Success rates of the first inferior alveolar nerve block administered by dental practitioners

  • Background: Inferior alveolar nerve block (IANB) of the mandible is commonly used in the oral cavity as an anesthetic technique for dental procedures. This study evaluated the success rate of the first IANB administered by dental practitioners. Methods: Volunteer dental practitioners at Mahidol University who had never performed an INAB carried out 106 INAB procedures. The practitioners were divided into 12 groups with their advisors by randomized control trials. We recorded the success rate via pain visual analog scale (VAS) scores. Results: A large percentage of the dental practitioners (85.26%) used the standard method to locate the anatomical landmarks, injecting the local anesthetic at the correct position, with the barrel of the syringe parallel to the occlusal plane of the mandibular teeth. Further, 68.42% of the dental practitioners injected the local anesthetic on the right side by using the left index finger for retraction. The onset time was approximately 0-5 mins for nearly half of the dental practitioners (47.37% for subjective onset and 43.16% for objective onset), while the duration of the IANB was approximately 240-300 minutes (36.84%) after the initiation of numbness. Moreover, the VAS pain scores were $2.5{\pm}1.85$ and $2.1{\pm}1.8$ while injecting and delivering local anesthesia, respectively. Conclusions: The only recorded factor that affected the success of the local anesthetic was the administering practitioner. This reinforces the notion that local anesthesia administration is a technique-sensitive procedure.

  • A large diameter sampler was developed to take undisturbed samples from not only soft ground but also sandy and weathered ground. The large diameter sampler which was developed in Korea Institute of Construction Technology(KICT-type large diameter sampler) was manufactured based on the principle of triple core barrel sampling. A specially designed cutting device was used to cut and contain various kinds of samples in the sampler during a sampling and retrieval procedure. By adjusting the stiffness of the spring located at the top of the sampler, the distance between the cutting shoe and auger can be controlled in accordance with the ground condition. In order to investigate the applicability of the developed sampler and compare the quality of the samples taken by the sampler with that by the traditional thin-walled tube sampler, samples were taken at various sites according to the ground condition. And a series of laboratory tests such as the unconfined compress ion test, triaxial compression test, oedometer test, large diameter Rowe cell consolidation test (D: 150 mm) were performed. The test results showed that the samples by the KICT-type large diameter sampler show higher quality than the samples by the thin-walled tube sampler. And the validity and applicability of the developed KICT-type large diameter sampler was confirmed accordingly.

  • Piezo-ceramics are special materials which transform energy from mechanical to electrical forms and vice versa. Bender elements are composite materials consisting of thin piezo-ceramics and elastic shims, and are widely used as actuators and transducers in the field of electronics, robotics, autos and mechatronics utilizing the effectiveness of energy transformation capability. In geotechnical engineering, commercial bender elements are used in laboratory as source and receiver in the measurements of soil stiffness. The elements were built by using various metal shims sandwiched between piezo-ceramics and coating over the composite in the research. A pair of elements were buried in a concrete block and used as source and receiver to measure the stiffness of the concrete. The test results were verified by comparing with the resonant column testing results. In a preliminary stage of the development of an in-situ seismic testing equipment using bender elements for soft clay materials, shear waves were generated and measured by burying the elements in the barrel of kaolinite and water mixture. The measured shear wave signals were so distinct for the first-arrival pick that applicability of the elements in the field measurements could be very promising.

  • An integrated analysis of kerosine/LOX based KSR-III rocket body/plume flowfield has been performed. The analysis has been executed employing three kind of gas thermo-chemical models including calorically perfect gas, multiple species chemically reacting gas, and chemically frozen gas models and their effect on rocket flowfield has been accessed to provide the most appropriate gas thermo-chemical model which meets a specific purpose of performing rocket body and plume analysis. The finite-rate chemically reacting flow solution exhibited higher temperature throughout the flowfield than other gas models due to the increased combustion gas temperature caused by the chemical reactions within the nozzle. All the reactions were dominated only in the shear layer and behind the barrel shock reflection region where the gas temperature is high and the effect of finite-rate chemical reactions on the flowfield was found to be minor. However, the present plume computation including finite-rate chemical reactions revealed major reactions occurring in the plume and their reaction mechanisms and as well.

A Study on the Middle Age Costume Expression of France (프랑스 <마카이요프스키 성경(Maciejowski Bible)>의 중세복식 표현에 관한 연구)

  • In picture bible, the picture is helpful for showing story more vividly and specifically and it is helpful for finding the meaning of bible hidden behind familiarity. It plays the role of the most powerful testimony and expression, with which human provides himself. The purpose of study was to grasp the relation among historical background of the middle age, human's characteristic and symbolic color by considering the fact that the costume expressed in Maciejowski Bible, which is a picture bible of the middle age and observing type, color and decoration of costume. Among 46 folios organizing Maciejowski Bible, 129 pictures explaining the Book of Genesis, the Book of Exodus, the Book of Leviticus, the Book of Numbers, the Book of Deuteronomy, the Book of Joshua and the Book of Judges, which belong to the former 400 years of the old Testament, were analyzed for people's costumes. The pictures shown in Maciejowski Bible portrays object, costume and behavior on the background of France of the 13th century so we can find out the precious natures of middle age's costume, weapon and armor. Especially, the portrait about finger armor of chain mail attached in hauberk becomes the outstanding evidence that can encroach the discussion over whether it was used in the end of 1300s. Regarding costumes of main characters, they wore chemise and braies as underwear, wore tunic or cotte having various lengths from the length reaching knee to the length reaching 8round as basic costume and wore mantle with different-colored lining and hose with various colors. In the war picture, we can see hauberk made with chain mail and cyclas with long slit, which was worn over hauberk, barrel helmet, norman helmet and steel school cap. Regarding cap. man usually wore coif, beret, shade-shaped cap, straw hat and woman wore the decoration using veil and hairlace and used the method to wrap whole hair with net. As the colors used in the costume of the picture bible, scarlet, blue, green and brown were usually used in order. They usually wore costumes decorated splendidly with the colors haying hish brightness and chroma.

Influence of the cylinder height on the elasto-plastic failure of locally supported cylinders

  • Frequently, steel silos are supported by discrete supports or columns to permit easy access beneath the barrel. In such cases, large loads are transferred to the limited number of supports, causing locally high axial compressive stress concentrations in the shell wall above the supports. If not dealt with properly, these increased stresses will lead to premature failure of the silo due to local instability in the regions above the supports. Local stiffening near the supports is a way to improve the buckling resistance, as material is added in the region of elevated stresses, levelling these out to values found in uniformly supported silos. The aim of a study on the properties of local stiffening will then be to increase the failure load, governed by an interaction of plastic collapse and elastic instability, to that of a discrete supported silo. However, during the course of such a study it was found that, although the failure remains local, the cylinder height is also a parameter that influences the failure mechanism, a fact that is not properly taken into account in current design practice and codes. This paper describes the mechanism behind the effect of the cylinder height on the failure load, which is related to pre-buckling deformations of the shell structure. All results and conclusions are based on geometrically and materially non-linear finite element analyses.

Starch Liquefaction and Residence Time Distribution in Twin-Screw Extrusion of ${\alpha}$-Starch (호화전분의 쌍축형 압출성형에서 전분액화 및 체류시간 분포)

  • ${\alpha}$-Waxy corn starch was used as a feed for twin-screw extrusion in order to enhance starch liquefaction with added thermostable ${\alpha}$-amylase (derived from Bacillus licheniformis). The residence time distribution and starch liquefaction were investigated. The starch liquefaction was analyzed in terms of reducing sugar contents, molecular size from gel permeation chromatography (GPC), and microstructure from scanning electron microscopy (SEM). The use of ${\alpha}$-starch contributed to the production of more reducing sugar than the use of raw starch use alone. From GPC, the effect of ${\alpha}$- starch on the molecular size reduction was shown to be small. From SEM, irregular and damaged surface were observed on the extrudate from ${\alpha}$-starch, as compared to those from raw starch. The spread of residence time distribution curves was greater with feed of ${\alpha}$-starch than raw starch, indicating that ${\alpha}$-starch was hard to flow forward during extrusion. This could be improved by increasing the feed moisture content and barrel temperature of extruder.

  • Escherichia coli AcrAB-TolC is a multidrug efflux pump that expels a wide range of toxic substrates. The dynamic nature of the binding or low affinity between the components has impeded elucidation of how the three components assemble in the functional state. Here, we created fusion proteins composed of AcrB, a transmembrane linker, and two copies of AcrA. The fusion protein exhibited acridine pumping activity, suggesting that the protein reflects the functional structure in vivo. To discern the assembling mode with TolC, the AcrBA fusion protein was incubated with TolC or a chimeric protein containing the TolC aperture tip region. Three-dimensional structures of the complex proteins were determined through transmission electron microscopy. The overall structure exemplifies the adaptor bridging model, wherein the funnel-like AcrA hexamer forms an intermeshing cogwheel interaction with the ${\alpha}$-barrel tip region of TolC, and a direct interaction between AcrB and TolC is not allowed. These observations provide a structural blueprint for understanding multidrug resistance in pathogenic Gram-negative bacteria.

The Study on the Silver Fashion Icon Iris Apfel's Fashion Style (실버 패션 아이콘 Iris Apfel의 패션 스타일에 관한 연구)

  • This study considers aesthetic characteristics by examining the fashion style of the silver fashion icon Iris Apfel. The research methods were a quantitative and qualitative analysis of Iris Apfel's images that were collected from 2015 to 2019 on various web-sites according to four criterions following advanced research analysis of fashion style. The results of the study are as follows. The analysis results on the fashion style of Iris Apfel, an icon of silver style, showed that cocoon, barrel and A-line silhouettes appeared most in terms of silhouettes. Second, in terms of colors, achromatic colors dominated among solid colors while one particular vivid color appeared most it came to mixed color. In terms of multi colors, these appeared according to the patterns applied to her clothing, in particular, colorful colors were used to emphasize splendor. Third, flower, bird and geometric patterns appeared most in terms of material patterns. Lastly, it was found that white short cut hair, large necklaces or bangle bracelets, over-sized black glasses and fur mufflers or canes were used in terms of hair and accessories. The features derived through analysis of the fashion style of Iris Apfel, an icon of silver style, are as follows. The first feature is exaggeration through splendid primary colors and over-sized silhouettes. The second feature is the hybrid of modern composition methods using natural images and exotic preferences. The third feature is her representation of identity using fixed items.

Dynamic Pressure Characteristics of Pulse Gun Device for Combustion Stability Rating of Liquid Rocket Engines (액체 로켓엔진 연소 안정성 평가를 위한 압력 교란 장치 특성 연구)

  • For the assessment of combustion stability of a liquid rocket engine, a device called "Pulse Gun" should be developed first, which can induce artificial perturbations that may lead to excitations of pressure oscillations in a combustion chamber. A model chamber has been used for identifying design parameters of a pulse gun that defines its characteristics. Dynamic pressure measurements showed that shock waves generated from pulse guns are axisymmetric around the axis of a pulse gun barrel. Pressure waves perturbed by a pulse gun induce resonant acoustic frequencies of a model chamber. This fact indicates that successful pressure field perturbations of the KSR-III combustion chamber can be performed by a newly developed pulse gun device. A maximum value of dynamic pressure peaks measured at the opposite point against a pulse gun outlet becomes stronger as charge mass of pulse gun powder increases.
