1인실과 다인실 입원 병상 이용에 관한 병상 선택 요인과 환자 만족도 분석 (Use of Single Bedrooms versus Multiple Bedrooms: Selection Factors and Comparison of Patient Satisfaction)

  • Background: It is widely acknowledged that single bedrooms have many potential advantages compared to multiple bedrooms. However, Korea has a reimbursement system that patients have to pay the additional fee if they will use single bedroom or pay-bed (1-3 bedroom). There is little research on patients' bed selection and relationship between patient satisfaction and bed type in the Korean setting. Methods: Using the 2017 Korea Health Panel (KHP) Survey data, we modified bed type by two dichotomous variables: single bedroom vs. multiple bedroom (2+ bedrooms) and pay-bed (1-3 bedroom) vs. reimbursed bed (4+ bedroom). Multivariate logistic regression is performed to determine the factors affecting the patient's choice of room types. Multivariate regression analysis was conducted to examine how hospital room types are affecting patient satisfaction. Results: Single room and pay-bed (1-3 bedroom) use was influenced by patient age (19- years old), male, a person at work, hospitalizing in a clinic, and birth. After controlling variables of the behavioral model of medical utilization, the association between patient satisfaction and use of single bedroom & pay-bed (1-3 bedroom). Cause of hospitalization, major treatment, and recognition of unnecessary care are statistically significant variables on patient satisfaction. Conclusion: Although the single room is about the standard for newly built hospitals in western countries, it remains unclear that single room or pay-bed (1-3 bedroom) has positive effects on patient's outcomes and satisfaction. In this empirical study, the difference in patient satisfaction by bedroom type was not noticeable. In terms of bed management by hospital staff, securing patient credibility in hospital care is an important task. Patients' perception of whether medical staffs have encouraged unnecessary treatment or tests has a significant impact on patient's satisfaction.

LES를 이용한 복단면 개수로의 바닥전단응력 분포특성 분석 (Analysis of Bed Shear Stress Distributions in Compound Open Channels Using Large Eddy Simulation.)

  • 하천설계에서 제방과 홍수터의 안정성 확보를 위해서는 바닥전단응력을 고려하는 것이 필수적이다. 본 연구에서는 다양한 하폭과 수심에 따른 복단면의 바닥전단응력을 모의하여 분포 특성을 분석하였다. 바닥전단응력 분포에 지배적인 영향을 주는 이차류를 모의하기 위하여 OpenFOAM의 large eddy simulation (LES)를 적용하였으며 하폭과 수심을 고려하여 12개의 케이스를 모의하였다. 모의 결과를 이용하여 시간 평균 유속 분포, 이차류 분포, 바닥전단응력 분포 등의 특성에 대하여 분석하였다. 홍수터 바닥전단응력 분포는 전체적으로 홍수터 좌안에서 높은 값이 나타나고 우안 방향으로 감소하는 경향을 확인하였으나 이차류에 의해 상당한 국부적인 변화가 나타남을 확인하였다. 홍수터의 수심이 얕은 경우에는 홍수터의 바닥전단응력이 평균전단응력의 0.8배 이하로 낮은 값이 나타나고 있으나 홍수터의 수심이 깊은 경우에는 평균전단응력의 1.2-1.4배의 높은 값이 나타남을 확인하였다. 홍수터의 폭이 좁은 경우에는 홍수터 우안 측벽의 영향으로 국부적으로 높은 값이 나타나는 것도 확인할 수 있었다.

딸기 수경재배에서 베드 피복유무에 따른 시들음병과 점박이응애 발생 양상 (Occurrence of Fusarium wilt and Twospotted Spider Mite under Plastic Mulched and Non-Plastic Mulched Bed in Hydroponic Culture of Strawberry)

  • 딸기 수경재배시스템은 국내에서 매년 증가추세에 있다. 대부분의 딸기농가는 수경재배에서 베드를 멀칭하여 9월 초순 딸기묘를 정식한다. Fusarium oxysporum f. sp. fragariae에 의한 시들음병과 점박이응애 Tetranychus urticae는 정식 후 높은 온도조건에서 발생이 증가하는 경향이 있다. 따라서 베드 녹색 polyethylene(PE)필름 멀칭과 무멀칭처리에서 시들음병과 점박이응애 발생을 비교하기 위해 시험을 실시하였다. 녹색 PE필름 멀칭처리에서 시들음병 발생은 10월 초순부터 시작되었고 무멀칭처리보다 더 증가하였다. 녹색 PE피름 멀칭 처리는 점박이응애의 피해율이 무멀칭처리보다 높았다. 녹색 PE필름 멀칭처리의 상토내 온도는 무멀칭처리보다 증가했지만 대기내 상대습도는 감소하였다. 이런 결과로 딸기 수경재배에서 베드의 무멀칭처리는 시들음병과 점박이응애 발생을 낮추는 효과가 있다고 판단된다.

Nail bed defect reconstruction using a thenar fascial flap and subsequent nail bed grafting

  • Background Full-thickness nail bed defects with significant exposure of the distal phalanx are typically challenging to reconstruct. We describe a novel method of nail bed defect reconstruction using a thenar fascial flap combined with nail bed grafting. Methods Full-thickness nail bed defects were reconstructed in a 2-stage operation involving the placement of a thenar fascial flap and subsequent nail bed grafting. A proximally-based skin flap was designed on the thenar eminence. The flap was elevated distally to proximally, and the fascial layer covering the thenar muscle was dissected proximally to distally. The skin flap was then closed and the dissected fascial flap was turned over (proximal to distal) and inset onto the defect. The finger was immobilized for 2 weeks, and the flap was dressed with wet and ointment dressings. After 2 weeks, the flap was divided and covered with a split-thickness nail bed graft from the great toe. Subsequent nail growth was evaluated on follow-up. Results Nine patients (9 fingers) treated with the novel procedure were evaluated at follow-up examinations. Complete flap survival was noted in all cases, and all nail bed grafts took successfully. Five outcomes (55.6%) were graded as excellent, three (33.3%) as very good, and one (11.1%) as fair. No donor site morbidities of the thenar area or great toe were observed. Conclusions When used in combination with a nail bed graft, the thenar fascial flap provides an excellent means of nail bed reconstruction.

지역병상수급계획 실효성 제고를 위한 수요공급 현황 분석 (Analysis of Demand-Supply Status for Improving the Effectiveness of Plans for Supply and Demand of Reginal Patient Beds)

  • Background: The purpose of this study was to analyze the demand and supply status of patient beds by type of medical institution, categorized into 70 clinical privilege, in order to understand the regional bed supply situation. Methods: Utilizing the 70 clinical privilege defined by the Ministry of Health and Welfare, we calculated bed demand and supply quantities from 2019 to 2021 using data from Statistics Korea and the Health Insurance Statistical Yearbook. The bed demand calculation formula was based on the detailed guidelines for the medical sector by the Korea Development Institute and the 3rd edition of bed supply basic policies announced by the Ministry of Health and Welfare. Additionally, to mitigate distorted bed supply situations caused by factors such as regional levels and patient outflows, we classified bed supply types using the population decrease index indicator published by the Ministry of Public Administration and Security. Results: Among the 70 clinical privilege, it was analyzed that a relatively balanced bed supply situation exists overall, irrespective of the type of healthcare institution. However, in medical institutions at or above the level of hospitals, regions with bed supply ratios exceeding 20% compared to demand, particularly in institutions at or above the level of general hospitals, showed a relatively high rate of demand diversion. Conclusion: We have identified the bed supply types in the 70 clinical privilege in South Korea. Based on the results of this study, we emphasize the need for bed supply policies that consider regional characteristics. It is expected that this research can serve as fundamental data for future efforts aimed at managing or rectifying bed supply imbalances on a regional basis.

  • In this paper, an Intelligent Bed Robot System (IBRS) is proposed, that is a special bed equipped with robot manipulator. To assist a patient using IBRS, pose and motion estimation process is fundamental. It is designed to help the elderly and the disabled for their independent life in bed without other assistants. For this purpose, we use the pressure sensor distributed mattress for detecting the change of motion on the bed. Using that data, we control the robot arm to move to the appropriate position and serve to the user. In addition, we can estimate the user's intention based on the change of pressure and use those data to control the robot arm guide.

소각로내의 폐기물층 연소특성 파악을 위한 연소모델 개발 (A Development of Combustion Model for the Investigation of the Waste Bed Combustion Characteristics in a Waste Incinerator)

  • This study is to establish a waste bed combustion model that can be available to assist the design of incinerators for efficient operation control of municipal waste incinerators. An unsteady one -dimensional bed combustion modeling was developed which incorporates the various sub-process models and solves the governing equations for both gases and solids in the waste bed combustion phenomena. The combustion characteristics and the properties of the combustion gas released from the bed were investigated by using a developed model. Besides, a sub-model which predicts the formation and destruction of nitrogen oxides in the waste bed was also developed as a post-processor for the waste combustion model. It is found that the reduction rate of nitrogen oxides is enhanced in the char layer.

에폭시-그래나이트재를 이용한 연삭기 베드의 설계 및 동특성 해석 (The Design and Dynamic Characteristics Analysis of the Grinder Bed using Epoxy-Granite)

  • In this study, the griding machine bed was designed and analyzed by using epoxy-granite. Finite element technique was employed to design and analyze the composite bed structure. Von Mises stress and relative displacements between a tool and a workpiece were checked to compare a cast iron bed structure with a composite bed structure. Dynamic features of structures were also studied according to the process of modal analysis through natural frequency and mode shape measurement. Another improved model was also designed using the accumulation of knowledge based on the use of the structural analysis and experiments. Comparing with the cast iron bed structure, the composite bed structure maintains better functions.

A simple and rapid approach to modeling chromium breakthrough in fixed bed adsorber

  • A simple mathematical model for predicting fixed bed adsorption dynamics is described. The model is characterized by a linear adsorption isotherm and a linear driving force expression for mass transfer. Its analytic solution can be approximated with an algebraic equation in closed form which is easily evaluated by spreadsheet computation. To demonstrate one application of the fixed bed model, a previously published adsorption system is used as a case study in this work. The adsorption system examined here describes chromium breakthrough in a fixed bed adsorber packed with imidazole functionalized adsorbent particles and is characterized by a nonlinear adsorption isotherm. However, the equilibrium behavior of the fixed bed adsorber is in essence governed by a linear adsorption isotherm due to the use of a low influent chromium concentration. It is shown that chromium breakthrough is predicted reasonably well by the fixed bed model. The model's parameters can be easily extracted from independent batch experiments. The proposed modeling approach is very simple and rapid, and only Excel is used for computation.

제철 소결공정의 철광석-코크스 베드에서의 연소와 열전달 해석 (Prediction of Combustion and Heat Transfer in the Sintering Bed of Iron Ore)

  • Sintering bed of iron ore in the steel making process is one of typical applications of solid fuel bed, which has relatively uniform progress of fuel and simple processes of combustion. The sintering bed was modelled as an unsteady one-dimensional progress of fuel layer containing the two phases of solid and gas. Cokes added to the raw mix of which the amount is about 3.5% of the total weight was assumed to form a single particle with other components. In the early predition results presented in this paper, the flame propagation within the bed was not sustained after the top surface of the bed was ignited with hot gas. It suggests that the model should be extended to consider the multiple solid phase, which can treat the ore particles and the coke particles separately.
