적 후유증을 남길 수 있어 적절한 치료와 함께 조기 예방의 중요성이 부각되고 있다. 이에 저자는 우리나라 신생아의 특발성 비용혈성 고빌리루빈혈증의 임상 경과와 위험 인자, 뇌 MRI 결과를 조사함으로써 황달의 역학 조사와 치료 방침 설정의 기초 자료를 얻고자 본 연구를 시행하였다. 방법: 2006년 1월부터 2009년 9월까지 고빌리루빈혈증으로 진단받고 경북대학교병원 신생아 집중치료실에 입원한 환아 중 재태연령이 35주 이상, 총혈청 빌리루빈이 20 mg/dL 이상이면 서 용혈성 질환이 없으면서 감염, 대사 질환등 황달의 원인이 없는 79명의 환아를 대상으로 의무 기록을 후향적으로 조사하였다. 결과: 평균 재태주령은 $38^{+3}{\pm}1^{+4}$주였고 내원 당시 나이는 8.8$\pm$4.0일이었다. 내원 당시 체중은 3,105$\pm$479 g으로 출생 체중 3,174$\pm$406 g과 비교하여 평균 2.8$\pm$6.4% 감소하였다. 모유수유, 두혈종, 경막하출혈 및 ABO 부적합증과 같은 위험인자가 있는 환아와 위험인자가 없는 환아를 비교하였으며 최고 총혈청빌리루빈, 광선요법의 시행 기간 및 반응 정도에서 유의한 차이를 보이지 않았다. 출생 체중과 입원 당시 체중을 비교하여 체중이 증가한 군과 감소한 군의 비교에서는 재태주령, 내원 당시 연령과 최고 혈청 빌리루빈치가 유의한 차이를 보였다. 뇌 MRI를 시행한 39명의 환아 중 21명은 T1 강조 영상에서 창백핵의 음영이 피각 및 인접한 피질척수로의 음영과 비교하여 증가된 소견을 보였다. 43명의 환아가 ABR을 시행하였는데 5명의 환아에서 이상 소견이 보였다. 결론: 우리나라에서 발생하는 특발성 비용혈성 고빌리루빈혈증은 수유량 부족이 원인이 될 수 있으므로 수유방법에 대한 충분한 교육과 퇴원 후 정기적인 추적 관찰이 필요하다. 본 연구는 많은 대상군을 포함하지 않아 더 많은 대상군과 대조군의 비교분석이 필요하고 향후 장기적인 추적 관찰을 통해서 만성 빌리루빈 뇌증에 대한 연구도 필요할 것으로 생각된다.

한.양약 복용이 간기능이상에 미치는 영향에 대한 연구 (Effects of Herbal and Western Medicines on Liver Functions)

  • Objectives : The aim of this study was to evaluate effects of herbal medicine and other associated factors for abnormal liver function tests(especially total bilirubin, AST, ALT, and ${\gamma}GT$) levels in outpatients at an oriental medical clinic. Methods : A cross-sectional study based on clinical records was conducted on 1,871 patients at one Oriental medical clinic in Seoul, Korea. 504 patients received a liver function tests for screening and 497 patients ranging from the age of 4 to 74 were chosen for the study. Patients with basal liver disease or DM were excluded during the screening process. Patients were classified into case(abnormal) and control(normal) groups by normal liver function test references. Results and Conclusions : In this study, 33.0% of the patients were females, and 67.0% were males. The mean age was $34.7{\pm}11.9$ years old. The mean total protein value was $6.78{\pm}0.66g/dL$(male $6.79{\pm}0.61$, female $6.76{\pm}0.76$), albumin $3.89{\pm}0.50g/dL$(male $3.94{\pm}0.47$, female $3.81{\pm}0.54$), total bilirubin $0.51{\pm}0.35mg/dL$(male $0.54{\pm}0.39$, female $0.45{\pm}0.23$), AST $23.31{\pm}18.22U/L$(male $26.37{\pm}20.73$, female $17.09{\pm}8.72$), ALT $33.49{\pm}36.36U/L$(male $40.56{\pm}41.77$, female $19.13{\pm}12.64$), LDH $258.07{\pm}74.84mg/dL$(male $263.68{\pm}73.77$, female $246.70{\pm}75.92$), and ${\gamma}GT$ $39.64{\pm}59.16U/L$(male $50.15{\pm}69.43$, female $17.83{\pm}8.36$). The percentage of abnormal total bilirubin levels(>1.0) in these patients was 2.6%, abnormal AST levels(>39m, >29f) 8.5%, ALT levels(>47m, >32f) 18.8%, and ${\gamma}GT$ (>50m, >40f) 19.9%.

대방풍탕(大防風湯)이 galactosamine으로 유도(誘導)한 간중독(肝中毒) 흰쥐에 미치는 영향(影響) (Effects of Daebangpungtang on galactosamine induced hepatoxity in rats)

  • In order to study the effects of Daebangpungtang on galactosamine induced hepatoxity in rats, the experimental rats divided five groups (Normal, Control, Sample A, B, C groups). Under the same condition, normal and control groups were administered normal saline for 16 days, control group was injected to abdomen with galactosamine at 8th day (800mg/kg). Sample A group was administered the Daebangpungtang aqueous solution($200m{\ell}/kg$ p.o) for 8 days and injected galactosamine(800mg/kg i.p) for the last day and was administered normal saline for 8 days. Sample B group was treated as same as group A for 8 days, and then was administered the Daebangpungtang aqueous solution($200m{\ell}/kg$ p.o) forfurther 8 days. Sample C group was administered the Daebangpungtang aqueous solution($200m{\ell}/kg$ p.o) for 16 days. The activity of GOT, GPT, ${\gamma}$-GTP, ALP, LDH and total bilirubin in serum were measured at 8th and last day. The results were obtained as follows: 1. The activity of serum GOT of the sample A group decreased effectively at the 8th day and sample B group decreased effectively at 16th day as compared with the control group. 2. The activity of serum GPT of the sample A group decreased effectively at the 8th and 16th day and sample B group decreased effectively at 16th day as compared with the control group. 3. The activity of serum ${\gamma}$-GTP of the sample B group decreased effectively at the 16th day as compared with the control group. 4. The activity of serum ALP of the sample A group increased respectively at the 8th and 16th day and sample B group decreased effectively at 16th day as compared with the control group. 5. The activity of serum LDH of the sample A, B groups decreased effectively at 16th day as compared with the control group. 6. The activity of serum total bilirubin of the sample A, B groups decreased effectively at 16th day as compared with the control group. 7. The activity of GOT, GPT, ${\gamma}$-GTP, ALP, LDH and total bilirubin in serum of the sample C group were analogous with thats of normal group.

알코올성 간염 환자의 불면과 피로 개선에 대한 한양방 병용치료 치험 1례 (A Case Report of Insomnia and Fatigue in Alcoholic Hepatitis Patients Treated with Korean Medicine and Western Medicine)

  • Objectives: This study aimed to report a case that showed improvements in the symptoms of patients with alcoholic hepatitis without any indication of deterioration of the disease. Methods: Western medicine with Urusa tablets and Godex capsules and Korean medicine therapeutic approaches, including Shihogayonggolmoryo-tang, acupuncture, and moxibustion, were administered to a patient during the period of treatment. Blood tests were used to determine levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), AST/ALT ratio, alkaline phosphatase (ALP), gamma glutamyltransferase (GGT), total bilirubin, direct bilirubin, and total cholesterol. Fatigue was measured using the numeric rating scale (NRS), and the patient's total sleeping time was checked, daily. Results: After the combined treatment, the AST/ALT ratio and the AST, ALT, ALP, GGT, total cholesterol, and direct bilirubin levels were decreased. Through Oriental medicine for the purpose of improving symptoms, NRS of fatigue decreased from 10 to 5, and the amount of sleeping time increased from 2 to 5 hours. Conclusions: The herbal medicine had no effect on the hepatoprotective drugs such as Urusa tablets and Godex capsules used to treat alcoholic hepatitis, and no adverse reaction from the combined administration was observed. To reduce fatigue and insomnia in patients with alcoholic hepatitis, it might be helpful to combine Western medications with Korean medicine treatments, including acupuncture, moxibustion, and Shihogayonggolmoryo-tang.

말기암환자에서 혈장 내독소 농도가 생존기간에 미치는 영향 (The Effects of Plasma Endotoxin Level on Survival Time of Terminally Ill Cancer Patients)

  • 목적: 말기암환자의 여명 예측은 치료의 이득과 위해를 판단하는 잣대가 되고, 적절한 의료 중재 제공 및 환자의 자율성에 기초한 의사결정에 중요한 기준이 된다. 특히 많은 수의 말기암환자는 다발성 장기 부전으로 사망에 이르기 때문에 본 연구에서는 이를 반영할 수 있는 혈장 내독소 농도와 생존기간과의 연관성을 알아보고자 하였다. 방법: 2009년 4월부터 10월까지 6개월 동안, 만 20세 이상의 말기암환자 56명을 대상으로 혈장 내독소 농도를 측정하고, 생존기간을 조사하였다. 나이, 성별, 원발 암 부위, 암 치료 경력, 전이여부, 투약상황 및 활력 증후, Karnofsky 수행지수 등의 신체검사, 백혈구 수, 혈색소, 적혈구용적률, aspartate aminotransferase, alanine aminotransferase C-반응성 단백질, 총 빌리루빈, 내독소 등의 혈액검사를 시행하였다. 결과: 혈장 내독소 농도, 성별, 나이, 백혈구 수, 혈색소, 적혈구 용적률, AST, ALT, 총 빌리루빈, C-반응성 단백질, 통증강도를 단변량 분석한 결과 혈장 내독소 농도는 생존기간과 통계적으로 유의한 관계를 보이지는 않았으나, 단변량 분석에서 생존기간에 유의한 영향을 미치는 성별, 백혈구 수, 총 빌리루빈을 포함하여 시행한 다변량 분석에서 혈장 내독소 농도는 생존기간과 통계적으로 유의한 관계가 있는 것으로 나타났다. 결론: 다발성 장기 부전과 관련이 있는 혈장 내독소 농도는 생존기간 예측인자로서 쓰일 수 있는 가능성을 보여 주었다.

삼두해정탕이 사염화탄소(四鹽化炭素) 및 d-Galactosamine에 의(依)하여 유발(誘發)된 실험적(實驗的) 흰쥐의 간손상(肝損傷)에 미치는 영향(影響) (The effects of Samduhaejungtang on liver injury of rats induced by $CCl_4$ and d-galactosamine)

  • In order to investigate the protective effect of Samduhaejungtang on the liver injury of rats induced by $CCl_4$ and d-galactosamine, the serum transaminase(GOT& GPT), alkaline phosphatase(ALP), lactic dehydrogenase(LDH) for enzyme activities and triglyceride, total bilirubin amounts for serum component were measured. All animals were divided into 4 groups, those were normal group(untreated), control group(treated with vehicle 0.9% Saline solution), sample Ⅰ group(1000mg/kg administrated), sample Ⅱ group(2000mg/kg administrated). The results were as follows : 1. The inhibitory effects of the serum GOT activities in rats induced by $CCl_4$ were noted in both sample Ⅰ group(p<0.01) and sample Ⅱ group(p<0.001). In serum GPT activities, sample Ⅱ group(p<0.05) only showed the inhibitory effects. 2. The inhibitory effects of the serum ALP activities in rats induced by $CCl_4$ were noted in both sample Ⅰ group(p<0.01) and sample Ⅱ group(p<0.001). 3. The inhibitory effects of the serum LDH activities in rats induced by $CCl_4$ were noted in sample Ⅱ group(p<0.05). 4. The increases of the serum triglyceride content level in rats induced by $CCl_4$ were inhibited in both sample Ⅰ group(p<0.01) and sample Ⅱ group(p<0.001). 5. The increases of the serum total bilirubin content level in rats induced by $CCl_4$ were inhibited in both sample Ⅰ group(p<0.01) and sample Ⅱ group(p<0.001). 6. The inhibitory effects of the serum GOT& GPT activities in rats induced by d-galactosamine were noted in sample Ⅱ group(p<0.001), but sample Ⅰ group was not recongnized. 7. The significantly inhibitory effects of in the serum LDH activities in rats induced by d-galactosamine were note in sample Ⅱ group(p<0.05). 8. The increases of the serum triglyceride content level in rats induced by d-galactosamine were inhibited in sample Ⅱ group(p<0.01). 9. The increases of the serum total bilirubin content level in rats induced by d-galactosamine were inhibited in sample Ⅱ group(p<0.05). According to the above results, it is considered that Samduhaejungtang has protective effect against liver injury in rats induced by $CCl_4$ and d-galactosamine.
