Factors Influencing Nursing Practices of Physical Restraint Use among Nurses working in Long-term Care Hospitals (요양병원 간호사의 신체적 억제대 사용과 관련된 간호실무에 영향을 미치는 요인)

  • This study was undertaken to identify knowledge, attitude, perception and nursing practice toward use of physical restraints, among nurses in long-term care hospitals. We further explored the impact of factors associated with nursing practice toward use of physical restraints. This was a cross-sectional descriptive study in which self-administered questionnaires were collected from 128 nurses working at 11 long-term care hospitals. Descriptive statistics, including t-test, one-way ANOVA, Welch's test, Pearson correlation coefficients and multiple regression, were applied to analyze the data collected. Our results reveal that the participants possess moderate levels of knowledge, attitude, perception and nursing practice toward use of physical restraints. Significant positive correlations were observed between nursing practice and knowledge. Multiple regression analysis showed that the factors significantly related to nursing practice and use of physical restraint were knowledge (${\beta}=0.27$, p=0.002) and attitude (${\beta}=-0.17$, p=0.044), having a total explained variance of 9.0%. The findings from this study suggest that knowledge is the strongest predictor on nursing practice toward use of physical restraints. Thus, we propose that developing and applying evidence-based educational intervention programs to reduce the use of physical restraints in long-term care hospitals are required.

Convergence relationships between Leisure Satisfaction, Job Stress and Nursing Performance of Nurses (간호사의 여가만족, 직무스트레스 및 간호업무성과 간의 융합관계)

  • This study aimed to identify the factors affecting the nursing performance and the convergence relationships of leisure satisfaction, job stress, and nursing performance among nurses. Participants included 150 nurses working at 2 hospitals in C city from August 3 to September 15, 2018. The data were analyzed using descriptive statistics, independent t-test, one-way ANOVA with Scheffe's test, Pearson correlation coefficients, and multiple regression by SPSS 23.0 program. Nursing performance were significantly differences in age (F=4.78, p=.003), marital status (F=2.74, p=.007), length of clinical career (F=3.34, p=.012), and health condition (F=5.23, p=.002). Nursing performance was a positively correlated with leisure satisfaction (r=.32, p<.001) and a negatively correlated with job stress (r=-19, p=.020), and job stress was a negatively correlated with leisure satisfaction (r=-.32, p=.005). The factors affecting the nursing performance of clinical nurses were age (${\beta}=.20$, p=.008) and leisure satisfaction (${\beta}=.28$, p<.001), and these variables explained about 19.6% of the variance in nursing performance. In order to improve nursing performance, it is necessary to provide working environment and institutional support for increasing leisure satisfaction and reducing job stress and age-differentiated approaches.

  • The European Economic Community four plate test(EEC 4-plate test, FPT, EU) has been used for monitoring antimicrobial drug residues in meat by Local Veterinary Service Center in Korea. This study was performed to evaluate sensitivity and group specificity of some antimicrobial drugs in FPT and to compare FPT with Charm II test. The minimal detectable levels of targeted antimicrobial drugs tested with standard solutions were 0.025∼1.0 ppm for 7 beta-lactams, 0.5∼1.0 ppm for 4 aminoglycosides, 0.05∼0.5 ppm for 5 macrolides, 0.05∼0.25 ppm for 3 tetracyclines and 0.25&1.0 ppm for 6 sulfonamides. In comparison of FPT and Charm II test, the results of FPT were not accord with those of Charm II test having the group specificity of seven families of antimicrobial drugs in meat samples except some families like tetracyclines.

The Effects on the Level of $\beta-endorphin$ and Pain Threshold according to each TENS and MENS Application (경피신경 자극과 미세전류신경근 자극이 $\beta-endorphin$ 과 동통역치에 미치는 영향)

  • The purpose of this study was to examine the amount of $\beta-endorphin$ and pain threshold according to time sequences in applying experimented TENS and MENS(pre treatment, posttreatment, 25 minutes after the treatment). This test was to aim at showing the effects of the experimental $\beta-endorphin$ and pain threshold applied to the randomly selected twenty healthy men in their twenties. The subjects were divided into two group(ten for TENS and the other ten MENS). The results obtained are as follows : 1. There was no significant difference in the $\beta-endorphin$ between TENS and HENS(p>0.05). There was no difference in the pain threshold at pretreatment and posttreatment(p>0.05), but there was some difference at 25 minutes after the treatment(p<0.05). 2. The group of TENS in the experimental $\beta-endorphin$ had the highest level of $14.40{\pm}3.098$ at posttreatment, but the level decreased a little according to time passed. And in the experimental pain threshold, the level went to the highest plateau of $2.92{\pm}0.483$ at 25 minutes after the treatment. 3. The group of MENS in the experimental $\beta-endorphin$ had the highest plateau of $14.20{\pm}3.967$ at posttreatment, but the level decreased a bit according to time passed also. And in the experimental pain threshold, the level went to the highest plateau of $2.49{\pm}0.617$ posttreatment. 4. There were some differences of the experimental $\beta-endorphin$ in TENS group at pretreatment and posttreatment(p<0.05). There were some differences in the experimental pain threshold between pretreatment and posttreatment as well as between pretreatment and 25 minutes after the treatment(p<0.05) MENS did not influence the experimental $\beta-endorphin$ and pain threshold. This experiment showed that TENS increased the levels of the experimental $\beta-endorphin$ at posttreatment and increased the levels of the experimental pain threshold untill 25 minutes after the treatment. Therefore, the time of sustaining pain in TENS group was longer than that of MENS group. Also, MENS showed that it increased each level of the experimental $\beta-endorphin$ and pain threshold, but these levels were not statistically meaningful.

The Influence of Life Stress, Ego-Resilience, and Spiritual Well-Being on Adaptation to University Life in Nursing Students (간호대학생의 생활스트레스, 자아탄력성 및 영적안녕이 대학생활 적응에 미치는 영향)

  • This study examined the factors influencingthe adaptation to university life of nursing students. Data were collected from 235 nursing students in three nursing schools in B and C city from November 15 to December 10, 2016, and analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and stepwise multiple regression. Correlations were found between adaptation to university life and life stress (r=-0.657, p<0.001) and between ego-resilience (r=0.455, p<0.001) and spiritual well-being (${\beta}=0.210$, p=0.025)(r=0.494, p<0.001). The adaptation to university life in participants showed a significant correlation between life stress (r=-0.657, p<0.001), ego-resilience (r=0.455, p<0.001), and spiritual well-being (r=0.494, p<0.001). The significant factors influencing the adaptation to university life were life stress (${\beta}=-0.418$, p<0.001), health status(${\beta}=0.213$ p<0.001), spiritual well-being (${\beta}=0.210$, p=0.025), satisfaction with major (${\beta}=0.146$, p=0.003), and ego-resilience (${\beta}=0.109$, p<0.001); these factors explained 56.5% of the variance. The findings suggest that adequate educational strategies are needed to reduce the life stress and increase health status, spiritual well-being, satisfaction on major, and ego-resilience in nursing students to promote their adaptation to university life.

The Clinical Significance of Serum $Beta_2-microglobulin$ Levels in Patients with Various Liver Diseases (각종(各種) 간질환자(肝疾患者)에서 혈청 $Beta_2-microglobulin$ 치(値)의 임상적(臨床的) 의의(意義))

  • To evaluate the significance of serum $beta_2-microglobulin$ in patients with various liver diseases, serum $\beta_2m$ levels were measured in 44 cases of normal controls, 32 cases of asymptomatic HBsAg carriers and 134 patients with various liver diseases, by radioimmunoassay using Phadebas $Beta_2-micro$ test kits. The following results were obtained: 1) The mean level of serum $\beta_2m$ was $1.39{\pm}0.25mg/l(Mean{\pm}S.D.)$ in normal controls ($1.39{\pm}0.23mg/l$ in 24 males, $1.38{\pm}0.27mg/l$ in 20 females). 2) The serum levels of $\beta_2m$ in patients with various liver diseases and asymptomatic HBsAg carriers were as follows; $1.40{\pm}0.27mg/l$ in asymptomatic HBsAg carriers, $2.42{\pm}0.37mg/l$ in 45 patients with acute viral hepatitis, $2.10{\pm}0.26mg/l$ in 46 patients with chronic persistent hepatitis, $2.60{\pm}0.34mg/l$ in 23 patients with chronic active hepatitis, and $2.60{\pm}0.49mg/l$ in 20 patients with liver cirrhosis. Serum $\beta_2m$ levels of each disease group were significantly higher than that of normal controls(p<0.001). 3) There was significant correlation between the levels of serum $\beta_2m$ and the degrees of lymphocytic infiltration in patients with chronic active hepatitis(p<0.001). 4) Significant correlations were observed between the levels of serum $beta_2-microglobulin$ and serum alanine aminotransferase(r=0.68, p<0.05) and bilirubin(r=0.63, p<0.05) in 15 patients with acute viral hepatitis. In conclusion, the serum $beta_2-microglobulin$ levels were increased in patients with various liver diseases, and it may serve as a new index of liver disease activity.

Immunomodulatory Activities of Oat Bran Extracts with Different Extraction Conditions (추출조건에 따른 귀리 추출물의 면역활성)

  • Immunomodulatory activities of crude ${\beta}$-glucans extracted from oat bran under different conditions, fractions A ($55^{\circ}C,\;5%,\;pH\;6$), B ($45^{\circ}C,\;15%,\;pH\;6$), C ($50^{\circ}C,\;20%,\;pH\;7$), D ($50^{\circ}C,\;0%,\;pH\;7$), and E ($50^{\circ}C,\;10%,\;pH\;9$) were investigated. All crude ${\beta}$-glucan fractions stimulated macrophages, producing nitric oxide dose-dependently, and, efficiently promoted nitric oxide production in presence of IFN-${\gamma}$. Except for fraction C, in vivo test indicated fractions B, D, and E (100 mg/kg) substantially enhanced carbon-phagocytic indices of blood macrophages by oral administration of crude ${\beta}$glucan for 7 days prior to carbon injection. These immunomodulatory effects could be determined with extraction conditions of crude ${\beta}$-glucan.

Cognition Enhancing Effect of Muskmelon (Cucumis melo) Extracts on Scopolamine-induced Memory Impairment in Mice (참외추출물이 스코폴라민 유도 기억상실 흰쥐의 인지능 회복에 미치는 효과)

  • The methanol extract of muskmelon (Cucumis melo) has been investigated for its cognition enhancing effects by evaluation of inhibitory activities on acetylcholinesterase, a degrading enzyme of acetylcholine, a brain neurotransmitter, and ${\beta}$- secretase, which forms the ${\beta}$-amyloid toxic protein from its precursor protein. A passive avoidance task, one of the animal model experiments for learning and memory, was also performed. As a result, the melon extract showed 15.8% and 35.3% inhibition on acetylcholinesterase and ${\beta}$-secretase, respectively, with a final concentration of 100 mg/ml. In the animal model test, melon extract significantly (p<0.05) lengthened the step-through latency time by 22.7% compared to the control group, suggesting that melon extract has, indeed, an effect on cognition enhancement.

The Relationship among Subjective Health Status, Wisdom, and Self-care Agency in Korean Older Adults

  • Purpose: This study identified the relationships among subjective health status, wisdom, and self-care agency of Korean older adults. Methods: The participants were 274 older adults from Daegu, aged over 65 years. Data were collected by visiting two social welfare centers, two senior welfare centers, and nine senior centers in Daegu from January 18 to February 5, 2013. Data were analyzed through one-way ANOVA, independent t-test, Pearson's correlation, and stepwise multiple linear regression using the IBM SPSS/WIN 19.0 statistical program. Results: A significant positive correlation was found among older adults' subjective health status, wisdom, and self-care agency. Self-care agency was affected by wisdom (47%, ${\beta}=.55$), the physical component summary (18%, ${\beta}=.31$) and mental component summary (1%, ${\beta}=.12$) of subjective health status, and the absence of disease (2%, ${\beta}=.13$). Conclusion: This study suggests that nursing interventions to promote self-care agency in older adults should be developed based on the characteristics of their subjective health status and wisdom.

Experience of Attending Sexual Health Education, Knowledge and Healthcare Attitude of Nurse Clinicians (임상간호사의 성교육 경험, 성지식 및 성 건강 간호수행태도)

  • Purpose: This study aimed to identify if participating in a sexual education program, influences knowledge, and healthcare attitude among attitudes of nurse clinicians in Korea. Methods: A cross-sectional study was conducted with 210 Korean nurse clinicians. Data were collected through self-reported questionnaires from July to August, 2011 from five hospitals in S and I cities. Data were analyzed using t-test, Pearson correlations, and multiple linear regression with SPSS/WIN 21.0. Results: This study reported that the majority of nurse clinicians had attended sexual programs about sexual harassment, reproductive system and function. There were significant correlations between experience of attending sexual education(r=.32, p=.003) and sexual knowledge(r=.14, p=.047) with sexual healthcare attitude. Sexual education on the job(${\beta}=.13$, p=.004), supplementary sexual education(${\beta}=.17$, p=.009), sexual knowledge(${\beta}=.16$, p=.048), and obstetrics and gynecology department carrier(${\beta}=.29$, p<.001) explained 14.0% of sexual healthcare attitude. Conclusion: This study's results demonstrate that advanced and continuous sexual health program is necessary for nurse clinicians. Proper sexual knowledge and positive attitude may promote nursing intervention and counseling concerning patients' sexual problems.
