Posterior type II SLAP Lesion Combined with Posterior Bankart Lesion - A Case Report - (후방 Bankart 병변을 동반한 후방 II형 SLAP 병변 - 증례보고 -)

  • SLAP(Superior labrum anterior to posterior) lesion is found in superior labrum injury alone and also combined with extension of the Bankart lesion(anteroinferior labral tear) in recurrent shoulder dislocation patients and rarely accompanied by the posterior Bankart lesion. There have been reports about SLAP lesions associated with various lesions, however, posterior type II SLAP lesion associated with posterior Bankart lesion has been rarely reported. In such a case, there are important technical tips in inserting anchors and suturing during arthroscopic repair. We experienced a rare case of posterior type II SLAP lesion associated with posterior Bankart lesion, occurred not after repetitive throwing(common mechanism) but after trauma in slipping down with the arm stretched during riding a bicycle. The satisfactory result was obtained after arthroscopic repair in this case.

  • This is a review article about range of motion, stretching, and aerobic exercise in accelerated rehabilitation of knee and shoulder. If the joint was immobilized for a long time after injury, it would cause stiffness and atrophy. Therefore, this program includes various exercise techniques; range of motion for joint stiffness, and stretching for muscle relaxation, and cardiovascular training (e.g., swimming, upper body extremity, stationary bicycle) for prevention of cardiopulmonary function decrease. In accelerated rehabilitation, It is very important factor to make interaction between clinical exercise specialist and patients. Also, we recommend that they should discuss with sports medicine doctor as a team members the following; pain, adaptation of exercise, fitness level, and progression of program.

Students', Teachers', and Parents' Safety Concerns Affecting Students' Safety Behaviors of Elementary Schools in Korea (학생, 부모, 교사의 안전 관심도가 초등학교 어린이의 안전행동에 미치는 영향)

  • Objectives: More than 80% of unintentional injury was related to risk-taking behaviors involved in child accidents. Therefore, diverse care and concerns on safety should be provided from teachers as well as parents to build child safety behaviors. The purpose of this study was to identify the effects of safety concerns from students, teachers, and parents on elementary students' safety behaviors in Korea. Methods: The 5th and 6th grade students participated in this study in 14 elementary schools in Korea and the total number of students was 1,033. The study schools were selected by the stratified cluster sampling method throughout 7 metropolitan areas in Korea. Data were collected by the self-administered survey and the questionnaires were delivered to the vice-principals of the designated schools by ground postal service and the vice-principals conducted survey data collection. The questions on students' safety concerns, teachers' safety concerns, parents' safety concerns, safety practices, safety education, and demographics were included in the survey. All survey responses were encoded into SPSS program and t-test, ANOVA, and multiple regression analysis were utilized. Results: The students taking accidents one time more per year were more than 60% and the two thirds of them recognized that their accidents were due to their risk behaviors. Parents' education on safety practices was more frequent than teachers' education; furthermore, teachers' concerns on safety behaviors were poorer than students' or parents' concerns. In terms of safety behaviors, the scores of fire and home-related safety behavior were high but the scores of traffic and bicycle, in particular, helmet wearing practice were low. Three concerns from students, parents, and teachers all were significantly related to children's safety behaviors. Conclusions: In conclusion, safety concerns from the close networks are the important indicator of child safety behaviors and, the safety training programs, therefore, for parents and teachers as well as for students need to be developed for improving children's safe behaviors.

Clinical Study of Old-aged Patients in Traffic Accidents and Admitted For Emergency Treatment (도심 지역에 위치한 일개병원의 고 연령 교통사고 환자에 대한 임상적 연구)

  • Purpose: For prevention and suitable administration, the effect of age on the severity of injuries in traffic accidents should be considered when evaluating a patient, but there have not been enough epidemiological studies that evaluate the age factor in traffic accidents. For that reason, we investigated old-aged patients who were involved in traffic accidents (65 years old or more) and who were admitted to the emergency department of a college hospital in an urban city of Korea. Methods: We collected data from traffic-accident patients who came to the emergency room of a university hospital in Seoul from Jan.1, 2004 to Dec.31, 2005. We compared their abilities to ambulate and the RTSs (Revised trauma scores) by using a LSD (least significant difference), linear regression. Results: A total of 1460 patients were included. The mean RTS of all traffic-accident patients was $7.77{\pm}0.280$. The scores for drivers and passengers, motor-cycle drivers and passengers, bicycle drivers and passengers, and pedestrians were $7.79{\pm}0.21$, $7.78{\pm}0.22$, $7.54{\pm}0.25$, $7.77{\pm}0.20$, and $7.80{\pm}0.21$ respectively (p=0.000). There was no statistically significant difference between the RTS of patients over 65 years and that of other patients. In a regression analysis, the number of patients over 45 ages who were able to ambulate was lower than that of younger people, independently of other influencing factors (B=-0.330, R-square = 0.243, p=0.000). Conclusion: We expected that RTS of old age group more than 65 years old will significantly lower than that of others, but there was no statistically significant difference.

  • A 55 year-old man hit a vehicle while riding a bicycle. He was diagnosed as left hemopneumothorax, multiple rib fracture, cerebral hemorrhage, and skull fracture. Initially he suffered from hypoxia requiring 100% oxygen with a mechanical ventilator. Finally he became hypotensive. Venovenous extracorporeal membrane oxygenation (ECMO) was initiated to support patient's gas exchange. Because hypotension and left ventricular dysfuction persisted, we converted the mode of support to veno-arterio-venous ECMO. Over four days of intensive care, we could wean off ECMO. The patient went to rehabilitation facility after 45 days of hospitalization. Although trauma and bleeding are considered as relative contraindication of ECMO, careful decision making and management may enable us to use ECMO for trauma-related refractory heart and/or lung failure.

Association Between Transport Accident Type And Mortality In Elderly Inpatients : Using Korean National Hospital Discharge In-depth Injury Survey Dataset (퇴원손상심층조사자료를 이용한 노인 입원 환자의 운수사고 유형과 사망 사이의 연관성)

  • This study analyzed the association between the type of transport accident and the associated mortality of elderly inpatients. The findings will contribute to the development and establishment of a systematic and effective policy according to the type of transport accident to reduce the mortality of inpatients. The data on elderly inpatients with transport accidents was extracted from the 2013-2017 Korean National Hospital Discharge In-depth Survey dataset. The data was analyzed by descriptive statistics analysis, chi-square tests and multiple logistic regression analysis. After adjustment for sociodemographic, disease, injury and policy factors, the elderly inpatient deaths due to transport accidents were significantly higher for pedestrian accidents (OR: 2.522 95%, CI: 1.291-4.972), bicycle/cart accidents (OR: 2.809, 95% CI: 1.328-5.942) and motorcycle accidents (OR: 2.330, 95% CI: 1.226-4.819) rather than that for car accidents. Likewise, elderly inpatients have a higher risk of death from other types of transport accidents than those caused by car accidents. However, Korean policies related to transport accidents of elderly inpatients are concentrated on car accidents. Effective policy is needed according to the characteristics of each type of transport accident to reduce the transport accident mortality of elderly inpatients.

Acetabular Labral Tear in Sports Injury (스포츠 손상에서의 비구순 파열)

  • Purpose: The purpose of this study is to evaluate diagnosis and treatment of acetabular labral tears in sports injuries. Materials and Methods: From March 1995 to January 2000, We treated 15 patients with acetabular labral tear by sports injuries. There were 6 men and 9 women. Mean age was 41 years old. Running injuries are 5 cases, fighting 3, aerobic exercise 3, swimming 2, climbing 1, bicycle 1. For conservative treatment, we performed medication and observation at least for 1 year. For the patients with arthroscopic surgery, we performed Harris Hip Score (pain and function) preoperatively and postoperatively at 6, 12 months, and evaluated patient’s pain by JOA pain scoring system and postoperative subjective satisfaction. Results: The mean score of HHS improved 15 points with conservative treatments and 32 points with hip arthroscopy. In 4 cases of conservative treatment, there was 1 scale improvement of JOA pain scoring system. In arthroscopic partial labrectomy, all cases were improved to more than 2 scale. Conclusion: We considered that acetabular labral tears occurred associated with sports injuries. So it is important to be interested in enthusiastic diagnosis and appropriate treatment for hip pain caused by sports injuries, especially about acetabular labral tears.

The impact of COVID-19 on trauma patients and orthopedic trauma operations at a single focused training center for trauma in Korea

  • Purpose: The objective of this study was to determine the effects of coronavirus disease 2019 (COVID-19) on the volume of trauma patients, the number of orthopedic trauma operations, and the severity of injuries. We also investigated the correlations between social distancing and these variables. Methods: This was a retrospective review of trauma patient cases at a single focused training center for trauma in Korea from January 2017 to April 2021. The COVID-19 group included patients treated from January 1 to April 30 in 2020 and 2021, and the control group included patients treated during the same months from 2017 to 2019. The volume of trauma patients according to the level of social distancing was evaluated among patients treated from August 2, 2020 to November 23, 2020. Results: The study included 3,032 patients who presented to the emergency department with traumatic injuries from January to April 2017 to 2021. The average number of patients was 646.7 and 546.0 in the control and COVID-19 groups, respectively. The percentage of patients injured in traffic accidents (TAs) decreased from 25.0% to 18.2% (P<0.0001). The proportions of in-car TAs and pedestrian TAs also decreased from 6.7% and 10.8% to 3.5% and 6.0%, respectively (P=0.0002 and P<0.0001). The percentage of bicycle TAs increased from 2.4% to 4.0% (P=0.0128). The proportion of patients with an Injury Severity Score above 15 and the mortality rate did not change significantly. As the level of social distancing increased, the number of trauma patients and the number of trauma injuries from TAs decreased. The number of orthopedic trauma operations also depended on the social distancing level. Conclusions: The number of trauma patients presenting to the emergency department decreased during the COVID-19 period. The volume of trauma patients and orthopedic trauma operations decreased as the social distance level increased.

Comparison of Biomechanical Characteristics for the Skill Level in Cycle Pedaling

  • Objective: This study aimed to compare biomechanical data between elite and beginner cyclists during cycle pedaling by performing a comparative analysis and to provide quantitative data for both pedaling performance enhancement and injury prevention. Methods: The subjects of this study included 5 elite cyclists (age: $18{\pm}0years$, body mass: $64.8{\pm}9.52kg$, height: $173.0{\pm}4.80cm$) and 5 amateur cyclists (age: $20{\pm}0years$, mass: $66.6{\pm}2.36kg$, height: $175.6{\pm}1.95cm$). The subjects pedaled on a stationary bicycle mounted on rollers of the same gear (front: 50 T and rear: 17 T = 2.94) and cadence of 90. The saddle height was adjusted to fit the body of each subject, and all the subjects wore shoes with cleats. In order to obtain kinematic data, 4 cameras (GR-HD1KR, JVC, Japan) were installed and set at 60 frames/sec. An electromyography (EMG) system (Telemyo 2400T, Noraxon, USA) was used to measure muscle activation. Eight sets of data from both the left and right lower extremities were obtained from 4 muscles (vastus medialis oblique [VMO], vastus lateralis oblique [VLO], and semitendinosus [Semitend], and lateral gastrocnemius [Gastro]) bilaterally by using a sampling frequency of 1,500 Hz. Five sets of events ($0^{\circ}$, $90^{\circ}$, $180^{\circ}$, $270^{\circ}$, and $360^{\circ}$) and 4 phases (P1, P2, P3, and P4) were set up for the data analysis. Imaging data were analyzed for kinematic factors by using the Kwon3D XP computer software (Visol, Korea). MyoResearch XP Master Edition (Noraxon) was used for filtering and processing EMG signals. Results: The angular velocity at $360^{\circ}$ from the feet was higher in the amateur cyclists, but accelerations at $90^{\circ}$ and $180^{\circ}$ were higher in the elite cyclists. The amateur cyclists had greater joint angles at $270^{\circ}$ from the ankle and wider knee joint distance at $0^{\circ}$, $180^{\circ}$, and $360^{\circ}$ than the elite cyclists. The EMG measurements showed significant differences between P2 and P4 from both the right VLO and Semitend. Conclusion: This study showed that lower body movements appeared to be different according to the level of cycle pedaling experience. This finding may be used to improve pedaling performance and prevent injuries among cyclists.

Analysis of Traffic Accidents Injury Severity in Seoul using Decision Trees and Spatiotemporal Data Visualization (의사결정나무와 시공간 시각화를 통한 서울시 교통사고 심각도 요인 분석)

  • The purpose of this study is to analyze the main factors influencing the severity of traffic accidents and to visualize spatiotemporal characteristics of traffic accidents in Seoul. To do this, we collected the traffic accident data that occurred in Seoul for four years from 2012 to 2015, and classified as slight, serious, and death traffic accidents according to the severity of traffic accidents. The analysis of spatiotemporal characteristics of traffic accidents was performed by kernel density analysis, hotspot analysis, space time cube analysis, and Emerging HotSpot Analysis. The factors affecting the severity of traffic accidents were analyzed using decision tree model. The results show that traffic accidents in Seoul are more frequent in suburbs than in central areas. Especially, traffic accidents concentrated in some commercial and entertainment areas in Seocho and Gangnam, and the traffic accidents were more and more intense over time. In the case of death traffic accidents, there were statistically significant hotspot areas in Yeongdeungpo-gu, Guro-gu, Jongno-gu, Jung-gu and Seongbuk. However, hotspots of death traffic accidents by time zone resulted in different patterns. In terms of traffic accident severity, the type of accident is the most important factor. The type of the road, the type of the vehicle, the time of the traffic accident, and the type of the violation of the regulations were ranked in order of importance. Regarding decision rules that cause serious traffic accidents, in case of van or truck, there is a high probability that a serious traffic accident will occur at a place where the width of the road is wide and the vehicle speed is high. In case of bicycle, car, motorcycle or the others there is a high probability that a serious traffic accident will occur under the same circumstances in the dawn time.
